Anomalous origin of a coronary aortic artery is a rare cardiac anomaly. Although it can cause angina, syncope, and palpitations, most patients are asymptomatic. This anomaly requires surgical treatment or intervention because it is associated with sudden death. Several surgical techniques, such as coronary reimplantation, coronary artery bypass grafting (CABG), unroofing, and neo-ostium formation, have been proposed as treatments. We report a case surgically treated with neo-ostium formation in anomalous origin of the left coronary artery from the right coronary sinus.
Sixty-one consecutive patients with coronary artery bypass graft for myocardial revascularization were retrospectively reviewed to analyze various pattern of postoperative complication and death during hospital stay from Nov. 1988 to Oct. 1992. Fortytwo of the patients were male and nineteen female. The mean age was 56 and 51 years in male and female. Preoperative diagnosises were unstable angina in 14 of patients, stable angina in 28, postmyocardial infarction state in 15, and state of failed percutaneous transluminal coronary angioplasty in 4. 141 stenosed coronary arteries were bypassed with use of 20 pedicled internal mammary artery and 124 reversed saphenous vein grafts. Postoperative complications and perioperative death were as follows: 1. Of 61 patients undergoing operation, peri and postoperative over all complication occured in 15 patients [ 25% ]; newly developed myocardial infarction in 4, intractable cardiac arrhythmia including atrial fibrillation and frequent ventricular premature contraction in 3, bleeding from gastrointestinal tract in 2, persistent vegetative state as a sequele of brain hypoxia in 1, wound necrosis in 1, left hemidiaphragmatic palsy in 3 and poor blood flow through graft in 2. 2. Operative mortality was 8%[5 patients]. 3 out of these died in operating room; 1 patient by bleeding from rupture of calcified aortic wall, 1 by air embolism through left atrial vent catheter, 1 by low cardiac output syndrome. 2 patients died during hospital stay; 1 by acute respiratory distress syndrome with multiuple organ failure, 1 by brain death after delayed diagnosis of pericardial tamponade.
67세 남자환자가 6개월 전부터 반복되는 배부 동통을 주소로 내원하였다. 환자는 11년 전 Stanford B형 대동맥 박리로 하행 대동맥 스텐트 도관 삽입술을 시행 받았다. 대동맥 컴퓨터 단층촬영에서 좌 쇄골하동맥 기시부에서 시작되어 복부 대동맥 분지 부위까지 진행된 대동맥 박리, 스텐트 도관 근위부의 제 1형 endoleak 및 하행 대동맥의 낭종성 확장을 관찰하였다. 체외 심폐 순환 하에 무명동맥, 좌 총경동맥, 좌 쇄골하동맥의 탈분지 및 재혈관화술을 시행하고, 수술 후 13일째 대동맥 궁에서 하행대동맥까지 스텐트 도관을 삽입하는 Hybrid 술식을 성공적으로 시행하였고, 수술 후 시행한 대동맥 컴퓨터 단층촬영에서 제 1형 endoleak의 완전한 소실을 보였으며 퇴원 후 8개월간 특이 합병증 없이 경과 관찰 중이다.
심혈관계 수술에 있어서도 무혈수술에 대한 관심이 높아져가고 있으나, 현재까지의 무혈 심혈관계 수술은 성인 환자 층에 주로 국한되어 있다. 특히 심폐회로를 이용한 소아의 심혈관계 수술 시에는 혈액희석의 문제로 무혈수술은 기피되어 왔다. 저자들은 최근 여호와의 증인 신자를 부모로 둔 2.8 kg 환아에서 심폐회로를 이용하여 대동맥축착성형술, 심방중격결손 폐쇄술을 무혈수술로 시행한 1예를 경험하여 이에 대해 보고하고자 한다.
This is a report on four cases of successful surgical correction of coarctation of the aorta [COA] in Department of the Thoracic & Cardiovascular Surgery, Hanyang University Hospital. The first case was a postductal type of coarctation of the aorta associated with Patent ductus arteriosus [PDA], Persistent left superior vena cava [LSVC] and richly developed collateral circulation. Blood pressure was measured to be hypertensive at the arm, but hypotensive at the legs. The coarctation of the aorta was corrected with following procedure: Partial resection of the aortic wall with diaphragmatic structure lust above and below the coarctating line of the aorta, and then the defect of the aortic wall was closed by lateral aortographic suture. PDA was closed by ligation procedure. The second case a preductal type of coarctation of the aorta associated with PDA, LSVC, ventricular septal defect [VSD] and poorly developed collateral circulation. Normal blood pressure was measured at the arm, but hypotension was observed at the legs. Correction of coarctation of the aorta was performed under the establishment of tube bypass because of poor collateral circulation. After resection of coarctating short segment, end to end anastomosis was performed without any tension. PDA was closed by division procedure. Simple suture closure of VSD was performed by open heart surgery two weeks after correction of COA. The third case was a long segment COA without any other anomaly. Blood pressure was measured to be hypertensive at the arm, but hypotensive at the legs. Vascular prosthesis was performed using Teflon graft tube after resecting coarctating long segment [6.5 cm] of the aorta. The fourth case was a long segment COA associated with aortic insufficiency and richly developed collateral circulation. Normal blood pressure was measured at the arm, but hypotension was observed at the legs. Vascular prosthesis was performed using Teflon graft tube after resecting coarctating long segment [6.0 cm] of the aorta. Both blood pressure and peripheral pulse on the arm and the legs returned to normal postoperatively in all patients.
Aneurysm of the Aorta is a grave disease mostly producing disabling symptoms and ultimate death by rupture and hemorrhage without surgical intervention. The author recently experienced one case of surgical correction of descending thoracic aortic aneurysm treated with excision of the aneurysm and replacement of Dacron artificial vessel under temporary external by pass technique in November, 10th, 1975. 9mm internal diameter arterial cannula was inserted into upper and below the aneurysm. Bypass time was about 1 hour. The case was 35 years old women who had small egg sized saccular aneurysm in the upper third of the descending thoracic aorta involving the 1t. subclavian artery. Histopathological diagnosis was arteriosclerotic. Immediate postoperative course had been uneventful except low pressure and pulse of the left arm. The follow-up was possible up to date about 3 months. The patient has been doing well with ordinary activities except mild left chest discomfort.

Purpose: A recent study analyzing several cytokines reported that long cardiopulmonary bypass (CPB) time and long aortic cross clamp (ACC) time were accompanied by enhanced postoperative inflammation, which contrasted with the modest influence of the degree of hypothermia. In this present study, we aimed to examine the effect of CPB temperature on the clinical outcome in infants undergoing repair of isolated ventricular septal defect (VSD). Methods: Of the 212 infants with isolated VSD who underwent open heart surgery (OHS) between January 2001 and December 2010, 43 infants were enrolled. They were classified into 2 groups: group 1, infants undergoing hypothermic CPB ($26^{\circ}C-28^{\circ}C$; n=19) and group 2, infants undergoing near-normothermic CPB ($34^{\circ}C-36^{\circ}C$; n=24). Results: The age at the time of the OHS, and number of infants aged<3 months showed no significant differences between the groups. The CPB time and ACC time in group 1 were longer than those in group 2 (88 minutes vs. 59 minutes, P =0.002, and 54 minutes vs. 37 minutes, P =0.006 respectively). The duration of postoperative mechanical ventilation was 1.6 days in group 1 and 1.8 days in group 2. None of the infants showed postoperative neurological and developmental abnormalities. Moreover, no postoperative differences in the white blood cell count and C-reactive protein levels were noted between two groups. Conclusion: This study revealed that hypothermic and near-normothermic CPB were associated with similar clinical outcomes and inflammatory reactions in neonates and infants treated for simple congenital heart disease.
During the period from September 1989 through December 1992, 118 cases of coronary arterial bypass graft were performed at Department of Cardiothoracic Surgery, Asan Medical Center. Twenty-one of these had history of recent or remote percutaneous transluminal coronary angioplasty. They consisted of 13 males[age,58.7 + 5.4 years] and 8 females[age, 63.6 + 2.8years] with the mean age of 60.6. History of old myocardial infarction was noted in 24%[5/21] of the patients and congestive heart failure in 2 cases. The angina by type of presentation is unstable in all of the patients. The patterns of involvement of coronary arterial disease were left main disease[1], single vessel disease[5], double vessel involvement[10], and triple vessel involvement[5]. We performed 4 cases of single bypasses, 7 cases of double, 8 cases of triple, and 2 cases of quadruple bypasses. Total of 51 grafts[LIMA:12, RSVG:39] were inserted in 21 cases with average of 2.4 grafts per patient. The methods of myocardial protection were cold blood cardioplegia[8 cases], intermittent aortic occlusion[11], and continuous coronary perfusion with local coronary sharing[2]. There were no operative or late death. The only cardiac complication was 1 case of low cardiac output required IABP. The other complications were 1 case of sternal wound infection and 1 case of postoperative bleeding required reoperation. And there was no case of perioperative myocardial infarction. Postoperatively, 3 cases of recurrent angina were detected at 5, 7, and 18months after surgery. One of them was managed successfully with repeat PTCA[who was recurred 18 months postoperatively], and the other two with medication. I conclude that we can approach the patients more aggressively with PTCA, because of our acceptable operative risks.
In order to assess the correlation of the myocardial damage and the duration of cardiopulmonary bypass, measurement of creatine kinase [CK], lactate dehydrogenase [LDH], asparatate aminotransferase [AST], and MB band of CK [CK-MB] were carried out on the first, third, fifth, seventh, and ninth day in 44 patients following open heart surgery [POD 1,3,5,7,9]. And the patients were divided into three groups according to the duration of aortic cross clamp time [ACT]: Group I [ACT< 60 minutes. n=19], Group II [60 minutes < ACT< 90 minutes, n=7] and Group III[90 minutes > ACT, n=18]. 1. The leakage of CK in total patients increased to the highest level at POD 1, with rapid decrease and recovery at POD 7. The leakage of CK in Group III were greater than in Group I from POD 1 to POD 3 [P < 0.01]. The recovery time of CK level was shorter in Group I [POD 3] than in Group II and III [POD 7]. 2. The serum levels of LDH in total patients increased to the highest level at POD 1, with slow recovery until POD 9. The levels of LDH in Group III were higher than in Group I until POD 9 [P < 0.005]. The levels of LDH in Group I and II recovered but not in Group III. 3. The serum levels of AST in total patients increased to the highest level at POD 1, with rapid decrease and recovery at POD 7. The levels of AST in Group III were greater than in Group I from POD 1 to POD 5 [P < 0.05]. The recovery time of AST level was shorter in Group I and II [POD 5] than in Group III [POD 7]. 4. The positive cases for CK-MB in 36 patients were 22 [61.1 %] as a whole, 5[41.6%] in Group I, 4[57.1 %] in Group II, 13[76.4 %] in Group III at POD 1, and a case in each group at POD 3, and only a case in Group Ill at POD 5. It is concluded that the myocardial injury was closely related with the duration of cardiopulmonary bypass in open heart surgery.
연구배경 : 개심술 후의 부정맥은 빈번하게 발생하는 합병증이며 그 종류도 다양할 뿐 아니라, 수술후 발생하는 부정맥은 심박출량의 저하 등 심각한 결과를 초래할 가능성이 있다. 재료 및 방법 : 본 연구는 이러한 부정맥의 예방과 치료의 방침을 결정하는데 기본적인 자료를 제공하고자 1994년 6월부터 1995년 5월까지 1년간 서울대학교병원 흉부외과에서 개심술을 시행 받은 성인 환자들을 대상으로 술후 부정맥의 양상을 전향적으로 분석하여 위험인자를 유추하였다. 결과 : 총 302명을 대상으로 하였는데, 그 중 남자가 150명이었고 여자는 152명이었으며, 평균 연령은 43.9세 (16세부터 75세까지) 였다. 대상환자 모두 술전 및 술후 표준 12-lead EKG 및 중환자실에서의 24시간 심전도 감시장치로 부정맥을 진단하였으며 수술직후 집중감시병동에서는 동맥혈 가스분석 및 혈중 potassium 농도를 측정하여 이상이 있으면 교정하였고 단순히 산혈증이나 저칼륨혈증에 의한 부정맥은 연구대상에서 제외하였다. 술후 부정맥의 전체 발생률은 58.3%이었는데, 판막 재수술의 경우 부정맥이 77.8%에서 나타났고, 단순 판막 수술, 관상동맥 우회술, 대동맥 수술, 선천성 심기형의 수술후의 부정맥 발생률들은 각각 70.8%, 45.3%, 40.0%, 29.5% 이었다. 연령별 발생은 의미있는 차이를 보이지 않았으며 심정지액의 종류도 의미있는 차이는 보이지 않았다. 반면에 수술의 종류, 술전 부정맥의 유무, 체외순환 및 대동맥 차단시간, 그리고 술전 시행한 심초음파상의 좌심실 확장기말과 수축기말 내경, 좌심방의 내경 등은 부정맥의 발생률과 통계적으로 유의한 상관관계를 보여주었다 (p< 0.05). 결론 : 향후 질병, 수술방법 등이 균질화된 집단을 선정하여 전향적인 연구를 진행함으로써 개심술후 부정맥의 발생, 치료 및 예방에 관한 보다 정확한 결론에 접근할 수 있을 것으로 생각한다.
