아동용 외상관련 증상 평가척도의 효용성 - 성폭력 피해 아동과 일반 아동의 비교 - (The Effectiveness of Traumatic Symptom Checklist for Children(TSCC) - Comparisons of Sexually Abused Children and Nonabused Normal Children -)

  • Objectives : The research is designed in order to know how much Traumatic Symptom Checklist fur Children(TSCC) is effective in the case of child abuses, especially in the estimation of the psychopathology of child sexual abuse. Methods : A sample includes a group of thirty children of 4th, 5th and 6th grade whose sexual abuse have confirmed and also a group of thirty four normal kids who have no abuse. Along with TSCC, Children's Depression Inventory (CDI) and Revised Children's Manifest Anxiety Scale (RCMAS) were used to children and child behavior checklist (K-CBCL), sexual behaviors (CSBI), and Kiddie-Schedule for Affective Disorder and Schizophrenia-Present and Lifetime version (K-SADS-PL) record sheets were used to kids' parents. Results : In case of the sexual abuse group, the most meaningful difference from the analyzed result is shown at sexual concerns scale which is one of the clinical scale in TSCC (p<.01). Also, the sexual abuse group has the meaningful higher score than the general group's one in the stress scale after the other anxiety, depression, anger and dissociation (p<.01). Conclusion : The scale of TSCC has relatively shown the characteristic symptom and severity which were gained from the children who have experienced trauma, especially sexual abuse.

일부 제조업 근로자의 스트레스와 정신건강에 관한 연구 (Work Stress and Mental Health of the Industrial Workers)

  • Information on work stress and mental health status of industrial workers was required for optimum staffing and health care management. This study dealt with the relationship between mental health status examined by PSI(psychiatric symptom index) and socio-demographic variables. Especially this study attempted to find relevant determinants of the stress and mental health through multiple stepwise regression analysis based on data obtained from 687 occupational workers. The findings of this study were as follows; Mean scores of symptom dimension were higher in youth, female, more educated, unmarried and divorced, and workers in the noisy condition and short duration in job. And it showed high scores in long duration in job for anxiety; college educated for anger; unmarried for cognitive disturbance. The factors affecting the mean scores of psychiatric symptom index were varied according to the types of symptom: age, sex and duration in job for anxiety; age, sex, living with parent, education and marital status for anger; sex and marital status for depression; noisy condition and sex for cognitive disorder.

심장질환자의 심리사회적 요인 및 증상 경험간의 관계 : 6개월 추적조사 (Relationship between Psychosocial Factors and Symptom Experience in Patients with Cardiovascular Disease: Over a Six-month follow-up Period)

  • Purpose: The purpose of this study was to identify the relationship between psychosocial factors and symptom experience of patients with cardiovascular disease over a 6-month follow-up period. Method: Baseline data for each of the 138 patients were collected. Eighteen patients were dropped for a final total of 120 patients in the present study. To monitor patients' symptom experience after discharge, each patient was interviewed by telephone, and interviewed directly with a questionnaire at each clinic visit. Results: Mean scores for hostility, anger, anxiety, and depression were 51.63, 22.87, 48.95, and 41.21, respectively. Mean scores for the level of symptom experience at discharge, 3 months and 6 months after discharge were 32.83, 24.79, and 26.70, respectively. There were significant differences in the level of hostility by gender, BMI, job, and monthly income. Also the differences in the level of anxiety and depression were statistically significant according to gender and existence of spouse. In the regression analysis, depression was identified as the significant factor associated with symptom experience at discharge, 3 months, and 6 months after discharge. Conclusion: This study presented baseline data to elucidate whether psychosocial factors were associated with the progression of cardiovascular diseases. Accordingly, it suggests that nurses need to take into consideration psychosocial factor as well as physiological factor in symptom management for patients with cardiovascular disease.

한국판 우울과 신체 증상 평가 척도(Depression and Somatic Symptom Scale, DSSS)의 신뢰도와 타당도 (Reliability and Validity of Korean Version of Depression and Somatic Symptom Scale(DSSS))

  • Objectives : We examined the reliability and validity of the Korean version of the Depression and Somatic Symptom Scale (DSSS) in Korean patients with depressive symptoms. Methods : Participants were 55 outpatients diagnosed with major depressive disorder, a depressive episode of bipolar I disorder, somatoform disorder, panic disorder, generalized anxiety disorder, or post-traumatic stress disorder according to the DSM-IV criteria. We assessed them using the Korean versions of the DSSS, Hamilton Depression Rating Scale (HDRS), and Quick Inventory of Depressive Symptomatology Self Report (QIDS-SR). Results : The Korean version DSSS had a Cronbach's alpha of 0.90. Moreover, each item's correlation with the total score was statistically significant (r=0.24-0.71, p<0.01). The test-retest correlation coefficient (r=0.83, p<0.01) was relatively high, and the DSSS correlations with the HDRS and QIDS-SR were 0.77 and 0.74, respectively. Conclusion : These results demonstrate that the Korean version of the DSSS could be a reliable and valid tool for screening and assessing depressive patients. The Korean version of the DSSS will be a useful tool for screening both depressive and somatic symptoms in Korea.

우울, 불안증상을 동반한 인후두역류증 환자 한방치험 1례 (A Case Report on Laryngopharyngeal Reflux Combined with Depressive and Anxiety Disorder)

  • Objective: This study was performed to report the effect of traditional Korean medicine as a treatment for laryngopharyngeal reflux (LPR) combined with depressive and anxiety disorder. Methods: We treated this patient with traditional Korean medicine and measured symptom severity using the reflux symptom index (RSI), Beck depression inventory (BDI), and Beck anxiety inventory (BAI). Results: After 3 weeks of treatment, most symptoms had decreased. The RSI score dropped from 21 to 8, BDI from 27 to 14, and BAI from 29 to 15. Conclusions: Traditional Korean medicine may be effective as a treatment for LPR combined with depressive and anxiety disorder, and a correlation may exist between LPR and psychological factors. However, more rigorous studies are required to identify exactly what treatment is most efficient for relieving LPR combined with depressive and anxiety disorder and whether LPR and psychological factors are clearly correlated.

공황장애 환자에서 공존 주요 우울증과 연관된 임상요인들 (Clinical Factors Associated with Comorbid Major Depressive Disorder in Patients with Panic Disorder)

  • Objective : Panic disorders are frequently accompanied by major depressive disorder (MDD). There is insufficient information about which clinical factors in panic disorder are associated with comorbid MDD. The aim of this study is to identify clinical factors related with comorbid MDD in patients with panic disorder. Methods : Two experienced psychiatrists diagnosed panic disorder based on DSM-IV criteria. This diagnosis in the 275 subjects was confirmed again by Mini-International Neuropsychiatric Interview (MINI). Lifetime comorbid psychiatric diagnoses were examined by MINI. The Hamilton Depression Scale (HAMD), Hamilton Anxiety Scale (HAMA) and Panic Disorder Severity Scale (PDSS) were used to assess the severity of depressive, anxiety and panic symptoms. Results : The result of MINI showed that 95 patients (34%) with panic disorder satisfied the diagnosis of MDD. Multivariate logistic regression model showed that comorbid generalized anxiety disorder (GAD) and the symptom of "fear of losing control or going crazy" were associated with MDD in patients with panic disorder. In female patients, the "chills or hot flushes" symptom was also associated with comorbid MDD. Conclusion : These results showed that coexisting GAD and certain symptoms of panic are associated with comorbid MDD.

퇴원시 간호정보 제공이 개심술 환자의 지식, 신체증상, 상태불안 및 일상활동에 미치는 효과에 관한 연구 (The Effects of Nursing Information on knowledge, physical symptoms, state anxiety and daily activity levels of patients recovering from open heart surgery)

  • This study investigated the effects of nursing in formation given verbally and a booklet on the knowledge, Physical symptoms, anxiety and daily activities of patients recovering from open heart surgery from just before discharge to six weeks after discharge. The convenience sample was of patients who had undergone open heart surgery in Seoul National University Hospital. The first 25 patients who agreed to participate in the study became the control group. They did not receive the intentional information but the usual nursing care. The next 28 patients became the experimental group who received a booklet about their post-discharge care which was discussed with them by the researcher before discharge. This study used a non equivalent control group non - synchronized quasiexperimental design. The tools included a 30 item knowledge scale, a 9 item physical symptom scale, Spielberger's state anxiety scale, and a 28 item daily activity scale. The instruments were applied before discharge and at two and six weeks after discharge. For data analysis, frequency, t-test, Pearson's Correlational Coefficient and Cronbach's $\alpha$ were used. The results were as follows : 1. Information given through the booklet was effective in increasing the knowledge of patients recovering from open heart surgery. The experimental group's knowledge was higher at 2 weeks after discharge than prior to discharge (P<0.05). 2. There was no significant difference in physical symptoms between the two groups at two and six weeks after discharge. 3. Information given through the booklet was effective in decreasing the State Anxiety two weeks after discharge (P<0.001) but at six weeks after discharge, there was no significant difference in anxiety between the control and the experimental groups. 4. There was no significant difference in daily activity between the two groups at two and six weeks after discharge. 5. There was a negative correlation between knowledge at discharge and at 2 weeks and anxiety at 2 weeks after discharge. There was a positive correlation between knowledge at discharge and daily activity at 2 weeks after discharge. There was a negative correlation between knowledge at 2 weeks and Physical symptom at 6 weeks after discharge.

기능성 상부 위장관 증상을 호소하는 환자의 스트레스, 불안 및 우울 (Stress, Anxiety, and Depression of the Patients Who Complained of Functional upper Gut Symptoms)

  • Objective : The sensation of dysphagia, heartburn, globus hystericus, and functional dyspepsia are common symptoms of the functional upper gut disorders. This study was designed to investigate quantity of perceived stress, depression, and anxiety in the patients with functional upper gut symptoms whose esopahgeal manometry(EM) and gatroesophageal reflux (GERT) test were normal. Methods : A total of 38 patients who complained of the symptoms had been tested with 24-hour ambulatory EM and conventional GERT in our gastrointestinal clinic. Thirty patients whose tests had been normal(patients group) were assessed with Symptom Checklist-90-Revision(SCL-90-R), Beck Depression Inventory(BDI), and Spielberger Stait-Trait Anxiety Inventory(STAI) and compared with 30 patients(control group) without functional upper gut symtpoms in the hepatobiliary clinic. The two groups were also assessed by quantity of perceived stress during the last year through self-report. Results: 1) These patients tended to be predominently female, older, and possessed a lower education than control group. 79% of 38 patients who had been tested were normal. 2) Compared to the control group, the patients had significantly higher mean scores on four subscales(somatization, depression, anxiety, and positive symptom distress index). 3) The patient group had significantly more perceived stress than the control group. 4) The patients group had significantly higher levels of depression than the control gorup, but there was not any significant difference in the STAI. 5) There were significant positive correlations between the BDI score and the STAI-trait, the STAI-stait and the STAI-trait, the quantity of perceived stress and the STAI-trait. Conlusion : The patients with functional upper gut symptoms displayed more, psychological distress, sornatization, anxiety, and deperssion. Among them, patients had higer depression than control group. Functional upper gut symtoms could be more appropriately viewed as somatic symptoms of depression. These findings suggest that such patients need to have psychiatric intervention and treatment.

우울 및 불안장애 환자에서 아동기 학대와 정서증상 및 리질리언스와의 관계 (Relationship of Affective Symptoms and Resilience with Childhood Abuse in Patients with Depressive or Anxiety Disorders)

  • Objective : The aim of this study is to identify the relationship between childhood abuse and affective symptom including resilience in patients with depression or anxiety. Methods : A total 256 outpatients diagnosed with depressive disorder or anxiety disorder according to DSM IV-TR, were evaluated with Beck Depression Inventory (BDI), State-Trait Anxiety Inventory (STAI), Parent-Child Conflict Tactics Scale (PCCTS), Connor-Davidson Resilience Scale (CD-RISC). Independent t-test, Chi-square analysis and analysis of covariance (ANCOVA) were performed to identify the demographics of patients and the relationship between affective symptoms including resilience and childhood abuse. Results : Among demographic and clinical variables, patients with childhood abuse history were significantly higher rate in patients who were living alone and unemployed. In affective symptoms, patients with childhood abuse history were significant more severe in depressive symptoms, and state anxiety score than patients without history of childhood abuse. Patients with childhood abuse history had higher score for trait anxiety and lower score for resilience than patients who had no history of childhood abuse. Conclusion : These finding suggest that history of childhood abuse might be risk factor on depressive and anxiety symptoms severity. And this might be a predictable factor of poor treatment outcome.

강박장애 환자에서의 성격 5요인 모델 분석 (Analysis of the Five-Factor Model of Personality in Obsessive-Compulsive Disorder)

  • Objective : The aim of this study is to evaluate the five-factor model of personality in patients with obsessive-compulsive disorder (OCD) related to obsessive-compulsive symptom severity and the distinct symptom subgroups. Methods : We recruited 95 patients with OCD and 116 normal controls in the study. We used the short version of Revised NEO Personality Inventory and the Yale-Brown Obsessive Compulsive Scale (Y-BOCS) for the assessment. Results : Patients with OCD showed significantly higher scores in neuroticism than normal controls. On multiple linear regression analysis, we found that agreeableness and conscientiousness were associated with the total Y-BOCS scores. On subscale analysis, agreeableness and neuroticism were associated with the obsession subscale scores and only conscientiousness was associated with the compulsion subscale scores. Furthermore, we found that patients who have contamination/cleaning or symmetry/ordering/counting/arranging as a main symptom presentation had significantly higher mean scores in conscientiousness than patients who have harm due to injury/violence/aggression as a main symptom presentation. Conclusion : In this study, we found that specific personality factors are associated with the obsessive-compulsive symptom severity. In addition, this is the first study to investigate the relationship between the personality factors in the five-factor model and the distinct symptom subgroups in OCD.
