Psychiatric Symptoms Among Female Adult Victims of Sexual Molestation : Comparison with Rape Victims (성인 여성 성추행 피해자들의 정신적 증상 : 강간 피해자들과의 비교)

  • Objectives : According to the recent Korean National Police Agency report, rape victims were 17.1%, but sexual molestation victims were more than the rape victims by 78.0%. Despite many international reports about the occurrence of severe psychiatric symptoms in sexual molestation victims, there is no domestic research. Therefore in this study, we investigated psychiatric symptoms of sexual molestation victims, and we also compared it with psychiatric symptoms in rape victims. Methods : 58 women who visited Busan Smile Center within 3months after sexual violence were the study subjects. Questionnaire about sociodemographic and sexual violence related characteristics were retrospectively investigated. Of the psychiatric symptoms, Beck Depression Inventory(BDI), Beck Anxiety Inventory(BAI) for severity of depression and anxiety, and Impact of Event Scale-Revised(IES-R) to check the presence of posttraumatic stress disorder(PTSD) symptoms were used. Results : Of 58 sexual violence victims, sexual molestation victims were 36(62.1%) and rape victims were 22(37.9%). In sexual molestation victims, 80.6% had more than moderate severity of depression, 83.3% had more than moderate severity of anxiety, and 94.4% had significant scores at PTSD screening test. Compared with rape victims' psychiatric symptoms(each 95.5%, 95.5%, 95.5%) there were no significant difference. Conclusions : The majority of sexual molestation victims were also accompanied by depression, anxiety and PTSD symptoms as rape victims. There results suggest that appropriate assessment and early treatment for psychiatric symptom must be made in the early stage of injury in sexual molestation victims.

The Effect of Spiritual Nursing Intervention on Anxiety of the Hospice Patients (영적 간호중재가 호스피스 환자의 불안에 미치는 효과)

  • Purpose : Surveying the effects on drop the anxiety of the hospice patients in spiritual nursing intervention with a quasi-experimental design using non-equivalent contrast group non-synchronized design to try in order to give the support which provide a holistic and individualizational nursing to comfort of hospice patients. Method : The results of survey were collected from 67 patients(67 subjects comprised 37 hospice patients of the experimental group and 30s of contrast) who were given hospice care from July to September of 2000 at the General Hospital in Cheon Ju city. The tool was used Spielberger's State Anxiety Scale, and the difference in the level of dropping anxiety among patient groups was analyzed with the mean, standard deviation, $x^2-test$. t-test and paired t-test. The spiritual nursing intervention was carried out through Hymn, Scripture, prayer, the therapeutic use of self over a period of three weeks. Results : 1) In general characters, men were a many more of the objects and the average age of the experimental and contrast group was 59.6, 55.9 respectively. The family of living together was $2{\sim}3$ members of most part. 2) There were not significant differences in the general, disease and therapeutic, religional characters between the experimental and contrast group. 3) The majority of the objects were cancer patients in disease and therapeutic characters(Experimental : 92%, Contrast : 95%). 4) After the spiritual nursing intervention state anxiety of the experimental group were remarkably lower than those of the contrast (t=-5.987, P=0.000). 5) Decreasing rate in the anxiety scores of before and post facto of the experimental group were remarkably lower than those of the contrast (t=6.237, P=0.000). Conclusion : The hospice patients who were offered spiritual nursing intervention became much lower than those who were not offered it in anxiety. Spiritual nursing intervention can be suited to field with one program of an effective that that relieved their anxieties. It is not only a very short time but has quite a little findings in part of spiritual nursing intervention. Therefore, further study in this field is necessary to concrete and substantial investigate in order to more and more increasing hospice patients in 21st century.

Food and nutrient intake status of Korean elderly by perceived anxiety and depressive condition: data from Korean National Health and Nutrition Examination Survey 2013~ 2015 (한국 노인의 주관적 불안·우울 상태에 따른 식품 및 영양소 섭취 실태 : 2013~ 2015년 국민건강영양조사 자료를 이용하여)

  • Purpose: This study examined the food and nutrient intake of Korean elderly according to the anxiety and depressive condition using the data from the Korea National Health and Nutrition Survey (KNHANES) from 2013 to 2015. Methods: The participants were 3,504 elderly people over 65 years of age (1,523 in men and, 1,981 in women). The dietary information was analyzed using the 24-hour recall data. The anxiety and depressive state was assessed using the self-reported scale EQ-5D in the quality of life dimension. The subjects were divided into the anxiety depression group (AD) and non-anxiety depression group (NAD) according to their anxiety and depressive conditions. Results: In the male elderly, the AD group had a significantly lower education and economic level and higher proportion in living alone than the NAD group. The percentage of eating lunch and dinner alone in the male AD group was higher than that of the NAD group. The female AD group showed less a lower frequency of dinner than the NAD group. The male AD group had a lower consumption of total foods, fish and shellfishes, seaweeds, mushrooms, oils and fats, and seasonings than the NAD group. With regard to the nutrient intake, the male elderly NAD group had more sufficient nutrient intakes than the AD group. In particular, the daily intakes of dietary fiber, riboflavin, niacin, potassium and iron were significantly lower in the AD group. To compare with the nutrient density of the two groups, the vitamin C and niacin intakes were lower in the AD group than in the NAD group. Overall, the nutritional status of the male AD group was significantly lower than that of the NAD group. Meanwhile, the female elderly had showed a smaller difference in nutrient intake according to their anxiety and depressive condition. Conclusion: These results of this study show that more nutritional education and emotional support are needed to improve the nutritional status and health of the male elderly with anxiety or depression.

Patient-Centredness, Job Satisfaction and Psychological Distress: a Brief Survey Comparing Oncology Nurses and Doctors

  • Background: We aimed to explore whether levels of patient-centredness, job satisfaction and psychological distress varied between oncology nurses and doctors. Materials and Methods: In a cross-sectional study using self-administered questionnaires, a total of 24 nurses and 43 doctors were assessed for patient-centredness, psychological distress, and job satisfaction using the Patient-Practitioner Orientation Scale, Hospital Anxiety and Depression Scale, and Job Satisfaction Scale. Data were analysed using descriptive statistics, independent samples t-test and MANCOVA, with p<0.05 considered significant. Results: Overall response rate was 95.6% (43/45) for physicians and 85.7% (24/28) for nurses. Even after adjusting for known covariates, our principal finding was that doctors reported greater psychological distress compared to nurses (p=0.009). Doctors also reported lower job satisfaction compared to nurses (p = 0.017), despite higher levels of patient-centredness found in nurses (p=0.001). Findings may be explained in part by differences in job characteristics and demands. Conclusions: Mental health is an important concern not just in cancer patients but among healthcare professionals in oncology.

Infertility Stress, Depression, and Resilience in Women with Infertility treatments (난임 치료 과정 중인 여성의 난임스트레스, 우울 및 회복탄력성)

  • Purpose: The purpose of this study was to determine the relationship among infertility stress, depression and Resilience of infertile women during infertility treatment. Methods: As a descriptive study, we surveyed 129 infertile women in 1 fertility center from November 2014 to January 2015. The data was analyzed in SPSS WIn 18.0 program. Results: Infertility stress, depression, and resilience averaged $3.23{\pm}.32$ (range of scale 1~6), $1.42{\pm}.39$ (range of scale 1~4), and $3.45{\pm}.35$ (range of scale 1~5), respectively. There was significant difference in infertility stress by job, abortion experience after pregnancy with infertility treatments, anxiety of high risk pregnancy, burden of pregnancy, important of having child. There was significant difference in depression by job, Economy burden on infertility treatment, burden of pregnancy. Infertility stress had a positive correlation with depression (r=.192. p<.029) Resilience had a negative correlation with depression (r=-.349. p<.001). Conclusion: It is necessary to provide infertile women with the counseling on their infertility stress and depression, and the intervention programs for infertile women is expected to help them cope and adapt with their personal and marital problems, reduce their negative emotions, and thus promote their quality of life.

Effects of Specific Exercise on Chronic Neck Pain in Elderly Women

  • Purpose: The main causes of chronic neck pain (CNP) are wrong postures and degenerative changes. This study investigated the effects of specific exercise on elderly women with CNP. Methods: 29 elderly women who complain of CNP were classified into experimental group (14) and control group (15). The experimental group was treated with specific exercise and minimal conventional therapy, whereas the control group was treated only with conventional therapy. Then the effects on the visual analogue scale (VAS), neck disability index (NDI), self-rating anxiety scale (SAS), Korean form of geriatric depression scale (KGDS), and range of motion (ROM) were compared between the two groups. Results: The experimental group showed significantly improved results in VAS, NDI, SAS, KGDS, and ROM after intervention (p<0.05), as did the control group (p<0.05). The comparison of changes in the experimental group before and after intervention showed superior results in the SAS, NDI, and ROM (flexion, rotation, lat. flexion) results when compared with the control group (p<0.05). Conclusion: Specific exercise is effective in the improvement of SAS, NDI, and ROM for elderly women with CNP.

Cognitive Behavioral Therapy in Childhood Tic Disorder : A Case of Report (틱 장애 아동의 인지행동치료 증례: 5회기 습관뒤집기 훈련과 호흡훈련을 중심으로)

  • Chronic tic disorder or Tourette syndrome is known to be a chronic neuro-behavioral disease for which cognitive behavioral therapy (CBT) strategies have recently been introduced. Here, we report the effectiveness of CBT in a case of childhood chronic tic disorder, which is very common in clinical settings. The DSM-IV clinical diagnosis was applied by a child psychiatrist. The Yale Global Tic Severity Scale, Kovac's children's depression inventory, Spielberger State-Trait Anxiety Inventory, Abbreviated Conners' Rating Scales, and the Dupaul ADHD Rating Scales were used. This case involved a pharmacological treatment-refractory patient over the previous year. Thus, psychiatric consultation was undertaken. Subsequently, we administered five sessions of CBT for four weeks, consisting of symptom evaluation and planning, habit reversal training, and ventilation training. Following four weeks of CBT administration, there were improvements in the scores of the Yale Global Tic Severity Scale and the Clinical Global Improvement scale. Our observations indicate that CBT may be effective in the treatment of childhood tic disorder.

Differences in Depression, Anxiety, Self-esteem According to the Degree of Obesity in Women -A Pilot Study- (여성에 있어서의 비만도에 따른 우울, 불안, 자아존중감의 차이 -기초연구-)

  • Objects and Methods : BDl, STAI and Self-Esteem scale were completed by 28 female subjects who composed of nurses of Kyung Hee Medical Center and students of Kyung Hee Oriental Medical college. At the same time, current weight, height and BMI(Body Mass Index), ideal weight, height and BMI, the difference of current weight and ideal weight, the difference of current height and ideal height, and the difference of current BMI and ideal BMI were measured. Result : 1. There was a significant difference between obese and normal weight group in the difference between the current weight and the ideal weight. The difference of the obese group were higher than the normal weight group. 2. There was a significant difference between obese and normal weight group in the mean BDI score. The mean BDI score of the obese group were higher than that of the normal weight group. 3. There was no significant difference between obese and normal weight group in the mean STAI score. 4. There was no significant difference between obese and normal weight group in the mean Self-Esteem Scale score. Conclusion : There are a tendency; the more weight, the more depression in women.

Effects of Neck and Back Touch on Ultrasonic Vocalization and the Rat Grimace Scale in Rats Receiving Intramuscular Injections (목과 등의 접촉이 근육주사를 투여 받는 흰쥐의 초음파발성과 Rat Grimace Scale에 미치는 효과)

  • Purpose: To determine the effects of neck and back touch on Ultrasonic Vocalization (UV) and the Rat Grimace Scale (RGS) in rats touched by the human hand as an intervention for reducing pain and anxiety associated with intramuscular injections. Methods: This study was a randomized, controlled, pre-test/post-test design study. Twenty male Sprague-Dawley rats were assigned to either the experimental or control groups (n= 10 rats/group). The experimental group was touched in a manner similar to massaging the rat's neck and back twice a day for one week. The two groups received two intramuscular injections. The 22kHz and 50kHz UV and the RGS scores were measured before and after each intramuscular injection. Results: The 22kHz UV and RGS score were significantly higher in the control group than in the experimental group when the first intramuscular injection was administered; whereas, no difference between the experimental and the control groups were observed when the second intramuscular injection was administered. There was no statistical difference in the 50kHz UV between the two groups when the first and second injections were administered. Conclusion: This study found that touching was an effective intervention method to reduce negative emotions and pain in rats receiving their first intramuscular injection.

Drug-Induced Dyskinesia Treated with Korean Medicine: A Case Report

  • Drug-induced dyskinesia is an involuntary muscle movement caused by various dopamine receptor-blocking drug exposure, such as antipsychotics, antidepressants, and antiemetics. Causative drug removal is the main treatment for drug-induced dyskinesia whenever possible because its pathophysiology lacks a universally accepted mechanism; however, the symptoms can persist for years or decades in many patients even after causative drug removal. Herein, we present a case of drug-induced dyskinesia in a 61-year-old female patient who consumed medication for approximately 10 years for her depression, anxiety, and insomnia. Cervical and facial dyskinesia was suggested to be related to perphenazine and levosulpiride administration. The patient received acupuncture, pharmacopuncture, herbal medicine, and chuna treatment for 81 days during hospitalization. The symptoms were evaluated using the Abnormal Involuntary Movement Scale, Toronto Western Spasmodic Torticollis Rating Scale, Tsui's score, and Numeric Rating Scale, which revealed remarkable improvement, suggesting the effectiveness of combined Korean medicine for drug-induced dyskinesia.
