권백이 U937의 세포독성에 미치는 영향 (Effect of Selaginella tamariscina on U937 Cytoxicity)

  • 천연물에서 직접적인 암세포 살해능을 가지며, 동시에 중증의 백혈구 감소증을 억제시킬 수 있는 물질을 탐색하기 위하여 암세포주인 U937 human lymphoma cell과 정상면역 세포인 lymphocyte에 대한 권백 수침 엑기스의 암세포 살해능과, 현재 임상에서 응용되고 있는 화학요법제인 retinoic acid, doxorubicine, cyclopho-sphamide와의 병용요법이 암세포(U937)와 정상세포(lymphocyte)에 미치는 영향을 MTT 분석법을 이용하여 비교 관찰하여 다음과 같은 결과를 얻었다. 권백 수침 엑기스 단독첨가시 권백 수침 엑기스는 암세포인 U937 human iymphoma cell의 성장을 유의적으로 억제시켰으며, 정상면역세포인 lymphocyte에는 영향을 미치지 않았따. 권백 수침 엑기스와 호학요법제를 병용 첨가시 화학요법제를 단독 처리했을 때 보다 권백 수침 엑기스(1mg/ml)와 화학요법제를 병용하였을 때, U937의 세포살해능의 현저히 증가되었고, 한편 lymphocyte의 세포살해능은 감소하였다. 이상의 결과에서 화학요법제와 권백을 병용 투여함으로서 암세포 살해능은 증가되고, 정상세포 살해능은 감소되었으므로 화학요법제의 가장 큰 부작용인 immunosupression 중 $leucopeenia^{20~30}$를 감소시킬 수를 감소시킬 수 있을 것으로 사료된다.

  • The proliferation of bone marrow stem cell compartment is thought to be under both positive and negative controls by cytokines and colony stimulation factors. Macrophage inflammatory $protein-1{\alpha}(MIP-1{\alpha})$ has been assessed for its potential to protect hematopoietic stem cells from cytotoxic effects of a cycle-specific antineoplastic agents. We have tested the ability of $MIP-1{\alpha}$ to suppress the proliferation of stem cell line Du.528.101 in variety of active status by using $[^{3}H]-thymidine$ incorporation test. The results were as follows. 1. The effect of $MIP-1{\alpha}$ on steady-state Du.528.101 cell represented the cell growth suppression at the concentration of 10, 50, 100nM of $MIP-1{\alpha}$(P<0.001). 2. $MIP-1{\alpha}$ stimulated the proliferation of Du.528.101 cells previously treated with IL-1 at the concentration of 5, 50nM of $MIP-1{\alpha}$(P<0.01). 3. The suppression effect of MIP-1 on Du.528.101 cells at the concentration of 5, 50nM was shown when cells were treated with $MIP-1{\alpha}$ before activation with $IL-1{\beta}(P<0.01)$. 4. The growth rate of synchronized cells were slower than that of non-synchronized ones, and $MIP-1{\alpha}$ represented the similar suppression effect on both synchronized and non-synchronized cells.

L5187Y 세포에 대한 불화피리미딘 대사억제제 독성에 관한 Benzylacyclouridine의 영향 (The Effects of 5-benzylacyclouridine on the Cytotoxicities of Fluorinated Pyrimidine Antimetabolic Agents in L5178Y Cells)

  • Benzylacyclouridines (BAU and BBAU)는 uridine phosphorylase (UrdPase)의 선택적이고 강력한 상경억제제이다. 보고된 바에 의하면 (Cancer Res., 44: 1852, 1984) Benzylacyclouridines가 5-fluoro-2'-deoxyuridine (FdUrd)의 인체 암세포에 대한 독성을 증가시켜 준다고 하였지만, L5187Y 세포를 사용한 본 실험에서는 Benzylacyclouridines가 FdUrd를 포함하여 5-fluorouridine (FUra) 모두에 대해 조금도 세포 독성을 증가시키지 못하였을 뿐만아니라, 오히려 세포를 그들 독성으로부터 투여량에 비례하여 보호하였다. 복강내 주사에 의한 생체실험에서도 Benzylacyclouridines는 5-fluorinated pyrimidine에 의한 L5187Y를 지닌 쥐(mouse)의 life-span을 연장시켜 주지 못하였다. 본 실험에서 Benzylacyclouridines가 기대했던 fluorinated pyrimidine 항암제의 효과를 증진시키지 못한 이유는 nucleosides의 anabolism이 UrdPase와 orotate phosphoribosyltransferase이 의한 sequential 작용에 의하던가 또는 Benzylacyclouridines에 의한 nucleosides의 수송억제에 의하던가, 아니면 두가지 다 복합적으로 작용한 결과로 생각된다.

Anticancer Effects of Curcuma C20-Dialdehyde against Colon and Cervical Cancer Cell Lines

  • Background: Recent attention on chemotherapeutic intervention against cancer has been focused on discovering and developing phytochemicals as anticancer agents with improved efficacy, low drug resistance and toxicity, low cost and limited adverse side effects. In this study, we investigated the effects of Curcuma C20-dialdehyde on growth, apoptosis and cell cycle arrest in colon and cervical cancer cell lines. Materials and Methods: Antiproliferative, apoptosis induction, and cell cycle arrest activities of Curcuma C20-dialdehyde were determined by WST cell proliferation assay, flow cytometric Alexa fluor 488-annexin V/propidium iodide (PI) staining and PI staining, respectively. Results: Curcuma C20 dialdehyde suppressed the proliferation of HCT116, HT29 and HeLa cells, with IC50 values of $65.4{\pm}1.74{\mu}g/ml$, $58.4{\pm}5.20{\mu}g/ml$ and $72.0{\pm}0.03{\mu}g/ml$, respectively, with 72 h exposure. Flow cytometric analysis revealed that percentages of early apoptotic cells increased in a dose-dependent manner upon exposure to Curcuma C20-dialdehyde. Furthermore, exposure to lower concentrations of this compound significantly induced cell cycle arrest at G1 phase for both HCT116 and HT29 cells, while higher concentrations increased sub-G1 populations. However, the concentrations used in this study could not induce cell cycle arrest but rather induced apoptotic cell death in HeLa cells. Conclusions: Our findings suggest that the phytochemical Curcuma C20-dialdehyde may be a potential antineoplastic agent for colon and cervical cancer chemotherapy and/or chemoprevention. Further studies are needed to characterize the drug target or mode of action of the Curcuma C20-dialdehyde as an anticancer agent.

Treatment results of radiotherapy following CHOP or R-CHOP in limited-stage head-and-neck diffuse large B-cell lymphoma: a single institutional experience

  • Purpose: This study evaluated outcomes of radiotherapy (RT) after chemotherapy in limited-stage head-and-neck diffuse large B-cell lymphoma (DLBCL). Materials and Methods: Eighty patients who were treated for limited-stage head-and-neck DLBCL with CHOP (n = 43) or R-CHOP (n = 37), were analyzed. After chemotherapy, RT was administered to the extended field (n = 60) or the involved field (n = 16), or the involved site (n = 4). The median dose of RT ranged from 36 Gy in case of those with a complete response, to 45-60 Gy in those with a partial response. Results: In all patients, the 5-year overall survival (OS) and disease-free survival (DFS) rates were 83.9% and 80.1%, respectively. In comparison with the CHOP regimen, the R-CHOP regimen showed a better 5-year DFS (86.5% vs. 73.9%, p = 0.027) and a lower rate of treatment failures (25.6% vs. 8.1%, p = 0.040). The volume (p = 0.047) and dose of RT (p < 0.001) were significantly reduced in patients treated with R-CHOP compared to that in those treated with CHOP. Conclusion: The outcomes of RT after chemotherapy with R-CHOP were better than those of CHOP regimen for limited-stage head-and-neck DLBCL. In patients treated with R-CHOP, a reduced RT dose and volume might be feasible without increasing treatment failures.

Baseline Serum Interleukin-6 Levels Predict the Response of Patients with Advanced Non-small Cell Lung Cancer to PD-1/PD-L1 Inhibitors

  • Although various studies on predictive markers in the use of PD-1/PD-L1 inhibitors are in progress, only PD-L1 expression levels in tumor tissues are currently used. In the present study, we investigated whether baseline serum levels of IL-6 can predict the treatment response of patients with advanced non-small cell lung cancer (NSCLC) treated with PD-1/PD-L1 inhibitors. In our cohort of 125 NSCLC patients, the objective response rate (ORR) and disease control rate (DCR) were significantly higher in those with low IL-6 (<13.1 pg/ml) than those with high IL-6 (ORR 33.9% vs. 11.1%, p=0.003; DCR 80.6% vs. 34.9%, p<0.001). The median progression-free survival was 6.3 months (95% confidence interval [CI], 3.9-8.7) in the low IL-6 group, significantly longer than in the high IL-6 group (1.9 months, 95% CI, 1.6-2.2, p<0.001). The median overall survival in the low IL-6 group was significantly longer than in the high IL-6 group (not reached vs. 7.4 months, 95% CI, 4.8-10.0). Thus, baseline serum IL-6 levels could be a potential biomarker for predicting the efficacy and survival benefit of PD-1/PD-L1 inhibitors in NSCLC.

대장암 항암 화학요법의 처방 오류 평가 및 개선안 제시 (Guideline of Improvement and Evaluation of Prescribing Errors in Colorectal Chemotherapy)

  • Background: Colorectal cancer shows a significant increase in South Korea due to westernization of diet, lack of dietary fiber, drinking and smoking, irregular defecation. There are surgery, chemotherapy, radiation therapy in treatment of colorectal cancer. There may be a medication errors in the process of chemotherapy because of its high toxicity, narrow therapeutic index and the health status of cancer patients. Consequently medication errors can cause increasing the risk of death, prolonging hospital stay and increasing the cost. Among medication errors on medication use process, prescribing errors are of particular concern due to higher risk of serious consequences. It is important for pharmacist to prevent the prescribing errors before reaching patient. Therefore we analyzed the prescriptions of colorectal cancer, classified prescribing errors, suggested guideline to reduce prescribing errors and verified the importance of pharmacist's role in prevention of medication errors activity. Methods: We collected the numbers of prescriptions of colorectal cancer(n=2,373) through anti cancer management program and EMR and analyzed the errors of prescriptions by categories from Oct 1st 2011 to Sep 30th 2012 at Chungbuk National University Hospital. We reviewed the prescriptions as follows - patients' characteristics, the result of test, previous prescriptions, characteristics of antineoplastic agents and patients' allergy, drug sensitivity, adverse events. Prescriptions are classified into inpatient and outpatient and analyzed the errors of prescriptions by categories (dosage form, dose, input, diluents, regimen, product). Results: Total prescription number of inpatient and outpatient of colorectal cancer was 1,193 and 1,180 and that of errors was 107(9%) and 22(1.9%), respectively. In case of errors of categories, the number of errors of dosage form is 69 and 8, errors of dose is 15 and 5, errors of input is 9 and 9 in inpatient and outpatient prescriptions, respectively. Errors of diluents is 8, errors of regimen is 3, errors of product is 3 in only inpatient prescriptions. In case of errors of categories by inpatient department, the number of errors of dosage form is 34 and 35, errors of dose is 7 and 8, errors of input is 6 and 3, errors of diluents is 4 and 4, errors of regimen is 2 and 1, errors of product is 2 and 1 in SG and HO, respectively. In case of outpatient department, the number of errors of dosage form is 8 in HO, errors of dose is 5 in HO, errors of input is 5 and 4 in SG and HO, respectively. Conclusions: The rate of errors of inpatient is higher than that of outpatient. Junior doctors are engaged in prescriptions of inpatient and pharmacist need to pay attention to review all prescriptions. If prescribing errors are discovered, pharmacist should contact the prescriber and correct the errors without delay. The guideline to reduce prescribing errors might be upgrading software of anti cancer management program, education for physicians as well as pharmacists and calling prescriber's attention to preventing recurrence of errors.

반모가 수종의 인체 암세포에 미치는 영향 (Study of Mylabris Phalerata on Anti-cancer Effects in Some Kinds of Cancer Cells)

  • Object : Objective: This study was conducted to investigate the anti-cancer effects of Mylabris phalerata (반모) in some kinds of cancer cells. Materials and Methods: Some kinds of cancer cells lines were treated. We used nine kinds of cancer cell lines, such as stomach cancer cells (Kato), lung cancer cells (Calu-1, NCI-H 1395), urinary bladder cancer cells (HS789T), bone cancer cells (Saos-2), brain cancer cells (SK-N-MC), liver cancer cells (Hep-G2), skin cancer cells (Mo-1) and prostate cancer cells (PC-3) with the water decoction of Mylabris phalerata. The histological changes of all cell lines in the media (RPMI-1640) containing the decoction of Mylabris phalerata were observed and we examined cell death assay by trypan blue exclusion testing was examined. Finally, the change of mitochondrial membrane potential was measurd and the inhibitory effect of Mylabris phalerata on cell increase was examined by analyzing the cell cycle. Results: In histologic change all cancer cell lines showed withdrawn and floating appearance that is typical in cellular impairment. Most of the cell lines showed over 50% death rate after 24 hours in trypan blue exclusion tests. Especially the stomach, urinary bladder. brain and liver cell lines showed over 30% death rate after 12 hours. All cell lines treated with Mylabris phalerata were less stained than the control group and the mitochondrial membrane potential in the Mylabris phalerata treated cell lines was markedly lower than that in the control group. The measurement of DNA quantity in all cell lines showed the disappearance of the peak and the thickened left axis, which suggests that all cellular DNA degraded. Conclusion: Mylabris phalerata had cytotoxicity on various kinds of cancer cell lines and the mechanism of that was the impairment of mitochondria by the breakdown of the mitochondrial cell membrane. We propose that this is in part attributable to the destruction of DNA in cancer cells.

  • 본 교실에서는 우측 악하부와 우측 안와 하부의 종창으로 내원한 53세 여자 환자에서 Fine Needle Aspiration Biopsy로 림프절의 반응성 과증식으로 진단되어 다시 절제 생검을 시행하여 multi-centric Castleman's disease로 진단하였다. 조직 검사상 유리질 혈관형과 형질세포형이 공존하는 복합형으로 나타났으며 병소의 적출술만을 시행한 후 1년이 지난 지금까지는 별다른 소견없이 좋은 경과를 나타내고 있으며 앞으로도 계속 주의깊은 관찰이 필요로 한다.

Usefulness of cyclic thermal therapy and red blood cell scintigraphy in patients with chemotherapy-induced peripheral neuropathy

  • Background: Pharmacological and non-pharmacological therapies have been used to treat patients with chemotherapy-induced peripheral neuropathy (CIPN). However, the effect of therapies in cancer patients has yet to be investigated comprehensively. We hypothesized that cyclic thermal therapy would improve blood flow and microcirculation and improve the symptoms driven by CIPN. Methods: The criteria of assessment were blood volume in region of interest (ROI) in the images, and European Organization for Research and Treatment of Cancer-Quality of Life Questionnaire-Chemotherapy-Induced Peripheral Neuropathy 20 questionnaire scores. The blood volume was quantified by using red blood cell (RBC) scintigraphy. All patients were treated 10 times during 10 days. The thermal stimulations, between 15° and 41°, were repeatedly delivered to the patient's hands. Results: The total score of the questionnaires, the score of questions related to the upper limbs, the score of questions closely related to the upper limbs, and the score excluding the upper limbs questions was decreased. The blood volume was decreased, and the variance of blood volume was decreased. During cooling stimulation, the blood volume was decreased, and its variance was decreased. During warming stimulation, the blood volume was decreased, and its variance was decreased. Conclusions: We suggest that cyclic thermal therapy is useful to alleviate CIPN symptoms by blood circulation improvement. RBC scintigraphy can provide the quantitative information on blood volume under certain conditions such as stress, as well as rest, in peripheral tissue.
