개 자궁축농증에서의 원인균 분리 및 항생제 감수성 검사 (Identification and antimicrobial susceptibility of bacteria from the uterus of bitches with pyometra)

  • 본 연구는 1998년부터 1999년까지 서울대학교 수의과대학 부속동물병원에 내원한 개중 자 궁축농중으로 진단된 개의 자궁내용물로부터 균을 분리, 동정하고 항생제 감수성 검사를 실시하여 개 자궁축농증 치료를 위한 기초 자료로 사용하고자 하였다. 위 병원에 내원하여 자궁축농증으로 진단된 개는 16두였으며, 이들로부터 자궁적출술을 이용하여 자궁을 적출한 후 원인균을 분리하였고, 분리된 균에 대하여 15종의 항균제에 대하여 감수성 검사를 실시하였다. 16두의 개로부터 분리된 균은 Escherichia coli 11주, Serratia marcescens 2주, Staphylococcus aureus 및 Salmonella spp 각 l주 씩 이었다. 이들 균주들에 대하여 항균제 감수성 검사를 실시한 결과, 대부분의 균주들이 enrofloxacin, norfloxacin, chloramphenicol, nalidixic acid, gentamicin, Trimethprim-sulfamethazole, tetracycline에 대하여는 높은 감수성을 나타내었고, carbenicillin, amikacin, ampicillin, neomycin에 대하여는 중등도의 감수성을 나타내었으나, vancomycin, streptomycin, bacitracin, colistin에 대하여는 저항성을 나타내었다.

개에서 분리된 호기성 병원성 세균의 항생제 감수성 (Aerobic Antimicrobial Susceptibility Patterns of Bacteria Isolated from Dogs)

  • 질병을 유발하는 세균의 분리와 동정, 그리고 그 세균에 대한 항생제 감수성 검사는 치료에 효과적인 항생제를 선택하는 데에 있어서 중요하다 본 연구의 목적은 항생제 감수성의 경향을 알아보고 치료를 위한 효과적인 항생제 선택을 위해 세균을 동정하는 것이었다. 1999년 7월부터 2000년 9월 사이에 서울대학교 수의과대학 부속동물병원에 내원한 개로부터 실험재료를 채취하여 호기배양을 실시하였다. 피부(63개), 뇨(45개), 외이도(31개), 안결막(18개)으로부터 채취한 총 157개 시료를 이용하였다 내원한 환자의 피부, 외이도, 결막에서 가장 많이 분리된 세균은 S. intermedius였다. 내원한 환자의 뇨에서 가장 많이 분리된 세균은 E. coli 였다. 분리된 그람 양성 세균의 대부분은 ampicillin(80.6%), erythromycin(68.8%), penicillin(86.2%), tetracycline(89.2%) 에 대해 저항성을 나타내었다. 반면, 분리된 그람 음성 세균의 대부분은 ampicillin(73.4%), trimethoprim-sulfa(53.3%) 에 대해 저항성을 나타내었다 E. coli는 ciprofloxacin(6l.5%), piperacillin(69.2%) 에 대해 저항성을 나타내었다 분리 동정된 세균의 채취부위에 따른 항생제 감수성의 차이는 뇨중의 S. aureus를 제외하고는 특이할 만한 사항이 없었다.

최근 8년간 혈액배양에서 분리된 미생물의 항균제 감수성 양상: 2010~2017 (Antimicrobial Susceptibility Patterns of Microorganisms Isolated from Blood Culture during the Last 8 Years: 2010~2017)

  • 검사는 2010년부터 2017년까지 162,551 검체중 11,233 검체(6.9%)가 배양 양성이었으며, 이중 11,865 균주가 배양되었다. 배양된 균종 중 그람양성구균이 47.8%, 그람음성간균 38.8%, 그람양성간균 4.2%, 진균 6.8%, 혐기성균이 2.3%를 나타냈다. 배양결과를 성별로 비교하면 남성에서 분리된 균이 55.0% (2,732/4,969)로 여성에서 분리된 45.0% (2,237/4,969) 보다 많았다. 또한 연령별로는 70대가 28.7% (1,426/4,969)로 가장 많이 분리되었으며, 10대의 1.2% (62/4,969)와 많은 차이를 보였다. MRSA에 대한 내성률은 2016년 66.7%에서 2017년에는 46.8%로 많은 감소를 보였으며, E. faecium의 vancomycin 내성률은 35.0% (48/137)로 나타났다. 한편 장내세균에서 E. coli의 ESBL 양성률은 2010년 17.2%에서 2017년 28.8%로 증가하였으나, K. pneumoniae에서는 오히려 양성률이 감소하였다. P. aeruginosa의 MRPA는 11.8% (14/119), A. baumannii에서의 MRAB는 64.3% (161/252)로 높은 내성을 나타내었다. 혈액 검체에서 분리되는 미생물의 균종과 항균제 감수성 양상은 시기, 지역 및 대상환자 등에 따라 다르므로 병원균을 주기적으로 분석하고 이에 대한 항균제 감수성 정도의 변화양상을 파악하는 것이 병원에서 내성균의 감독과 조절에 큰 도움이 될 것으로 판단된다.

Genotyping, Phage Typing, and Antimicrobial Resistance of Salmonella Typhimurium Isolated from Pigs, Cattle, and Humans

  • Salmonella enterica serovar Typhimurium (ST) is one of the most common serovars isolated from humans and animals. It has been suggested that ST infections in Koreans are largely due to the consumption of contaminated pork and beef. To investigate the genotypes, phage types, and antimicrobial resistance patterns for ST isolates of different origins, a total of 70 ST strains, including 19 isolates from humans, 44 isolates from pigs, and 6 isolates from cattle, were analyzed using pulsedfield gel electrophoresis (PFGE), phage typing, and antimicrobial susceptibility tests. Forty-three distinct PFGE patterns were generated from 70 ST isolates, which were grouped into 14 PFGE groups (from A to N) at the level of 75% similarity. The most prevalent group was the A (A1-A17 subtypes) group, encompassing 54.5% (38/70) of ST isolates. ST isolates from pigs and cattle mostly belong to groups A and L, whereas ST isolates from humans mostly belong to groups F and C. Antimicrobial susceptibility tests using 11 antimicrobial agents showed that resistance to tetracycline (TE) (81.4%) was highly prevalent, followed by streptomycin (S) (64.3%) and nalidixic acid (NA) (31.4%) resistance. A total of seventeen antimicrobial resistance patterns were observed. Only 8.6% of isolates, including a reference strain, were susceptible to all antimicrobial agents tested. The most prevalent resistance pattern was TE-S (37.1%), which was seen in 66.6% of bovine, 40.8% of swine and 21.1% of human isolates. Three ST isolates from humans (15.9%) showed resistance to 7-8 antimicrobials. The most predominant phage type (PT) was U302 (64.3%), followed by DT170 (10.0%). PFGE types did not coincide with antimicrobial resistance patterns and phage types; therefore, the combination of those types allowed for further differentiation between tested ST isolates.

Antimicrobial Susceptibility of Viridans Streptococci Plaque Isolates in Korea

  • It has been reported that the antimicrobial susceptibility patterns of viridans streptococci vary according to geographical region. Although several studies on the antibiotic resistance of viridans streptococci in foreign countries have been reported, little is known about the distribution of resistance among viridans streptococci in Korea. In this study, 88 isolates of viridans streptococci from Korean students' dental plaque were identified as 12 different species. The susceptibility of these isolates to 8 antibiotics was investigated. The in vitro antibiotic activity of penicillin G, ampicillin, vancomycin, streptomycin, gentamicin, erythromycin, amoxicillin, and tetracycline was measured by the broth microdilution method. The range of the minimum inhibitory concentrations (MIC), MIC50, MIC90, and the percentage of the susceptible isolates were determined. Streptococcus mutans and Streptococcus salivarius were susceptible to the 8 antibiotics. Isolates with resistance to vancomycin, streptomycin, and amoxicillin were not found. The overall resistance rates of the 88 isolates to penicillin G, ampicillin, gentamicin, erythromycin, and tetracycline were 12.5%, 62.5%, 62.5%, 26.1%, and 26.1%, respectively.

젖소 유방염으로부터 분리한 그람음성균의 분포 및 항생제 감수성 (Antimicrobial susceptibility and prevalence of gram-negative bacteria isolated from bovine mastitis)

  • Environmental mastitis has increased particularly in well-managed or low somatic cell countherds that have successfully controled contagious pathogens. Major pathogens of environmental mastitisare Escherichia coli (E. coli) and Streptococcus uberis. The present study was conducted to investigate1,865 quaters of 241 Korean dairy farms from 2001 to 2004. Prevalence of major gram-negative bacteriaisolated from mastitis milk were E. coli (22.7%) and Enterobacter spp. (16.3%) in coliforms and Pseudomoassp. (10.3%) and Serratia spp. (7.9%) in non-coliforms. The results on antibiotic susceptibility by agardifusion test against these pathogens were 86.7% in piperaciliin, 94.6% in cefepime, 85.5% in amikacin,87.7% in gentamicin and so on. In contrast, the susceptibility against ampicillin (41.9%), cephalothin (9.9%),streptomycin (39.9%) and tetracycline (46.7%) appeared to be below 50%. Gram-negative bacteria showed(96.8%). Acording to year, distribution of high $256{\sim}64{\mu}g/ml$ on cephalothin get increased, but the othersare diferent. These findings demonstrate that major gram-negative bacteria were E. coli and Enterobacterspp. isolates, and often encountered the diverse antibiotic resistant patterns.

닭 도계 및 가공공정 중 유해미생물의 분포와 항생제 감수성 (The distribution and antimicrobial susceptibility of pathogenic microorganisms isolated from chicken slaughtering and processing procedure)

  • This study was performed to analyze the distribution and antimicrobial resistance of pathogenic microorganisms isolated from the carcass and environments of chicken processing plant located in Gyeonggi province from October to November in 2010. Chicken slaughterhouse was visited 3 times and totally 40 samples were collected from chicken carcass before and after washing (n=14), chicken cuts (n=7), cooling water (n=8), brine (n=2), cutting knives (n=7) and working plate (n=2). Whole-chicken rinsing technique (for chicken carcasses) and swab technique (for working plate and knives) were used to analyze the distribution of pathogenic microorganisms. In addition, brine and chilling water from storage tanks were gathered using sterilized tubes and used as samples. The matrix-assisted laser desorption/ionization mass spectrometry (MALDI MS) for whole cell fingerprinting in combination with a dedicated bioinformatic software tool was used to identify the isolated microorganisms. The pathogenic microorganisms, such as Bacillus cereus (n=8) and Staphylococcus aureus (n=9), were isolated form the chicken processing process (chicken carcasses of before and after chilling, chicken cuts, and working plate). The antimicrobial susceptibility of those isolated microorganisms was analyzed using 21 antimicrobial agents. In the case of B. cereus, it showed 100% of resistance to subclasses of penicillins and peptides, and it also resistant to cephalothin, a member of critically important antimicrobials (CIA), however there was no resistance (100% susceptible) to vancomycin and chloramphenicol. S. aureus showed 100% resistance to subclasses of peptides and some of penicillins (penicillin and oxacillin), however, it showed 100% susceptibility to cephalosporins (cefazolin and cephalothin). All of the tested pathogens showed multi drug resistance (MDR) more than 4 subclasses and one of B. cereus and S. aureus showed resistance to 9 subclasses. After the ban on using the antimicrobials in animal feed in July 2011, there would be some change in microbial distribution and antimicrobial resistance, and it still has a need to be analyzed.

소아 요로 감염에서 Escherichia coli에 대한 항생제 감수성의 변화에 대한 연구 (Escherichia coli Susceptibility to Antimicrobials in Children with Urinary Tract Infection)

  • Purpose : This study was performed to identify longitudinal changes in the prevalence of organisms isolated from urinary tract infection(UTI) and in the pattern of Escherichia coli susceptibility to antibiotics during the past 10 years in children with UTI. Methods : We performed a retrospective study of a total of 192 urine cultures from children with UTI in the Department of Pediatrics, Seoul Adventist Hospital over two periods(1st: 1995-2000, 2nd:2001-2005). Antimicrobial susceptibility of the isolates was compared between the two groups. Results : The pathogens of UTI in the two groups were similar. In the first period, E. coli was the leading uropathogen(66.2%) followed by Klebsiella pneumoniae(7.8%), Enterobacter cloacae(6.5%), and others(19.5%). In the second period, E. coli was the leading uropathogen(67%) followed by K. pneumoniae(12.2%), E. cloacae(3.5%), Enterobacter aerogenes(3.5%), and others(13.8%). The susceptibility pattern of E. coli to amoxicillin/clavulanate(87.5%, 81.0%) did not present any statistically significant difference between the two periods(P>0.05). The susceptibility of E. coli to TMP/SMX(52.4%, 50.0%) was still low with no significant difference between the two periods(P>0.05). Conclusion : Our results suggest that the use of amoxicillin/clavulanate is still an excellent therapeutic option in children with UTI. The low rate of susceptibility to TMP/SMX against uropathogens suggest that TMP/SMX may be reevaluated as the first-line therapeutic drug for UTI.

육계에서 분리한 Salmonella gallinarum 의 약제내생 및 PFGE 양상 (Antimicrobial resistance and pulsed -field gel electrophoresis patterns of Salmonella gallinarum isolated from broiler)

  • Fowl typhoid (FT) is a septicemic disease caused by Salmonella gallinarum. The purpose of this study was to investigate the antimicrobial resistance and pulsed -field gel electrophoresis (PFGE) patterns of S gallinarum isolated from broiler. During 1999 to 2004, there was isolated a total of 26 strains in liver and spleen. The biochemical characteristics of S gallinarum isolates was nonmotile, no production of $H_2S$, glucose gas, non-fermented rhamnose, indole-negative, fermentation of dulcitol, mannitol, maltose, and ornithine decarboxylase. At antimicrobial susceptibility, all of isolates were susceptible to amoxicillin/clavulanic acid, amikacin, neomycin, kanamycin, and cephalothin. Twenty-six isolates were divided into 19 resistant patterns and 6 strains was 8-multi-drug resistance. PFGE of Xba I restriction fragments of S gallinarum isolates was 22 patterns.

심한 근관 감염에서의 항생제 선택 (Antimicrobial choice of severe endodontic infection)

  • Objectives : The purpose of our study was to evaluate penicillin as a still drug of choice for severe endodontic infection, by analyzing the antimicrobial susceptibilities from endodontic infections with swelling to figure out appropriate antibiotics as empirical treatment. Materials and methods : This study involved 18 patients who attended for emergency treatment because of facial or periapical swelling associated with root canal infections. Identification and antimicrobial susceptibility test of each pathogen were performed by Vitek2 Systems (bioM$\acute{e}$rieux, Marcy l'Etoile, France). Results : The most frequent bacteria was Streptococcus spp.(77%), and the resistance against penicillin was 35% in overall patients, followed by clindamycin and erythromycin (17%), which was much higher than previous studies. Conclusions : In our study, the higher resistance made penicillin alone not to be chosen as the first antibiotic drug for severe endodontic infections. Combinations with other drug, penicillin with wider spectrum of activity, or changing to other antibiotics was considered while remembering the increased risk of resistant microorganism.
