Phytotherapy in periodontics as an effective and sustainable supplemental treatment: a narrative review

  • Purpose: Periodontal disease is a chronic condition caused by microbial infection and mediated by the host's immune response. Phytotherapy is a therapeutic approach that utilizes a renewable resource capable of supplying less expensive medicines for the world's growing population. This review aimed to present clinical evidence on the use of complementary medicinal herbs in the treatment of periodontal diseases. Methods: Different databases were searched using the terms "herbal" and "periodontitis." All included studies were examined with a focus on herbal indications, type, and prescription length. Dentists' therapeutic and prophylactic herbal prescribing habits were also assessed. Results: Various herbs such as turmeric, neem, aloe-vera, pomegranate, catechu, tulsi, cloves, lemon grass, green tea, tea tree oil, peppermint, garlic, pineapple, oak bark, babul, bakul, sage, coriander, moringa, amla, guava, and grape seed extract have been used in the treatment of periodontitis. These herbs have been reported to exhibit a range of therapeutic effects, including anti-inflammatory, antiplaque, antihalitosis, antiresorptive, antioxidant, antibacterial, antifungal, antiviral, and antimicrobial properties. These components can be utilized in various forms such as mouth rinse, gel, oil, toothpaste, aqueous extract, mouthwash, or tooth powder. Conclusions: Several readily available herbal formulations are now available on the market and have been shown to be effective as supplemental periodontal phytotherapy. However, these should be used under the supervision of a dental professional to ensure optimal benefits and effectiveness. Therefore, it is necessary to improve the understanding of suggested herbal prescription practices among dental professionals.

발효식품 추출물과 미생물을 활용한 참깨 종자전염성 병 방제 (Effect of Extracts and Bacteria from Korean Fermented Foods on the Control of Sesame Seed-Borne Fungal Diseases)

  • 참깨 종자전염성 병해를 방제하기 위하여 다양한 유용미생물을 함유하는 김치, 고추장, 된장, 간장, 막걸리를 채집하여 식품 추출물을 조제하고 종자전염성병원균에 대한 억제 효과를 조사하였다. 추가로 식품유래 미생물의 참깨 종자전염성 병해 억제 효과를 in vitro 및 in vivo 조건하에서 조사하였다. 50개 식품추출물에 처리한 참깨 종자를 샤레상에 처리하였을 때 20개 식품추출물이 유묘썩음증상을 92% 이상 감소시켰고, 7개 식품 추출물 처리는 온실조건에서 지상부 출현율을 58.3-66.8% 향상시키는 것으로 나타났다. 여러 발효식품으로부터 얻은 218종의 발효미생물 중 7종의 종자전염성 병원균에 대하여 높은 항균활성을 보였던 29균주를 처리하여 종자소독효과를 검정한 결과, 20균주가 병 억제 효과가 있는 것으로 나타났다. 20균주 중 13균주는 모썩음 증상을 유의성 있게 감소시켜 건전주율을 향상시키는 것으로 나타났다. 16S rDNA region의 염기서열분석을 통해 in vitro 및 in vivo에서 항균활성 및 종자소독 효과를 보인 식품유래 유용미생물 16종에 대한 동정을 수행하였다. 그 결과 14균주는 Bacillus 속 세균으로, 막걸리로부터 분리된 2균주는 Saccharomyces 속으로 동정되었다. 발효식품에서 분리된 선발 균주는 대부분 B. amyloliquifaciens인 것으로 나타났다. 선발 균주 중 B. amyloliquefaciens Gcj2-1와 B. amyloliquefaciens Gcj3-1를 코팅제로 bentonite, kaolin, talc 및 zeolite를 첨가 혼화하여 제제화하였을 때 두 균주 공히 talc와 kaolin 제형이 억제 효과가 우수하였고 그 다음이 zeolite 제형이었다. 그러나 bentonite 제형의 경우에는 타 제형에 비해 억제 효과가 낮게 나타났다. 선발된 발효식품유래 미생물은 in vitro와 온실조건에서 참깨 종자전염성병 방제에 있어서 효과적인 것으로 확인되었다. 이상의 결과를 근거로 국내산 발효식품 추출물 및 발효식품 유래 미생물은 참깨 종자소독용 방제제로서 사용될 수 있을 것으로 생각된다.

대계근에서 분리한 Polyacetylene계 화합물을 이용한 밀 이삭마름병 방제 (Biological Control of Fusarium Head Blight on Wheat by Polyacetylenes Derived from Cirsium japonicum Roots)

  • 이삭마름병은 지금까지 화학살균제에 의해 방제되어 왔지만 잔류농약 문제로 인하여 합성농약의 사용은 곡류 수확 30일 전까지로 그 처리기간이 제한되어 있다. 따라서 수확 직전까지 처리가 가능한 새로운 생물농약의 개발이 필요하다. 본 연구에서는 이삭마름병 방제를 위하여 F. graminearum에 대하여 항균활성을 보이는 식물추출물을 선발하기 위하여 총 85과에속하는 225 한약재시료의 ethanol 추출물을 대상으로 F. graminearum 포자발아 억제효과를 조사하였다. 그 결과 높은 억제활성을 가진 대계근을 선발한 후, 용매 분획, 컬럼 크로마토그래피와 생물검정을 거쳐 F. graminearum 포자발아 억제활성을 가진 두 개의 화합물을 분리하였다. 두 물질은 질량분석과 핵자기공명분석을 통하여 ciryneol C와 1-heptadecene-11,13-diyne-8,9,10-triol의 polyacetylene계 화합물로 동정되었다. F. graminearum의 포자발아 저해활성을 나타내는 $IC_{50}$은 1-heptadecene-11,13-diyne-8,9,10-triol이 $3.17{\mu}g/ml$, ciryneol C는 $28.14{\mu}g/ml$로 1-heptadecene-11,13-diyne-8,9,10-triol의 저해활성이 ciryneol C보다 더 높았다. 또한 온실에서의 대계근 EtoAc 추출물 분말수화제의 밀 이삭마름 병방제효과는 250배와 500배희석액처리시각각 78.92%와 31.56%로 유효성분의 농도가 높을수록 방제효과도 높아졌다. 본 연구결과는 polyacetylene계 화합물을 함유하는 대계근의 조추출물이 이삭마름병 방제를 위한 식물유래 천연물 살균제로 사용이 가능하다는 것을 제시하고 있다.

HaCaT 세포에서 회향 열매의 피부장벽기능과 hyaluronic acid 생성에 미치는 영향 (The Effects of the Fruits of Foeniculum vulgare on Skin Barrier Function and Hyaluronic Acid Production in HaCaT Keratinocytes)

  • 회향은 미나리과에 속하는 다년생식물인 Foeniculum vulgare Mill.의 성숙한 과실로서 항염, 진통, 피부노화방지 등의 효과를 가지고 있어 다양한 질환에 우수한 치료효과를 나타내는 것으로 알려져 왔다. 따라서 본 연구에서는 기능성이 우수한 회향 열매 추출물을 이용하여 피부 질환 치료제 및 피부장벽 기능 개선 소재로서의 적용가능성을 확인하였다. 이를 위해 각질형성세포주인 HaCaT에 회향 열매 메탄올 추출물과 그 분획물(hexane, methyl chloride, ethyl acetate, butanol)을 처리한 후, involucrin, loricrin, filaggrin의 발현과 hyaluronic acid의 생성 및 β-defensin -1, -2, -3, LL-37의 mRNA 발현을 측정하였다. 그 결과butanol 분획물 50 μg/ml을 24시간 동안 처리시 involucrin과 filaggrin단백질 발현을 각각122.8%, 105%로 유의하게 증가시킴을 확인하였다. Elisa assay를 통해 분석한 결과, ethyl acetate와 butanol 분획물은 대조군에 비해 hyaluronic acid의 생성을 각각17%, 11% 증가시켰으며, 이는 hyaluronic acid synthesis-1의 mRNA 발현 증가에 의한 것임을 확인하였다. 그러나 회향 열매 메탄올 추출물과 분획물 모두에서 β-defensin -1, -2, -3, LL-37의 mRNA 발현은 증가되지 않았다. 이러한 결과는 회향 열매 butanol 분획물이 각질형성세포에서 물리적 장벽을 형성하고 보습인자 조절을 통해 피부장벽 기능을 강화하는데 효과적임을 의미한다.

야콘(Smallanthus sonchifolius) 추출물이 흰쥐의 췌장 섬유화에 미치는 영향 (The Effects of Yacon (Smallanthus sonchifolius) Extract on Pancreatic Fibrosis in the Rat)

  • 야콘(Smallanthus sonchifolius)은 저혈당에 민간요법으로 이용해 오고 있는 구근작물이다. 최근에는 야콘잎이 항산화, 항균, 항진균 활성과 세포보호 기능이 있다고 보고되고 있다. 본 실험에서는 이러한 생리활성을 지니는 야콘잎을 이용하여 흰쥐에 DBTC (8 mg/kg)를 주사하여 췌장염을 유발시킨 후 1% 야콘추출물이 췌장의 섬유화에 미치는 영향을 연구하였다. 유발 21일째 처리군에서 췌장의 실질조직의 많은 부분이 collagen으로 재구성되어 있었으나 유발군에서는 처리군에 비해 현저히 감소되어 있었다. COX-2 발현에서, 대조군은 반응이 나타나지 않거나 매우 약한 반응이었으나, 유발군에서는 14일째 매우 증가되었으며, 특히 21일째는 침윤하고 있는 많은 염증세포에서 COX-2의 발현이 확인되었다. 처리군은 유발군에 비해 발현이 감소되었다. TGF-${\beta}1$ 발현은 21일째 염증세포에서는 유발군이 처리군에 비해 현저한 증가 현상이 나타났으나, 샘꽈리세포에서의 TGF-${\beta}1$의 발현은 처리군에서 증가되었다. VEGF 발현은 TGF-${\beta}1$의 발현과 거의 유사한 경향으로 나타났다. 그러므로 야콘추출물이 DBTC로 유도된 췌장염의 섬유화 진행을 억제하는데 매우 효과적임을 확인하였다.

콩 Isoflavone의 생리활성 기능과 함량 변이 (Physiological Function of Isoflavones and Their Genetic and Environmental Variations in Soybean)

  • Soyfoods have potential roles in the prevention and treatment of chronic diseases, most notably cancer, osteoporosis, and heart disease. There is evidence that carcinogenesis are supressed by isolated soybean derived products in vivo such as a protease inhibitor, phytic acid, saponins and isoflavones. It is believed that supplementation of human diets with soybean products markedly reduces human cancer mortality rates. Especially, recent papers recognize the potential benefit of soybean isoflavone components for reducing the risk of various cancers. Isoflavones exhibit a multitude of medicinal effects that influence cell growth and regulation, which may have potential value in the prevention and treatment of cancer. In addition to potential biological effects, soybean isoflavones have the important physiological functions such as the induction of Bradyrizobium japonicum nod genes and the responses of soybean tissues to infection by Phytophthora megasperma as well as biochemical activities such as antifungal and antibacterial actions. Genistin, daidzin, glycitin and their aglycone (genistein, daidzein, glycitein) are the principal isoflavones found in soybean. Malonyl and acetyl forms have also been detected but they are thermally unstable and are usually transformed during the processing in glucoside form. Most soy products, with the exception of soy sauce, alcohol-extracted soy protein concentrate, and soy protein isolate, have total isoflavone concentrations similar to those in the whole soybean. Soybean-containing diets inhibit mammary tumorigenesis in animal models of breast cancer, therefore, it is possible that dietary isoflavones are an important factor accounting for the lower incidence and mortality from breast cancer. Of the total soybean seed isoflavones, $80\~90\%$ were located in cotyledons, with the remainder in the hypocotyls. The hypocotyls had a higher concentrations of isoflavones on a weight basis compared with cotyledons. Isoflavone contents were influenced by genetics, crop years, and growth locations. The effect of crop year had a greater impact on the isoflavone contents than that of location. The climate condition might be the attribution factor to variation in isoflavone contents. Also, while the isoflavone content of cotyledons exhibited large variations in response to high temperature during seed development, hypocotyls showed high concentration in isoflavone content. So, it is concluded that one of the factors affecting isoflavone content in soybean seeds is temperature during seed development. High temperature, especially in maturity stage, causes lower isoflavone content in soybean seed. It is also suggested that there may exist a different mechanism to maintain isoflavone contents between cotyledon and seed hypocotyls. In a conclusion, soy foods may be able to have a significant beneficial impact on public health.

제충국화(除?菊花) 추출물(抽出物)의 항진균작용(抗眞菌作用) (Antifungal Activity of Extracts from Pyrethrum Cinerariaefolium V.)

  • Although numerous drugs are available for the treatment of superficial fungi infections of skin, and yet the clinical effects of most of such drugs are not satisfactory. In the hope of searching for effective drugs for superficial fungi infections, the authors observed fungistatic effects of Pyrethri Flos, a common herb in Korea, with water extract (PFWE), ethanol extract (PFEE), and methanol extract (PFME) from Pyrethrum cinerariaefolium V. In in vitro studies, the spores of fungi were inoculated on Sabouraud's glucose agar media which contained Pyrethri Flos extracts in each concentrations of $500\;{\mu}g/ml$, $1,000\;{\mu}g/ml$ and $5,000\;{\mu}g/ml$ respectively, and the growth of the fungi was observed for 3 weeks. The species of the fungi used in these experiments were Epidermophyton floccosum, Microsporum canis, Microsporum nanum, Microsporum gypseum, Microsporum cookei, Trichophyton rubrum, Trichophyton mentagrophytes, Trichophyton tonsurans and Trichophyton verrucosum. The results of these studies were as follows; 1. The growth of M. nanum & T. rubrum was slightly inhibited by PFWE $500\;{\mu}g/ml$, and the growth of M. nanum, M. cookei & T. rubrum was slightly inhibited by PFWE $1,000\;{\mu}g/ml$. The growth of E. floccosum, M. gypseum & M. cookei was slightly inhibited, however the growth of M. canis, M. nanum, T. mentagrophytes, T. rubrum & T. tonsurans was significantly inhibited by PFWE $5,000\;{\mu}g/ml$. With $500\;{\mu}g/ml$ of PFEE, the growth of M. canis, M. nanum, T. mentagrophytes, T. rubrum & T. tonsurans was significantly inhibited, and moderate inhibition of M. cookei growth and slight inhibition E. floccosum & M. gypseum were observed. The growth of M. canis, M. nanum, T. mentagrophytes, T. rubrum & T. tonsurans was significantly inhibited, and the growth of E. floccosum, M. gypseum & M. cookei was moderately inhibited by PFEE $1,000\;{\mu}g/ml$. Significant inhibitions of the growth of E. floccosum, M. canis, M. nanum, M. gypseum, M. cookei, T. mentagrophytes, T. rubrum & T. tonsurans were observed by PFEE $5,000\;{\mu}g/ml$. 3. The growth of E. floccosum & M. cookei was moderately inhibited, and the growth of M. canis, M. nanum, M. gypseum, M. mentagrophytes, T. rubrum & T. tonsurans was significantly inhibited by PFME $500\;{\mu}g/ml$. But the growth of E. floccosum, M. canis, M. nanum, M. gypseum, M. cookei, T. mentagrophytes, T, rubrum & T. tonsurans was significantly inhibited, and the growth of T. verrucosum was slightly inhibited in both PFME $1,000\;{\mu}g/ml$ and $5,000\;{\mu}g/ml$.

구강 내 세균에 대한 Essential oil의 항균효과에 관한 연구 (ANTIMICROBIAL EFFECT OF ESSENTIAL OILS ON ORAL BACTERIA)

  • Essential oil은 식물로부터 추출한 휘발성의 호지방성 화합물로서 essential oil은 잠재적으로 항균, 항진균, 진경련, 항말라리아성, 곤충퇴치능력 등의 생물학적 효과를 가지고 있다. 본 연구에서는 다양한 방향식물에서 추출한 Citral, Pineole, Linalool, Eugenol, Limonene, Pinene 등으로 구성된 다섯종류의 essential oil을 사용하였으며, 여덟 군의 중요한 병원성 세균인 Streptococcus mutans(S. mutans), Staphylococcus aureus(S. aureus), Streptococcus sanguis(S. sanguis), Streptococcus anginosus(S. anginosus), Actinobacillus actinomycetemcomitans(A. actinomycetem- comitans), Streptococcus sobrinus(S. sobrinus), Staphylococcus epidermidis(S. epidermidis), Esherichia coli (E. coli)에 대한 저항성으로서 항균능력을 평가하였다. Essential oil의 항균능력은 용액희석방법을 사용하여 검사하였으며, essential oil 농도범위는 10 mg/mL, 5 mg/mL, 2.5 mg/mL, 1.25 mg/mL, 0.625 mg/mL, 0.516 mg/mL, 0.3125 mg/mL, 0.078 mg/mL, 0.039 mg/mL, 그리고 0.015 mg/mL였다. Essential oil은 용량 의존적으로 시험 균주의 성장을 효과적으로 억제하였으며, NM을 제외하고 R, LG, FR, O oil은 저농도에서 구강 병원성 미생물의 성장을 억제하였다. 또한 사용된 essential oil은 E.coli와 S. epidermidis의 성장에는 탁월한 억제효과를 보이지는 못했으나 구강 병원성 미생물에 대해서는 특별한 성장 억제효과를 나타냈다.

토복령알코홀추출물의 항진균작용 (Effects of Chinae Rhizoma Extracts on the Growth of the Dermatophytes)

  • The majority of drugs used in the treatment of superficial fungal infections has limited values due to its low efficacy or development of resistance. For the purpose of searching efficacious agent on the superficial fungal infections induced by dermatophytes which is regarded as the most malicious one, authors examined whether Chinae Rhizoma Extracts have significant on it. Extracts from Smilax china Linne used for the study are water extract (CRWE), ethanol extract (CREE) and methanol extract (CRME). In in vitro studies, the spores of the dermatophytes were inoculated on Sabouraud's glucose agar media which contained three extracts of Chinae Rhizoma in each concentration of $500\;{\mu}g/ml$, $1,000\;{\mu}g/ml$ and $5,000\;{\mu}g/ml$ respectively, and also $1,000\;{\mu}g/ml$ of salicylic acid and undecylenic acid $1,000\;{\mu}g/ml$ as comparable drugs. The growth of the dermatophytes were observed for 3 weeks. The species of the dermatophytes used in this experiment were Epidermophyton floccosum, Microsporum canis, Microsporum cookei, Microsporum gypseum, Microsporum nanum, Trichophyton mentagrophytes, Trichophyton rubrum, Trichophyton tonsurans and Trichophyton verrucosum distributed from The Institute of Tropical Medicine in Belgium. The results of the studies were as follows: 1. The growth of M. canis, M. nanum, T. mentagrophytes, T. rubrum & T. tonsurans were slightly inhibited in CRWE $1,000\;{\mu}g/ml$ and CRWE $5,000\;{\mu}g/ml$, and only slight inhibition on the growth of E. floccosum, M. canis and M. gypseum were observed in CRWE $5,000\;{\mu}g/ml$. 2. Complete inhibition of T. rubrum, moderate inhibition of M. nanum & T. tonsurans, and slight inhibition of E. floccosunl, M. canis, M. cookei & T. mentagrophytes in growth were observed in concentration of CREE $500\;{\mu}g/ml$. The growth of M. gypseum was slightly inhibited, moderate inhibition on the growth of M. canis, M. cookei & T. mentagrophytes, and complete inhibition of E. floccosum, M. nanum, T. rubrum & T. tonsurans in growth were observeed by CREE $1,000\;{\mu}g/ml$. With $5,000\;{\mu}g/ml$ of CREE, the growth of E. floccosum, M. canis, M. cookei, M. gypseum, T. mentagrophytes, T. rubrum & T. tonsurans were completely inhibited except T. verrucosum being showed slight inhibition. 3. In CRME $500\;{\mu}g/ml$, slight inhibition of T. verrucosum, moderate inhibition of M. gypseum and complete inhibition of E. floccosum, M. canis, M. cookei, T. mentagrophytes, T. rubrum & T. tonsurans in growth were observed. The growth of E. floccosum, M. canis, M. cookei, M. gypseum, M. nanum, T. mentagrophytes, T. rubrum & T. tonsurans were completely inhibited except T. verrucosum being showed moderate inhibition in both CRME $1,000\;{\mu}g/ml$ and CRME $5,000\;{\mu}g/ml$. 4. In $1,000\;{\mu}g/ml$ of undecylenic acid, slight inhibition of T. verrucosum and complete inhibition of E, floccosum, M. canis, M. cookei, M. gypseum, M. nanum, T. mentagrophytes, T. rubrum & T. tonsurans in growth were observed. From the above results, it was found that Chinas Rhizoma Alcoholic Extracts(CREE & CRME) exerted significant antifungal activity, and their effects were probably derived from the pharmacological actions of triterpenoidal saponin and steroidal saponin.

Production of Rapamycin in Streptomyces hygroscopicus from Glycerol-Based Media Optimized by Systemic Methodology

  • Rapamycin, produced by the soil bacterium Streptomyces hygroscopicus, has the ability to suppress the immune system and is used as an antifungal, anti-inflammatory, antitumor, and immunosuppressive agent. In an attempt to increase the productivity of rapamycin, mutagenesis of wild-type Streptomyces hygroscopicus was performed using ultraviolet radiation, and the medium composition was optimized using glycerol (which is one of the cheapest starting substrates) by applying Plackett-Burman design and response surface methodology. Plackett-Burman design was used to analyze 14 medium constituents: M100 (maltodextrin), glycerol, soybean meal, soytone, yeast extract, $(NH_4)_2SO_4$, $\small{L}$-lysine, $KH_2PO_4$, $K_2HPO_4$, NaCl, $FeSO_4{cdot}7H_2O$, $CaCO_3$, 2-(N-morpholino) ethanesulfonic acid, and the initial pH level. Glycerol, soytone, yeast extract, and $CaCO_3$ were analyzed to evaluate their effect on rapamycin production. The individual and interaction effects of the four selected variables were determined by Box-Behnken design, suggesting $CaCO_3$, soytone, and yeast extract have negative effects, but glycerol was a positive factor to determine rapamycin productivity. Medium optimization using statistical design resulted in a 45% ($220.7{\pm}5.7mg/l$) increase in rapamycin production for the Streptomyces hygroscopicus mutant, compared with the unoptimized production medium ($151.9{\pm}22.6mg/l$), and nearly 588% compared with wild-type Streptomyces hygroscopicus ($37.5{\pm}2.8mg/l$).
