This study was performed to investigate the opinions of various related-industries on ban of antibiotics growth promoters (AGPs) in commercial mixed feed. The answers on a total of 21 questions were summarized by response number and percentage. 93% of those surveyed were in agreement of a ban of dietary AGPs. The agreement reasons were the livestock safety (61.5%), the reduction of antibiotic use (23.1%), and decrease of antibiotic-resistant bacteria (11.5%). The negative effects expected by the ban of AGPs were poor growth performance (44.2%), elevated disease emergence (31.4%), increasing the feed cost (18.6%), and quality degradation of livestock (5.8%). As the efficient plans for decline of AGPs use, the feeding environment improvement was the highest with 43%, and farmer training and the consolidation inspection of residual substance on antibiotics in livestock product was 27.9% and 22.1%, respectively. 46.5% of respondent are considering the modification of feed spec and 39.5% of those surveyed have staged a modified feed spec. In conclusion, livestock related-industries approve a ban of AGPs, and they assert that the policy support, improvement of management and environment in the farm, providing technology from related-industries are multiply essential for a stable settlement of a ban policy of AGPs.

Antibiotics are manufactured and used for specific physiological functions, hence they may exert adverse ecological consequences when they are in contact with nontarget organisms. In the last decade, many reports have been made on the occurrences of various antibiotic compounds in surface water, and their potential impact to the environment has become an increasing concern. This study was conducted to prioritize antibiotic substances with potential environment risk in Korea. Human use antibiotics with an EIC (Expected Introduction Concentration) value greater than $1{\mu}g/l$, US FDA's action limit criteria, were selected. In order to calculate a worst-case EIC for each substance, annual production volume (in kg) of each antibiotic substance was derived using the Korea Pharmaceutical Manufacturers Association (KPMA)'s monetary database. Sixteen substances were preliminarily selected. The EICs of the 16 antibiotic substances were refined with the excretion rate of the parent substances. Ten antibiotic substances were identified to have EIC-corrected greater than $1{\mu}g/l$, which include Amoxicillin ($15.8{\mu}g/l$), Cefaclor ($10.1{\mu}/l$), Roxithromycin ($4.2{\mu}g/l$), Cephradine ($4.5{\mu}g/l$), Cefatrizine ($2.6{\mu}g/l$), Cefadroxil ($3.3{\mu}g/l$), Aztreonam ($2.3{\mu}g/l$), Ceftazidime ($2.8{\mu}g/l$), Ribostamycin ($1.3{\mu}g/l$), and Ceftezole ($1.3{\mu}g/l$). Additional risk assessments for these antibiotic substances are suggested.

The purpose of this article is to cope with the abuse of antibiotics in the clinic, to determine the necessity of antibiotic administration, to share information on the selection and proper use of appropriate antibiotics, and to increase the appropriateness of antibiotic prescription through continuous monitoring. In line with the latest research and guidelines trends of various agencies, we will supplement the antibiotic prescription guidelines and use them for the treatment in Apple Tree Dental Hospital. Specially, by history taking and the penicillin allergy test, amoxicillin is prescribed as a primary selective antibiotic for 1 day. The complaints and treatment effects of the first antibiotic should be evaluated at the next visit. If the primary antibiotic was ineffective, we replaced it with a broad-spectrum antibiotic. If there was no improvement in symptoms, the patient would be referred to upper grade hospital. The staff of the Apple Tree Dental Hospital regularly monitored and educated antibiotic prescriptions. The current guidelines should be supplemented continually and positively affect the abuse of antibiotics and the habit of dental practice.
Since many years, continuous low dose antibiotic prophylaxis (CAP) has been used for children at a risk for recurrent urinary tract infection (UTI), especially those with vesicoureteral reflux (VUR). The incidence of recurrent UTI has been shown to be higher in children with VUR with bladder and bowel dysfunction (BBD) than in those with VUR without BBD. Therefore, CAP has been recommended for children with BBD and VUR because of the increased risk of UTI. However, the use of CAP has become highly controversial because of bacterial resistance developed due to antibiotic over-usage. The preventive effects of probiotics have been proved in various adult urogenital infections, and the antimicrobial activities of lactobacilli against uropathogens have been demonstrated in previous in vitro studies. However, a critical assessment of their efficacy in children with UTI is lacking. The importance of the use of urogenital probiotics is that it is a natural approach that replenishes the depleted normal flora to create a better environment to fight off uropathogens. Probiotics have a great potential, particularly today with the increasing threat of antibiotic-resistant microorganisms.

Background : The Clostridium difficile is the most important identifiable cause of nosocomial infectious diarrhea and colitis, which lengthens hospital stay. Recently incidence of C. difficile has been increasing in an university hospital, and an intervention for prevention and control of C. difficile associated diarrhea (CDAD) was in prompt need. Methods : Subjects were the patients in the neurosurgical intensive care unit(NCU) where C. difficile was most frequently isolated. To increase participation of various departments, we used the CQI method, because management of CDAD requires a wholistic approach including control of antibiotics, barrier precaution and environmental cleaning and disinfection. Duration of the CQI activities was 9 months from April to December 1999. Results : The identified problems were misuse and overuse of antibiotics, lack of consciousness of medical personnels and the possibility of transmission from the contaminated environment and tube feeding. Education for proper use of antibiotics and management of C. difficile infection, use of precaution stickers, supplement of handwashing equipments, emphasis on environmental disinfection, and the change of the process of tube feeding were done. The CDAD rate in NCU was significantly decreased after the CQI program (8.6 case per 1,000 patient days from January to April 1999 vs 4.8 from May to December 1999). The distribution of neurosurgical wards including NCU among the total number of isolated C. difficile from the clinical specimens dropped from 49.4% in January to April to 33,7% in May to December. The average hospital stay of the neurosurgical department changed from 19.6 days to 15.2 days. Also, the effect of the CQI activities for C. difficile may have affected the incidence of vancomycin resistant enterococci (VRE). Duration and dosage of certain antibiotics used in the NS department were decreased. The distribution of neurosurgical department in the number of VRE isolated patients declined from 18.4% to 11.1%. Conclusion : Infection control of resistant organisms such as C. difficile is likely to be successful when management of environmental contamination an collaborative efforts of decreasing the patients' risk factors such as antibiotics management and decreasing the length of hospital stay come simultaneously. For this work, related departments need to actively participate in the entire process under a common target through discussions for identifying problems and bringing up solutions. In this respect, making use of a CQI team is an efficient method of infection control for gathering participation and cooperation of related departments.
The effects of pharmacist's intervention for asthma patients using self-monitoring of peak expiratory flow rate in medication teaching model was evaluated for 3 months in improving clinical outcomes including emergency visits, hospitalizations, antibiotics use, symptoms and sleep disturbance. Twenty seven patients were enrolled in study and twenty three patients completed the follow-up schedules. The selected patients were given the pre-designed instruction for medication including appropriate use of medication, metered-dose inhaler(MDI) technique, identifying and controling asthma triggers and recognizing early signs of deterioration. There were significant improvements in clinical outcomes, in terms of emergency visits, hopitalizations, antibiotics use, symptoms and sleep disturbance. There were also significant improvements in the MDI use, environmental control, and medication knowledges. There was a progressive increase in peak expiratory flow rate during the three-month intervention. In conclusion, pharmacist's intervention using self-monitoring of peak expiratory flow rate has a significant impact on improving clinical outcomes in asthma patients.

Veterinary antibiotics have been widely used to increase feed efficiency, to prevent disease, and to promote growth as well as to control disease. The antibiotics administered can be excreted through the urine and feces. One of the major routes of veterinary antibiotics entering soil and water environment is via the application of animal manure to agricultural land as an organic fertilizer source. Since little is known about impacts of antibiotics on the environment, this study was conducted to prioritize the veterinary antibiotics based on the consumption and potential to reach the environment. Among 83 veterinary antibiotics consumed in Korea, ten antibiotics were used at the greater dose than 25 Mg in 2004. Potential to reach the environment was determined according to excretion rate after administered to animals and sorption affinity to soil solids after applied to agricultural land. Seventeen antibiotic active ingredients (Ais) were classified as 'High' priority in terms of the potential to reach the environment. An overall priority score was determined by combining priority score based on consumption with the degree of potential environment exposure. Twenty veterinary antibiotic AIs were classified as 'Very high' or 'High' priority requiring detailed assessment. The antibiotic AIs were identified four aminoglycosides, two macrolides, two penicillins, five sulfonamides, three tetracyclines, two quinolones, and two miscellaneous. Eight veterinary antibiotic AIs including amoxicillin, carbadox, chlortetracycline, neomycin, oxytetracycline, sulfamethazine, sulfathiazole, and tylosin were identified to have a greater priority of environmental risk in Korea.

Fish pathology is one of the main scientific bases upon which this expansion in aquaculture has been dependent and requires a wide knowledge of the environmental constraints, the physiology and characteristics of the various pathogens, the responses of the host and the methods by which they may be controlled. The primary disease and parasite problems in aquaculture animals relate to viral, bacterial, fungal and protozoan epizootics. Parasitic nematodes, trematodes and cestodes are commonly found in aquaculture animals, but seldom are they present in concentrations sufficinet to cause significant problems. When an epizootic does occur and chemical treatment is indicated, the appropriate chemical must be selected and properly applied. We have antibiotics, sulfa, nitrofuran and other chemicals for treatment of fish diseases. Some may be mixed with the fred during formulation, added to the pellets of feed as a surface coating, given in the dorm of an injection or used as a bath. Even though a drug or chemical has been officially approved for use in aquaculture, the substance should never be used unless there is a clear need. Some of the reasions for this view are as follows: (1) the constant use of antibiotics can lead to the development of resistant strains of bacteria, (2) biofilter efficiency may be impaired or destroyed by chemicals added to closed recirculating water systems, and (3) the injudicious use of chemicals can have a damaging effect on the environment as well as on human.
This study was performed in order to evaluate the risk factors for nosocomial urinary tract infection and the frequencies of organisms isolated, and to provide the epidemiologic and basic data of hospital acquired urinary tract infection in intensive care unit. A prospective analysis was performed with 1,235 urine samples following urinary bladder catheterization in 569 patients, who had no evidence of UTI at the time of catheter insertion, admitted to intensive care unit in Pusan P hospital between June 1997 and May 1998. To identify risk factors for UTI, clinical characteristics of infected patients were analyzed. We analyzed these data by percentage, chi-square and odd ratio. Obtained results were as follows: A total of 569 patients (male 341 and female 228) were an average age of 50.8 years and catheterization of 8.04 days. Incidence of UTI was 16.1% (199/1,235) and The risk factors of UTI were duration of catheterization over 7 days, no use of systemic antibiotics, summer and female, and During the first 7 days these risk factors were no use of systemic antibiotics, summer, place of first catheter insertion (ICU) and type of intensive care unit (NSICU). A total of 220 the isolated strains were Gram negative rod 83 (37.7%), yeast like fungi 74 (33.6%) and Gram positive cocci 63 (28.6%). The common organisms isolated were Enterococcus faecalis 23 (10.5%), Serratia marcescens 19 (8.6%), Pseudomonu spp.17 (7.7%), E. ooh 16 (7.3%), Staphylococcus epidemidis 11 (5.0%) mdklebsiellapneumoniae 8 (3.6%). Therefore, in these results 199 of 569 (35%) patients in ICU with indwelling urinary catheter developed UTI. The risk factors for UTI are prolonged duration of catheterization, no use of systemic antibiotics, summer, and female.
This review provides an overview of dietary nucleotides as an alternative to in-feed antibiotics for weaning pigs. Dietary nucleotides are composed of DNA or RNA molecules and are normally contained in protein-rich feed ingredient, brewer's yeast, yeast extract, and milk. Weaning pigs are suffering from several stresses, such as environmental challenges (i.e. crowding, transportation, and feeding). Such stressors can damage the intestinal epithelium and cause an invasion by Escherichia coli, secondary inflammatory responses, and post weaning diarrhea. To overcome weaning disorder, people often use antibiotics which reduce symptoms and boost growth performance. However, since antibiotics were banned due to concerns of antibiotic resistant bacteria, researchers are studying alternative materials to antibiotics. Dietary nucleotides are one of the alternative materials for replacing antibiotics and can be used in abnormal conditions, such as weaning diarrhea, low digestibility, and disease condition. Nucleotides have substances that have important roles in cell division and cell growth, affecting growth performance, intestinal condition, and immunological effect at the weaning stage. However, nucleotides' composition is very different between sources and this aspect makes it difficult to utilize nucleotides at the weaning stage. Therefore, this review paper focuses on i) the characteristics and functions of dietary nucleotides and ii) the effect of dietary nucleotides on the growth performance and immune system of pigs.
