Renal Subcapsular Abscess Associated with Pyometra in a Dog

  • A 12-year-old intact female Pomeranian dog was presented with vulvar discharge, lethargy, polyuria, and polydipsia. A complete blood count revealed leukocytosis and anemia. The serum biochemical analyses showed elevated serum alkaline phosphatase activity and blood urea nitrogen. Survey abdominal radiography revealed an enlarged left kidney. Abdominal ultrasonography identified multiple rounded hypoechoic subcapsular lesions in the left kidney. The bilateral uterine horn was enlarged, and contained echogenic fluid. Percutaneous ultrasonographic-guided aspiration of the renal subcapsular lesions was performed on the left kidney. Cytological examination revealed bacterial cocci, degenerated neutrophils, and suppurative inflammation. Bacterial culture produced growth of Escherchia coli. Left nephrectomy and ovariohysterectomy were performed. Dilated uterine fluid was also evaluated for bacterial culture, and showed growth of E. coli. Urine was examined by bacterial culture, and showed negative results. The treatment consisted of antibiotic therapy according to the antibiotic sensitivity test. Based on these findings, the diagnosis was renal subcapsular abscess associated with pyometra. This study suggests that pyometra should be considered as the cause of renal abscess.

Clinical Characteristics of Symptomatic Clostridium difficile Infection in Children: Conditions as Infection Risks and Whether Probiotics Is Effective

  • Purpose: This study investigated the clinical presentations of symptomatic Clostridium difficile infection (CDI) in children. Methods: We reviewed the medical records of 43 children aged <20 years who showed either positive C. difficile culture or C. difficile toxin test results between June 2010 and April 2014. Results: Of the 43 patients (mean age 6.7 years), 22 were boys. Sixteen patients (37.2%) showed both positive C. difficile culture and toxin test results. Seventeen out of 43 children (39.5%) had preexisting gastrointestinal diseases, and 26 children had other medical conditions that were risk factors for CDI. Twenty-eight children had a history of antibiotic treatment for >3 days, and the most frequently prescribed antibiotic was amoxicillin-clavulanate (35.7%). Twenty-eight patients were diagnosed with CDI despite taking probiotic supplements, most commonly Lactobacillus acidophilus (53.6%). The most common symptom was diarrhea (72.1%) at the time CDI was diagnosed. C. difficile was eradicated in 11 patients (25.6%) after treatment with oral metronidazole for 10-14 days, and in the two patients (4.6%) who required two courses of oral metronidazole. Sixteen patients (37.2%) showed clinical improvement without any treatment. Conclusion: This study showed the various clinical characteristics of CDI in children and that preexisting clinical conditions favored the development of CDI. In addition, CDI was found to occur in a number of patients even after probiotic prophylaxis given in conjunction with antibiotic therapy.

A comparative analysis of odontogenic maxillofacial infections in diabetic and nondiabetic patients: an institutional study

  • Objectives: The increased prevalence of antibiotic resistance is an outcome of evolution. Most patients presenting with odontogenic space infections also have associated systemic co-morbidities such as diabetes mellitus resulting in impaired host defense. The present study aims to compare the odontogenic spaces involved, antibiotic susceptibility of microorganisms, length of hospital stay, and the influence of systemic comorbidities on treatment outcome in diabetic patients. Materials and Methods: A 2-year prospective study from January 2012 to January 2014 was conducted on patients with odontogenic maxillofacial space infections. The patients were divided into two groups based on their glycemic levels. The data were compiled and statistically analyzed. Results: A total of 188 patients were included in the study that underwent surgical incision and drainage, removal of infection source, specimen collection for culture-sensitivity, and evaluation of diabetic status. Sixty-one out of 188 patients were found to be diabetic. The submandibular space was the most commonly involved space, and the most prevalent microorganism was Klebsiella pneumoniae in diabetics and group D Streptococcus in the nondiabetic group. Conclusion: The submandibular space was found to be the most commonly involved space, irrespective of glycemic control. Empiric antibiotic therapy with amoxicillin plus clavulanic acid combined with metronidazole with optimal glycemic control and surgical drainage of infection led to resolution of infection in diabetic as well as nondiabetic patients. The average length of hospital stay was found to be relatively longer in diabetic individuals.

외과계 입원환자에 대한 Vancomycin의 약물사용 평가 (Drug Use Evaluation of Vancomycin in Hospitalized Patients of Surgery Departments)

  • Over the last 50 years, a number of antibiotic agents have been developed and clinically used in the area of infectious diseases. Due to antimicrobial resistance problems and increasing health care costs, the rational use of antibiotics has been required. As a drug of choice to treat infections caused by MRSA, vancomycin has been extensively prescribed since the late 1970's. Recently, reports of vancomycin-resistant organisms such as VRE and VRSA have been increased to draw medical concerns. The objectives of this study were to evaluate the rational use of vancomycin and the appropriateness of the Restrictional Program of Antibiotic Utilization (RPAU) which has been operated at Samsung Medical Center. A retrospective chart review was performed in 132 hospitalized patients treated with vancomycin in the surgery departments from. January to June 1998. The guidelines of ASHP and HICPAC for vancomycin were modified and used as our criteria to determine the vancomycin DUE. In one hundred out of the patients, uses of vancomycin were approved by the Department of Infectious Diseases (DID) based on the RPAU. Vancomycin was appropriately used in $62.5\%$ of the 100 patients according to the criteria of justification of use, while $60.0\%,\;60.0\%,\;79.0\%,\;and\;51.0\%$ of the patients showed appropriate according to those of lab reports such as applicable culture obtained, pretreatment SCr, WBC and serum drug concentration monitoring, respectively. Although the rest 32 patients were not approved to receive vancomycin by the DID, twenty two percent continued receiving vancomycin treatment. This might result from the fact that the RPAU was started not before the use of antibiotics but in the middle of antimicrobial therapy. Continual education should be provide to the related health professionals and the RPAU should be simultaneously modified in order to increase the rate of appropriate uses of antibiotics.

한양방 병행치료를 시행한 양측성 중복 요관 환자의 재발성 신우신염 치험 1례 (A Case Report of Recurrent Pyelonephritis in a Patient with a Bilateral Bifid Ureter Treated with Korean-Western Cooperative Treatment)

  • Introduction: In this case report, we addressed the effects of a Korean medicine treatment on recurrent pyelonephritis in a female patient with a duplicated collecting system. Case report: A 22-year-old female with acute pyelonephritis was treated with Korean medicine, including herbal medication, Insampaedok-san plus Oryeong-san, and with antibiotic therapy. We evaluated the improvements in her symptoms by measuring C-reactive protein, WBC count, body temperature, and a numeric rating scale for nausea. The symptoms of fever and nausea improved after two days of hospitalization. From the 4th day of taking the herbal medicine, the antibiotic IV infusion was changed to oral agents. Conclusions: This clinical case study suggests that modified Insampaedok-san might have shortened the duration of intravenous antibiotic infusion and hospitalization for recurrent pyelonephritis in a female patient with a bifid ureter.

ESKAPE Pathogens in Oral and Maxillofacial Infections

  • Purpose: Most cases of oral and maxillofacial infections are usually easily treated by proper diagnosis, elimination of causative factors, and antibiotic therapy. However, the emergence and the increase of multidrug-resistant bacteria make treatment challenging. "ESKAPE" pathogens are the most common opportunistic organisms in nosocomial infections and have resistant to commonly used antibiotics. There are many medical reviews of ESKAPE pathogens, but few in dentistry. This study focuses on oral and maxillofacial infection especially with ESKAPE pathogens. The purpose of this study is to prepare feasible data about tracing and treatment of infection related to pathogens that may be beneficial to clinicians. Methods: A total of 154 patients with oral and maxillofacial infections were reviewed by analyzing retrospectively hospitalized data in the Department of Oral and Maxillofacial surgery, Chosun University Hospital, Korea, past 5 years from January 2014 to December 2018. Based on the medical records and microbiological tests, the results were divided into two groups: infections with ESKAPE pathogens and other bacteria. Results: A total of 22 species were isolated from 154 patients. The proportion of ESKAPE pathogens among all bacterial isolates collected from infected patients was 39.6%. Causative factors, especially in post-operative infection, showed a statistically significant correlation to ESKAPE infections (29 cases). And average of treatment period in ESKAPE group was longer than non-ESKAPE groups. Overall, Klebsiella pneumoniae (60.7%) was the most frequently isolated ESKAPE pathogen. And high antibiotic resistance rates had been detected in the ESKAPE during the five-year period. Conclusions: Infections with ESKAPE pathogens are now a problem that can no longer be overlooked in Dentistry. Based on results of this study, ESKAPE pathogens were highly associated with post-operative or opportunistic infections. Clinicians should be careful about these antibiotic resistant pathogens and use appropriate antibiotics to patients while having dental treatments.

  • Helicobacter pylori는 사람의 위점막에서 발견되는 전염성이 있는 중요한 병원균이다. 장기간 기생하면서 만성위염, 소화성궤양, 위 변연부 B세포 림프종, 그리고 위암을 일으키는 세균으로 알려져 있다. 국내에서는 프로톤펌프 억제제(proton pump inhibitor, PPI)와 두 가지 항생제(amoxicillin, clarithromycin)를 포함하는 표준 삼제 요법을 1차 치료로 사용하여 왔으나, 점점 증가하고 있는 항생제 내성으로 인해 제균율은 점차 감소하고 있다. 여기서는 H. pylori의 현재의 치료법들과 이들 치료법들의 문제점들을 검토하고, 표적치료의 필요성과 표적치료에 활용할 수 있는 약물전달체로서의 점토광물의 가능성에 대해 알아보고, 이들을 이용한 새로운 치료 방향에 대한 향후 연구계획 등에 대해서 논하고자 한다.

광역동요법을 병행한 두피 모낭염 한방치험 1례 (A case of scalp folliculitis treated by Korean Medicine with photodynamic therapy(PDT))

  • Objectives : The purpose of this study is to report the effect of combination therapy of photodynamic therapy (PDT) with Korean medical treatments on scalp folliculitis, which has been difficult to treat by using antibiotic.Methods : A 32-year-old male patient with scalp folliculitis, received treatment with acupuncture, herbal medicine and PDT and so on. We used Visual Anologue Scale(VAS) to evaluate recovery from erythema, pruritus and pain. Also, the photos of patient were taken before and after treatments.Results : Visible symptoms to the naked eye and VAS scale effectively decreased after Korean medicine treatments for two months.Conclusions : It is expected that Korean medical treatments and PDT were thouht to be effective for scalp folliculitis.

Treatment of the Mycobacterium chelonae Infection after Fat Injection

  • For recent years, use of autologous fat injection has increased significantly in facial contouring surgery. Along with such increase in use, complications like atypical mycoplasma infection have been also on the increasing trend. The authors report two cases of Mycobacterium chelonae infection that occurred after autologous fat injection. Patients were treated as infection that resistant to common antibiotics and results were negative to routine culture and Gram staining. Acid-fast bacillus stain, polymerase chain reaction (PCR) test and mycobacterial cultures were conducted for diagnosis under suspicion of atypical mycoplasma infection. Then, combination antibiotics therapy, surgical treatment, and steroid injection were performed for treatment. Both patients were diagnosed with Mycobacterium chelonae in PCR test. They were positive to mycobacterial cultures. Combination antibiotics therapy was repeated to improvement of symptom. However, they could not be free from side effects such as deformation in facial contour, scar and pigmentation even after full recovery. When chronic wound infections after autologous fat injection, we must suspect atypical or mycobacterial infection and conduct examinations for a early diagnosis and proper antibiotic therapy that is effective to the nontuberculous mycobacteria.

은 이온도입이 세균성장에 미치는 영향 (Inhibition of Bacterial Growth with Silver Wire Iontophoresis)

  • The silver cation has broad-spectrum antibiotic activity toward Gram-positive, Gram-negative, fungal. aerobic and anerobic micro-organisms. Silver has been used to care of infected wound. pyogenic arthritis, and chronic osteomyelitis. The purpose of this study was to determine whether pure silver wire iontophoresis using milliamperage direct current has an inhibitory effect on growth in vitro of 3 different species of bacteria-Staphylococcus aureus, Escherichia coli, and Pseudomonas aeruginosa. Using agarose based media, silver iontophoresis performed at 0, 1, 2, 4, 8 mA for 15 minutes. All experiments were performed in triplicate. Following iontophoresis, inhibition zone width of bacterial growth was measured with calliper. The inhibition of bacterial growth occured at the anodal silver electrode. Inhibition zone width of bacterial growth was significantly increased in all three bacterial species (p<0.05). Between bacterial species, inhibition zone width was not significantly different. Inhibition gone and amperage showed a highly significant positive linear relationship (p<0.001). The result of this study showed that pure silver wire iontophoresis with milliamperage direct current, as well as microamperage direct current, can inhibit bacterial growth in vivo.
