Same-Day versus Overnight Observation after Outpatient Pediatric Percutaneous Liver Biopsy: A Retrospective Cohort Study

  • Purpose: Percutaneous liver biopsy (PLB), a diagnostic procedure to identify several hepatobiliary disorders, is considered safe with low incidence of associated complications. While postoperative monitoring guidelines are suggested for adults, selection of procedural recovery time for children remains at the discretion of individual operators. We aim to determine if differences exist in frequency of surgical complications, unplanned admissions, and healthcare cost for children undergoing outpatient PLB for cohorts with same-day vs. overnight observation. Methods: We performed a retrospective cohort study in children 1 month to 17 years of age undergoing ultrasound-guided PLB from January 2009 to August 2017 at a tertiary care, pediatric referral center. Cohorts were defined by postprocedural observation duration: same-day (${\leq}8$ hours) vs. overnight observation. Outcomes included surgical complications, medical interventions, unscheduled hospitalization within 7 days, and total encounter costs. Results: One hundred and twelve children met study criteria of which 18 (16.1%) were assigned to same-day observation. No differences were noted in demographics, anthropometrics, comorbidities, biopsy indications, or preoperative coagulation profiles. No major complications or acute hospitalizations after PLB were observed. Administration of analgesia and fluid boluses were isolated and given within 8 hours. Compared to overnight monitoring, same-day observation accrued less total costs (US $992 less per encounter). Conclusion: Same-day observation after PLB in children appears well-tolerated with only minor interventions and complications observed within 8 hours of procedure. We recommend a targeted risk assessment prior to selection of observation duration. Same-day observation appears an appropriate recovery strategy in otherwise low-risk children undergoing outpatient PLB.

Nonalcoholic fatty liver disease in obese and nonobese pediatric patients

  • Purpose: Obesity is risk factor for nonalcoholic fatty liver disease (NAFLD). However, nonobese patients are also increasingly susceptible to NAFLD. The aim of this study was to compare the clinical characteristics of obese and nonobese pediatric patients with NAFLD. Methods: We retrospectively studied 68 patients who were diagnosed with NAFLD between January 2010 and October 2016 at 10-18 years of age. Body mass index ${\geq}95th$ percentile for age and sex was defined as obesity. Abdominal ultrasonography and laboratory, anthropometrics measurements were evaluated. Results: Among the 68, 26 (38.2%) were nonobese patients. The ratio of male to female was 5.8:1, and the median age at diagnosis was 13 years (range, 10-17 years). Significant higher triglyceride (223.0 mg/dL vs. 145.9 mg/dL, P=0.047) and total cholesterol levels (211.6 mg/dL vs. 173.2 mg/dL, P=0.011) were shown in nonobese than obese patients. High-density lipoprotein cholesterol level <40 mg/dL (hazard ratio [HR], 6.5; 95% confidence interval [CI], 2.13-7.10; P=0.048), total cholesterol level >200 mg/dL (HR, 5.6; 95% CI, 1.23-15.31; P=0.038) and abdominal obesity (HR, 2.53; 95% CI, 1.22-4.68; P=0.013) were significant risk factors for NAFLD in nonobese patients. Conclusion: Nonobese patients present a substantial proportion of pediatric NAFLD cases. Significant abnormal lipid concentrations were found in nonobese and abdominal obesity was important risk factor for nonobese NAFLD.

Association of Dietary Intake with 10-Year Risk for Coronary Heart Disease Predicted from Framingham Risk Score in the Korean Adults: Korea National Health and Nutrition Examination Survey 2019-2020 (한국 30세 이상 성인에서 식사 섭취와 Framingham risk score에 의한 10년 관상심장질환 위험과의 연관성: 2019-2020년 국민건강영양조사 자료 활용)

  • This study investigated the association between dietary intake and 10-year risk for CHD predicted from Framingham risk score in Korean adults using the Korea National Health and Nutrition Examination Survey 2019-2020. Eight thousand subjects (3,382 men and 4,618 women) aged 30 years or older were classified into three groups according to the 10-year CHD risk (%). The sociodemographics, anthropometrics indices, blood profiles, and dietary intake were collected. After adjusting for confounding variables, the mean adequacy ratio (MAR) decreased for both men and women as the disease risk increased. The high-risk group was below the MAR criteria, indicating that the overall quality of the meal was not good. The highest consumers of legumes (OR 0.56, 95% CI 0.37-0.84), fruits (OR 0.67, 95% CI 0.48-0.93), and fish (OR 0.60, 95% CI 0.42-0.87) had a 44%, 33%, and 40% lower 10-year CHD risk than lowest consumers in men, respectively. In women, there is no significant relationship between food groups and disease risk. Therefore, improving lifestyle habits such as weight control, increased activity, and adequate food intake, especially legumes, fruits, and fish rich in antioxidant nutrients and bioactive substances, appears to have a potential association with preventing coronary heart disease in Korean adults.

Development and Evaluation of an Integrated Health Management Program for Psychiatric Patients with Metabolic Syndrome (정신질환자를 위한 대사증후군 통합건강관리 프로그램 개발 및 평가)

  • Purpose: This study developed an integrated health management program for metabolic syndrome in psychiatric patients and examined its effects on self-efficacy, healthy lifestyle, physiological indicators, knowledge of metabolic syndrome, attitudes toward healthy behavior, and social support. Methods: A non-equivalent control group pretest posttest design was used. The participants were 65 psychiatric patients with metabolic syndrome in psychiatric rehabilitation centers, with 33 in the experimental group and 32 in the control group. The experimental group participants engaged in daily mobile application and walking exercises three times a week for more than 40 minutes over 8 weeks, while those in the control group were provided education booklets. The outcomes were measured using self-report questionnaires, anthropometrics, and blood analyses. Intervention effects were analyzed using the independent t-test, Mann-Whitney U test, ANCOVA, and Ranked ANCOVA. Results: The experimental group showed a significant increase in self-efficacy (F = 8.85, p = .004, ηp2 = .13) and knowledge of metabolic syndrome (t = 2.60, p = .012, d = 0.60) compared to the control group. Additionally, the experimental group demonstrated a significant decrease in waist circumference (Z = - 2.34, p = .009, d = 0.58) and body mass index (Z = - 1.91, p = .028, d = 0.47) compared to the control group. Conclusion: The integrated health management program for psychiatric patients with metabolic syndrome is effective in improving self-efficacy and knowledge of metabolic syndrome and decreasing physiological indicators such as waist circumference and body mass index.

A Study on the Korean Fit Test Panel and Static Headform Chamber (한국형 테스트 패널과 Static Headform Chamber 개발연구)

  • Objectives: A fit test panel is needed to identify the fit performance of a respirator and its face seal. This is a criterion for selecting subjects that can represent the facial characteristics of users. Although anthropometry data has been developed for people in United States and China it is not yet present in Korea. This study aimed to develop a Korean fit test panel and test headform. Methods: For the 7th and 8th waves of the Size Korea anthropometry data, facial measurements of 11,429 people aged 15 to 69 years were used for analysis. PCA and bivariate panel were classified using the ISO16976-2:2022(E) anthropometrics analysis method. Based on this result, a static headform was developemed and a fit test chamber was constructed. Results: Of the 11,429 Korean people used for principal component analysis, 11,300 were included in the ellipse, marking an acceptance rate of 98.87% on PCA panel. The face types were classified into five types. Among them, a large, medium, and small static headform were printed using a 3D printer. In addition, 10,985 people (96.12%) were included in the bivariate panel based on face length and face width. The y-axis (face length) boundary was 97.87 to 134.59 mm, and the x-axis (face width) boundary was 120.75 to 158.23 mm. Conclusions: Compared to the ISO analysis, the Korean principal component was narrower in the width item (PC1) and longer in the length item (PC2). For the future, it is necessary to conduct a fit test using the developed headform and chamber device to confirm the usefulness of this Korean test panel. Therefore, this study is considered valuable as basic research for Korean test panels.

Recognition of Body Weight Loss according to Age and Gender (연령 및 성별에 따른 체중 감량에 대한 인식 조사)

  • Among current health-related issues, obesity is considered one of the foremost, and the importance of this subject has fostered a national interest in body weight loss. In this study, the differences in recognition of body weight loss according to age and gender are investigated. The subjects of the study were 720 (male: 360 and female: 360) aged between 10-60 years, who had experienced to try body weight loss during 6 months prior to this study. Anthropometrics, general characteristics, personal reasons for body weight loss, comprehension of body weight loss, and a knowledge of diet-related issues were assessed through a questionnaire. The anthropometric measurements showed significant differences in height, body weight (present and desired) and BMI (p < 0.05) by age and gender. The difference between desired body weight and actual body weight was greater for younger subjects or female, regardless of whether they had under- or normal body weight (p < 0.05). Reasons for body weight loss varied; younger subjects and female tended to lose body weight "to enhance their appearances", whereas the older subjects and male desired "to improve their health" (p < 0.05). Subjects had different concepts concerning body weight loss; younger subjects and female considered body weight loss as "maintenance of a slender figure, or becoming more lean". On the other hand, older subjects and male thought body weight loss to be "effects approaching normal body weight" (p < 0.05). From our studies, it can be concluded that attitude on concerning body weight loss varied according to a age and gender. Thus, consideration of this individual differences would be vital in developing contents of a particular nutritional education program for body weight loss.

Nutritional Status and Constipation Rate among Female College Students Practicing Weight Control (다이어트를 하고 있는 여대생들의 변비유병율과 식이섭취상태)

  • This study was conducted to investigate the effects of diet on the nutritional status and constipation rate of female college students. A total of 251 female college students living in Gyeonggi province participated in this study. Dietary intake was ascertained from a 3-day dietary record, including 2 weekdays and 1 weekend day. Anthropometrics, general characteristics, diet experiences, dietary habits and prevalence of constipation were analyzed using a questionnaire. The subjects were divided into two groups, a control group (n=165, 46.2%) and a diet group (n=135, 53.8%). There was no significant difference in age, height, weight or BMI between groups. The prevalence of constipation was higher in the diet group than the control group. The intakes of energy, dietary fiber and water of the diet group were significantly lower than those of the control group. Correlation analysis revealed that the prevalence of constipation was negatively associated with exercise score, dietary fiber intake and water intake. It is postulated that dietary habits might be important factors contributing to constipation. Accordingly, nutrition education aimed to improve intakes of fiber and water may be necessary during weight control periods.

Study on the Status in Body Weight Loss Practice According to Age and Gender of Subjects with Experiences of Attempting to Lose Weight in Seoul (서울지역에 거주하는 체중 감량 시도자의 성별, 연령별 체중 조절 실태)

  • Among current health-related problems, the issue of over-weight is considered one of the most important, fostering a national interest in body weight loss. In this study, an awareness of one's current body weight and previous experiences of body weight loss were analyzed according to age and gender. The subjects of the study were 720 people (male: 360 and female: 360) aged $10{\sim}60$ years with previous experiences of attempting to lose weight 1 year prior to this study. Anthropometrics, general characteristics, awareness of one’s current body weight, and the body weight loss practice were analyzed through a questionnaire. Approximately 73.2% of the subjects recognized themselves as obese, and 83.5% wished to lose weight. The older the subjects were, the frequency of attempting to lose weight was lower, and those who succeeded in losing weight maintained their reduced body weight for a longer period. Compared to male subjects, more female reported attempts of losing weight, but the period of maintaining their reduced weight was shorter. The older subjects reported a higher satisfaction rate of the weight loss methods they used, and few of them gained back the weight they lost. This phenomenon was especially more stronger among male than female subjects. From our studies, it may be concluded that attitudes concerning a person’s current body weight and body weight loss practice vary according to age and gender.

Factors to Influence Consumption Pattern of Snacks of Middle School Students in Ilsan Area (일산 지역 중학생의 간식 섭취에 영향을 미치는 요인)

  • The purpose of this study was to analyze factors that influence consumption pattern of snacks of middle school students. The subjects of study were 607 middle school students (male: 316, female: 291) in Ilsan area. The subjects were surveyed by a self-administered questionnaire about anthropometrics, mother's employed status, parents' education level, and snack consumption pattern. Male students showed higher consumption in 'milk and milk products' (p<0.05), 'instant noodles' (p<0.01), and 'cereal' (p<0.05). In the other hand, female students showed higher consumption in 'biscuits, cookies' (p<0.01) and 'candy and chocolates' (p<0.01). Subjects in underweight group showed higher frequency in the consumption of 'biscuits, cookies' (p<0.05) and 'candy and chocolates' (p<0.001) compared with normal weight or overweight subjects. The subjects with mother not having a job showed higher consumption of snacks under controlling parents (p<0.001), and higher consumption in 'fruit and fruit juice' (p<0.05) and 'milk and milk products' (p<0.01) compared to the subjects with mother having a job. The subjects with parents having higher education level showed higher of consumption in 'fast food'. These results suggest that general characteristics of the subjects and characteristics of family environment may affect consumption pattern of snacks in middle school students.

Relationship between Corrected QT Interval and Cardiovascular Risk Factors in Young Healthy Adults: The Kangwha Study (젊은 성인에서 교정 QT간격과 심혈관질환 위험요인의 관련성: 강화연구)

  • Objectives : Prolongation of the heart rate-corrected QT (QTc) interval has been reported to be associated with cardiovascular morbidity and mortality. However, few studies have examined the relationship between the QTc interval and cardiovascular risk factors in young healthy people, The aim of this study was to examine the associations between the QTc interval and cardiovascular risk factors in young healthy adults. Methods : This study was performed as part of the Kangwha study, which started in 1986, and is an on-going follow-up study on blood pressure and related cardiovascular risk factors. In follow-up examinations during 2005, cardiovascular risk factors, including anthropometrics, blood pressure, blood chemistry and carotid ultrasonography, were measured, and questionnaires on health behaviors completed by 127 men and 149 women aged 25 years. The QTc interval was measured on the resting 12-lead electrocardiogram using an automatic analysis program. Results : The mean QTc interval was significantly longer in women $(419{\pm}17ms)$ than in men $(405{\pm}17ms)$ (p<0.001). A significant positive correlation was found between the QTc interval and waist-hip ratio (p=0.030) in men. Women showed a positive correlation between the QTc interval and systolic blood pressure (p=0.017). On a multiple regression analysis, the QTc interval was positively associated with the waist-hip ratio in men (p=0.012) and with the systolic blood pressure (p=0.020) in women. Conclusions : In young healthy Korean adults, the QTc interval was independently associated with the waist-hip ratio in men and with the systolic blood pressure in women.
