Breast and Cervical Cancer Screening and Associated Factors among Older Adult Women in South Africa

  • Background: Little is known about the cancer screening prevalence and correlates in older adults from different racial backgrounds. In the context of heightened efforts for prevention and early diagnosis, we collected information on screening for two major types of cancers: cervical and breast cancer in order to establish their prevalence estimates and correlates among older South African women who participated in the Study of Global Ageing and Adults Health (SAGE) in 2008. Materials and Methods: We conducted a national population-based cross-sectional study with a multi-stage stratified cluster sample of 3,840 individuals aged 50 years or older in South Africa in 2008. In this analysis, we only considered the female subsample of (n=2202). The measures used included socio-demographic characteristics, health variables, anthropometric and blood pressure measurements. Multivariable regression analysis was performed to assess the association of socio-demographic factors, health variables and cancer screening. Results: Overall, regarding cervical cancer screening, 24.3% ever had a Papanicolaou (PAP) smear test, and regarding breast cancer screening, 15.5% ever had a mammography. In multivariate logistic regression analysis, younger age, higher education, being from the White or Coloured population group, urban residence, greater wealth, and suffering from two or more chronic conditions were associated with cervical cancer screening, and higher education, being from the White or Indian/Asian population group, greater wealth, having a health insurance, and suffering from two or more chronic condtions were associated with breast cancer screening. Conclusions: Cancer screening coverage remains low among elderly women in South Africa in spite of the national guideline recommendations for regular screening in order to reduce the risk of dying from these cancers if not detected early. There is a need to improve accessibility and affordability of early cervical and breast cancer screening for all women to ensure effective prevention and management of cervical and breast cancer.

한국 성인의 식생활 지침 실천도와 이상지혈증의 관련성 (Association between Compliance with Dietary Guidelines and Dyslipidemia among Koreans)

  • While metabolic syndrome(MS) is rapidly expanding and dietary pattern, the known risk factor of MS, goes through heavy transition to western diet, not many researches have been done on the association between dyslipidemia and dietary factors in Korean adults. The purpose of this study was to investigate the association between compliance with dietary guidelines and dyslipidemia among Koreans. The subjects of 399 adults who visited health examination center were classified into dyslipidemia(n=180) and control(n=219). Diagnosis of dyslipidemia was based on NCEP-ATPIII criteria(triglyceride>=150mg/d, HDL-C<50mg/dl for male, HDL-C<40mg/dl for female). A questionnaire based interview was done to collect information on compliance with dietary guidelines, general characteristics and health related behaviors. Anthropometric variables were measured during the survey. Mean compliance score of dietary guideline was significantly lower in dyslipidemia group than in control. It was associated negatively with waist circumference and positively with serum HDL-cholesterol(p<0.05). Risks of dyslipidemia were significantly decreased in the group with highest dietary guideline score, high serum triglyceride levels(OR=0.484, 95% CI=0.268-0.875), abdominal obesity(OR=0.296, 95% CI=0.159-0.553), and dyslipidemia(OR=0.481, 95% CI=0.266-0.869). These results indicated that increasing compliance with dietary guidelines could be an effective strategy to lower the risk of dyslipidemia among Koreans.

Effects of daily quercetin-rich supplementation on cardiometabolic risks in male smokers

  • Limited information from human studies indicates that dietary quercetin supplementation influences blood lipid profiles, glycemic response, and inflammatory status, collectively termed cardiometabolic risks. We tested the hypothesis that quercetin-rich supplementation, derived from onion peel extract, improves cardiometabolic risk components in healthy male smokers in a randomized, double blinded, placebo-controlled parallel design. Randomly assigned subjects were instructed to take either the placebo (n=43) or 100 mg quercetin capsules each day (n=49) for 10 weeks. Anthropometric parameters and blood pressure were measured, and blood lipids, glucose, interleukin-6, and soluble vascular cell adhesion molecule-1 (sVCAM-1) were determined at baseline and after 10 weeks of quercetin supplementation. Quercetin-rich supplementation significantly reduced serum concentrations of total cholesterol (P<0.05) and LDL-cholesterol (P<0.01), whereas these effects were not shown in the placebo group. Furthermore, significant increases were observed in serum concentrations of HDL-cholesterol both in the placebo (P<0.005) and quercetin-rich supplementation group (P<0.001); however, changes in HDL-cholesterol were significantly greater in subjects receiving quercetin-rich supplementation than the placebo. Both systolic (P<0.05) and diastolic blood pressure (P<0.01) decreased significantly in the quercetin-rich supplementation group. Glucose concentrations decreased significantly after 10 weeks of quercetin-rich supplementation (P<0.05). In contrast, no effects of quercetin-rich supplementation were observed for the inflammatory markers-IL-6 and sVCAM-1. Daily quercetin-rich supplementation from onion peel extract improved blood lipid profiles, glucose, and blood pressure, suggesting a beneficial role for quercetin as a preventive measure against cardiovascular risk.

Obesity, Diet and Physical Inactivity and Risk of Breast Cancer in Thai Women

  • To evaluate the relationship between obesity, diet, physical activity and breast cancer in Thai women, we conducted a case control study with 1,130 cases and 1,142 controls. Informed consent was obtained from all participants and a structured questionnaire was performed by trained interviewers to collect information on demographic and anthropometric data, reproductive and medical history, residential history, physical activity and occupation as well as dietary habits. A significant positive association with an increased risk of breast cancer was observed in women body mass index (BMI) of ${\geq}25mg/m^2$ (OR=1.33, 95%CI 1.07-1.65), the risk being higher in postmenopausal women (OR=1.67, 95%CI 1.24-2.25). In addition, underweight BMI at ages 10 and 20 years showed an inverse association in all women (OR=0.70, 95%CI 0.56-0.88 and OR=0.74, 95%CI 0.59-0.93, respectively) and in those with a premenopausal status (OR=0.69, 95%CI 0.51-0.93 and OR=0.76, 95%CI 0.56-0.99, respectively). Regular exercise was associated with a decreased risk of breast cancer (OR=0.78, 95%CI 0.68-0.98). Interestingly, analysis by type of activity revealed significant protective effects for women who reported the highest levels of walking for shopping (OR=0.58, 95%CI 0.38-0.88). High consumption of vegetables and fruit were associated with a decreased risk of breast cancer, while high consumption of animal fat showed an increased risk in postmenopausal women. In conclusion, our results indicate that obesity and high consumption of animal fat are associated with breast cancer risk, particularly in postmenopausal women, while recreational physical activity has protective effects. It seems that primary prevention of breast cancer should be promoted in an integrated manner. Effective strategies need to be identified to engage women in healthy lifestyles.

청소년 전기 여학생의 하의 치수 규격에 관한 연구 (The Apparel Sizing System of Early Adolescent Girls - Focusing on Lower Garments -)

  • The purpose of this study was to develop a new size range and size interval for early adolescent girls. For this purpose, a total of 529 girls aged between 10 and 14 were measured and data were collected from 42 anthropometric measurements and 41 photographic measurements per a person. SAS 8.1 was used in data analysis including means, standard deviations, and frequency analysis. The stature was divided at 5cm intervals as in KS into 9 sizes from the lowest 130cm to the highest 171cm. If waist circumference were divided at the same intervals, the sizes cannot reflect the body growth of adolescent girls at these ages. Thus this study set intervals between sizes irregularly based on the mean of waist circumference by the type of body shape. Based on the results, this study proposed: for Type A - 6 sizes (140A-58, 145A-54, 145A-62, 150A-58, 150A-62, 155A-62); for Type X- 9 sizes (150X-59, 155X-63, 155X-66, 160X-59, 160X-63, 160X-66, 165X-59, 165X-63, 165X-66): and for Type H - 7 sizes (145H-68, 150H-68, 150H-70, 155H-68, 155H-73, 160H-68, 160H-73). For the sizes selected for each type, reference measurements were decided - centering on items necessary for manufacturing clothes. Reference measurements suggested for lower garments 8 items including waist circumference, hip circumference, slacks length and crotch length. The suggested sizes are distributed in a wider range, so they are considered to be helpful for students to find clothes fitting their bodies.

대구지역 임신부의 식품 및 영양섭취실태 (The Dietary Intake of Pregnant Women in Daegu Area)

  • This study was carried out to figure out the dietary intake of pregnant women in Daegu area. A total of 467 pregnant women was recruited at two hospitals in this area to collect the information on dietary intakes, general characteristics, and anthropometric measurements using an interview. Forty-seven women (10.1%) were in the first trimester, 101 women (21.6%) in the second trimester, and 319 women (68.3%) in the third trimester. The mean age, prepregnant weight and height of subjects were not significantly different among the trimester groups. The mean daily nutrient intakes of energy, fat, protein, carbohydrates, phosphate, sodium, vitamin B$_1$, vitamin B$_2$, and vitamin C were significantly different among the trimester groups, but the other nutrients were not different. For the calcium and iron, the percentage of subjects consumed less than 75% of the Korean RDA was over 80%, and for other nutrients such as energy, vitamin B$_1$, and vitamin B$_2$ over 50% of subjects consumed less than 75% of the Korean RDA in the first trimester. The correlations between nutrient intakes and weight gain during pregnancy showed different patterns among the trimester groups. The mean adequacy ratio and dietary variety scores were not significantly different among the trimester groups. The amount of food intake from cereals and cereal products were higher during the second and the third trimester compared to the first trimester, but those of the other food groups were not significantly different. In conclusion, nutrient intakes of the pregnant women for iron, calcium and some vitamins were inadequate and nutritional management programs for these nutrients are necessary. 농촌 성인 남녀의 단백질 섭취 실태 및 급원과 식습관, 기호품과의 관계 (A Survey of Protein Nutrition of Rural Adults in Korea and A Correlation between Their Protein Nutrition and Their Food Habits)

  • The purpose of this study is to provide useful information which will aid in the promotion of nutritional policy in the Korean rural area which are derived from a survey of intake and source of protein in some Korean rural adults and the correlations between their protein nutrition and various nutrients, the uses of tobacco, alcohol, coffee, etc. The survey was conducted from July, 24 through July, 18, 1989. The healthy subjects were 45 males(average age 42.3, average BMI 22.1kg/m2) and 55 females (44 years, 21.2 kg/m2) residing in Sungjoo Kyun, Chulanam-Do, Korea. The subjects were examined for the anthropometric, food Intake and food habits. Their daily diets were measured by 24-hr recall method. The results can be summarized as follows : The daily mean protein intakes of male and female subjects were 9595 of RDA(66, 5g) (16. 7% of total food intake per day) and 102.3Bh of RDA(61g) (14.8% of total intake per day) respectively. The order of sources of animal protein in all subjects was fish(47.9%), meat(29.8%), milk (12%), and egg(10.3%). Among protein sources the intake frequencies of fork and chicken were higher than those of others. The protein nutrition of the subjects showed positive correlations with energy and fat, carbohydrate, fiber at the level of significance of 1%. The protein nutrition of the male subjects showed no correlation with age, BMI, uses of alcohol, coffee, medicine, but the plant protein nutrition showed a positive correlation with smoking and exercise(at 5%). And the protein nutrition of the male subjects showed no correlations with uses of tobacco, alcohol, coffee, medicine and their opinion of their present state of their health, but the age of the male subjects showed negative correlations with milk(at 1%) and egg(at 5%). In conclusion, the daily protein Intake was good and the main sources of animal protein were fishes in rural adults. The protein nutrition of the subjects showed a correlation with energy, fat, carbohydrate and in the case of female, milk and egg intakes showed the negative correlation with age.

대한지역사회영양학회지 게재 논문에서의 한국인 비만 판정에 관한 연구 - 1996~2011년 게재 논문 분석 - (A Study on Classification of Obesity for Koreans based on the Articles in the Korean Journal of Community Nutrition - Articles Enlisted from 1996 to 2011 -)

  • The purpose of this study was to provide information on obesity assessment for Koreans. Among total of 1012 research papers enlisted in the Korean J Community Nutrition form 1996 to 2011, 248 articles were examined in which subjects were divided into more than 2 groups by obesity rate. About the method of anthropometric data collection, more than half of the research papers examined 52.5% and 28.7% of studies utilized the directly measured data and self-described data, respectively. About the utilization of obesity assessment methods, indirect methods of weight-height index (BMI, BMI percentile, and R$\ddot{o}$hrer index) and PIBW (WLR, Broca index, and KDA) were 62.4% and 23.2%, respectably, and the direct method of percent body fat assessment was only 9.3%. The most frequently utilized methods were WLR in under primary and primary school children, and BMI in the middle and high school students and in adults. For primary school students, WLR was the most frequently utilized method up to 2007, but it changed to BMI percentile afterward. Broca Index was no longer utilized since 2008. There were no articles utilizing BMI percentile and R$\ddot{o}$hrer index for obesity assessment in adults. Criteria for obesity assessment were not consistent among research papers: for example, % body fat, 19~40%; BMI, 20~30; BMI percentile, 85th or 95th. In the case of PIBW, 120% of ideal weight was the most frequently utilized criterion for obesity. Based on these findings, we suggest that proper methods and criteria of obesity assessment for each age group should be determined and proclaimed.

일부 농촌 지역에서 회합 급식을 하는 여자 노인의 우울 정도, 신체계측 및 영양섭취실태 조사 (Depression Symptom, Anthropometric Assessment, and Nutrient Intake of Elderly Females Who Eat Congregate Meals at Lunch in Rural Area)

  • This study was carried out to determine the depression symptoms and nutritional status of elderly females that ate congregate meals at lunch in rural Pocheon. The subjects were 18 elderly females aged over 65 ($75.7{\pm}4.6$ years) and information on smoking, subjective health status, depression symptoms and malnutrition risk were collected using a questionnaire administered with the help of trained research assistants. Measurements included mid-arm circumference and calf circumference. Food consumption for 3 days during breakfast and supper were determined by 24-hour recall and food consumption at lunch was determined as the difference between the amounts served and the plate waste. All data were compared between two age groups (${\leq}75$ and > 75). The subjects had the lowest nutrient adequacy ratio (NAR) for vitamin $B_6$ (0.523), followed by calcium and niacin and the lowest index of nutrient quality (INQ) for calcium (0.738). The older elderly had significantly lower NAR for vitamin $B_1$, vitamin $B_2$, vitamin $B_6$ and niacin as well as significantly lower mean adequacy ratio (MAR) compared to the young elderly. They also had significantly lower INQ for vitamin $B_1$ and vitamin C and significantly thinner arm circumference (28.3 cm vs. 31.3 cm). They consumed more than 40% of their daily intake for each nutrient at lunch. Higher proportions of nutrient intake from lunch provided evidence of the importance of congregate meals, suggesting that the government and society should support congregate meals to improve nutritional status.

중·고령인구에서 비만과 만성질환 관리를 위한 비만 측정 지표 비교 (Comparisons of obesity indicators for obesity and chronic diseases management among older adults)

  • Objectives: This study was to examine the associations between obesity level and major chronic diseases in older population in Korea, using different obesity indicators. Methods: Data was from the 2008 KloSA Biomarker pilot, a nationwide sample of 514 non-institutionalized subjects (age $63.6{\pm}9.8$ years; women 57.2%). Anthropometric information was collected by home visiting nurses. Portable bioimpedence devices(Omran HBF359) were used for measuring fat mass. Obesity cut-offs used the Asian criteria of $BMI{\geq}25$, $WC{\geq}90/85cm$, and $fat%{\geq}25/35$ for men and women. Chronic disease prevalence was defined by physician-diagnosed history of 8 specified diseases. Results: Prevalence of chronic diseases significantly increased with increment of obesity level by BMI, WC and fat %. Odds ratios of acquiring chronic diseases remained significant in the obese group (BMI OR 2.76, 95%CI 1.82-4.19; WC OR 2.73, 95%CI 1.81-4.11; Fat OR 1.87 95%CI 1.26-2.78), after adjusting for age, sex, marital status, education, work participation, household income, smoking, drinking, and exercise. Conclusions: Obesity measured by all three indicators, BMI, WC, and fat mass cannot be disregarded, accentuating the significant effect on increases in disease risks among older population. BMI and WC measure appeared better to assess the risks of chronic diseases.
