Orthodontic intrusion treatment of mandibular anterior teeth in a periodontal patient with hyperdivergent skeletal pattern: 8-year follow-up (장안모를 보이는 치주 질환자에서 하악 전치의 압하를 통한 과개 교합 치료: 8년 경과 관찰)

  • Patients who have extruded anterior teeth and deep bite with pathologic tooth migration, it is necessary not only periodontal treatment for reduce inflammation, but also orthodontic treatment for intrusion of anterior teeth. However, it is difficult to place the orthodontic brackets due to the deep bite, and there is a problem that the extrusion of the posterior teeth occurs more easily than the intrusion of the anterior teeth biomechanically. In particular, in patients with long face, relative intrusion of the anterior teeth by extrusion of the posterior teeth causes the clockwise rotation of the mandible and makes the facial profile worse. Therefore the biomechanical consideration and appliance design that can block these problems are required from the treatment plan. This is a patient who had a deep overbite with extruded anterior teeth, treated by periodontal treatment and intrusion of mandibular anterior teeth using cute brackes and miniscrews, and resulted in favorable maintenance during 8-year retention.

  • The purpose of this case report was to present an example of an esthetic and functional rehabilitation of anterior teeth with tetracycline discoloration and minor morphological abnormality of a 39-year old female. A chairside computer-aided design/computer-aided manufacturing (CAD/CAM) system with CEREC AC was applied for the prosthetic procedure and all ceramic crowns made with lithum disilicate (IPS e.max CAD) restored the esthetic and functional features of sixteen anterior teeth successfully.

  • Widths between the first permanent molars and the mesio-distal widths of the four anterior teeth were measured on 32 lower and 33 upper dental casts of young adults aged 18-25 yerars. The casts were grouped in five categories-two where no anterior crowding existed, and three in which varying degrees of crowding were present. Subjects with a difference between arch width and anterior tooth widths greater than 4mm in the upper arch, or 10mm in the lower arch, rarely had crowding. As arch width changes very little from age 7 onwards, measurements at this age may be used as a screening mechansim for future arch crowding.

Full mouth rehabilitation of the patient with severely worn dentition: a case report

  • The severe wear of anterior teeth facilitates the loss of anterior guidance, which protects the posterior teeth from wear during excursive movement. The collapse of posterior teeth also results in the loss of normal occlusal plane and the reduction of the vertical dimension. This case report describes 77-year-old female, who had the loss of anterior guidance, the severe wear of dentition, and the reduction of the vertical dimension. Occlusal overlay splint was used after the decision of increasing vertical dimension by anatomical landmark, facial and physiologic measurement. Once the compatibility of the new vertical dimension had been confirmed, interim fixed restoration and the permanent reconstruction was initiated. This case reports that a satisfactory clinical result was achieved by restoring the vertical dimension with an improvement in esthetics and function.

  • Purpose: The aim of augmentation of the alveolar ridge is to restore absorbed alveolar ridges for future implant site or esthetic prosthodontic restoration. The present clinical report describes the anterior maxillary augmentation cases using a soft tissue rotated palatal flap, and considers various problems of before and after surgery. Method: First & second patients were treated by vascularized interpositional periosteal-connective tissue(VIP-CT) flap for horizontal soft tissue augmentation. Especially second patient was progressed with bone grafting at the same time. Third patient was treated by the same flap with bone graft and implant placement in single tooth missing premaxillary area. Result: The obtained horizontal augmentation width measured $0.5{\sim}2.7\;mm$. Conclusion: This technique constitutes a viable approach for augmentation the anterior sector of alveolar ridge with the placement of dental implants. But it needs correct diagnosis preparation and careful surgery skill.

Anterior stafne bone cyst mimicking periapical cyst: a case report (치근단낭과 유사한 stafne 골낭종의 증례 보고)

  • Stafne bone cyst (SBC) is a bone defect usually located in the posterior portion of the mandible or mandibular angle below the inferior alveolar nerve. The cases of SBC involving multiple anterior tooth apices and penetrating the mandibular bone are extremely rare. Here we present a case of an anterior-positioned SBC mimicking periapical cyst, which penetrated the mandibular bone, with a review of the differential diagnosis.

  • A study was made primarily to investigate vertical and horizontal distortion of the image at the apical region of the dental roots in orthopantomographs. The subjects consisted of two dry skulls with radiopaque materials attached to root surface. Measuring of the width and length of each predetermined point at 23 teeth was performed in dry skulls and radiographic films. The results obtained were as follows; 1. There was overall magnification of image in the vertical dimension. And anterior portion had greater magnification rate than posterior portion, while lower anterior portion had less magnification rate than upper anterior portion. 2. There was reduction of the image in the horizontal dimension of the teeth, because of the position relation between dry skull and image layer of the orthopantomograph. 3. There was a significant difference in distortion rate between the oposite teeth. 4. Cervical portion of the tooth had more decreased rate of horizontal distortion than apical portion.

The effect of tooth brushing and thermal cycling on a luster change of ceromers finished with different methods

  • Statement of problem. Luster loss in esthetic anterior ceromer restoration can occur and can be related with rough surface texture. Understanding durability of surface finishing methods like polishing and surface coating have critical importance. Purpose. This study evaluated the effect of tooth brushing and thermal cycling on surface luster of 3 ceromer systems (Artglass, Targis, Sculpture) treated with different surface finishing methods. Material and methods. Seventy-two samples were prepared: 12 for control group Z100, 12 for Artglass, 24 for Targis, and 24 for Sculpture. Half of the Targis and Sculpture were polished according to the manufacturer's recommendation. The rest of the samples were coated with staining and glazing solution for Targis and Sculpture, respectively. All specimens were subjected to 10,000 cycles between $5^{\circ}C\;and\;55^{\circ}C$ with 30 seconds dwell time. Tooth brushing abrasion tests were performed in a customized tooth brushing machine with 500g back and forth for 20,000 cycle. Luster comparisons were based on grading after direct observation, and light reflection area was measured with Image analysis software. Results. All materials showed an decrease in luster grade after thermal cycling and tooth brushing. The post-tooth brushing results revealed that the glazed Sculpture had greater mean luster grade than did any other groups. While, the stained Targis group showed greatest changes after tooth brushing (p < 0.05), polished Targis and Sculpture did not show significant changes. However, glazed Sculpture showed discretely fallen out glaze resin. Conclusion. From the results of this study, all of the ceromer specimens were much glossy than control composite group after tooth brushing. coatings used for Targis and Sculpture had not durability for long term use.

A COMPARISON OF THE FORM OF THE FACE, THE DENTAL ARCH AND THE MAXILLARY CENTRAL INCISOR (안모, 치열궁 및 상악 중절치의 형태에 관한 상호 비교 연구)

  • The purpose of this study was to evaluate facial form, arch form and tooth form in young adults to determine if a correlation exists. 115 subjects who had healthy natural maxillary incisors and good occlusion consisted of 71 males and 44 females ranging from 20-30 years of age. Facial photographs and, intraoral photographs of upper anterior teeth and dental casts of upper jaws were taken to demonstrate facial form, dental arch form and tooth form. Form analysis is determined by comparing the ratio of the widths of the faces, dental archs and the teeth. The Chi-square test of independence between facial form, arch form and tooth form, was executed and the significance level determined. The results were as follows : 1. The distribution of facial forms was 66.1% square-tapering, 27.0% square, 5.2% ovoid, 2% tapering-square. 2. The distribution of tooth forms was 65.2% ovoid, 20.0% square-tapering, 11.3 % tapering-square, 3.5 % square. 3. The distribution of arch forms was 50.4 % square-tapering, 48.7 % tapering, 0.9 % reverse tapering-square. 4. There was no large differences in the distribution of facial forms, dental arch forms and tooth-forms between male and female. 5. No relationship existed between the tooth form and the facial form. 6. No relationship existed between the facial form and the dental arch form. 7. No relationship existed between the tooth form and the dental arch form. 8. This gave the impression that dental arch form and facial form could not be used as a true index in tooth selection.

The Use of Ovate Pontics in the Maxillary Anterior Esthetic Zone: A Clinical Report (Ovate Pontic을 이용한 상악 전치부 심미수복)

  • The ovate pontic was first described by Dewey and Zugsmith in 1933, but used clinically as a clinical alternative for esthetics in the late 1990s. The ovate pontic has been suggested as a more accurate duplication of emergence profile for natural teeth to provide an esthetic, cleanable prosthesis. If the resin temporary prosthesis with the ovate pontic is used during the healing period after the tooth extraction, it is possible to preserve the interdental papilla and eliminate or minimize the black triangle between the teeth. Ultimately it can become a esthetic final restoration without saliva leakage and phonetic discomfort. In this case we tried to treat the maxillary anterior area by the use of the ovate pontic and minimize the loss of the interdental papilla via duplicate the emergence profile of the natural tooth.
