계단 오름 보행 시 전족, 후족 착지 방법에 따른 하지의 근활성도와 발목 각도 비교 (Comparison of Ankle Angle and Lower extremity Muscle Activities Between Forefoot Strike, Heelfoot strike During the Stair Ascent Walking)

  • Purpose: The purpose of this study is to compare lower extremity muscle activities and ankle joint angles between different foot strike patterns (forefoot strike, heelfoot strike) during stair ascent walking. Methods: The subjects of this study were 22 males who walked in each foot strike pattern on ascent stairs at a speed of 85 beats/min. During stair walking with the two types of foot strike patterns, the muscle activities of the rectus femoris, tibialis anterior, medial gastrocnemius, hamstring, and gluteus medius were measured. Additionally, ankle joint angles for inversion, eversion, dorsi flexion, and plantar flexion were recorded. Each participant underwent the experiment three times, with the foot strike pattern randomized. Results were averaged according to the foot strike pattern. Results: Significant differences in ankle angles were observed across all phases according to foot strike pattern. Muscle activities in the lower extremities showed significant differences in all phases except the swing 1 phase. Moreover, differences in foot movement trajectory were noted depending on the foot strike pattern. Conclusion: Walking on ascent stairs elicited differences in lower extremity muscle activities and ankle joint angles based on foot strike pattern. These findings can serve as foundational data for selecting a suitable foot strike pattern tailored to individual patient conditions when training patients in walking on ascent stairs.

바벨 백 스쿼트 시 운동 속도 조건에 따른 하지근 활성도 및 무릎 관절의 부하량 비교 (Comparison of Lower Extremity Muscle Activity and Knee Joint Load according to Movement Speed Conditions during the Barbell Back Squat)

  • Objective: The purpose of this study was to compare the lower extremity muscle activity and knee joint load according to movement speed conditions during the barbell back squat. Method: Nine males with resistance training experience participated in this study. Participants performed the barbell back squat in three conditions (Standard, Fast, and Slow) differing movement speed. During the barbell back squat, muscle activity of the rectus femoris (RF), vastus lateralis (VL), vastus medialis (VM), biceps femoris long head (BFL), semitendinosus (ST), gluteus maximus (GM), gastrocnemius (GCN), and tibialis anterior (TA) was collected using an 8 channel wireless EMG system. The peak flexion angle of the lower extremity joints and the peak resultant joint force in each direction of the knee joint were calculated using eight motion capture cameras and ground reaction force plates. This study was to used the Friedman test and the Wilcoxon signed rank test, to compare lower extremity muscle activity and peak resultant joint force at knee joint according to movement speed conditions during the barbell back squat, and the statistical significance level was set at .01. Results: In the downward phase of the barbell back squat, the RF and TA showed the higher muscle activity in the fast condition, and in the upward phase, RF, VL, VM, BFL, ST, GM, and TA showed the higher muscle activity in the fast condition. As a results, analyzing of the load on the knee joint, in the downward phase, and in the upward phase, the higher peak compressive force of the knee joint was showed in the fast condition. Conclusion: The barbell back squat with fast movement speed was more effective due to increased muscle activity of lower extremity, but one must be careful of knee joint injuries because the load on the knee joint may increase during the barbell back squat with fast movement speed.

유형별 운동프로그램이 선 자세에서의 균형능력과 근활성도에 미치는 영향 (Effects of Exercise Program by Type on Balance Ability and Muscle Activity In A Standing Posture)

  • 본 연구는 20대 성인남성을 대상으로 세 운동군이 정적 및 동적 균형능력과 근활성도에 미치는 효과를 규명하기 위하여 수중 운동군, 체간 안정화 운동군, 균형 운동군으로 각각 15명씩 배치하였다. 2013년 6월부터 2013년 9월까지 연구가 진행되었으며, 6주 간 1주일에 3일, 1회 30분 동안 중재 후, 동일 대상자들의 균형능력 및 근활성도(앞정강근, 장딴지근)를 측정하여 비교하였다. 그 결과 집단 내 중재 전과 중재 후의 비교에서 세 운동군 모두 신체중심 이동면적과 총궤적길이의 변화는 유의한 차이가 있었고(p<.05)(p<.01), 동적 균형에서도 안정성 한계의 변화는 유의한 차이가 있었다(p<.05)(p<.01). 앞정간근의 근활성도의 변화에서 세 운동군의 좌 우측 모두 통계학적으로 유의한 차이가 있었고(p<.05)(p<.01), 장딴지근은 체간 안정화 운동군의 좌측을 제외한 모두 통계학적으로 유의한 차이가 있었다(p<.05)(p<.01). 수중 운동이 균형능력의 향상과 근활성도 증가에 효과적임을 규명하였고, 본 연구결과를 토대로 균형능력이 좋지 않은 노인이나 환자들에게 객관적인 수중 운동 효과를 밝힐 수 있는 연구가 진행되어 구체적인 수중 운동 프로그램 개발을 제안하고자 한다.

혈류제한 유산소운동 프로그램의 웰니스를 위한 효과검정 - 근활성도와 운동신경원을 중심으로 - (Effect for Wellness of Blood Flow Restriction Aerobic Exercise Program - Focusing on Mscle Ativity and Mtor Nurons -)

  • 본 연구는 정상인을 대상으로 건강을 위한 유산소 능력과 관계가 깊은 하지에 혈류제한 유산소 훈련을 시행함으로써 하지 근활성도와 운동신경원을 정량적으로 비교·분석하여 효과적인 혈류제한 운동프로그램의 효용성을 제시하는 기초자료를 제공하고자 한다. 압력 정도가 140 mmHg으로 혈류제한하여 트레드밀 위에서 유산소 운동을 적용한 집단 10명을 실험군I, 트레드밀 위에서 유산소 운동만 적용한 집단을 11명을 대조군으로 각각 무작위 배치한 후, 트레드밀에서 4주간, 주 3회, 1일 1회, 1회 30분간 중재 프로그램을 시행하였다. 중재 전 표면근전도를 활용하여 근활성도와 운동신경원을 측정하여 분석하였다. 연구 결과는 실험군I의 집단 내 전후 비교에서 넙다리곧은근, 넙다리두갈래근, 앞정강근 및 장딴지근의 근활성도가 유의하게 증가하였다(p<.001). 대조군의 집단 내 전후 비교에서는 넙다리곧은근, 넙다리두갈래근, 앞정강근 및 장딴지근의 근활성도가 유의하게 증가하였다(p<.001). 집단 간 변화 비교에서는 넙다리곧은근의 활성도가 유의한 차이가 있었다(p<.05). 하지 혈류제한과 병행하여 유산소운동을 접목하는 것은 엘리트선수 육성과 관절이 약한 노인 등 재활 트레이닝에 기능적인 활동을 회복시킬 수 있는 부상방지 운동프로그램 등으로 발전시킬 수 있을 것이며, 향후 연구에서 이러한 결과를 바탕으로 하여 정상인이 아닌 대상의 영역을 확대하고, 압력강도에 따른 다각적인 연구들이 필요할 것으로 사료된다.

Muscle Activities in the Lower Limbs for the Different Movement Patterns on an Unstable Platform

  • We performed experimental studies on the muscle activities in the lower limbs for the different movement patterns on an unstable platform. A training system for postural control using an unstable platform that we previously developed was applied for the experiments. This unstable platform provides 360 degrees of movement allowing for training of posture in various directions and provides simultaneous excitations to visual sensory, somatic sensation and vestibular organs. Compare with the stable platform, keeping body balance on the unstable platform requests more effective sensation from vision, vestibular sense and somatic sense. Especially, the somatosensory inputs from the muscle proprioceptors and muscle force are crucial. To study the muscle activities for the different movement patterns and find the best training method for improving the ability of postural control through training and improving the lower extremity muscular strength, fifteen young healthy participants went through trainings and experiments. The participants were instructed to move the center of pressure following the appointed movement pattern while standing on the unstable platform. The electromyographies of the muscles in the lower limbs were recorded and analyzed in the time and the frequency domain. Our experimental results showed the significant differences in muscle activities for the different movement patterns. Especially, the spectral energy of electromyography signals in muscle for the movement pattern in anterior-posterior direction was significantly higher than those occurred in the other patterns. The muscles in the lower leg, especially tibialis anterior and gastrocnemius were more activated compared to the others for controlling the balance of body on the unstable platform. The experimental results suggest that, through the choice of different movement pattern, the training for lower extremity strength could be performed on specific muscles in different intensity. And, the ability of postural control could be improved by the training for lower extremity strength.

10시 혹은 2시 방향의 경경골 대퇴 터널을 이용한 전방 십자 인대 재건술 후 2~4년 추시 결과 (Two to Four Year Follow-Up Results after Anterior Cruciate Ligament Reconstruction Using Transtibial Femoral Tunnel at 10 or 2 O'clock Position)

  • 목적: 10시 혹은 2시 방향의 경경골 대퇴 터널을 이용한 관절경적 전방 십자 인대 재건술 후 2-4년 추시 결과를 분석하고자 하였다. 대상 및 방법: 동종 경골건을 사용하여 전방 십자 인대 재건술을 시행하고 추시가 가능하였던 86예를 대상으로 하였다. 평가는 2000 International Knee Documentation Committee (IKDC) 주관적 슬관절 점수, Lachman 검사, Pivot-shift 검사, KT-1000 arthrometer 검사, 2000 IKDC 슬관절 검사 및 2차 관절경 검사를 포함하였다. 결과: IKDC 주관적 슬관절 점수는 80점 이상이 75예(87.2%), Lachman 검사상 1+ firm end 이하의 전위는 83예(96.5%), Pivot-shift 검사상 음성은 78예(90.7%), KT-1000 전방 전위 검사상 차이가 5 mm 이하인 경우는 83예(96.5%)였다. IKDC 슬관절 검사상 정상 또는 거의 정상이 80예(93.0%)였다. 2차 관절경 검사는 15예에서 시행되었으며 9예(60%)에서 전방 십자 인대 이식물의 양호한 활액막 형성을, 4예(26.7%)에서 부분적인 활액막 형성을, 2예(13.3%)에서 불량한 활액막 형성을 보였고 이식물의 실패는 1예(6.67%)가 있었다. 결론: 10시 혹은 2시 방향의 경경골 대퇴 터널을 이용한 전방 십자 인대 재건술은 전후방 안정성뿐만 아니라 회전 안정성에서 양호한 결과를 보여주었다.

고유 수용성 신경근 촉진법과 시각 되먹임 기반 관절재현 훈련이 발목관절의 고유 수용성 감각 수준과 한 발 서기 균형 능력에 미치는 영향 (Effects of Proprioceptive Neuromuscular Facilitation and Visual-Feedback based Joint Position Reproduction Training on the Level of Ankle Proprioception and One-leg Standing Balance Ability)

  • Proprioception training has been considered a secondary method to facilitate postural control ability. This study investigated the effects of two different proprioception training methods - the proprioceptive neuromuscular facilitation (PNF) and visual feedback-based joint position and force reproduction (VF) - on postural control advancements. Sixteen healthy people volunteered for this study, and they randomly grouped two. Each group participated in the PNF and VF training for three weeks. We evaluated each subject's proprioception levels and balance ability before and after the training. We used a clinometer and electromyogram (EMG) for VF training. The joint position reproduction test was also used to evaluate the position and force aspects of the proprioception level. We analyzed the trajectory of the center of pressure (COP) while subjects were standing on the firm floor and balance board with one leg using a pressure mat. The improvement of the position aspect of the proprioception level of the VF group (4.93±4.74°) was larger than that of the PNF group (-0.43±2.08°) significantly (p=0.012). The improvement of the anterior-posterior COP velocity of the PNF group (0.01±0.01 cm/s) was larger than that of VF group(0.002±0.01 cm/s) significantly (p=0.046). Changes of position error in the PNF group (rho=0.762, p=0.028) and tibialis anterior force reproduction error in the VF group showed a significantly strong relationship with balance ability variables. These results showed that different PNF and VF have different effects on improving two aspects of proprioception and their relationship with the balance ability. Therefore, these results might be useful for selecting proprioception or balance rehabilitation considering the clinical and patients' situation.

보행 중 중족골 패드와 인솔의 생체역학적 영향성 평가 (Evaluation for Biomechanical Effects of Metatarsal Pad and Insole on Gait)

  • The purpose of this study was to evaluate the effects of metatarsal pad (MP) compared with barefoot and MP with using different insoles on gait. 15 healthy females who had no history of injury in the lower extremity with an average age of 22.7 year(SD=1.35), height of 160 cm(SD=3.4), weight of 48.8 kg(SD=5.52) and average foot size of 232.5 mm(SD=6.8) participated in this study as the subjects. The subjects walked on a treadmill under four different experimental conditions: 1) walking with barefoot, 2) walking wearing MP 3) walking wearing a soft insole with MP(SIMP), 4) walking wearing a rigid insole with MP(HIMP). During walking, foot pressure data such as force, contacting area, peak pressure, and mean pressure was collected using Pedar-X System(Novel Gmbh, Germany) and EMG activity of lower limb muscles such as tibialis anterior(TA), lateral gastrocnemius(LG), rectus femoris(RF), and musculus biceps femoris(MBF) was gathered using Delsys EMG Work System(Delsys, USA). Collected data was then analyzed using paired t-test in order to investigate the effects of each of experimental conditions. As a result of the analysis, when MP and HIMP were equipped, overall contacting area was increased while the force, peak pressure and the mean pressure were decreased. Especially, when the SIMP was equipped, every data were significantly decreased. In case of EMG, wearing MP, SIMP and HIMP made three muscles(TA, LG, RF)'s activity decrease. A result of the analysis will be able to apply for manufacturing functional shoes, diabetes shoes, senior shoes and lower extremity orthosis. Significance of the study due to a metatarsal pad and the insole is to analyze the changes in muscle strength.

Differences in the Joint Movements and Muscle Activities of Novice according to Cycle Pedal Type

  • Objective: The purpose of this study was to compare the joint movements and muscle activities of novices according to pedal type (flat, clip, and cleat pedal). Method: Nine novice male subjects (age: $24.4{\pm}1.9years$, height: $1.77{\pm}0.05m$, weight: $72.4{\pm}7.6kg$, shoe size: $267.20{\pm}7.50mm$) participated in 3-minute, 60-rpm cycle pedaling tests with the same load and cadence. Each of the subject's saddle height was determined by the $155^{\circ}$ knee flexion angle when the pedal crank was at the 6 o'clock position ($25^{\circ}$ knee angle method). The muscle activities of the vastus lateralis, tibialis anterior, biceps femoris, and gastrocnemius medialis were compared by using electromyography during 4 pedaling phases (phase 1: $330{\sim}30^{\circ}$, phase 2: $30{\sim}150^{\circ}$, phase 3: $150{\sim}210^{\circ}$, and phase 4: $210{\sim}330^{\circ}$). Results: The knee joint movement (range of motion) and maximum dorsiflexion angle of the ankle joint with the flat pedal were larger than those of the clip and cleat pedals. The maximum plantarflexion timing with the flat and clip pedals was faster than that of the flat pedal. Electromyography revealed that the vastus lateralis muscle activity with the flat pedal was greater than that with the clip and cleat pedals. Conclusion: With the clip and cleat pedals, the joint movements were limited but the muscle activities were more effective than that with the flat pedal. The novice cannot benefit from the clip and cleat pedals regardless of their pull-up pedaling advantage. Therefore, the novice should perform the skilled pulling-up pedaling exercise in order to benefit from the clip and cleat pedals in terms of pedaling performance.

두충(杜沖) 이 근육위축 흰쥐의 후지 근섬유형 및 MyoD 발현에 미치는 영향 (Effects of Eucommiae Cortex on Myofiber Type Transition and MyoD Expression in Hind Limb Muscle Atrophy of Rats)

  • Objectives : Eucommiae cortex is reported that it helps bone and skeletal muscle stronger. In case of bone, many report is presented, but reports related to skeletal muscle are rarely existed. So in order to investigate effects of Eucommiae cortex on the skeletal muscle atrophy following stroke, cerebral infarct was induced by the middle cerebral artery occlusion (MCAO) in the rats. Methods : In order to induce MCAO rats, nylon suture was advanced and then blocked middle cerebral artery(MCA). Water extract of Eucommiae cortex was treated for 15 days, once a day orally, after the MCAO. Effects were evaluated with muscle weights, muscle fiber type composition, cross-sectioned area of muscle fibers in soleus and gastrocnemius of the unaffected and affected hind limbs. And MyoD protein expression in gastrocnemius was demonstrated with immunohistochemistry and western blotting. Results : In the affected hind limb of the MCAO rats, muscle weight loss of gastrocnemius and tibialis anterior muscles were attenuated by Eucommiae cortex treatment. In soleus muscle of the affected hind limb of the MCAO rats, increase of type-I fibers and decrease of type-II fibers were induced by Eucommiae cortex treatment. In soleus muscle of the affected hind limb of the MCAO rats, decrease of cross-sectioned areas of type-I fibers was attenuated by Eucommiae cortex treatment. In gastrocnemius muscle of the affected hind limb of the MCAO rats, increase of type-I fibers and decrease of type-II fibers were induced by Eucommiae cortex treatment. In gastrocnemius muscle of the affected hind limb of the MCAO rats, decreases of cross-sectioned areas of type-I and type-II fibers were attenuated by Eucommiae cortex treatment. In gastrocnemius muscle of the affected and unaffected hind limb of the MCAO rats, MyoD expressions were increased by Eucommiae cortex treatment. Conclusions : These results suggest that Eucommiae cortex has a protective effect against muscle atrophy, through the inhibition of the muscle cell apoptosis, following the central nervous system demage.
