This is a report of 3 cases of tumors, which primarily originated from ribs and the sternum. In the first case of multiple myeloma, the patient was 67 year old male with a tumor located on the middle sternum invading the manubrium and the body of the sternum manifesting symptoms after a contusion of the anterior sternum. The sternum was entirely resected and was replaced by tantalum plate to reconstruct the defective chest wall in order to prevent the paradoxical movement during respiration. In the second case of osteogenic sarcoma, the patient was 43 year old male with a tumor located on the costochondral junction of the left 5th rib for 6 months. The left 5th rib was resected between the middle part and sternochondral junction of it including tumor and adjacent soft tissues. In the third case of chondrosarcoma, the patient was 36 year old male with a tumor located near the posterior angles of the right 7th and 8th ribs manifesting back pain on the area where the tumor was located. Resection of right lower lobe was performed since direct invasion of tumor was seen in the superior segment of right lower lobe. This was followed by the resection of both 7th and 8th ribs at the area between the costovertebral junction and the portion 10 em apart from the tumor including the tumor and intercostal soft tissues. Diagnoses of 3 cases of tumors described above were confirmed by histopathologic examination postoperatively. The postoperative courses were uneventful.
Subcondylar fractures have generally been treated by intermaxillary fixation except in cases where there is an absolute indication for open reduction. The reason behind a less aggressive surgical approach lies on the inherent difficulties in manipulating fragments in such a small area at the risk of damaging facial nerves or vessels such as the internal maxillary artery. However, long-term follow-up studies showed that conservative treatment of subcondylar fractures results in disturbances of occlusal function, deviation of the mandible, internal derangement of the temporomandibular joint, and ankylosis of the joint. We carried out open reduction of dislocated subcondylar fractures in 14 patients from 2000 to 2004 by a retromandibular approach. After the reduction of fractured bone, two H-shaped miniplates with 6 holes were fixed with screws at the anterior and posterior surfaces of the subcondyle. The retromandibular approach allowed good access and easy manipulation of the subcondyle. Immediate relief from malocclusion and correction of mandibular midline shifting were observed in all patients. Late temporomandibular dysfunction and ankylosis were not observed. Open reduction with plate osteosynthesis made it possible to avoid IMF in 7 of the 14 patients. The present study shows that open reduction through this retromandibular approach can produce good outcome in adult patients with subcondylar fracture.
The author has studied each landmark for successful interpretation in the radiograph of the head that have the complex anatomic structures, using panoramic radiograph, postero-anterior cephalometric radiograph, lateral cephalometric radiograph, Waters' radiograph of the skull. The anatomic structures of the human dry skull attached by radiopaque materials were taken radiographs and analysed comparatively. The results were as follows: 1. The overall anatomic structures of the mandible showed sharp images in the panoramic radiograph than other radiographs with relatively less distortion, superimposition, blurring of the image. 2. The anatomic structures were situated on sagital plane of the skull showed blurred images in panoramic radiograph than other radiographs. 3. The anatomic structures which were situated on the basal portion of the skull showed blurred and secondary images in the panoramic radiograph than other radiographs. 4. In the panoramic radiograph, the lower 3rd portion of the orbit appeared to be superimposed with the superior portion of the maxillary sinus and the medial and lateral surface of the nasal cavity showed extensively superimposition of the orbit and the maxillary sinus, which images showed blurring. 5. The inferior surface and posterior surface of maxillary sinus showed to be good image in the panoramic radiograph than other radiographs. 6. In the panoramic radiograph, line of maxillary bone between lateral pterygoid plate, line of maxillary bone between zygomatic bone showed distinct image with another structures.
Objective : Medpor porous polyethylene was used to reconstruct small bone defects (gaps and burr holes) along a craniotomy bone flap. The feasibility and cosmetic results were evaluated. Methods : Medpor Craniotomy Gap Wedges, V and T, were designed. The V implant is a 10 cm-long wedge strip, the cross section of which is an isosceles triangle with a 4 mm-long base, making it suitable for gaps less than 4 mm after trimming. Meanwhile, the Medpor T wedge includes a 10 mm-wide thin plate on the top surface of the Medpor V Wedge, making it suitable for gaps wider than 4 mm and burr holes. Sixty-eight pterional craniotomies and 39 superciliary approaches were performed using the implants, and the operative results were evaluated with respect to the cosmetic results and pain or tenderness related to the cranial flap. Results : The small bone defects were eliminated with less than 10 minutes additional operative time. In a physical examination, there were no considerable cosmetic problems regarding to the cranial bone defects, such as a linear depression or dimple in the forehead, anterior temporal hollow, preauricular depression, and parietal burr hole defect. Plus, no patient suffered from any infectious complications. Conclusion : The Medpor Craniotomy Gap Wedge is technically easy to work with for reconstructing small bone defects, such as the bone gaps and burr holes created by a craniotomy, and produces excellent cosmetic results.
Meningioangiomatosis (MA) is a rare congenital tumor that occurs mostly in 5-15 year old children. There have been only 5 cases previously reported that described the cystic nature within these tumors. We present a case of a MA accompanied by a separate macrocyst. A normally developed 2 year-old female patient presented with partial and generalized seizures. The brain computerized tomogram and magnetic resonance imaging revealed the presence of a calcified mass accompanied by a cyst in the right parietal area, surrounded by low density and high attenuation edema and hemorrhage. Upon right parietal craniotomy, a $1.6cm{\times}1.2cm{\times}0.5cm$ sized plate-like, gray-white, slightly hard mass was seen and it was completely excised. Approximately 1 cm from the mass in the anterior lateral direction, a cyst was found and subsequent biopsy of the cyst wall revealed no tumor tissue, and therefore the cyst was not removed. Pathologic report demonstrated the meningioangiomatosis. Follow up examination 2 years later showed no recurrence of the tumor, and there was no evidence of neurological deficits. Authors suggest that cysts that arise in the surrounding tissues of tumors may not be tumor cysts, and do not require surgical removal.
Orthodontic treatment planning of cleft lip and palate requires consideration of the characteristic features, growth pattern and functional disorders related to cleft lip and palate patients. Tissue deficiencies and constriction of the scar tissue in surgically treated cleft lip and palate results in disturbance of maxillary growth and deficiency of midfacial region with anterior and posterior crossbite. These patients often present congenital missing of teeth, supernumerary teeth, malformed teeth, or ectopic position of teeth, which should be treated by orthodontic treatment by expanding upper arch followed by fixed appliance. Proper use of retainer and continuous follow-up is needed to prevent relapse after orthodontic treatment has finished. Also we have to pay attention to correct speech disorder which is caused by the velopharyngeal insufficiency.
The labiomandibulotomy approch to the oral cavity and oropharygeal region was first described by Roux in 1836 and become popular for oral and oropharyngeal tumors in cases where there are no clinical and radiological signs of mandible invasion. Anterior labiomandibulotomy and swing procedure provides excellent access and facilitate a mandibular resection and subsequent repair. In last two years, 8 cases of oral and oropharyngeal tumors were treated by this approach at the Dept. of Oral & Maxillofacial Surgery, Holy Family Hospital, Catholic University of Korea. And we analyzed postoperative complications as well as functional evaluations, and the results were as follows; In 4 cases, marginal mandibulectomy were combined with labiomandibulotomy and in case of malignancies, neck dissection was performed simultaneously. In almost case, plate and miniscrew fixation was used for osteotomy sites. Histologic evaluation of the resection margins of the specimens revealed tumor free in all cases. The postoperative complications were occured in 3 cases, one case of nonunion, one case of orocutaneous fistula, and one case of wound dehisence. Occlusal stability, jaw movement and swallowing function were acceptable postoperatively in 3 months. From above results, we concluded that, this approach not only provides wide exposure, permitting radical removal of benign and malignant lesions but also preserves function with minimal complications.
Loss of postural balance can possibly lead to increased risk of slips and falls in work places. Present study was performed to investigate the effects of noisy environments on postural stability during standing. It is known that a sound is characterized by the frequency and pressure level of the sound. Therefore, effects of the frequency and pressure level on postural stability were of primary concern. Ten male subjects participated in the experiment. Subject's center of pressure(COP) position was collected on a force plate while they were exposed to different frequency and pressure levels of the sound. Measured COP was then converted into the length of postural sway path in both anterior-posterior(AP) and medio-lateral(ML) axis. Results showed that the length of sway path in AP axis was significantly affected by the frequency of sound. The length of sway path was lowest at frequency level of 2000Hz and increased below and above this frequency range. The sound pressure level, however, did not significantly affect the postural sway length in both AP and ML axis. The results imply that industrial workers in noisy environments should be aware that their abilities of postural balance can be disturbed significantly.
This study was aimed to understand the effects of perturbed floor surface on human postural stability while standing. Ten subjects were asked to stand quietly on the surface with two angles of inclination ($0^{\circ}$ and $5^{\circ}$), two contamination conditions (dry and oil-contaminated), and three commercial floor materials (ceramic tile, coated wood, and vinyl tile). During each trial, a force plate with data acquisition systems was used to collect subject's center of pressure (COP) position. Measured COPs were then converted into the length of postural sway path in both subject's anterior-posterior (AP) and medio-lateral (ML) axis. Results showed that the length of sway path in ML axis was significantly affected by the angle of inclination and the type of floor material. The sway length was increased significantly at the inclination angle of $5^{\circ}$ and on the vinyl tile, respectively. The contamination condition, however, did not significantly affect the postural sway length in both AP and ML axis. The results imply that a proper treatment of floor surface and material is critical to preserving postural balance while standing.
Postural stability can reduce the likelihood of critical slip and fall accidents in workplaces. The present study aimed to analyze the effect of shoes type on the ability of postural control during quiet standing. The effect of workload on the body balance was also of primary concern. Thirteen healthy male undergraduate students participated voluntarily in the experimental study. Standing on a force plate with wearing slippers, sports shoes, or safety shoes, two-axis coordinate on subjects' center of pressures (COP) was obtained in the two levels, rest and workload. For the workload level, subjects performed treadmill exercise to reach the predetermined level of physical workload. By converting the position coordinates of COPs, the postural sway length in both anterior-posterior (AP) axis and medio-lateral (ML) axis was assessed. ANOVA results showed that, in AP direction, wearing slippers significantly increased the postural sway length compared to wearing sports shoes or safety shoes. No significant difference in the mean sway length in AP axis was observed between sports shoes and safety shoes. In ML direction, both the workload and the shoes type did not significantly affect the mean length of postural sway. However, the postural sway length increased marginally with the slippers especially during the workload condition. This study explains wearing slippers may interfere with the ability of postural control during quiet standing. Physical workload decreases the ability of postural stability further.
