A Clinical Study on Rotational Path Removable Partial Denture (회전삽입로를 이용한 국소의치에 관한 임상적연구)

  • Path of insertion(1) can be defined that the direction of movement of an appliance from the point of initial contact of its rigid parts with the supporting teeth to the place of final rest. Krol(2) described that in the conventional path of insertion, all the rests are seated more or less simultaneously but in the use of the rotational path one segment of the partial denture is seated first then the remainder of the prosthesis is rotated into position. The rotational path of insertion is limited primarily to the tooth borne prosthesis. Its great advantages are the elimination of anterior clasps to improve ethetic and reduction of tooth coverage to minimize plaque accumulation. Either a rigid minor connector or proximal plate provides retention through its intimate contact with a proximal tooth surface below the height of contour as indicated at a o-degree tilt. A specially designed rest in conjunction with this retentive component satisfies the basic requirements of clasp design. The purpose of this study was a clinical evaluation of rotational path removable partial dentures. Author delivered rotational path removable partial dentures to three different cases of patients and evaluated function of the dentures, difficulties of removal and insertion of the dentures and supporting structures of the abutment teeth by means of clinical and X-ray examinations for eighteen months. According to the examination data author came to the conclusion that the prognosis of the rotational path removable partial dentures was excellent.

  • The anterior approach to the cervical spine surgery is associated with possible complications such as dysphagia, hoarseness and granuloma formation etc. Because of orthopedic metallic plates, the increasing or focal uptake pattern may be demonstrated in $^{18}F$-FDG PET/CT scan. A 67-year-old-man came to our department, complained of dysphagia during the 4 months. He underwent cervical spine surgery three years ago. The CT and MRI findings mimicked typical posterior pharyngeal wall cancer with cervical metastasis. Furthermore, the SUV in $^{18}F$-FDG PET/CT was 10.3. But he was finally diagnosed as a granuloma resulting from the metallic cervical implants. The clinical correlation and medical history should be taken into account to avoid false-positive findings in PET/CT and to avoid many erroneous diagnostic pathways.

  • A cephalometric study was performed to reveal differences between skeletal Class III malocclusion patients and cleft lip and palate patients, The material for this study consisted of 16 males (mean age 19.8, range 17-29) and 9 females(mean age 19.4, range 16-27) with cleft lip and palate, and 222 Skeletal Class III malocclusion patients(males 106, females 116), Cephalometric tracing and measurements were done by one investigator. Results were followed: 1. Cleft lip and palate group had more retrusive maxilla than the skeletal Class III malocclusion group. 2, Cleft lip and palate group had smaller effective maxillary and mandibular length than skeletal Class III malocclusion group, and the difference was more prominent in the mandible than in the maxilla. 3. Dental compensation was not observed in the upper incisors of cleft lip and palate group and in the lower incisors it was smaller than skeletal Class III group. 4, In the Gonial angle and lower anterior facial height values, there was no significant difference between cleft lip and palate and skeletal Class III malocclusion group. These results can be used in orthodontic treatment planning and orthognathic surgery for the cleft lip and palate patients.

  • Severe maxilla1y deficiency can be caused by cleft lip and palate(CLP), other craniofacial deformities, atrophy in the edentulous maxilla, and trauma. Patients with maxillary deficiency present a difficult treatment challenge. Traditionally, this skeletal deformity has been treated by Le Fort osteotomy, skeletal repositioning, and fixation with mini-plates and screws. The drawbacks of this method include a limited amount of anterior maxillary advancement often requiring simultaneous mandibular setback, the inability to create new bone, and minimal soft tissue adaptation to the new position, all of which increase the potential of relapse in case of large advancement. The alternative method of maxillary distraction osteogenesis offers promising results for successfully treatment of these patients while potentially minimizing the risk of relapse.

Effects of Lumbar Stabilization on the Trunk and Lower Limb Muscle Activity and Velocity of the Center of Pressure During Single Leg Standing

  • The aim of this study was to investigate the effects of lumbar stabilization on both trunk and lower limb muscle activity and center of pressure (COP) in single leg standing. Surface electromyography (EMG) was used to collect muscle activity data, the mean velocity of COP was measured using a force plate, and a pressure biofeedback unit was used for lumbar stabilization training. The findings of this study are summarized as follows: 1) The EMG activity of the erector spinae decreased significantly and the activity of the rectus abdominis, internal oblique, external oblique, gluteus maximus, and gluteus medius increased significantly with lumbar stabilization single leg standing. 2) No differences in activity in the tibialis anterior, medial gastrocnemius, rectus femoris, and medial hamstrings were found with single leg standing. 3) The mean velocity of COP in the antero-posterior and medio-lateral directions in the lumbar stabilization single leg standing decreased significantly compared with the preferred single leg standing. The findings of this study therefore indicate that lumbar stabilization can facilitate the co-activation of deep stabilization and global muscles that improve postural control capability during single leg standing.

  • Purpose : To introduce a more effective and efficient protocol for protraction of maxillary complex in CLP patient and demonstrate it by case presentation. Methods : Miniplates are placed on zygomatic buttress and anterior part of mandible, at the time of secondary alveolar bone graft. During the night time, the orthopedic force is applied by wearing elastics from maxillary miniplates to facemask (500 g per side). During the day time, the intermaxillary elastics connecting maxillary miniplates to mandibular miniplates are applied (200 g per side). During the orthopedic treatment, dental alignment is possible. Results and conclusion : By the new protocol, the intermaxillary relation and occlusion are improved due to the stable intraoral anchorage and better cooperation of patients.

A Case of Amblyomma testudinarium Tick Bite in a Korean Woman

  • A case of tick bite was found in the inguinal region of a 74-year-old Korean woman. She was attacked by the tick while working in her vegetable garden in the vicinity of mountain located in Suncheon City, the southern coastal area of the Korean Peninsula. On admission she complained of mild discomfort and itching around the bite area. The causative tick was 23 mm long and had slender pedipalps. The scutum was quite ornate and had eyes at the edge. The genital aperture was located anterior to the level of the coxa II. The spiracular plate was comma-shaped and the anus was surrounded posteriorly by the anal groove. The coxa I had subequal 2 spurs; the external one slightly larger. The spur of coxa IV was slightly longer than those of coxae II and III. The tarsus IV had 2 distinct subapical ventral spurs. It was identified as the fully engorged adult female of Amblyomma testudinarium. This is the first human case of Amblyomma bite in Korea.

  • In treatment of dentigerous cyst, complete enucleation, histopathologic examination and postoperative care are important to prevent the potential complications (mural ameloblastoma, squamous cell carcinoma). On the other hand, a maxillary impacted supernumerary anterior tooth are removed surgically, owing to the possibility of the cyst formation in future. After the cyst enucleation and extraction of the involved tooth, the wound area sutured and removable resin plate is then applied. In this operation, the postoperative bleeding and infection is likely to occur owing to postoperative accumulation of hematoma & seroma, psychologic stress and other contaminated factor. So, the authors established the immediate rubber & iodoform gauze drainage into the sutured wound of cyst enucleation & tooth extraction for the prevention of postoperative bleeding and infection. The removable resin splint are not used because of the poor cooperation and economic factor. The results were more favorable without the postoperative bleeding & wound infection in a cerebral palsy patient.

Ultrastructure of the flagellar apparatus in cryptomorphic Cryptomonas curvata (Cryptophyceae) with an emphasis on taxonomic and phylogenetic implications

  • Cryptomonas curvata Ehrenberg is a photosynthetic freshwater flagellate and the type species of the genus Cryptomonas. We examined the flagellar apparatus of cryptomorphic C. curvata by transmission electron microscopy. The major components of the flagellar apparatus are the non-keeled rhizostyle (Rhs), striated fibrous root (SR), striated fiber-associated microtubular root (SRm), mitochondrion-associated lamella (ML), and two types of microtubular roots (3r and 2r). The non-keeled Rhs originate at the ventral basal body and consist of two types of microtubule bands extending together into the middle of the cell. The SR and SRm extend parallel to the left side of the cell. The ML originates from the ventral basal body and is a plate-like fibrous structure associated with mitochondria. The 3r extends from the dorsal basal body toward the dorsal anterior of the cell. The 2r originates between the two basal bodies and extends shortly to the left of the cell. The overall configuration of the flagellar apparatus is most similar to that previously reported for C. pyrenoidifera. These results demonstrate that the features of the flagellar apparatus are useful for distinguishing closely related species and inferring phylogenetic relationships among taxa.
