  • Intrinsic discoloration of permanent anterior teeth is a continuing esthetic problem. Tetracycline ingested during amelogenesis has long been recognized as predisposing factor in intrinsic staining. Methods for restoring original tooth color have included (1) complete PFM crown coverage (2) porcelain laminate veneers (3) direct bonding of composite resin, and (4) bleaching. In the case of tetracycline-stained upper anterior teeth, We authors, obtained the satisfactory results by means of gingivectomy, preliminary bleaching and porcelain laminate veneer restoration.

  • Comprehensive prosthetic treatment requires considerations from various points of view. The anterior guidance is important factor in prosthodontic treatment of anterior teeth. Lingual surface contour of anterior restoration is so critical that a small mistake of laboratory or clinical process can cause discomfort of patient and disharmony of entire dentition. There are no guidelines for lingual surface contour that fit all patients. Therefore the lingual surface of provisional restoration is most accurately described as a customized one. The dentist transfers the exact information of anterior guidance that has made through long term provisional restoration to the technician. This case introduce that the duplication technique of CAD/CAM system to reproduce the anterior guidance of provisional restoration. This method can improve satisfaction of both patient and dentist.

The Results of Bankart Repair for Anterior Instability of the Shoulder - Arthroscopic versus Open Bankart Procedure - (견과절 전방 불안정성에 대한 Bankart 술식의 결과-관절경적 술식과 관혈적 술식의 결과 비교-)

  • Purpose : The purpose of this study was to compare patients with anterior shoulder instability who were treated with an open Bankart procedure with those treated with an arthroscopic procedure, and to evaluate factors influencing the final outcomes and recurrence. Materials & Methods : One hundred seven shoulders underwent open Bankart repair, and fifty-one shoulders were treated arthroscopically. Average followup for open group was 34 months, and for arthroscopy group was 25 months. The Bankart Rating System by Rowe was used to evaluate the clinical outcome of the procedure. And, the patients were asked about any changes concerning their sports and professional activities. Results: According to Bankart Rating system by Rowe, open group had 97% fair to excellent results with 2 recurrent dislocation(1.8%) and 4 recurrent subluxation(3.6%), and arthroscopy group had 94% fair to excellent results with 3 recurrent dislocation(5.8%) and 4 recurrent subluxation(8%). In open group, 9 shoulders(8.4%) had the mild limitation of range of motion at the time of followup, and 2 shoulders(3.9%) in arthroscopy group. Age and gender do not seem to be a significant factor contributing to an increased re-recurrence rate. The incidence of re-recurrence seems to be affected by dominance, frequency, and patient's activity. The size of Bank art lesion might be also considered as a contributing factor. Conclusion: Either open or arthroscopic Bankart procedures are safe and effective methods with acceptable results if an adequate patient's selection, precise surgical technique and proper postoperative care are done. And arthroscopic surgery could be considered if the anterior instability is non-dominant, non-athlete, traumatic unidirectional and Bankart lesion has minimal erosion of the glenoid and it has thick and mobile labrum.

  • A tooth solely can not be an exclusive factor to attain our goal, esthetics. Other consideration, such a proportion of individual tooth, harmonized arrangement of the dentition and surrounding tissues including gingiva, lip and face should be taken into account. Esthetic gingival pattern and incisal level in the anterior area are the very important factors for esthetic restoration of the smile. We often propose adequate periodontal and prosthodontic approaches in order to improve the esthetics of the anterior area. Here, I would like to share some cases which include soft tissue management for ovate pontic placement, osteoplasty, resin filling for severe cervical caries, and bleaching approach to discolored root caused by endodontic treatment, and so on.

  • 견관절의 ABER position에서 posterosuperior rotator cuff 가 posterior glenoid에 contact 하여 internal impingement가 일어나는 현상은 운동선수가 아닌 일반인에게도 발생하는 physiologic phenomenon이며 과도한 반복 동작으로 인하여 증상이 발현한다. Internal impingement는 증상이 모호할 뿐만 아니라 견관절 내 여러 병소가 함께 존재하기 때문에 각각의 pathology에 대한 자세하고 정확한 evaluation이 필수적이다. 아직 controversy가 있지만 internal impingement의 pathologic factors로서는 anterior capsular laxity (true anterior laxity), posteroinferior capsular tightness (pseudo anterior laxity)와 이로 인한 glenohumeral internal rotation deficit (GIRD), 그리고 SICK scapula syndrome (scapular dyskinesis) 등으로 요약할 수 있다. 진단은 주로 병력 청취와 신체 검사로 내려지며 ultrasonogram, MRA등으로 확진한다. Sleeper stretching exercise 등과 같은 보존적 치료에 반응이 좋지만 6개월간의 재활 치료에도 호전이 없다면 증상과 직접 관련된 주 병소(appropriate pathology)를 찾아 이에 대한 적절한 수술이 이루어져야 한다.

  • The critical factors affecting the esthetics of anterior implants can be summarized as following: 1) Correct positioning of implant fixture 2) Enough amount of alveolar bone 3) Optimum volume of soft tissue. The position of implant is probably the most important factor in obtaining esthetic treatment outcome. The 3-dimensional orientation of implant is determined by the position on the alveolar ridge and its direction. Clinicians often try to mimic natural teeth when fabricating restorations. During the course of esthetic diagnosis and treatment, however, one should not forget to consider the correlation between facial pattern, lips, gingiva, alveolar ridge, as well as remaining dentition. Since anterior region is biologically unfavorable when compared with posterior region, one minor discrepancy in positioning of implant can cause esthetically undesirable treatment outcome. If one understands the biological and prosthetic meaning of implant's 3-dimensional position, he or she can achieve superior esthetic outcome in anterior region.

A STUDY ON THE FACTORS AFFECTING TO TOOTH WEAR (치아교모에 영향을 미치는 요인에 관한 연구)

  • This study was performed to investigate the factors related to tooth wear. For this study, 78 patients with temporomandibular disorders and 76 dental students without any signs and symptoms of temporomandibular disorders were selected as the patients group and as the normal group, respectively. Preferred chewing side, Angle's classification, lateral guidance pattern, head and shoulder posture were observed clinically. Electromyographic activity of anterior temporalis and masseter muscle were recorded with $BioEMG^{(R)}$ and occlusal status were recorded with $T-Scan^{(R)}$. Wear facet area of each tooth was measured from working model of upper arch corresponding to the occlusal status from T-Scan. Wear facet area were measured with planimeter in $mm^2$. Total area were divided into incisal, canine, posterior tooth area. Anterior wearfacet area was incisor area plus canine area, and unilateral area was anterior area plus posterior area. The data collected were analyzed by SAS statistical program and the results of this study were as follows: 1. There was no significant difference between the two groups in total werafacet area, and male subjects showed tendency to have larger area in the normal group but female subjects showed tendency vice versa. 2. There was no significant difference related to preferred chewing side and Angle's classification, however some difference was observed by lateral guidance pattern. Anterior wear facet area in subjects of canine guidance was the largest in the three subgroups. 3. Subjects with head tilting to right side had larger posterior and total area, and subjects with higher shoulder in right side had larger canine and anterior area than any other subgrous. 4. Electromyographic activity of masseter muscle was more correlated with wear facet area than anterior temporalis muscle, and tooth contact number and force were significantly correlated with wear facet area, but the most important factor affecting tooth attrition was age.

Postoperative Clipping Status after a Pterional versus Interhemispheric Approach for High-Positioned Anterior Communicating Artery Aneurysms

  • Objective : When treating high-positioned anterior communicating artery (ACoA) aneurysms, pterional-transsylvian and interhemispheric approaches are both viable options, yet comparative studies of these two surgical approaches are rare. Accordingly, this retrospective study investigated the surgical results of both approaches. Methods : Twenty-four patients underwent a pterional approach (n=11) or interhemispheric approach (n=13), including a unilateral low anterior interhemispheric approach or bifrontal interhemispheric approach, for high-positioned ACoA aneurysms with an aneurysm dome height >15 mm and aneurysm neck height >10 mm both measured from the level of the anterior clinoid process. The clinical and radiological data were reviewed to investigate the surgical results and risk factors of incomplete clipping. Results : The pterional patient group showed a significantly higher incidence of incomplete clipping than the interhemispheric patient group (p=0.031). Four patients (36.4%) who underwent a pterional approach showed a postclipping aneurysm remnant, whereas all the patients who experienced an interhemispheric approach showed complete clipping. In one case, the aneurysm remnant was obliterated by coiling, while follow-up of the other three cases showed the remnants remained limited to the aneurysm base. A multivariate analysis revealed that a pterional approach for a large aneurysm with a diameter >8 mm presented a statistically significant risk factor for incomplete clipping. Conclusion : For high-positioned ACoA aneurysms with a dome height >15 mm and neck height >10 mm above the level of the anterior clinoid process, a large aneurysm with a diameter >8 mm can be clipped more completely via an interhemispheric approach than via a pterional approach.

A Study on the Product differentiation Process by the Structuring of Design Factors (디자인 인자의 구조화에 의한 제품 차별화 프로세스 연구)

  • In this study design information was separately defined form general product information and thus factors reflected in product design ion the basis of values and roles were extracted. The following is a classification of 5 different types of design factors divided according to their disposition. ·Innovation factor - element which previously did not exist or element related with explicit reformation ·Open factor - active element which not only improves current performance but also induces new functions through understanding of usage situations and new possibilities. ·Anterior factor - element which prolongs and develops the early development requirements of products through C.I. and P.I. related elements and characteristics of previous models and design strategy. Self-evidence factor - element related with function visualization through product structure which may make possible the consolidation of shape and function. Rigid factor - element, based on the human factors engineering, related with the safety and efficiency of users. This classification was obtained by defining major characteristics of products considering the target consumer and market characteristics. In this classification factor structuring design process which efficiently deducted a differentiated final product by synthesizing factors of higher importance as dominant factors was proposed. With this kind of factor structuring process, product differentiation may be achieved by bestowing individual characteristics to each product by combining design dominant factors associated with the product for a specific purpose from the stages of product concept development. Moreover, this may be used as an approach to actively correspond to the various and specific demands of the comsumer.

Comparison of somatotypes from various classification methods - Between 18 and 24 years old Korean Women - (체형분류 방법에 따른 체형 유형 간 비교 - 18~24세 여성을 대상으로 -)

  • The purpose of this study was to compare somatotypes from various classification methods, to analyze the interrelation among each somatotype or each high frequency type, and to suggest the basis to interpret body size and shape more accurately. As a sample, the subjects were 97 Korean females between 18 and 24 years old. They were measured both anthropometric and photographic measuring in November, 1999. Their somatotypes were classified by three kinds of classification methods. The first method was based on the lateral view of body, the second involved Factor and Cluster analysis with the photographic measurements of anterior and lateral body, and the third involved Factor and Cluster analysis with the anthropometric measurements of whole body. The upper body was classified into three types, and the lower body was classified into 6 types from the lateral view of body. The bend-forward/q-2 was found to be the 'High-frequency type from the lateral view of body', and the Straight/n-1 was found to be the 'Straight type from the lateral view of body'. From the classification by the analysis of photographic measurements, the anterior body was classified into three types, the lateral was classified into 4 types. The X/${\varepsilon}$ type was found to be the 'High-frequency type from the analysis of photographic measurements of anterior and lateral body'. From the classification by the analysis of anthropometric measurements, the whole body was classified into three types. The i type was found to be the 'High-frequency type from the analysis of anthropometric measurements of whole body'. The significant interrelation was certified among some somatotypes or some High-frequency types. We found that both the view of body and the statistical analysis would make the clear definition of each somatotype possible. In order to certify the representativeness of High-frequency type, further analysis would be required of subjects who were in the High-frequency type and their body parts were in the High-frequency range.
