발목 염좌 및 만성 발목 불안정성 환자들에 대한 4주간의 고유수용감각 운동 프로그램의 효과 (The Effect of 4-Week Proprioceptive Exercise Program in Patients with Ankle Sprain and Chronic Ankle Instability)

  • Background : Ankle inversion sprains are one of the most common injuries in sports and activities of daily living that mostly concern physically active individuals. In most researches, proprioceptive deficit, muscle weakness and/or absent coordination have been regarded as a contributing factors. Despite the high incidence of ankle sprain and instability, therapeutic approaches to properly manage the symptoms have rarely been investigated. This study aimed to identify the effect of proprioceptive exercise program that is easy to integrate in normal training program. Methods : Subjects were randomly allocated to control group and experimental group consisting of 11 and 10 patients, respectively. The conservative treatment for the control group consisted of hot packs, ultrasound and TENS. In addition, the experimental group performed 7 exercises to enhance proprioceptive function of ankle joint. The therapeutic intervention of the controland experimental groups was performed a total of 20 exercise sessions, averaging 50 hour each, 5 times per week for 4 weeks. To compare the two groups, the level of ankle disability was assessed by using the ankle injury score scale in pre- and post-treatment. Results : On assessment of post-treatment, there were statistically significant differences in the scores of all sub-items, except for ankle laxity and range of motion, and the total score of ankle injury score scale between the two groups(p<0.05). In comparison between pre- and post-treatments, the significant difference in the scores of all sub-items and total score didn't appear for the control group, while the scores of most sub-items and total score of the experimental group were shown the statistically significant difference(p<0.05). Conclusion : The findings suggest that the proprioceptive exercise program is more effective for relieving ankle disability than conservative treatment therefore, the program to improve proprioceptive function should be recommended for prevention and rehabilitation of recurrent ankle inversion injuries.

Changes of One-Leg Standing Balance of Ipsilateral and Contralateral Lower-Limb Following Unilateral Isokinetic Exercise of Ankle Joint in Young Adults

  • Purpose: The purpose of this study was to investigate the effects of a four week unilateral isokinetic exercise program applied to ankle on the one-leg stance balance performance of ipsilateral and contralateral lower-limbs. Methods: Subjects were randomly assigned to either a right ankle training program (n=12) or a control group (n=12). The training group received unilateral ankle isokinetic exercise of the dominant side for 4 weeks, whereas control group did not. Ipsilateral and contralateral one-leg balance were measured before and after intervention using the Biodex Balance System. Results: Improvements of stability scores, such as APSI, MLSI, and OSI, from pre-test to post-test were significantly different greater for the training group when the control. Conclusion: The results of this study suggest unilateral ankle strengthening exercise transfers benefit to the untrained limb by a cross-education effect, and that this type of exercise should be considered to improve one-leg standing balance of trained and untrained lower-limbs.

중간볼기근 강화운동을 포함한 가정운동 재활 프로그램이 발목기능 및 삶의 질 향상에 미치는 영향: 무작위 대조 실험 (The Effects of Home-Exercise Rehabilitation Program with Gluteus-Medius Strengthening Exercise on the Ankle Function and Quality of Life: Randomized Controlled Trial)

  • Background: The purpose of this study is to investigate the effect of ankle strengthening and proprioceptor exercise, including gluteus-medius strengthening exercise, on ankle strength, dynamic and static balance, ankle function, and quality of life in 24 adults without ankle instability. Methods: After randomly classifying 12 people per group into the experimental group and the control group, Home-exercise rehabilitation program was conducted for a total of 2 weeks. The control group performed ankle strengthening and proprioceptor exercise 5 times a week for 15 minutes, and the experimental group was performed by adding 15 minutes of gluteus-medius strengthning exercise to the exercise performed by the control group. The SPSS 27.0 program was used for statistical analysis. Results: Both groups showed statistically significant differences in the plantar flexion muscle strength, Y-Balance test, side hop test, quality of life. But, only experimental group showed statistically significant differences dorsi-flexion muscle strength and hip abduction muscle strength. There was no statistical difference in comparison of difference values between groups. Conclusion: These exercises can provide useful home rehabilitation programs for sports populations seeking to prevent injury in sports.

발목재활로봇을 위한 6축 힘/모멘트센서 설계 (Design of Six-Axis Force/Moment Sensor for Ankle-Rehabilitation Robot)

  • Most serious patients who have the paralysis of their ankles can't use of their feet freely. But their ankles can be recovered by an ankle bending rehabilitation exercise and a ankle rotating rehabilitation exercise. Recently, the professional rehabilitation therapeutists are much less than stroke patients in number. Therefore, the ankle-rehabilitation robot should be developed. The developed robot can be dangerous because it can't measure the applied bending force and twisting moment of the patients' ankles. In this paper, the six-axis force/moment sensor for the ankle-rehabilitation robot was specially designed the weight of foot and the applied force to foot in rehabilitation exercise. As a test results, the interference error of the six-axis force/moment sensor was less than 2.51%. It is thought that the sensor can be used to measure the bending force and twisting moment of the patients' ankles in rehabilitation exercise.

Comparison of the Immediate Effect of Ankle and Hip Joint Thera-band Exercise on the Balance Ability

  • PURPOSE: This study compared the effect of training ankle joint and hip joint thera-band exercise on balance. METHODS: The participants were divided into two groups of 11 each. Group A performed hip exercise after ankle exercise, and Group B performed ankle exercise after hip exercise. Using a green thera-band, the dorsiflexion and plantarflexion and hip flexion and hip extension were exercised repeatedly for 15 seconds three times with a five-second rest between each set. After the exercise and measurement of one area were complete, the exercise and measurement of the other area were performed at one-day intervals. The balance ability was assessed using a Tetrax and Y-balance test and repeated three times; the best values were taken. RESULTS: In the stability index (ST) of the static balance, the hip joint exercise group (HTG) during the follow-up of normal eye open (NO) revealed notable improvement over the ankle joint exercise group (ATG), and in the follow-up of the normal eye closed (NC), the ATG showed significant improvement over the HTG. In the pillow with eye closed (PC) follow-up, the ATG showed significant improvements over the HTG. At the left (Lt) and Y-balance test (YBT), the ATG showed significant improvements in the follow-up over the HTG (p <.05). CONCLUSION: In static balance, the ATG showed significant improvement in the follow-up of NC and PC over the HTG. In the dynamic balance, the Lt. dynamic balance on the non-dominant side in the ATG showed significant improvement in the follow-up over the HTG.

Effects of Floss Bands on Ankle Joint Range of Motion and Balance Ability

  • Background: The range of motion (ROM) and balance ability of the ankle joint affect the stability of the ankle and prevent injuries or hurts from falling. In the clinical tests conducted recently, the floss band is widely used to enhance the range of joint motion and exercise performance, and there are many studies that have applied it to ankle joint increasing dorsi flexion (DF) angle. Objects: This study compared the effects on the range of ankle motion and static/dynamic balance ability of the ankle through three conditions (before floss band intervention, after floss band intervention, and after active exercise intervention) for adults. Methods: One intervention between floss band and active exercise was applied randomly and another intervention was applied the next day. After each intervention, the ROM of the ankle joints and the static balance was checked by measuring conducting one leg test. And the dynamic balance was checked by conducting a Y-balance test. Results: In the case of DF, the range of joint motion showed a significant increase after floss band intervention compared to before floss band intervention (p < 0.05). Static balance ability showed a significant increase after the intervention of floss band and active exercise compared to before the intervention of floss band (p < 0.05). The dynamic balance ability showed a significant increase after the intervention of the floss band compared to before intervention of the floss band and after active exercise intervention (p < 0.05). Conclusion: Based on these results, it was confirmed that the application of floss band to the ankle joint increases DF and improves the static and dynamic balance ability. Based on this fact, we propose the application of a floss band as an intervention method to improve the ROM of the ankle joint and improve the stability of the ankle in clinical field.

Effects of Ankle and Hip Strategy Training on Improving the Center of Pressure Movements and Limits of Stability in Stroke Patients

  • Background: Stroke patients have leg muscle weakness and impaired balance resulting in compensatory changes. To restore balance in these patients, functional training using postural strategy is needed. Objective: To examine the effects of ankle and hip strategy training on the center of pressure (COP) movement and limits of stability (LOS) in standing posture in stroke patients. Design: The study was an assessor-blinded and randomized-controlled clinical trial. Methods: Thirty patients were randomly assigned to an ankle strategy training group and a ankle/ hip strategy training group. Patients in the ankle strategy training group underwent ankle strategy exercise for 30 min, and those in the ankle/ hip strategy training group underwent 15 min of ankle strategy exercise and 15 min of hip strategy exercise. Both groups underwent training thrice a week for four weeks. Forward, backward, paretic side, and non-paretic side COP movements and LOS were measured using BioRescue. Results: After the intervention, except for the backward area in the ankle strategy training group, the COP movement area and the LOS were significantly improved in both the groups. In addition, these improvements were significantly higher in ankle/ hip strategy training group than that in the ankle strategy training group. Conclusions: Ankle strategy training in addition to hip strategy training improves COP movement (forward-backward, paretic side area, and non-paretic side area) and LOS in stroke patients.

족관절 염좌에 SSP 전자침 요법을 병행한 동기침법(動氣鍼法)의 효과 (The Effects of Alphabet Exercise with SSP Electrotherapy on Lateral Ankle Sprain)

  • The multiple acupuncture techniques have been used to relieve the pain of lateral ankle sprain. Recently, early functional rehabilitation is emphasized in rehabilitative concepts using active ROM, facilitated proprioception, alphabet exercise and so forth. Similarly, in oriental rehabilition medicine, traditional movement technique called "Dong-Ki" has been used to relieve pain of ankle sprain. So we used "Dong-Ki" technique with SSP(silver spike point) electrotherapy transformed from alphabet exercise and evaluated the effects in method of randomized controlled trials. Methods : 27 outpatients with lateral ankle sprains were randomized into two groups : 15 samples and 12 controls. Sample group was treated with "Dong-Ki" technique(writing his name with foot) with SSP electrotherapy to relieve pain during additionally. "Dong-KI". Outcomes were measured by 10cm VAS and ankle circumference(cm). Results : In terms of VAS, Sample group had inclination of pain relieving but not significantly meaningful. In terms of ankle circumference measurement, Sample group showed significantly improved.

Joint Position Effects on Biceps Femoris and Peroneal Muscle Activation and Ankle Evertor Strength

  • Background: The peroneus longus (PL) and peroneus brevis (PB) function as the primary muscles of eversion, a movement closely associated with tibial external rotation for ankle mortise stability. Ankle motion and tibial rotation vary based on different ankle and knee positions. Objects: This study aimed to investigate the PL, PB, and biceps femoris (BF) muscle activation and eversion strength during side-lying isometric eversion exercise based on different ankle positions (neutral [N] and plantarflexion [PF]) and knee positions (90° flexion [KF] and extension [KE]). Methods: Thirty healthy adults with an Ankle Joint Functional Assessment Tool score of ≥ 22 were recruited (mean age = 24.8 ± 3.1 years). Maximal isometric eversion strength and submaximal muscle activation of the PL, PB and BF were measured during isometric eversion exercise in side-lying. A 2 × 2 repeated measures analysis of variance was performed to investigate differences in muscle activation and strength. Results: The PL and PB muscle activation showed significant main effects with the knee and ankle positions (p < 0.05); activation was greater in the KE and PF positions than in the KF and N positions. The BF muscle activation showed a significant interaction effect with knee and ankle positions, which was greater in knee extension and ankle plantarflexed (KEPF) position than in knee flexion and ankle plantarflexed (KFPF) position (p < 0.05). Eversion strength showed a significant main effect only in ankle position (p < 0.05) and was greater in the N position than in the PF position. Conclusion: The results of this study indicate that the KEPF position can be recommended to facilitate contraction of the PL and PB during side-lying eversion exercise. Furthermore, the effects of the knee-ankle positions should be considered for measuring ankle eversion strength and implementing the isometric submaximal side-lying eversion exercise.

상상연습을 병행한 발목 운동이 자세정렬에 미치는 영향 (Effects of Ankle Exercise combined with Mental Practice on Postural Alignment)

  • Purpose : Various studies effects of mental practice. However, there is a lack of research on the effects of practice on postural alignment. Therefore this study to the effect of ankle exercise combined with mental practice on postural alignment of legs. Method : Subjects were randomly assigned to mental practice group (experimental group n=15) and general exercise group (control group n=15). Postural alignment was the hip, knee, and ankle joints. When viewed from the side, an arbitrary point in front of the malleolus makes a straight line with the plumb line. Exercise was performed a week for weeks. Exercise programs included muscle strengthening, relaxation, and proprioception exercise. The experimental group mental practice. Result : Both groups showed significant differences in postural alignment ankle joint, knee joint, and hip joint. In particular, the experimental group showed a larger change than the control group. However, significant difference in postural alignment change only the knee joint (p<0.05), and there was no significant difference the hip joint (experimental group=$0.77{\pm}0.81$, control group=$0.87{\pm}1.13$) and ankle joint (experimental group=$0.52{\pm}0.63$, control group=$0.48{\pm}0.41$). Conclusion : This study suggests that mental practice is effective as an exercise method postural alignment. Mental practice also expected to be musculoskeletal disorders. Therefore, additional studies should be conducted to verify the effect of mental practice on the alignment of various parts.
