역방향 에르고미터 훈련이 편마비 환자의 족관절 가동범위와 하지 근력 및 족저압에 미치는 영향 (The Effect of Backward Pedaling Ergometer Training on Ankle ROM, Lower Extremities Strength, Foot Pressure in Hemiplegia)

  • PURPOSE: The purpose of this study was to identify the effects of backward pedaling ergometer training on ankle ROM, lower extremities strength, foot pressure in hemiplegia. METHODS: The subjects consisted of control group(n=10) and experimental group(n=10), subjects were trained 5 times a week for 2 weeks each group. Control group were trained forward, experimental group were trained backward pedaling with physical therapy in both groups. Each group measured ankle joint ROM with DUALER IQ and ankle and knee joint flexor and extensor muscle strength by Manual Muscle Test System and foot pressure by Gait view system. RESULTS: The result of this study between pre and post test that experimental group had statistically significantly differences in ankle joint range of motion and lower extremities strength. But foot pressure had not statistically significant differences. There was not significantly difference of variation between groups. CONCLUSION: Therefore these results mean that backward and forward pedaling ergometer training effected an improvement of lower extremities function in hemiplegia.

급성 발목 염좌에 대한 족관절의 관절가동술이 보행과 고관절 근력에 미치는 영향 (The Effect of Ankle stability exercise and Mobilization on Hip Muscle Strength and Gait in Patients with Acute Ankle Sprain)

  • Background: In most human lives, 80 percent have problems with the ankle and can be solved with a treatment that is objective in proper assessment. Discrepacts in the ankle are also associated with walking patterns and affect hip and knee joints. An evaluation of hip flexion and extensor muscles was performed to check the strength of hip joints after ankle sprain patients application of arthesis. Methods: In the hospital in Bucheon, 20 outpatients who visited the hospital for treatment with ankle sprain were tested with 10 male and 10 female patients. The criteria for selection of the study subjects were randomly divided into those with joint movement techniques applied to the ankle joints and those with conservative physical therapy. Results: In applying arthrography and preservation physical therapy to patients with ankle sprain, a difference in muscle strength between hip flexion and extensor was noted in post-evaluation comparisons. There were significant differences in the assessment of walking speed and walking time between the two counties in the gait analysis assessment (p<.05). Conclusion: Studies have shown that applying arthrography to people with ankle sprain has a greater therapeutic effect than using conservative physical therapy.

신발 밑창 형태가 무릎 및 발목 근육의 근활성화에 미치는 영향 (Effect of Shoes Sole Form on Knee and Ankle Muscle Activity)

  • PURPOSE: This study was to examine changes in muscle activity of lower extremity shoes sole form (high heels, shoes with curved out sole, house shoes). METHODS: The subjects of this study were 12 women in their twenties. They put three kinds of shoes (high heels, shoes with curved out sole, house shoes) and walked 5m. The muscles activities of lower extremity muscles (rectus femoris, vastus medialis, tibialis anterior, gastrocnemius medial part) were measured using a wireless electromyogram (EMG). Rectus femoris was attached to 1/2 position at the distance between ASIS and knee bone and vastus medials was attached to 2cm from upper inside of knee bone. Tibialis anterior was attached to 75% position above line connecting knee joint and ankle joint and gastrocnemius medial part was attached to 3 5% position above knee joint and ankle. RESULTS: It was found that there were significant differences in changes of muscles activities of lower extremity muscles (rectus femoris, vastus medialis, tibialis anterior, gastrocnemius medial part) on shoes sole forms (p<.05). All lower extremity muscles were showed high muscles activities, when high heels wear (p<.05). Wearing shoes with curved out sole was showed high muscle activity of tibialis anterior and lower muscle activity of gastrocnemius medial part compared with wearing house shoes (p<.05). CONCLUSION: Shoes sole form should be considered when patients with knee and ankle joint problems choose shoes because muscles were showed different activities according to shoes sole forms.

정상발과 평발에서의 목말밑 관절가동범위와 등쪽굽힘근의 근활성도 비교 (Comparison of Subtalar Joint Range of Motion and Dorsiflexor Muscle Activity Between Normal and Pes Planus Feet)

  • PURPOSE: The imbalance of pretibial muscles can be a factor contributing to the development of pes planus. However, no study has yet compared the muscle activity of the tibials anterior (TA) to that of the extensor digitorum longus (EDL). The purpose of this study was to determine whether there are differences in the electromyographic (EMG) TA and EDL amplitude indexes (AIs) between normal and pes planus feet. METHODS: A total of 14 subjects with normal feet and 15 subjects with bilateral pes planus participated in this study. TA and EDL muscle activities were measured using a wireless EMG system and the angles of ankle dorsiflexion and eversion of the subtalar joint were measured using a universal goniometer during active ankle dorsiflexion in the prone position. AI was calculated as follows: $\text{amplitude_{TA}-amplitude_{EDL}/(amplitude_{TA}+amplitude_{EDL})}/2{\times}100$. RESULTS: The AIs of the TA and EDL were significantly lower in pes planus feet than in normal feet (p<.05). The angle of subtalar eversion was significantly greater in pes planus feet than in normal feet during active ankle dorsiflexon (p<.05). However, there was no significant difference in the angle of ankle dorsiflexion between normal feet and pes planus feet (p>.05). CONCLUSION: This study showed that TA muscle activation was lower in pes planus feet than in normal feet, resulting from greater eversion range of motion during active ankle dorsiflexion. We suggest that the imbalance of ankle dorsiflexors must be considered in pes planus management.

자동차 페달 반복 사용 시 보조 발판이 하지근육 활동과 피로에 미치는 영향 (The Effect of the Heel Rest on the Lower Leg Muscle Activity and Fatigue During Repetitive Pedaling)

  • This study examined the effect of ergonomic heel rest that was designed for drivers who have physical handicap in the low leg muscles or have to drive prolonged hours with frequent foot pedaling. An experiment was designed to test the ergonomic heel rest with traditional foot pedal. Forty subjects participated in the experiment. Electromyography(EMG) was used to monitor the muscle activity and fatigue of right leg, and Electro-goniometer was used to measure the ranges of motions of the knee and ankle. A simulator of driver's seat was built for the experiment and the heel rest was installed on it. In order to examine the low muscle activity and range of motion, subjects used the foot pedal for 15 minutes repetitively for each experimental condition. Another 15 minutes test without the heel rest was also performed for comparison. The Root Mean Square(RMS) and Mean Power Frequency(MPF) Shift were used to quantify the level of muscle activity and local muscle fatigue. In results, statistically significant decreases of muscle activity and fatigue were found in all the low leg muscles. The range of motion of the knee and ankle joint also decreased when the heel rest was used. The mechanism of the heel rest effect was discussed in this study. This type of heel rest can be applied to real driving situation after ensuring the safety, or overcoming the psychological discomfort possibly due to unfamiliarity.

기구운동과 복합운동이 발목염좌 경험이 있는 태권도학과 학생의 발목 안정성에 미치는 효과 (The Effects of Board Training and Complex Training on Ankle Stability in Taekwondo Students with a History of Ankle Sprain)

  • Purpose: The purpose of this study is to compare the effects of board training and complex training on ankle stability in taekwondo college students with a history of ankle sprain. Methods: Twenty-seven taekwondo college students were randomly assigned into a board training (BTG, n=9), complex training (CTG, n=9), or control groups (CG, n=9). BTG carried out disk and trampoline training 3 times a week for 8 weeks. CTG carried out resistive and plyometric training 3 times a week for 8 weeks. All subjects completed ankle stability test for static and dynamic balance in anterior/posterior and medial/lateral stability with New Balance System (BIODEX Medical System, USA). Results: There were no significant differences between three groups in ankle stability. Those who participated in BTG significantly improved static balance of left foot in anterior/posterior stability, and dynamic balance of both feet in mediolateral stability, but there were no change in CTG. Conclusion: Board training is more improved ankle stability in taekwondo college students with a history of ankle sprain than complex training.

태극권이 노인의 균형과 센서 USN을 이용한 발목관절 근활성도에 미치는 영향 (Effects of Tai Chi on Balance and Muscle Activity of Ankle Joints with USN sensor in Elderly People)

  • 본 연구는 태극권이 노인의 균형과 센서 USN을 이용하여 발목관절 근활성도에 미치는 영향을 알아보기 위해 다음과 같이 실시하였다. 대상자는 65세 이상 노인 36명을 태극권을 시행한 실험군과 태극권을 하지 않은 대조군을 각각 18명씩 무작위 배정하였다. 실험군은 태극권을 주 3회 6주 동안 18회 실시하였다. 모든 대상자는 실험 전과 실험 후에 정적 및 동적균형검사와 센서 USN을 이용하여 발목관절 근활성도 변화를 측정하였다. 정적균형은 기능적 뻗기 검사, 동적균형은 외발서기검사 그리고 발목관절 근활성도 측정은 눈을 감고 발을 모아 서있는 자세에서 안뜰계에 평류안뜰 자극을 적용한 상태에서 측정하였다. 연구결과, 정적 및 동적균형 모두 두 집단 간 통계적인 차이를 나타냈고, 발목관절 근활성도 분석도 통계적으로 유의한 차이가 나타났다. 따라서 태극권이 노인에게 균형과 발목관절 근활성화에 효과적임을 알 수 있었다.

Comparison of Tibialis Anterior Muscle Thickness with 4 Different Toe and Ankle Postures: Ultrasonographic Study

  • Purpose: Ankle dorsiflexion is an essential element of normal functions, including walking, activities of daily living and sport activities. The tibialis anterior (TA) muscle functioned as a dorsiflexor and as a dynamic stabilizer of the ankle joint during walking and jumping. This study aimed to compare TA muscle thickness using ultrasonography according to the four different toe and ankle postures for the selective TA strengthening exercise. Methods: This study were recruited 26 (males: 15, females: 11) aged 20-30 years, with no injury ankle and calf in the medical history, had normal dorsiflexion and inversion range of motion (ROM). The thickness of the TA muscle was measured by ultrasonography in the four different toe and ankle postures: 1. Ankle dorsiflexion with all toe extension and ankle inversion (ITEDF); 2. Ankle dorsiflexion with all toe flexion and ankle inversion (ITFDF); 3. Ankle dorsiflexion with all toe extension and neutral position (NTEDF); 4. Ankle dorsiflexion with all toe flexion and neutral position (NTFDF). One-way repeated analysis of variance (ANOVA) and Bonferroni correction were used to confirm the significant difference among conditions. The level of statistical significance was set at α=0.01. Results: TA muscle thickness with ITFDF was significantly greater than in any other ankle positions, including ITEDF, NTFDF, and NTEDF (p<0.01). Conclusion: Among the four toe and ankle postures, isometric contraction in ITFDF postures showed the greatest increase in thickness of TA rather than ITEDF, NTEDF, and NTFDF postures. Based on these results, ITFDF can be recommended in an efficient way to selectively strengthen TA muscle.

Comparison of the Difference of Ankle Range of Motion and Hip Strength in Both Legs of Fencing Athletes with Ankle Instability

  • Background: Ankle sprain in the Lead Leg Side (ALLS) is common in fencing athletes, and studies comparing the ankle range of motion (ROM) and strength of both legs are insufficient. Objectvies: To compare the ankle ROM and hip strength between two legs in fencing athletes who has ankle instability in the lead leg side. Design: Cross-sectional design. Methods: Seven fencing athletes with ankle instability participated in this study, and they randomly assigned into ankle in the Lead Leg Side (ALLS) and ankle in the Rear Leg Side (ARLS). Instability was determined by the Cumberland Ankle Instability Tool (CAIT), and then joint ROM and hip muscle strength were measured. Results: There were significant differences in dorsiflexion ROM, hip strength (extension and abduction) between the ALLS with ankle instability and ARLS (P<.05). Conclusion: This study suggests that the ankle ROM and hip muscle strength of ARLS are greater than ALLS in fencing athletes with ankle instability.

근에너지 기법과 스트레칭 기법의 발목관절 적용이 발등굽힘 제한이 있는 만성 뇌졸중 환자의 발목 관절가동범위, 균형 능력, 보행 능력에 미치는 영향 비교 (Comparison of the Effects of Applying Muscle Energy Techniques Versus Stretching Techniques to the Ankle Joint on Ankle Joint Range of Motion, Balance Ability and Gait Ability of Chronic Stroke Patients with Limited Ankle Dorsiflexion)

  • PURPOSE: This study compared the effect of the muscle energy technique (MET) and stretching technique on ankle dorsiflexion passive range of motion, balance, and gait ability of stroke patients with limited ankle dorsiflexion. METHODS: Forty-four post-stroke patients participated. The participants were randomized into the MET group (METG; n = 22) and the stretching group (STG; n = 22). The METG was subjected to the MET to relax the dorsiflexion, while the STG was subjected to the dorsiflexion stretching technique. Both groups completed standard neurological physical therapy for 30 min per session. The intervention was conducted five times a week over 3 weeks for a total of 15 times. All participants underwent ankle dorsiflexion passive range of motion measurement and Berg Balance Scale score determination and completed a 10-m walking test and the timed up and go test before and after the intervention. RESULTS: After the 3-week intervention, both groups showed significant improvement after the intervention (p < .05). METG participants showed greater improvements in ankle dorsiflexion passive range of motion and 10-m walking test results compared to STG participants (p < .05). CONCLUSION: Both interventions improved ankle dorsiflexion passive range of motion, balance, and gait ability in stroke patients with limited ankle dorsiflexion. Moreover, the MET was superior to ankle dorsiflexion passive range of motion on the 10-m walking test.
