과로와 전치부 교합과 두개안면골 형태의 상관관계에 관한 연구 (A STUDY ON THE INTERRELATIONSHIP OF THE CONDYLAR PATH, ANTERIOR OCCLUSION AND CRANIOFACIAL MORPHOLOGY)

  • The stable occlusion in function is thought as important as the esthetics in form, in order to preserve the healthy oral condition. The stable occlusion requires the harmony between the condylar guidance factors and the anterior guidance factors. The aim of this study was to evaluate the quantitative relationship between the condylar guidance factors and the anterior guidance factors, estimating statistically the measurement of the condylar paths by Pantronic and those of the anterior guidance factors, craniofacial morphology by roentgenocephalometry in 46 relatively good functional occlusion. The results of this study were as follows. 1. The measurements of the protrusive condylar path inclinations were $36.41^{\circ}$ in the right, $35.63^{\circ}$ in the left, $36.28^{\circ}$ in the mean. The measurements of Fisher's angles were $8.17^{\circ}$ in the right, $6.43^{\circ}$ in the left, $6.87^{\circ}$ in the mean. 2. The anterior facial height and the lower anterior facial height made a negative correlation with the protrusive condylar path inclination. 3. The articular eminence angle relative to the artificial articulator plane showed a positive correlation with the maximum protrusive condylar path. 4. SNA and SNB made a negative correlation with the articular eminence angle, and AAP-GoMe, AAP-DcGn, the facial height ratio had a positive correlation with the articular eminence angle. 5. The angulation of maxillary incisor lingual slope, overbite and the ratio of overbite to overiet showed a positive correlation with the articular eminence angle. 6. The angulation of maxillary incisor lingual slope , overbite, and the ratio of overbite to overjet made a positive correlation with the inclination of occlusal plane, functional occlusal plane. 7. Overbite and the ratio of overbite to overjet had a positive correlation with the angulation of maxillary incisor lingual slope. 8. The anterior guidance factors were more influenced by the mean protrusive condylar path inclination and the maximum Fisher's angle, and the regression equations of those were made.

두부축방향방사선규격사진에 있어서 하악과두의 두부방사선계측학적 연구 (A ROENTGENOGRAPHIC CEPHALOMETRIC STUDY OF CONDYLAR HEAD ON THE SUBMENTOVERTEX CEPHALOGRAM)

  • The purpose of this study was to observe the anatomic variation of condylar head and the positional relationship between condylar head and other anatomic structures of head by means of cephalometry using the submentovertex cephalogram from each person in centric occlusion for producing a good quality of temporomandibular joint radiograph. The 100 submentovertex cephalograms of 100 Korean adults consisted of 50 females ranged from 20 to 24 years age and 50 males ranged from 22 to 30 years age, were studied and analyzed statistically. The results were as follows; 1. The mean of the horizontal angulation of condylar head to the transmeatal line (EE-LA) was 18.5° (S.D. 7.9°), and all measurements that indicate the horizontal angulation of condylar head showed considerable differences among individuals. 2. In the comparison of male vs. female in the measurements, statistically significant differences were found in the majority of measurement items, and with exception of R-LA and LA-LA, the rest of these measurements were larger in male than in female. 3. In the comparison of left vs. right in the measurements, statistically significant differences were found in the majority of measurement items, and with exception of PC-CC, the rest of these measurements were larger in right than in left. As above, because the majority of measurements that involve the horizontal angulation of condylar head varied among individuals, between male and female, and between left and right, the condition of various temporomandibular joint radiography should be determined by means of cephalometry using the submentovertex cephalogram for producing the accurate radiographic image of temporomandibular joint.

Analysis of the root position of the maxillary incisors in the alveolar bone using cone-beam computed tomography

  • Purpose: The purpose of this study was to measure the buccal bone thickness and angulation of the maxillary incisors and to analyze the correlation between these parameters and the root position in the alveolar bone using cone-beam computed tomography (CBCT). Materials and Methods: CBCT images of 398 maxillary central and lateral incisors from 199 patients were retrospectively reviewed. The root position in the alveolar bone was classified as buccal, middle, or palatal, and the buccal type was further classified into subtypes I, II, and III. In addition, the buccolingual inclination of the tooth and buccal bone thickness were evaluated. Results: A majority of the maxillary incisors were positioned more buccally within the alveolar bone, and only 2 lateral incisors(0.5%) were positioned more palatally. The angulation of buccal subtype III was the greatest and that of the middle type was the lowest. Most of the maxillary incisors exhibited a thin facial bone wall, and the lateral incisors had a significantly thinner buccal bone than the central incisors. The buccal bone of buccal subtypes II and III was significantly thinner than that of buccal subtype I. Conclusion: A majority of the maxillary incisor roots were positioned close to the buccal cortical plate and had a thin buccal bone wall. Significant relationships were observed between the root position in the alveolar bone, the angulation of the tooth in the alveolar bone, and buccal bone thickness. CBCT analyses of the buccal bone and sagittal root position are recommended for the selection of the appropriate treatment approach.

Effect of abutment angulation in the retention and durability of three overdenture attachment systems: An in vitro study

  • This in vitro study investigated and compared the durability and retention of three types of attachments. MATERIALS AND METHODS. Three commercially available attachments were investigated: $Clix^{(R)}$, Dalbo-$Plus^{(R)}$ and $Locator^{(R)}$. In total, 72 samples of these attachments were placed in the acrylic resin forms and subjected to mechanical testing (5400 cycles of insertion and removal) over the respective ball or Locator abutments immersed in artificial saliva at pH 7 and $37^{\circ}C$. The abutments were placed at angulations of $0^{\circ}$, $10^{\circ}$ and $20^{\circ}$. The retention force was recorded at the beginning and after 540, 1080, 2160, 3240, 4320 and 5400 insertion-removal cycles. RESULTS. The results revealed that there were significant differences in the average values of the insertion/removal force due to angulation ($F_{(2.48)}=343619$, P<.05) and the type of attachment ($F_{(7.48)}=23.220$, P<.05). CONCLUSION. Greater angulation of the abutments was found to influence the retention capacity of the attachments, and the fatigue test simulating 5 years of denture insertion and removal did not produce wear in the metal abutments.

Zerinke 모멘트와 신경망을 이용한 온라인 필기체 숫자 인식 (Recognition of Online Handwritten Digit using Zernike Moment and Neural Network)

  • 본 논문에서는 Zernike 모멘트와 backpropagation신경망을 이용한 온라인 필기체 숫자 인식 방법을 소개한다. 마우스로 통해 입력된 숫자 정보는 전처리를 통해 시간에 순서적이고, 연속적인 좌표 정보로 변환된다. 전처리된 입력 좌표는 Zernike 모멘트(moment)와 각도 특징(angulation feature)을 이용하여 각 숫자가 가지는 고유의 특징을 만들어 낸다. 이러한 특징은 크기, 모양, 틀어진 정도에 상관없이 항상 일정한 성질을 가진다. 제안된 방법으로 추출된 특징은 패턴 구분을 위해 back propagation 신경망의 입력으로 사용된다. 본 논문은 200개의 필기체 숫자 데이터베이스를 이용하여 실험을 한 결과, 제시된 방법은 적은 학습데이터만으로 학습이 가능할 뿐만 아니라 좋은 인식률을 보여준다.

Evaluation of mandibular buccal shelf characteristics in the Colombian population: A cone-beam computed tomography study

  • Objective: To evaluate the mandibular buccal shelf (MBS) in terms of the angulation and bone depth and thickness according to sex, age, and sagittal and vertical skeletal patterns in a Colombian population using cone-beam computed tomography (CBCT). Accordingly, the optimal site for miniscrew insertion in this area was determined. Methods: This descriptive, retrospective study included 64 hemi-arches of 34 patients. On CBCT images, the angulation, buccal bone depth (4 and 6 mm from the cementoenamel junction [CEJ] of MBS), and buccal bone thickness (6 and 11 mm from the CEJ of MBS) were measured at the mesial and distal roots of the mandibular first and second molars. Results: There were no statistically significant differences in the angulation, depth, and thickness of MBS between male and female patients. The values for the bone around the distal root of the mandibular second molar were significantly greater than the other values. The osseous characteristics were significantly better in participants aged 16-24 years. Class III patients exhibited the best osseous characteristics, with the bone depth at 6 mm being significantly different from that in Class I and Class II patients. Although values tended to be greater in patients with low angles, the difference was not statistically significant. Conclusions: MBS provides an optimal bone surface for miniscrew insertion, with better osseous characteristics at the distal root of the mandibular second molar, 4 mm from CEJ. Adolescent patients, Class III patients, and patients with a low angle exhibit the most favorable osseous characteristics in the MBS area.

Influence of Plate Design on the Accuracy of Tibial Alignment after Center of Rotation of Angulation-Based Leveling Osteotomy in Toy-Breed Dogs

  • The center of rotation of angulation-based leveling osteotomy (CBLO) has been introduced for the stabilization of cranial cruciate ligament rupture (CCLR) in small-breed dogs. This technique can be applied to the tibia without damaging its tuberosity. Although different designs of CBLO plates are available, tibial plateau leveling osteotomy (TPLO) plates have been still used for stabilization during CBLO. To the best of our knowledge, no studies have reported the effects of TPLO plates on the postoperative limb alignment after CBLO. Therefore, the present study (January 2020 to February 2021) aimed to compare the postoperative outcomes (postoperative tibial plateau angle [TPA] and tibial alignment) in patients receiving CBLO and TPLO plates during CBLO. Paired stifle joints (n = 16) were obtained from eight toy-breed cadaver dogs (mean weight, 4.4 kg) that underwent CBLO. The joints were randomly assigned to receive the CBLO (CBLO group) or TPLO plates (TPLO group). Pre-and postoperative radiographs were obtained, and the dissected tibiae were evaluated. The following postoperative parameters were compared to evaluate the surgical outcome: TPA, osteotomy location, mechanical medial proximal tibia angle, inter-segment gap, and tibial plateau translation. No significant differences were found in the postoperative alignment between the two groups. Therefore, TPLO plates may be considered as a viable alternative in toy-breed dogs undergoing CBLO.

Clinical predictors of potentially impacted canines in low-risk patients: A retrospective study in mixed dentition

  • Objective: To evaluate the null hypothesis that there is no difference in a set of clinical predictors of potentially impacted canines between low-risk patients with and without displaced canines. Methods: The normal canine position group consisted of 30 patients with 60 normally erupting canines ranked in sector I (age, 9.30 ± 0.94 years). The displaced canine group comprised 30 patients with 41 potentially impacted canines ranked in sectors II to IV (age, 9.46 ± 0.78 years). Maxillary lateral incisor crown angulation, inclination, rotation, width, height, and shape, as well as palatal depth, arch length, width, and perimeter composed a set of clinical predictors, which were evaluated on digital dental casts. Statistical analyses consisted of group comparisons and variable correlations (p < 0.05). Results: There was a significant association between sex and mesially displaced canines. Unilateral canine displacement was more prevalent than bilateral displacement. The crown of the maxillary lateral incisors was significantly angulated more mesially and rotated mesiolabially in low-risk patients with displaced canines, who also had a shallower palate and shorter anterior dental arch length. Lateral incisor crown angulation and rotation, as well as palatal depth and arch length, were significantly correlated with the canine displacement severity. Conclusions: The null hypothesis was rejected. Maxillary lateral incisor angulation inconsistent with the "ugly duckling" stage as well as a shallow palate and short arch length are clinical predictors that can significantly contribute to the early screening of ectopic canines in low-risk patients.

Errors in light-emitting diodes positioning when curing bulk fill and incremental composites: impact on properties after aging

  • Objectives: This study aimed to evaluate the effect of improper positioning single-peak and multi-peak lights on color change, microhardness of bottom and top, and surface topography of bulk fill and incremental composites after artificial aging for 1 year. Materials and Methods: Bulk fill and incremental composites were cured using multi-peak and single-peak light-emitting diode (LED) following 4 clinical conditions: (1) optimal condition (no angulation or tip displacement), (2) tip-displacement (2 mm), (3) slight tip angulation (α = 20°) and (4) moderate tip angulation (α = 35°). After 1-year of water aging, the specimens were analyzed for color changes (ΔE), Vickers hardness, surface topography (Ra, Rt, and Rv), and scanning electron microscopy. Results: For samples cured by single-peak LED, the improper positioning significantly increases the color change compared to the optimal position regardless of the type of composite (p < 0.001). For multi-peak LED, the type of resin composite and the curing condition displayed a significant effect on ΔE (p < 0.001). For both LEDs, the Vickers hardness and bottom/top ratio of Vickers hardness were affected by the type of composite and the curing condition (p < 0.01). Conclusions: The bulk fill composite presented greater resistance to wear, higher color stability, and better microhardness than the incremental composite when subjected to improper curing. The multi-peak LED improves curing under improper conditions compared to single-peak LED. Prevention of errors when curing composites requires the attention of all personnel involved in the patient's care once the clinical relevance of the appropriate polymerization reflects on reliable long-term outcomes.

함치성 낭의 조대술 후 이환치아의 맹출 양상 (ERUPTION PATTERN OF A CYST-ASSOCIATED MANDIBULAR PREMOLAR AFTER MARSUPIALIZATON OF A DENTIGEROUS CYST)

  • 사춘기 전 아동에서 흔히 발생되는 함치성 낭을 조대술로 치료한 후 낭에 이환된 치아의 맹출에 영향 미치는 제반요인을 분석하여 이환된 치아의 외과적 또는 교정적 치료 유무의 기준을 마련하고자 본 연구를 시행하였다. 하악 소구치 부위가 함치성 낭으로 이환되어 있으며 조대술 실시 후 8개월 이내에 촬영된 파노라마 관찰사진이 있는 5-12세 아동 총 22명의 증례를 대상으로 하였다. 초진과 재진 시의 관찰기간 동안에 촬영 한 파노라마 사진으로 트레이싱 용지에 필요한 구조물과 치아를 모두 그렸으며 사진 상의 수직, 수평적인 상의 확대 및 축소의 보상을 위하여 초진사진의 제1대구치 치관크기를 기준으로 모든 사진을 표준화시켰다. 각 사진 상에서 낭에 이환된 부위를 실험군, 동일 악궁의 반대측 부위를 대조군으로 설정하여 교두깊이, 각의 크기, 낭의 크기, 교두점 위치지수를 계측하여 측정값을 얻은 후, 각 치아의 발육단계, 이환치아의 경사도, 위치의 변위율, 낭종의 면적 감소량과 치아의 맹출과의 관계를 분석하고자 맹출 속도, 각의 변화율, 낭면적의 감소율, 실험군 내의 조대술 후의 평균 기간(month)을 산정하여 다음의 결론을 얻었다. 낭에 이환된 부위의 영구치의 맹출 속도는 비이환측에 비해 평균 3.5배 빨랐으며 치축의 변화는 평균 $2.7^{\circ}/month$였다. 치근의 성장 단계에 따른 변화에서는 치근의 성장단계가 낮을수록 더욱 빠른 맹출 성향을 보였으며 각의 변화에서도 치근의 성장이 1/4이하 군에서 현저히 높게 나타났으며 낭종내 이환치의 변위에 따른 변화에서는 교두점 위치지수 차이가 30%될 때까지는 맹출 속도가 증가하다가 그 이상에서는 다시 감소하는 경향을 보였다. 낭의 면적 감소율이 $80mm^2/month$이상 감소하는 경우 현저하게 높은 맹출 속도를 보였고 이환된 치아가 근심 변위한 경우 원심 변위한 치아보다 맹출 속도는 낮았지만 각의 변화율은 크게 나타났다. 이환된 치아의 각도 변위가 작을수록 맹출의 속도는 증가하였으며 $15^{\circ}$ 이하의 경우 각도 변화도 높게 나타났다. 조대술 후 대부분의 경우 급격한 맹출 속도와 치축 각의 변화는 6개월 이내에 일어났다.
