Mid-Term Outcomes and Angiographic Patency of Redo Coronary Artery Bypass Grafting: A Comparison between OffPump and On-Pump Surgery

  • Background: We evaluated the mid-term outcomes and angiographic patency of redo coronary artery bypass grafting (CABG). Methods: Of 2,851 patients who underwent isolated CABG at Seoul National University Hospital from 2000 to 2017, 88 underwent redo CABG. Patients' mean age at redo CABG was 66.0±8.0 years. The mean interval between the first-time and redo CABG was 113.0±62.4 months. The mean follow-up duration was 86 months. Early and mid-term clinical outcomes were evaluated. Angiographic patency rates were evaluated early (1-2 days), 1 year, and 5 years after surgery. Comparative analyses between on-pump and off-pump CABG were also performed. Results: The culprits for reoperation were previous grafts (65.6%), native coronary vessels (17.8%), and both (16.7%). Off-pump CABG was performed in 75 cases (85.2%), and the mean number of distal anastomoses was 1.8±0.8. The saphenous vein (39.7%) was used most frequently, followed by the right internal thoracic artery (28.4%), right gastroepiploic artery (21.3%), left internal thoracic artery (7.8%), and radial artery (2.8%). Operative mortality was 1.1%. The overall survival, cumulative incidence of cardiac death, and cumulative incidence of major adverse cardiac events were 71.3%,12.0%, and 23.3% at 5 years after surgery, respectively. The overall angiographic patency rates were 95.7%, 90.1%, and 92.2% on early, 1-year, and 5-year angiograms, respectively. The angiographic patency rates of saphenous vein grafts were 93.1%, 85.6%, and 91.3% on early, 1-year, and 5-year angiograms, respectively. No significant differences in clinical outcomes or angiographic patency rates were observed between the on-pump (n=13) versus off-pump (n=75) groups. Multivariable analysis revealed that age (hazard ratio [HR], 1.07; p=0.005) and chronic kidney disease (HR, 3.85; p=0.001) were risk factors for all-cause mortality. Conclusion: Redo CABG could mostly be performed using the off-pump technique and did not show increased operative mortality and morbidities.

Comparison of Long-Term Angiographic Results of Wide-Necked Intracranial Aneurysms : Endovascular Treatment with Single-Microcatheter Coiling, Double-Microcatheter Coiling, and Stent-Assisted Coiling

  • Objective : Endovascular treatment of intracranial aneurysms is challenging in case of wide-necked aneurysms because coils are prone to herniate into the parent artery, causing thromboembolic events or vessel occlusion. This study aims to compare long-term angiographic results of wide-necked aneurysms treated by stent-assisted, double-microcatheter, or single-microcatheter groups. Methods : Between January 2003 and October 2016, 108 aneurysms that were treated with endovascular coil embolization with a neck size wider than 4 mm and a follow-up period of more than 3 years were selected. We performed coil embolization with single-microcatheter, double-microcatheter, and stent-assisted techniques. Angiographic results were evaluated using the Raymond-Roy occlusion classification (RROC). All medical and angiographic records were reviewed retrospectively. Results : Clinical and angiographic analyses were conducted in 108 wide-necked aneurysms. The immediate post-procedural results revealed RROC class I (complete occlusion) in 66 cases (61.1%), class II (residual neck) in 36 cases (33.3%), and class III (residual sac) in six cases (5.6%). The final follow-up results revealed class I in 48 cases (44.4%), class II in 49 cases (45.4%), and class III in 11 cases (10.2%). Of a total of 45 (41.6%) radiologic recurrences, there were 21 cases (19.4%) of major recurrence that required additional treatment, and 24 cases (22.2%) of minor recurrence. The final follow-up angiographic results showed statistically significant differences between the stent-assisted group and the others (p<0.01). Conclusion : Long-term follow-up angiography demonstrated that the stent-assisted technique had a better complete occlusion rate than the other two techniques.

Evaluation of Stent Apposition in the LVIS Blue Stent-Assisted Coiling of Distal Internal Carotid Artery Aneurysms : Correlation with Clinical and Angiographic Outcomes

  • Objective : To evaluate the stent apposition of a low-profile visualized intraluminal support (LVIS) device in distal internal carotid artery (ICA) aneurysms, examine its correlation with clinical and angiographic outcomes, and determine the predictive factors of ischemic adverse events (IAEs) related to stent-assisted coiling. Methods : We retrospectively analyzed a prospectively maintained database of 183 patients between January 2017 and February 2020. The carotid siphon from the cavernous ICA to the ICA terminus was divided into posterior, anterior, and superior bends. The anterior bends were categorized into angled (V) and non-angled (C, U, and S) types depending on the morphology and measured angles. Complete stent apposition (CSA) and incomplete stent apposition (ISA) were evaluated using unsubtracted angiography and flat-panel detector computed tomography. Dual antiplatelet therapy with aspirin 200 mg and clopidogrel 75 mg was administered. Clopidogrel resistance was defined as fewer responders (≥10%, <40%) and non-responders (<10%) based on the percent inhibition (%INH) of the VerifyNow system. These were counteracted by a dose escalation to 150 mg for fewer responders or substitution with cilostazol 200 mg for non-responders. IAEs included intraoperative in-stent thrombosis, transient ischemic attack, cerebral infarction, and delayed in-stent stenosis. A multivariate logistic regression analysis was used to determine the predictive factors for ISA and IAEs. Results : There were 33 ISAs (18.0%) and 27 IAEs (14.8%). The anterior bend angle was narrower in ISA (-4.16°±25.18°) than in CSA (23.52°±23.13°) (p<0.001). The V- and S-types were independently correlated with the ISA (p<0.001). However, treatment outcomes, including IAEs (15.3% vs. 12.1%), aneurysmal complete occlusion (91.3% vs. 88.6%), and recanalization (none of them), did not differ between CSA and ISA (p>0.05). The %INH of 27 IAEs (13.78%±14.78%) was significantly lower than that of 156 non-IAEs (26.82%±20.23%) (p<0.001). Non-responders to clopidogrel were the only significant predictive factor for IAEs (p=0.001). Conclusion : The angled and tortuous anatomical peculiarity of the carotid siphon caused ISA of the LVIS device; however, it did not affect clinical and angiographic outcomes, while the non-responders to clopidogrel affected the IAEs related to stent-assisted coiling.

Early Restoration of Hypoperfusion Confirmed by Perfusion Magnetic Resonance Image after Emergency Superficial Temporal Artery to Middle Cerebral Artery Anastomosis

  • Objective : Emergency superficial temporal artery to middle cerebral artery (STA-MCA) anastomosis in patients with large vessel occlusion who fails mechanical thrombectomy or does not become an indication due to over the time window can be done as an alternative for blood flow restoration. The authors planned this study to quantitatively measure the degree of improvement in cerebral perfusion flow using perfusion magnetic resonance imaging (MRI) after bypass surgery and to find out what factors are related to the outcome of the bypass surgery. Methods : For a total of 107 patients who underwent emergent STA-MCA bypass surgery with large vessel occlusion, the National Institute of Health stroke scale (NIHSS), modified Rankin score (mRS), infarction volume, and hypoperfusion area volume was calculated, the duration between symptom onset and reperfusion time, occlusion site and infarction type were analyzed. After emergency STA-MCA bypass, hypoperfusion area volume at post-operative 7 days was calculated and analyzed compared with pre-operative hypoperfusion area volume. The factors affecting the improvement of mRS were analyzed. The clinical status of patients who underwent emergency bypass was investigated by mRS and NIHSS before and after surgery, and changes in infarct volume, extent, degree of collateral circulation, and hypoperfusion area volume were measured using MRI and digital subtraction angiography (DSA). Results : The preoperative infarction volume was median 10 mL and the hypoperfusion area volume was median 101 mL. NIHSS was a median of 8 points, and the last normal to operation time was a median of 60.7 hours. STA patency was fair in 97.1% of patients at 6 months follow-up DSA and recanalization of the occluded vessel was confirmed at 26.5% of patients. Infarction volume significantly influenced the improvement of mRS (p=0.010) but preoperative hypoperfusion volume was not significantly influenced (p=0.192), and the infarction type showed marginal significance (p=0.0508). Preoperative NIHSS, initial mRS, occlusion vessel type, and last normal to operation time did not influence the improvement of mRS (p=0.272, 0.941, 0.354, and 0.391). Conclusion : In a patient who had an acute cerebral infarction due to large vessel occlusion with large ischemic penumbra but was unable to perform mechanical thrombectomy, STA-MCA bypass could be performed. By using time-to-peak images of perfusion MRI, it is possible to quickly and easily confirm that the brain tissue at risk is preserved and that the ischemic penumbra is recovered to a normal blood flow state.

MDCT를 이용한 역동적 간 컴퓨터단층촬영 검사에서 정맥과 동맥 주입법에 따른 영상의 화질 및 선량 비교 (Comparison of Image Quality and Dose between Intra-Venous and Intra-Arterial Liver Dynamic CT using MDCT)

  • 본 연구에서는 정맥경유 역동적 간 조영 컴퓨터단층촬영 검사와 동맥경유 역동적 간 조영 컴퓨터단층촬영 검사의 영상의 질과 선량의 차이를 분석하고자 하였다. 간세포암종의 중재적 시술인 경도관동맥화학색전술 대상으로 정맥과 동맥 경유 역동적 간 조영 컴퓨터단층촬영 검사를 한 케이스를 후향적 블라인드 방법으로 신호대잡음비와 대조도대잡음비를 분석하였다. 또한 영상저장 및 전송체계에 저장된 Dose Length Product (DLP)값을 이용하여 유효선량을 구하여 두 검사의 선량 차이를 분석하였다. 신호대잡음비는 정맥경유 역동적 간 조영 컴퓨터단층촬영 검사가 간과 지라에서 높은 결과를 보였지만 대조도대잡음비는 동맥경유 역동적 간 조영 컴퓨터단층촬영 검사가 높았다. 하지만 DLP와 유효선량 비교에서는 두 검사 간 차이가 발생하지 않았다. 결론적으로 동맥경유 역동적 간 조영 컴퓨터단층촬영 검사가 정맥경유 역동적 간 조영 컴퓨터단층촬영 검사에 비해 선량차이가 발생하지 않으면서도 대조도대잡음비가 우수한 검사임을 확인하였다. 추가로 간세포암종의 중재적 시술에서 가장 중요한 부분이 섭식동맥의 구분이 명료한가에 대한 구분이 필요하기 때문에 간동맥의 삼차원 혈관조영 컴퓨터단층촬영 검사에 대한 분석이 필요하다고 사료된다.

인공 슬관절 전치환술 후 발생한 메이-터너 증후군 및 심부정맥 혈전증 (May-Thurner Syndrome with Deep Vein Trombosis after Total Knee Arthroplasty)

  • 메이-터너 증후군(May-Thurner syndrome)은 장골 정맥 압박 증후군으로 알려져 있고 좌하지의 총 정맥 유출로가 압박되어 부종, 통증 또는 혈전(심부정맥 혈전증)을 유발할 수 있는 상태이다. 특히 우측 총 장골 동맥과의 교차 지점에서 좌측 총 장골 정맥이 압박되는 형태가 전형적이다. 저자들은 우측 인공 슬관절 전치환술을 시행한 75세 여자 환자에서 메이-터너 증후군이 합병된 증례를 치료하였고 이를 보고하고자 한다. 수술을 시행한 후 좌측 하지의 부종과 통증에 대해 혈관 조영술 및 컴퓨터 단층촬영을 이용해 메이-터너 증후군을 진단하였다. 혈전용해제와 혈전제거술를 사용하여 혈전을 제거한 후 혈관 성형술 및 정맥 내 스텐트를 삽입하였다. 한국에서 인공 슬관절 전치환술 후 메이-터너 증후군이 합병된 증례는 보고된 바가 없다. 이에 저자들은 문헌고찰과 함께 보고하고자 한다.

관상동맥질환 중증도 분류에 따른 성별 위험요인 비교 (Comparison of Risk Factors for Men and Women According to Severity Classification in Patients with Coronary Artery Disease)

  • 본 연구는 관상동맥질환 자의 관상동맥질환 중증도 분류에 따른 성별 위험요인 차이를 비교하고자 실시하였으며, 일개 종합병원에 처음 입원하여 관상동맥조영술을 실시한 남성 340명, 여성 221명을 대상으로 간호 정보 조사지와 관상동맥질환 중증도 진단 결과 기록을 2차 분석한 후향적 조사연구이다. 대상자가 진단받은 관상동맥질환 중증도 분류에 따라 남녀의 관상동맥질환의 위험요인 차이를 분석한 결과, 남성의 관상동맥질환 중증도 분류에 따른 위험요인은 나이(p=004), 총콜레스테롤(p=.040), 중성지방(p=.049), 당화혈색소(p<.001), 흡연(p<.001), 음주(p=.002), 동반질환(p=.036)에서 통계적으로 유의한 차이가 있었다. 반면 여성의 위험요인은 나이(p=.002)와 동반질환(p=.018)에서만 유의한 차이가 있었다. 남성의 관상동맥질환 중증도 분류에 영향을 주는 유의한 위험요인은 1개 관상동맥에 질환이 있는 군에서는 총콜레스테롤(OR 0.97, 95% CI 0.96-1.00, p=.014), 2개 관상동맥에 질환이 있는 군은 음주(OR 52.47, 95% CI 2.99-91.95, p=.007), 3개 관상동맥에 질환이 있는 군에서 총콜레스테롤(OR 0.98, 95% CI 0.95-0.98, p=.026)이었다. 여성의 관상동맥질환 중증도 분류에 영향을 주는 유의한 위험요인은 3개 관상동맥에 질환이 있는 군에서 동반질환 (OR 0.30, 95% CI 0.11-0.82, p=.020)이었다. 본 연구 결과를 통해 남성 관상동맥질환 자 위험요인 관리에는 금연과 절주, 혈당 조절, 콜레스테롤 관리 및 동반 질환 관리 등의 간호 중재 필요성과 중요성을 알 수 있었다. 본 연구는 관상동맥 중증도 분류에 따라 성별 위험요인 차이를 비교하여 개별적 맞춤형 간호 중재의 근거를 마련했다고 생각한다.

연부조직 종양의 진단적 접근 (Diagnostic Approach to a Soft Tissue Mass)

  • 사지와 몸통의 연부조직 종양은 정형외과 의사가 직면할 수 있는 흔한 문제이다. 비록 연부조직 종양은 대부분 양성이지만 정형외과 의사는 양성과 악성 종양을 구별할 수 있는 특징을 알고 있어야 한다. 연부조직 종양의 임상적 특징 및 역학을 이해하게 되면 올바른 진단 및 수술적인 치료를 할 수 있게 된다. 종양의 크기와 깊이는 종양의 진단을 위해 가장 중요한 요소이다. 종양의 감별 진단을 하기 위해서 우선적으로 상세한 병력청취와 자세한 신체 검사가 필요하며, 이후 단순 방사선 촬영, 초음파, 자기공명영상(magnetic resonance imaging), 양전자 방출 단층촬영술(positron emission tomography), 컴퓨터 단층촬영(computed tomography), 뼈 스캔, 혈관 조영술 등의 다양한 영상 촬영법을 사용하여 종양을 진단하고 진단된 종양의 특성을 확인하여야 한다. 특히 초음파 검사는 외래에서도 쉽게 수행할 수 있어 유용하다. 그러나 검사자의 숙련도에 따라 검사 정확도의 차이가 발생할 수 있다는 단점이 있다. 종양의 생검을 통한 조직검사는 종양에 대한 모든 영상 검사를 시행한 후 최종적으로 시행하는 것이 원칙이다. 조직 검사를 시행할 때는 세심한 주의를 기울여야 하며, 최종적인 진단 후에는 치료를 위해 다각적인 접근을 시행하여야 하며 필요한 경우에는 경험 있는 근골격계 종양전문의사에게 의뢰하는 것이 필요하다.

경피적 관상동맥중재술 후 재협착 예방에 대한 한약의 효과 : 무작위배정 대조군 임상연구에 대한 체계적 문헌고찰 및 메타분석 (Effect of Herbal Medicines on Preventing Restenosis After Percutaneous Coronary Intervention: A Systematic Review and Meta-analysis of Randomized Controlled Trials)

  • Objective: This study was conducted to investigate the preventive effect of herbal medicines on restenosis after percutaneous coronary intervention (PCI) by reviewing randomized controlled trials (RCTs). Methods: RCTs were searched for herbal medicine treatment after PCI using eight online databases (PubMed, CNKI, Wanfang, J-STAGE, OASIS, ScienceON, KTKP, and KISS). Studies that confirmed restenosis through coronary angiography at follow-up were selected according to the inclusion and exclusion criteria. The primary outcome was the restenosis rate, and the secondary outcome was the angina recurrence rate. Data were extracted from the final selected studies according to the research methodology and then analyzed with Review Manager 5.4.1. Study quality was assessed using Cochrane's risk-of-bias (RoB) tool. Results: Of the 252 papers obtained through the primary search, nine studies that met the selection criteria were finally selected. In these nine studies, herbal medicine combined with western medicine was used for the experimental group, and western medicine treatment was used alone for the control group. The meta-analysis result revealed that the restenosis rate and angina recurrence rate were significantly lower in the experimental group than in the control group (RR=0.34, 95% CI: 0.22-0.53, p<0.00001, I2=0% and RR=0.47, 95% CI: 0.29-0.78, p=0.004, I2=0%, respectively). Furthermore, the quality of studies assessed by Cochrane's RoB was low. Conclusions: This study showed that the combined treatment of herbal medicine and western medicine was effective in preventing restenosis and angina after PCI. As the number of papers included in this study was small, a large number of high-quality clinical studies should be considered in the future.

내외전마비를 동반하는 특발성 양안사시 한의 증례보고 1례 (A Case Report of an Idiopathic Bilateral Strabismus with Internal and External Palsy)

  • Objectives : The purpose of this study is to report the effect of Korean medical treatments on idiopathic bilateral strabismus. Methods : We treated a 43-year-old male idiopathic bilateral strabismus patient who was told to wait without a prescription as there was no problem in intraocular pressure test, fundus test, brain computed tomography, brain magnetic resonance imaging and angiography test, and cerebrospinal fluid test. The patient was admitted to Cheonan Korean medical hospital for 3 months to get Korean medical treatments including acupuncture, electroacupuncture, vapor treatment, pharmacopuncture therapy. The degree of improvement in idiopathic bilateral strabismus was determined by the objective measures of internal and external rotation disorders, and the patient's subjective opinions on eye pain and double vision discomfort. Results : The results of this patient showed distinct improvements on objective measures of internal and external rotation disorders in both eyes. Distance from the outer corner of the right eye when abducted decreased from 7mm to 0mm, and left eye decreased from 7mm to 0mm. Distance from the inner corner of the right eye when adducted decreased from 5mm to 0mm, and left eye decreased from 4mm to 0mm. In addition, eye pain decreased from 10 to 0 and diplopia decreased from 10 to 2 according to the patient's subjective opinions. Conclusions : This study shows the possibility that such medical treatment in Koreans can improve the rare idiopathic bilateral perspective among strabismus patients.
