Incidental detection of myocardial ischemia during F-18 FDG CoDe PET for the evaluation of a solitary pulmonary nodule

  • The authors report a case of unsuspected myocardial ischemia detected during CoDe FDG PET (coincidence detection fluorodeoxyglucose positron emission tomogram) which was performed for the evaluation of a solitary pulmonary nodule. Camera-based FDG PET without attenuation correction often reveals false defect in the inferior wall of the left ventricle in normals due to excessive attenuation. However, this asymptomatic patient had increased uptake in the inferior wall suggesting ischemic myocardium. The scan finding was confirmed by Tl-201 myocardial SPECT and coronary angiogram. The patient then underwent successful PTCA of mild RCA and right ventricular branch followed by right upper lobectomy for small cell lung cancer.

  • Unilateral hypoplasia of the pulmonary artery is an uncommon anomaly, which commonly develops in combination with congenital cardiovascular defects such as tetralogy of Fallot, patent ductus arteriosus and septal defect of atrium or ventricle, but may also present as an isolated lesion. We have recently experienced a case of the left pulmonary artery hypoplasia in adult by chance of during the general health screen, which diagnosed by chest X-ray, chest spiral CT, lung perfusion and ventilation scan, digital substraction angiogram and bronchoscopy, then presented hereby with the review of relevant literature.

Bow Hunter's Stroke Caused by a Severe Facet Hypertrophy of C1-2

  • Bow hunter's stroke is a rare symptomatic vertebrobasilar insufficiency in which vertebral artery (VA) is mechanically occluded during head rotation. Various pathologic conditions have been reported as causes of bow hunter's stroke. However, bow hunter's stroke caused by facet hypertrophy of C1-2 has not been reported. A 71-year-old woman presented with symptoms of vertebrobasilar insufficiency. Spine computed tomography showed massive facet hypertrophy on the left side of C1-2 level. A VA angiogram with her head rotated to the right revealed significant stenosis of left VA. C1-2 posterior fixation and fusion was performed to prevent serious neurologic deficit from vertebrobasilar stroke.

  • The objective of this paper is to develop computer assisted learning system for the education of cardiac disease using multimedia. Total 20 cases of disease was selected and their asculation, X-Ray, ECG, Sanography, Angiogram and RI-Study results can be shown on the computer screen. The references about cardiology, surgical process can be also shown on the screen and the computer can test and score their records for the evaluation. This system will be widely applied for the education of medicine combined wi th the communication network.

관상동맥 우회술을 병행한 대동맥판막 치환술 치험 1례 (Aortic valve Replacement Concomitant with Aorto-Coronary Bypass Surgery -One case report-)

  • Patient with aortic valvular disease have increased left ventricular work and greater myocardial oxygen demand, which may aggravate the effect of concomitant coronary artery disease. Thus in patient who repair aortic valve replacement, concomitant aortocoronary bypass surgery is often performed when angiographically significant coronary artery disease is present. This approach is supported by reports that revascularization does not increase operative risk when associated coronary artery disease is present and significantly reduce the occurrence of late sudden death. Recently we have experienced one case of aortic valve replacement concomitant with aorta-coronary bypass surgery. The patient was 56 year-old male and admitted with complaint of anterior chest pain especially during his exercise. He was diagnosed as aortic valve stenosis and regurgitation [GIII] with proximal right main coronary artery occlusion We performed aortic valve replacement with aorta coronary bypass surgery by use of saphenous vein. Post operative course was uneventful and chest pain was relieved. Post operative coronary angiogram disclosed good patency of grafted vessel.

Sinus Valsalva Rupture 에 대한 외과적 치료 (Surgical Treatment of the Sinus Valsalva Aneurysm Rupture - 5 cases reviews -)

  • We experienced 6 patients with the sinus Valsalva aneurysm rupture during last 12 years [Jan. 1977-Sep. 1989]. Of them. 5 cases were reviewed. They consist of 3 males and 2 females, and the age ranged from 12 years to 40 years with the mean age of 25 years. 4 patients showed congestive heart failure symptoms. The diagnosis was made by 2D-Echo and cine-angiogram. In 4 patients. sinus Valsalva aneurysm ruptured from the Rt. coronary sinus to the Rt. ventricle, and in one from non-coronary sinus to the Rt. atrium. In 2 cases, resection of the aneurysm and simple stitch closure was made. Resection of the aneurysm k patch closure and AVR in one, closure of the fistula, AVR and patch closure of the associated VSD in one, and closure of the fistula, AVR k TVR in one were made in another 3 cases. There was no postoperative mortality case.

이첨 대동맥판막협착을 동반한 단일 관상동맥증 -1례 보고- (Single Coronary Artery Associated with Bicupid Aortic Valvular Stenosis -1 Case Report-)

  • The incidence of single coronary artery is extremely rare in a review of congenital anomalies of the coronary arteries. This 27-year-old male patient was referred for the evaluation of cardiac condition showing exertional dyspnea[NYHA class II-III] and chest discomfort for about 1 year. A complete catheterization study including angiogram disclosed large single coronary artery arising from left aortic sinus [Ogden classification L-4] associated with bicuspid aortic valvular stenosis and low grade supravalvular aortic stenosis. Calcified stenotic aortic valve was fully removed with caution and the 19mm St. Jude Medical valve was then implanted in the small nortic annulus. The patient had an uneventful recovery and was discharged on 13th postoperative day.

An Extra-adrenal Pheochromocytoma Presenting with Spontaneous Intracerebral Hematoma

  • We report a 18-year-old man, who has been taking antihypertensive medication for 1month in a local clinic, presented with a sudden onset headache followed by left blindness. He experienced palpitation and chest discomfort during physical exertion since 2years before admission, but unfortunately has been ignored. Brain CT showed intracerebral hemorrhage in the left temporoparietal area, but cerebral angiogram and magnetic resonance image revealed no vascular anomaly. He was managed conservatively, and headache and visual loss were improved over time. Subsequently, on the evaluation of hypertension, he was diagnosed as having extra-adrenal pheochromocytoma on left paraaortic area from the results of endocrinological evaluations, abdominal CT scan, and $^{131}I$-MIBG scintigraphy.

Brainstem Congestion due to Dural Ateriovenous Fistula at the Craniocervical Junction

  • Dural ateriovenous fistula (DAVF) at the craniocervical junction is rare. We report a patient presenting with brainstem dysfunction as an uncommon onset. Brainstem lesion was suggested by magnetic resonance image study. Angiogram revealed a DAVF at a high cervical segment supplied by the meningeal branch of the right vertebral artery, with ascending and descending venous drainage. Complete obliteration of the fistula was achieved via transarterial Onyx embolization. Clinical cure was achieved in the follow-up period; meanwhile, imaging abnormalities of this case disappeared. Accordingly, we hypothesize that a brainstem lesion of this case was caused by craniocervical DAVF, which induced venous hypertension. Thus, venous drainage patterns should be paid attention to because they are important for diagnosis and theraputic strategy.

심혈관 조영도에서의 관상동맥 자동영상분할 (Automatic segmentation of the artery in coronary angiogram)

  • 관상동맥 폐색증 환자들에 대해서 시술되는 stent 삽입 시술이나 관상동맥 우회로 시술 중에는 X-ray 등의 조영 영상이 시술의 기준이 되고 있다. 따라서 조영 영상에서 혈관을 빠르게 인식하는 것은 정확하고 효과적인 시술의 필수 조건이다. 이러한 시술 중 빠르고 정확한 혈관 인식을 위하여 본 논문에서는 심혈관 조영 영상으로부터 관상동맥의 형태를 자동적으로 영상분할하기 위한 방법을 제안한다. 우선 조영 영상에서 혈관을 분명하게 인식하기 위해서는 잡음을 제거하기 위한 필터링이 필요한 데, Anisotropic diffusion을 이용한 필터링은 이미지 내 물체의 경계선을 보존하고, 영역 내의 잡음을 제거하는 데 효과적이다. 정확한 영상분할을 수행하기 위해서는 대부분의 경우 사용자가 영상 내에 관심 영역을 지정하는 인터렉션이 필요하지만 이는 사용자에게 불편함을 줄 수 있다. 따라서 이러한 번거로움을 최소화하고, 정확한 결과를 유도하기 위해서 자동 씨드 영역 추출 알고리즘을 제안한다. 따라서 조영 영상에 필터링을 적용한 후 추출된 씨드 영역과 추출된 에지와 Adaptive region-growing을 복합적으로 사용하는 영상분할을 수행하게 되면 보다 효과적인 관상동맥 영상 분할의 결과를 얻을 수 있었다.
