Objective : We evaluate the effect of the copolymer-coated coils on immediate occlusion of the aneurysm, preventing rupture, and decreasing compaction or re-growth. Methods : Thirty-five aneurysms treated between September 2003 and December 2004 using Matrix detachable coil were reviewed. Study population consisted of 12 men and 23 women ranging in age from 34 to 75 years[mean, 55.1 years]. Twenty-two aneurysms were ruptured and 23 aneurysms were located in the anterior circulation. Follow-up angiography was obtained in 16 patients after 6 months from the procedure. Results : Initial complete occlusion was achieved in 17 aneurysms[48.6%], and the others remained as a residual neck in 8 aneurysms[22.8%] and residual sac in 10 aneurysms[28.6%]. Among these incompletely occluded aneurysms, 7 aneurysms were performed follow-up angiography. And 6 of them converted into complete occlusion. In the other hands, among 17 aneurysms achieved complete occlusion initially, 9 aneurysms were performed follow-up angiography. Recurrence due to coil compaction occurred in one aneurysm and the others maintained complete occlusion. There was one mortality case due to thromboembolic complication. Conclusion : In spite of difficulty in achieving complete occlusion with Matrix coil system, there is no rupture or re-rupture during follow-up period. Follow-up angiography shows many conversions of residual sac into complete occlusion. Embolization using Matrix coil system is safe and effective, but the effects of PGLA copolymer need further investigation.
Objective : The goal of this study was to document the influence of the treatment modality(surgery versus endovascular treatment) on the development of chronic shunt-dependent hydrocephalus in a series of 296 patients treated after aneurysmal subarachnoid hemorrhage(SAH). Methods : The following parameters were retrospectively analyzed for association with chronic shunt-dependent hydrocephalus : 1) Age and Sex, 2) Hunt and Hess grade, 3) Fisher computed tomographic grade, 4) aneurysm location, and 5) treatment modality(surgery versus endovascular treatment). Results : Thirty-six of 251 patients(14.3%) who survived the SAH and its neurological and/or medical sequelae underwent definitive shunting for treatment of chronic hydrocephalus. The rate of shunt dependency was positively correlated with a higher age, a higher Hunt and Hess grade, a higher Fisher computed tomographic grade, and aneurysms arising at the anterior communicating artery(p<0.05). Conclusion : The results of the present study indicate that the treatment modality used does not affect the risk of the later development of chronic shunt-dependent hydrocephalus(surgery, 16.2% [25 of 154] ; endovascular treatment, 11.3% [11 of 97] ; p=0.45).

Objective : Vertebral artery dissecting aneurysm (VADA) is a very rare subtype of intracranial aneurysms; when ruptured, it is associated with significantly high rates of morbidity and mortality. Despite several discussions and debates, the optimal treatment for VADA has not yet been established. In the last 10 years, flow diverter devices (FDD) have emerged as a challenging and new treatment method, and various clinical and radiological results have been reported about their safety and effectiveness. The aim of our study was to evaluate the clinical and radiological results with the use of FDD in the treatment of unruptured VADA. Methods : We retrospectively evaluated the data of all patients with unruptured VADA treated with FDD between January 2018 and February 2021 at our hybrid operating room. Nine patients with unruptured VADA, deemed hemodynamically unstable, were treated with FDD. Among other parameters, the technical feasibility of the procedure, procedure-related complications, angiographic results, and clinical outcomes were evaluated. Results : Successful FDD deployment was achieved in all cases, and the immediate follow-up angiography showed intra-aneurysmal contrast stasis with parent artery preservation. A temporary episode of facial numbness and palsy was noted in one patient; however, the symptoms had completely disappeared when followed up at the outpatient clinic 2 weeks after the procedure. The 3-6 months follow-up angiography (n=9) demonstrated complete/near-complete obliteration of the aneurysm in seven patients, and partial obliteration and segmental occlusion in one patient each. In the patient who achieved only partial obliteration, there was a sac 13 mm in size, and there was no change in the 1-year follow-up angiography. In the patient with segmental occlusion, the cause could not be determined. The clinical outcome was modified Rankin Scale 0 in all patients. Conclusion : Our preliminary study using FDD to treat hemodynamically unstable unruptured VADA showed that FDD is safe and effective. Our study has limitations in that the number of cases is small, and it is not a prospective study. However, we believe that the study contributes to evidence regarding the safety and effectiveness of FDD in the treatment of unruptured VADA.
A successful repair of aortic dissection of descending thoracic aorta was performed in a 48 year old man. The patient was visited ER because of abruptly onset chest pain. On admission, Chest film showed mediastinal widening and undertaken chest CT, echocardiogram and angiogram There was evidence of dilation on descending aorta with internal separation of intimal calcification. Aneurysmal sac with dissection was noted from just below left subclavian artery to 2cm above of diaphragm. He underwent thoracotomy and the impending ruptured aneurysm of the aorta was replaced with a Woven Dacron graft[20Yo Albumin preclotted] using LA-femoral bypass. Postoperative course was uneventful.
The Cox-Maze procedure was developed as a cure for atrial fibrillation. The recovery rate of both atrial contractility is reported low in the atrial fibrillation associated with mitral valvular heart disease than that of loan atrial fibrillation. We performed the Cox-Maze procedure (Maze III) in three cases who suffered from non-mitral heart diseases associated with atrial fibrillation: A ruptured sinus of Valsalva aneurysm, a ventricular septal defect, and an aortic stenoinsufficiency. The Cox-Maze procedure was performed concomitantly with correction of the underlying heart disease. Conversion to sinus rhythm was achieved in all three patients, and both right and left atrial mechanical activities could be identified echocardiographically after three postoperative months.
More than one cerebral aneurysm on the same artery is uncommon. Furthermore multiple aneurysms on the anterior communicating artery(ACoA) is very rare. We surgically experienced one case of two separate aneurysms on the ACoA ; one developed at the junction of the ACoA with the right second branch of anterior cerebral artery, and the other on inferior wall of the ACoA itself. The former, confirmed as ruptured on operation, projected superiorly and the latter directed inferiorly toward the prechiasmatic cistern. Radiographic and operative findings are presented with review of literature.
Surgical correction of patent ductus arteriosus is,under most circumstances,highly successful and carries a low mortality. But infected PDA is yet potentially dangerous due to its frequent recurrence and resistant organisms to antibiotics. And,in surgical correction,surgeon may face the possibility of tearing of ductus arteriosus arterial end due to friability and adhesion of its surrounding tissue.This report demonstrats another problem in treatment of infected patent ductus arteriosus.This thirteen years old female patient received susceptible combined antibiotics intravenously from the day of admission and remitted from 4th.week of therapy.This remission state continued for 12days without relapse.But the pulmonary artery ruptured in this remission period.In autopsy,bacteria was not found in ductal vegetation.Also,there was no pulmonary artery aneurysm,Our experience show that in infected PDA,pulmonary artery can rupture spontaneously during remission period without aneurysmal formation.
목 적 : 파열된 뇌동맥류 수술후 경련발작은 잘 알려진 합병증으로 항경련제 치료가 요구된다. 저자들은 뇌동맥류 파열에 의한 지주막하출혈 환자에서 수술후 경련발작의 위험인자를 조사하여 경련발작 치료에 임상적 의의를 비교 하고자 한다. 방 법 : 1990년부터 1996년까지 파열된 뇌동맥류로 개두술을 시행한 321명의 환자중 1년 이상 추적관찰(1~4.6년)하였던 206명을 대상으로 후향적 방법으로 조사하였다. 모든 환자는 수술후 3~18개월 동안 예방적 항경련제 치료를 받았다. 경련발작에 기여하는 요인으로서 연령, 성별, 내원 당시 신경학적 상태, 수술전후 전산화단층촬영소견, Hunt-Hess grade, Fisher grade, 뇌동맥류의 위치, 고혈압의 기왕력, 수술시기, 그리고 지연성 신경학적 결손 등을 조사하여 비교 분석하였다. 통계적 검증분석 방법으로 chi-square test와 Fisher's exact test를 이용하였다. 결 과 : 평균 추적관찰기간은 1.8년으로 206명중 18명이 경련발작 소견을 보여 8.7%의 발생률을 보였다. 연령에서 나이가 적을수록, 파열된 동맥류의 위치가 중뇌동맥인 경우에서 경련발작이 높게 나타났다. 지연성 신경학적 결손을 보인 42명의 환자에서 8명(19%)이 경련발작이 발생하여 높은 통계학적 유의성을 보였다. 또한 고혈압의 기왕력, 수술후 뇌전산화촬영상 뇌경색등도 이와 관련성이 있었고, 수술전후 뇌내혈종의 소견이 관찰되면 또한 경련발작이 높게 발생되었다. 그러나 Hunt-Hess grade, Fisher grade, Glasgow Outcome Scale, 그리고 출혈후 수술의 시기 등은 경련발작의 발생빈도와는 통계학적 유의성을 보이지 않았다. 결 론 : 뇌동맥류 파열에 의한 수술후 경련발작의 위험인자로 낮은 연령, 중뇌동맥류, 지연성 신경학적 결손, 수술후 뇌전산화촬영상 뇌경색소견, 고혈압, 그리고 수술전후 뇌내혈종의 유무 등이 연관되었고, 이러한 경련발작의 유발인자를 인지함으로서 수술후 경련발작에 대한 예방과 치료에 도움을 줄 수가 있으며 이에 대한 전향적 연구가 요구된다.
배경: Dubost 등이 1952년에 실시한 신동맥 하부 대동맥류 수술을 시작으로 복부 대동맥류의 근치적인 수술적 치료가 시작되었다. 최근에 경피적 대동맥 스텐트 삽입술의 비약적인 발전으로 외과적 수술이 감소하고는 있으나, 여전히 근치를 위한 치료로 인정되고 있다. 저자들은 지난 6년간 시행한 복부 대동맥류의 수술성적 및 그 예후에 미치는 영향인자에 대해 분석하여 보았다. 대상 및 방법: 2002년 3월부터 2008년 3월까지 복부 대동맥류로 수술 받은 환자 18명을 대상으로 하였다. 수술의 적응은 파열, 60 mm 이상의 최대직경, 크기의 증가, 내과적으로 조절 안되는 고혈압이나 통증이 있는 경우 등이었다. 결과: 환자들의 평균 나이는 $66.6{\pm}9.3$세$(49\sim81)$였고, 남자가 12명(66.7%), 여자가 6명이었다. 신동맥 상부까지 진행된 경우는 6명(33.3%), 장골동맥까지 진행된 경우는 13명(72.2%)이었다. 진단 당시 대동맥이 파열되어 있었던 환자들은 5명(27.8%)이었다. 대동맥의 평균 최대직경은 $72.2{\pm}12.9$ mm ($58\sim109$ mm)였다. 수술은 모두 정중 복부절개를 통한 복강 내 접근으로 시행하였고, 응급수술은 6명의 환자에서 시행하였다. 대동맥 겸자 후 양쪽 하지로 혈류개통이 될 때까지의 허혈시간은 평균 $82{\pm}42$분($35\sim180$분)이었다. 전체 환자 중 3명이 사망하여 전체 사망률은 16.7%였고, 파열된 환자의 사망률은 60%, 파열되지 않은 환자의 사망률은 0%였다. 수술 후 합병증으로는 신부전, 대퇴등 정맥 폐쇄, 창상감염이 각 1예씩 있었다. 생존 환자들은 $34{\pm}26$개월($4\sim90$개월)간의 추적관찰 기간동안 대동맥 내 문제는 없이 장기생존 중이다. 사망에 영향을 주는 인자로는 파열, 응급수술이 의미 있었다(p<0.05). 결론: 파열된 복부대동맥류에 대한 응급수술은 여전히 높은 사망률을 보이나, 파열되지 않은 복부대동맥류의 수술은 비교적 안전하게 진행할 수 있으며 수술 후 생존한 환자는 장기생존을 보인다. 비록 경피적 대동맥 스탠트 삽입술이 최근의 치료 경향이나 종래의 수술방법도 만족할 만 한다.
Objective : Very large (20-25 mm) and giant (${\geq}25mm$) intracranial aneurysms have an extremely poor natural course, and treatment of these aneurysms remains a challenge for endovascular and surgical strategies. This study was undertaken to describe our experiences of endosaccular treatment of very large and giant intracranial aneurysms with parent artery preservation. Methods : From January 2005 to October 2016, twenty-four very large or giant aneurysms in 24 patients were treated by endosaccular coil embolization with parent artery preservation. Nine (37.5%) aneurysms were ruptured and 15 were unruptured, and of these 15, 11 were symptomatic cases and 4 were incidentally discovered. The cohort comprised 17 women and 7 men of mean age 58.5 years (range, 26-82). Mean aneurysm size was 26.0 mm (range, 20-39) and 13 of the 24 aneurysms were giant. Results : Immediate angiographic results were complete occlusion in nine (37.5%) cases, remnant neck in six (25.0%), and remnant sac in nine (37.5%). Overall procedural related morbidity and mortality rates were 12.5% and 4.2%, respectively. Angiographic follow-up was available in 16 patients (66.7%). Mean and median follow-up periods were 27.2 (range, 2-77) and 10.5 months, respectively. In 12 cases (12/16, 75%) stable occlusion was achieved, four cases (4/16, 25%) had recanalized, and two of these were retreated with additional coiling. At clinical follow-up of the nine ruptured cases, three patients (33.3%) achieved a good clinical outcome (Glasgow outcome scale [GOS] score of 4 or 5), two (22.2%) a poor outcome (GOS score of 2 or 3), and four patients (44.4%) expired (GOS 1). On the other hand, of the 15 unruptured cases, 13 patients (86.7%) achieved a good clinical outcome (GOS 4 or 5), one patient a poor outcome (GOS score of 2 or 3), and one patient expired (GOS 1). Conclusion : The present study shows endosaccular treatment of very large or giant intracranial aneurysms with parent artery preservation is both feasible and effective with acceptable morbidity and mortality.
