2-day Infusor를 이용한 마약성 진통제와 국소마취제의 지속적인 경막외 투여와 Meperidine근주와의 통증 치료 비교 (Comparison of Continuous Epidural Infusion of Narcotic Analgesics and Local Anesthetics Using 2-day Infusor and Meperidine IM. on Postoperative Analgesia)

  • Recently, continuous epidural infusion of narcotics and local anesthetics have been used for postoperative pain relief. This study was designed to compare the analgesic efficacy and side effects of continuous epidural infusion of narcotics and local anesthetics with those of intramuscular administration of meperidine, for postoperative pain relief after cesarean section. Forty patients were divided into 2 groups of 20 patients each ; Continuous epidural group and control (IM meperidine) group. Before each operation, the epidural group had an epidural catheter placed (L1-2) and following each operation, a bolus of 1%~8ml of lidocaine was injected, followed by continuous infusion of morphine 3 mg/day, fentanyl 300g, 2% mepivacaine 20 ml, 0.5% bupivacaine 20 ml and normal saline 40 ml. The control group received meperidine 50mg IM injection as needed. We evaluated analgesic efficacy with VAS (Visual analogue scale) and side effect at 1, 6, 12, 24, 36 and 48 hour intervals after the operation. The results were as follows: 1) Continuous epidural group was superior to the control group with respect to postoperative analgesia. 2) Side effects (pruritus, nausea & vomiting) were more frequent in the epidural group.

Anesthetic efficacy in vital asymptomatic teeth using different local anesthetics: a systematic review with network meta-analysis

  • Objectives: This study aimed to evaluate the efficacy of various local anesthesia (LA) in vital asymptomatic teeth. Materials and Methods: Randomized controlled trials comparing pulpal anesthesia of various LA on vital asymptomatic teeth were included in this review. Searches were conducted in the Cochrane CENTRAL, MEDLINE (via PubMed), EMBASE, ClinicalTrials.gov, Google Scholar and 3 field-specific journals from inception to May 3, 2019. Study selection, data extraction, and risk of bias assessment using Cochrane Risk of Bias Tool were done by 2 independent reviewers in duplicate. Network meta-analysis (NMA) was performed within the frequentist setting using STATA 15.0. The LA was ranked, and the surface under the cumulative ranking (SUCRA) line was plotted. The confidence of the NMA estimates was assessed using the CINeMA web application. Results: The literature search yielded 1,678 potentially eligible reports, but only 42 were included in this review. For maxillary buccal infiltration, articaine 4% with epinephrine 1:100,000 was more efficacious than lidocaine 2% with epinephrine 1:100,000 (odds ratio, 2.11; 95% confidence interval, 1.14-3.89). For mandibular buccal infiltration, articaine 4% with epinephrine 1:100,000 was more efficacious than various lidocaine solutions. The SUCRA ranking was highest for articaine 4% with epinephrine when used as maxillary and mandibular buccal infiltrations, and lidocaine 2% with epinephrine 1:80,000 when used as inferior alveolar nerve block. Inconsistency and imprecision were detected in some of the NMA estimates. Conclusions: Articaine 4% with epinephrine is superior when maxillary or mandibular infiltration is required in vital asymptomatic teeth.

치과 국소마취와 관련된 하치조신경과 설신경 손상에 대한 연구 (Study for Inferior Alveolar and Lingual Nerve Damages Associated with Dental Local Anesthesia)

  • Background: Damages of trigeminal nerve, particularly inferior alveolar nerve and lingual nerve, could occur following dental procedures. In some cases, nerve damage may happen as a complication of the local anesthetic injection itself and not of the surgical procedure. Methods: From September 2006 to August 2010, 5 cases of inferior alveolar nerve and lingual nerve damages, which were assumed to happen solely due to local anesthesia, were reviewed. All cases were referred to Division of Oral and Maxillofacial Surgery, Department of Dentistry, Hanyang University Medical Center for legal authentication in the process of criminal procedure. Results: In all five cases, patients complained of altered sensation occurred in the distribution of the inferior alveolar or lingual nerve following block anesthesia. The local anesthetics were 2% lidocaine with 1 : 100,000 epinephrine and the amount of local anesthetics, which were used during injection, were varied. Most of patients experienced the electric stimulation during injection. Recovery was poor and professional supportive care was mostly absent. Conclusions: Dental practitioners should consider that the surgical procedure caused the trigeminal nerve damage, however, dental local anesthesia for inferior alveolar nerve and lingual nerve could be one of the causes for damages. The various mechanisms for nerve damages by local anesthesia are thoroughly discussed.

The Effect of Lidocaine.HCl on the Fluidity of Native and Model Membrane Lipid Bilayers

  • The purpose of this study is to investigated the mechanism of pharmacological action of local anesthetic and provide the basic information about the development of new effective local anesthetics. Fluorescent probe techniques were used to evaluate the effect of lidocaine HCl on the physical properties (transbilayer asymmetric lateral and rotational mobility, annular lipid fluidity and protein distribution) of synaptosomal plasma membrane vesicles (SPMV) isolated from bovine cerebral cortex, and liposomes of total lipids (SPMVTL) and phospholipids (SPMVPL) extracted from the SPMV. An experimental procedure was used based on selective quenching of 1,3-di(1-pyrenyl)propane (Py-3-Py) and 1,6-diphenyl-1,3,5-hexatriene (DPH) by trinitrophenyl groups, and radiationless energy transfer from the tryptophans of membrane proteins to Py-3-Py. Lidocaine HCl increased the bulk lateral and rotational mobility of neuronal and model membrane lipid bilayes, and had a greater fluidizing effect on the inner monolayer than the outer monolayer. Lidocaine HCl increased annular lipid fluidity in SPMV lipid bilayers. It also caused membrane proteins to cluster. The most important finding of this study is that there is far greater increase in annular lipid fluidity than that in lateral and rotational mobilities by lidocaine HCl. Lidocaine HCl alters the stereo or dynamics of the proteins in the lipid bilayers by combining with lipids, especially with the annular lipids. In conclusion, the present data suggest that lidocaine, in addition to its direct interaction with proteins, concurrently interacts with membrane lipids, fluidizing the membrane, and thus inducing conformational changes of proteins known to be intimately associated with membrane lipid.

굴곡건 수술에서 각성마취의 유용성 (Usefulness of Awake Anesthesia in Flexor Tendon Surgery)

  • Purpose: According to various medical publications, it is believed that epinephrine should not be injected in fingers. However numerous articles show the successful use of local anesthetic with epinephrine in the digits. Epinephrine-mixed lidocaine solution enables to maintain a bloodless field for operation and provides long duration of local anesthesia when patient was wide awake. Methods: From May 2009 to December 2009, ten patients underwent flexor tendon reconstruction with local anesthesia using epinephrine. No tourniquet was necessary. Before operation, all patients were injected with local anesthetics using 1% lidocaine 20 mL and 0.1% epinephrine 0.1 mL. Results: There was no case of digital necrosis nor gangrene in the epinephrine injection. All 10 patients actively could move the finger through a full range of motion. All procedures were performed without sedation nor tourniquet and we could obtain a good vision of operative field and patients were comfortable. The patient make his or her fingers move through a full range of active motion before the skin is closed. Phentolamine was not required to reverse the vasoconstriction in any patients. Conclusion: The assertation that epinephrine should not be injected into the fingers is clearly no longer valid. The epinephrine injection allowed the authors to adjust flexor tendon surgery without risks associated with general anesthesia. It also enables to ensure longer anesthetic duration and bloodless operative field, and prevent post operative complications. In case of flexor tendon surgery, the use of epinephrine injection is recommended because of the advantages of local anesthesia.

경막외 Lidocaine의 최대사용량과 혈중농도에 대한 고찰 (The Plasma Concentrations and Systemic Toxicity of Lidocaine after Maximal or Supramaximal Recommended Doses of Epidural Administration)

  • Background: The current maximal recommended doses of lidocaine are 7 mg/kg with $5\;{\mu}g/ml$ of epinephrine. But in clinical practice, sometimes more doses of lidocaine are required to produce adequate regional anesthesia. Method: Twenty-two healthy women patients were divided into two groups and pretreated with valium 5 mg p.o., morphine 5 mg i.m., and midazolam 2 mg i.v. before operation. Of these, 7 mg/kg of 2% lidocaine with $5\;{\mu}g/ml$ of epinephrine were given to 11 patients epidurally. Initial 3 ml of epinephrine mixed lidocaine was given as a test dose and remaining doses were given 5 ml/30 sec with 3 min intervals. Radial arterial blood were drawn at 5, 10, 15, 20, 30, 45, 60, 90, 120 min to measure plasma lidocaine concentrations. After confirming all of the peak plasma concentrations of 7 mg/kg lidocaine were absolutely under $5\;{\mu}g/ml$, the other 11 patients were given 10 mg/kg of 2% lidocaine with $5\;{\mu}g/ml$ of epinephrine epidurally and blood samplings were taken according to the same method of 7 mg/kg group. The peak plasma concentration ($C_{max}$), time to reach to $C_{max}$ ($T_{max}$), time to reach to $T_4$, maximal sensory block level, systemic toxicity, and vital sign changes were investigated. Result: $C_{max}$ was significantly higher in 10 mg/kg group ($5.1{\pm}1.3\;{\mu}g/ml$) than 7 mg/kg group($3.3{\pm}0.5\;{\mu}g/ml$), but $T_{max}$ ($10.5{\pm}2.7$ min vs $10.9{\pm}3.1$ min) was not different. Time to reach $T_4$ was significantly shorter in 10 mg/kg group ($9.5{\pm}2.7$ min) than 7 mg/kg group ($12.7{\pm}3.2$ min) but maximal sensory block level ($T_{3.7{\pm}0.7}$ vs $T_{2.7{\pm}1.0}$) was not different. In four patients of 10 mg/kg group, peak plasma concentrations exceeded $5\;{\mu}g/ml$, but no systemic toxicities appeared. No significant vital sign changes were observed. Conclusion: The current maximal recommended doses of lidocaine, merely based on body weight are not always appropriate. Further studies are needed to determine more precise guideline of maximal doses that include various pharmacokinetic components.

치과용 국소마취제에 포함된 Epinephrine이 당뇨 환자의 소수술시 혈당에 미치는 영향에 관한 연구 (A STUDY ON THE INFLUENCE OF EPINEPHRINE CONTAINED IN DENTAL LOCAL ANESTHETIC AGENTS ON THE BLOOD SUGAR OF DIABETES DURING MINOR ORAL SURGERY)

  • Recently, the number of diabetic patients have been increased with westernized living way and meal habitation. The regulation of blood sugar concentration is very important for diabetic patients to keep homeostasis and, to prevent acute or chronic complications. Epinephrine combined with lidocaine is used in dental clinic extensively. And epinephrine combined with lidocaine also effects on decreasing the blood leakage volume by constricting micro vessle and arteriole. But, So far there are few researches about the effect of epinephrine contained in dental local anesthetic agent on the blood sugar of diabetes during minor oral surgery. The purpose of this study was intended to investigate whether epinephrine which combined with dental local anesthetics influence body glucose level in diabetes patients by glucose monitoring. The subject of this study were 38 diabetic patients and 38 normal adults, each patient was checked body glucose after a meal 2 hours later with resting state, and injection 1:100000 epinephrine with dental lidocaine 54 ml or lidocaine 54 ml only. And then the body glucose level was checked 5 minutes and 30 minutes after injection. The results were analyzed by two way ANOVA test (p<0.05). The results were as follows: In the experimental group 1, the mean of the blood sugar level was 180.3 mg/dl before an anesthetic injection, 182.8 mg/dl after 5minutes of the anesthetic injection and 182.2 mg/dl after 30minutes of the anesthetic injection. The change of the blood sugar level was not statistically by two-way ANOVA tests (p<0.05). In the experimental group 2, the mean of the blood sugar level was 237 mg/dl before an anesthetic injection, 234.5 mg/dl after 5minutes of the anesthetic injection and 231.8 mg/dl after 30minutes of the anesthetic injection. The change of the blood sugar level was not statistically by two-way ANOVA tests (p<0.05). In the controlled group 1, the mean of the blood sugar level was 117.6 mg/dl before an anesthetic injection, 119.1 mg/dl after 5minutes of the anesthetic injection and 129.3 mg/dl after 30minutes of the anesthetic injection. The change of the blood sugar level was not statistically by two-way ANOVA tests (p<0.05). In the controlled group 2, the mean of the blood sugar level was 104.2 mg/dl before an anesthetic injection, 102 mg/dl after 5minutes of the anesthetic injection and 105.3 mg/dl after 30minutes of the anesthetic injection. The change of the blood sugar level was not statistically by two-way ANOVA tests (p<0.05).

Diazepam 전투여와 Lidocaine 투여용량이 혈중농도 및 심혈역학적 변화에 미치는 영향 (The Effect of Lidocaine Dose and Pretreated Diazepam on Cardiovascular System and Plasma Concentration of Lidocaine in Dogs Ansthetized with Halothane-Nitrous Oxide)

  • 전신마취, 부위 또는 국소 마취시, 그리고 향부정맥제로 술중 및 중환자실에서 흔히 쓰이는 lidocaine의 용량과, 국소 마취제로 인한 중추 신경계의 독성을 예방 또는 중단시키기 위해 사용되는 diazepam의 전투여시 이로 인한 심혈관계 변화 및 lidocaine의 혈중농도를 관찰, 측정하여 환자의 관리 및 치료에 도움을 얻고자 본 실험을 하였다. Nitrous oxide, halothane으로 마취된 개에서 근육이완제 사용 후 조절호흡하에서 혈중 이산화탄소를 35-45mmHg로 유지하면서 국소 마취체인 lidocaine의 용량을 100 mcg/kg/min, 200 mcg/kg/min, 300 mcg/kg/min로 각각 30분간 지속적 침제하면서, diazepam 전투여 유무에 따른 심혈 역학치의 변화 및 lidocaine의 혈중농도를 측정하고, 억제된 심혈관계에 $CaCl_2$를 투여하여 회복 정도를 관찰하였던 바 아래와 같은 결과를 얻었다. Lidocaine의 지속적 침제량이 증가됨에 따라 심혈관계의 억제가 심하게 나타났으며 평균 동맥압, 심장지수, 일회 박출지수, 좌심실 박출 작업지수, 우심실 박출 작업지수 등은 감소하였고 (p<0.05), 폐동맥 쐐기압, 중심 정맥압, 전신혈관 저항지수 등은 증가하였으나 (p<0.05), 심박동수, 평균 폐동맥압, 폐혈관 저항지수의 변화는 거의 없었다. Lidocaine 100 mcg/kg/min 지속적 침제시는 diazepam을 투여한 II군에서만 평균 동맥압의 의의있는 감소를 보였으며 (p<0.05), lidocaine의 혈중농도는 diazepam투여하지 않은 I군에서는 $3.97{\pm}0.22$에서 $4.48{\pm}0.36$ mcg/ml 범위였고 II 군에서는 $3.70{\pm}0.32$에서 $4.10{\pm}0.22$ mcg/ml범위였다. Lidocaine 200 mcg/kg/min의 지속적 침제시는 I군에서는 평균 동맥압의 감소, 중심정액압의 증가를 나타내었고 (p<0.05), II 군에서는 심장지수의 감소, 폐동백 쐐기압의 증가를 나타내었으며 (p<0.05), lidocaine의 혈중농도는 I군은 $7.50{\pm}0.66$에서 $7.91{\pm}0.77$ mcg/ml, II군에서는 $7.64{\pm}0.79$에서 $8.23{\pm}1.18$ mcg/ml범위로 증가하였다. Lidocaine 300 mcg/kg/min의 지속적 침제시는 I군에서는 평균동맥압, 일회 박출지수, 좌심실 박출 작업지수의 감소가 있었고 (p<0.05), 폐동맥 쐐기압, 중심 정맥압, 전신혈관 저항지수의 증가를 나타내었다 (p<0.05). II군에서는 심장지수, 일회 박출지수, 좌심실 박출 작업지수의 감소가 있었으며 (p<0.05), 폐동맥 쐐기압, 중심 정맥압, 전신혈관 저항지수의 증가를 나타내었다 (p<0.05). 그러나 심박동수, 폐동맥압의 변화는 거의 관찰할 수 없었다. 또한 이때의 혈중 lidocaine의 농도는 I군에서는 $11.30{\pm}2.11$에서 $11.83{\pm}0.59$ mcg/ml범위 로, II군에서는 $12.95{\pm}0.71$에서 $13.79{\pm}0.82$mcg/ml의 범위로 나타내었다. $CaCl_2$ 투여 후는 억제된 심장지수, 일회 박출지수, 전신혈관 저항지수, 폐혈관 저항지수, 좌심실 박출작업지수 및 우심실 박출 작업지수는 회복시켰으나 (p<0.05), 폐동맥 쐐기압, 중심정맥압 등은 오히려 억제시켰다 (p<0.05). 이상에서 자율신경계에 손상이 없으며 산혈증 및 과탄산증이 없는 개에서는 lidocaine의 높은 혈중 농도치에서도 혈역학에 내성이 있었으며, ljdocaine의 중추신경계 독성의 예방 및 치료 목적으로 사용되는 diazepam의 전투여 시에도 추가적인 심혈관계의 억제가 없음을 알 수 있었다. 그러나 자율신경계 이상이 있거나 자율신경계를 억압할 수 있는 약을 사용할 때 또는 산혈증, 과탄산증 및 저산소증이 동반된 환자에 사용할 때에는 세심한 주의가 요할 것으로 생각된다.

요통, 좌골 신경통 환자에서의 지속적 경막외 차단의 효과 (The Effect of Continuous Epidural Block in Lumbago and Sciatica)

  • Extradural block is a form of treatment described as early as the beginning of the present centuries. It has since had positive criticism from a number of authors in different countries. Epidural injections of steroids with or without local anesthetic have become an occasional method of conservative treatment in sciatica & lumbago, especially in acute case. We assess the results of continuous epidural block with steroids and local anesthetics in sciatica & lumbago. From July 1994 to June 1995, we treated 46 case of lumbago and sciatica using continuous epidural block with steroids and local anesthetics. After placement of 17-Gauge Tuohy needle in the epidural space by the technique of loss of resistance, 0.25% bupivacaine 5 cc and triamcinolone 40 mg was administered and then epidural catheter was placed and connected to multiday infusor(Paragon) using 1% lidocaine with continuous infusion rate of 1 ml/hour. Usually, the catheter was removed after 1~2 weeks and then treated with the physical therapy. At the time of patient's discharge, 69.5% of all cases showed excellent or good results. Of particular note, 26 of the 46 cases were followed up by telephone. At present, in using Of particular note, 26 of the 46 cases were followed up by telephone. At present, in using continuous epidural block procedure, a relief in symptoms showed in 65.5% of these 26 cases. Continuous epidural block provides shortening of the recovery time from pain, avoidance of long period bed rest and early physical therapy and exercise. Therefore, continuous epidural block is simple and safe in the treatment of lumbago and sciatica, especially in acute phase.

비글견에서 앞쪽목신경절 차단술법의 개발 (The Development of Cranial Cervical Ganglion Block in Beagle Dogs)

  • In human, sympathetic nerve blocks with local anesthetics are widely used to treat a variety of diseases in the innervating regions. However, its procedure in dogs is difficult to approach and process repeatedly because of anatomically location. Therefore, this study was designed to develop a new technique of sympathetic nerve block in beagle dogs. Fifteen healthy beagle dogs, which did not show any neurologic abnormalities and disease, were used for the study. Radiograghs were taken after injected radiopaque material mixed with 2% lidocaine at the cranial cervical ganglion and injected methylene blue using the same percutaneous technique to verify the reliability of this newly developed technique. The successful block rate of the cranial cervical ganglion block was present in 80% of all dogs and the stained cranial cervical ganglions were shown in all dogs. The results show that this new technique of the cranial cervical ganglion block is a reliable and simple method that can be used for clinical studies in dogs.
