발작성야간혈색소뇨증 1례 (A Case of Paroxysmal Nocturnal Hemoglobinuria)

  • Paroxysmal nocturnal hemoglobinuria (PNH) is an uncommon hematologic disease characterized by an abnormal sensitivity of blood cells to the lytic action of serum complement. We experienced one case of PNH in Yeungnam University Hospital from May 1983 to May 1989. The patients was followed up without severe complications for 4 years since diagnosis with the only consevating treatments such as washed blood transfusion, adrenal corticosteroids, androgens, folate and iron preparation, intermittently.

다배란 용량의 임마혈청성 고나도트로핀(PMSG)이 랫트의 자궁내 미세환경에 미치는 영향 (The effects of superovulatory doses of pregnant mare serum gonadotropin on uterine microenvironment of the rat)

  • 고나도트로핀제재에 의한 다배란처치는 난소 스테로이드 호르몬(estrogens, progestins 및 androgens)의 정교한 균형을 깨트림으로써 자궁에 바람직하지 못한 영향을 미친다. 따라서 이러한 다배란처치가 자궁조직에 미치는 영향을 검사하기 위하여, 28일령의 189마리 미성숙래트에 4IU, 20IU 또는 40IU의 임마혈청성 고나도트로핀 (PMSG)을 투여하고, 그후 10일까지 매 24시간 간격으로 실험동물을 희생시켰다 자궁조직의 장기적 효과는 4IU 또는 40IU의 PMSG를 투여한 12마리의 래트를 30일째에 희생시켜 검사하였다. 그리고 일부 성숙래트의 자궁은 PMSG를 투여한 미성숙래트의 자궁과 비교하는데 공시되었다. PMSG투여후 2일부터 5일까지 대조군(4IU) 자궁의 형태학적, 조직학적 변화는 성숙래트의 발정주기 동안의 변화와 거의 동일하였다. 그러나 대배란처치용량(20IU 또는 40IU)의 PMSG는 투여 후 2일째에는 자궁 간질조직의 hypertrophy와 3일째에는 자궁내강 상피세포의 focal papillary hyperplasia를 형성시켰다. 20IU와 40IU의 PMSG를 투여한 후 $17{\beta}$-estradiol 혈중농도는 투여 1일 후에 대조군(4IU)보다 현저하게(P<0.005 및 P<0.05) 증가하였고, androgen농도는 투여 1일 후에 baseline으로부터 현저하게(P<0.05 및 P<0.005) 증가하여 2일과 3일 사이에 최고에 도달하였다. 20IU PMSG 투여군에 있어서, hyperplasia현상은 투여 3일 후부터 점차 퇴행되어 10일까지는 완전히 소멸되었다. 그러나 40IU PMSG투여군에서의 hyperplasia는 투여 6일 후까지 뚜렷이 진행되었다. 이러한 결과는 혈중 estrogen 농도의 상승과 밀접한 관계가 있는 것으로 사료된다. 왜냐하면 40IU PMSG투여군에 있어서의 $17{\beta}$-estradiol 혈중농도가 투여 4일 후에 최고에 도달하였으며, 이는 4IU PMSG를 투여한 대조군과 20IU PMSG 투여군보다 현저하게(P<0.001) 높았기 때문이다. 그리고 40IU PMSG투여군에 있어서의 hyperplasia 현상은 투여 6일과 10일 사이에 약간씩 퇴행됨을 보였고, 30일까지는 완전히 소멸되었다. 본 연구결과는, 다배란처치로 인하여 난조조직으로부터 과잉 분비되는 estrogen 및 androgen에 대한 자궁조직의 사전노출이, 위에서 언급한 비정상적 hyperplasia형성의 가능한 원인적 요소가 됨을 시사한다.

MALDI Mass Spectrometric Analysis of Nonderivatized Steroids Using Cyclodextrin-supported 2,5-Dihydroxybenzoic Acid as Matrix

  • Sex hormones are important metabolites in vertebrates' development and reproduction. For rapid screening sex hormones, matrix-assisted laser desorption/ionization (MALDI) mass spectrometry (MS) is one of the promising analytical platforms, but MALDI MS faces many challenges in detecting steroids such as low ionization efficiency and matrix background interference. One potential strategy to overcome matrix interference in the low m/z region is using a cyclodextrin (CD)-supported matrix for steroid analysis since CD-supported matrixes are known to effectively suppress matrix-related ion signals. In this study, we aimed to find the optimal CD-supported matrix for the analysis of the nonderivatized sex steroids. Our results showed that the ${\alpha}CD$-supported 2,5-dihydroxybenzoic acid (DHB) matrix efficiently ionized all three major classes of sex hormones, estrogens, androgens, and progestagens, with low or no matrix background and also with high sensitivity. In addition, the ${\alpha}CD$-supported DHB matrix mainly generated molecular ions or protonated ions of sex hormones, and this enabled us to obtain information-rich tandem mass spectra which potentially lead to unambiguous identification of steroid species from complex metabolite mixtures.

Significant Association of Metabolic Indices, Lipid Profile, and Androgen Levels with Prostate Cancer

  • Objectives: To compare the metabolic indices, lipid profile, androgens, and prostate specific antigen between prostate cancer and BPH and between grades of prostate cancer in a cross-sectional study. Materials and Methods: The study enrolled 95 cases of prostate cancer and 95 cases of benign prostatic hyperplasia (BPH). Prostate gland volume was measured using transrectal ultrasound. We compared insulin, testosterone, dihydrotestosterone, prostate specific antigen levels and lipid profile between prostate cancer of different grades and BPH. Further, prostate cancer patients were classified into low grade and high grade. Unpaired t-test for normally distributed variables and Man-Whitney U test for non-normal variables were used to assess differences. Results: We found that prostate cancer patients had significantly higher levels of insulin, testosterone, PSA, cholesterol, triglycerides, low-density lipoprotein (LDL) and very low-density lipoprotein (VLDL) in comparison to their BPH counterparts. Higher levels of these parameters also correlated with a higher grade of the disease. Conclusions: We conclude that higher levels of insulin, testosterone, PSA, and cholesterol correlate with a higher risk of prostate cancer, and also with a higher grade of the disease.

A case of 17 alpha-hydroxylase deficiency

  • $17{\alpha}$-hydroxylase and 17,20-lyase are enzymes encoded by the CYP17A1 gene and are required for the synthesis of sex steroids and cortisol. In $17{\alpha}$-hydroxylase deficiency, there are low blood levels of estrogens, androgens, and cortisol, and resultant compensatory increases in adrenocorticotrophic hormone that stimulate the production of 11-deoxycorticosterone and corticosterone. In turn, the excessive levels of mineralocorticoids lead to volume expansion and hypertension. Females with $17{\alpha}$-hydroxylase deficiency are characterized by primary amenorrhea and delayed puberty, with accompanying hypertension. Affected males usually have female external genitalia, a blind vagina, and intra-abdominal testes. The treatment of this disorder is centered on glucocorticoid and sex steroid replacement. In patients with $17{\alpha}$-hydroxylase deficiency who are being raised as females, estrogen should be supplemented, while genetically female patients with a uterus should also receive progesterone supplementation. Here, we report a case of a 21-year-old female with $17{\alpha}$-hydroxylase deficiency who had received inadequate treatment for a prolonged period of time. We also include a brief review of the recent literature on this disorder.

Empirical medical therapy in idiopathic male infertility: Promise or panacea?

  • Male factors account for 20%-50% of cases of infertility and in 25% of cases, the etiology of male infertility is unknown. Effective treatments are well-established for hypogonadotropic hypogonadism, male accessory gland infection, retrograde ejaculation, and positive antisperm antibody. However, the appropriate treatment for idiopathic male infertility is unclear. Empirical medical treatment (EMT) has been used in men with idiopathic infertility and can be divided into two categories based on the mode of action: hormonal treatment and antioxidant supplementation. Hormonal medications consist of gonadotropins, androgens, estrogen receptor blockers, and aromatase inhibitors. Antioxidants such as vitamins, zinc, and carnitines have also been widely used to reduce oxidative stress-induced spermatozoa damage. Although scientifically acceptable evidence of EMT is limited because of the lack of large, randomized, controlled studies, recent systematic reviews with meta-analyses have shown that the administration of gonadotropins, anti-estrogens, and oral antioxidants results in a significant increase in the live birth rate compared with control treatments. Therefore, all physicians who treat infertility should bear in mind that EMT can improve semen parameters and subsequent fertility potential through natural intercourse.

Morphine induced inhibition of the activities of accessory reproductive ducts in male rats

  • Adult male albino rats were treated with 0.5 mg and 0.75 mg morphine/100 g body weight intraperitoneally for 30 days. All the animals were autopsied on $31^{st}$ day. Epididymis and vas deferens were dissected out, weighed and processed for histological and biochemical studies. Morphine has caused a reduction in the weight of epididymis and vas deferens in both the doses of drug treated groups. The total cholesterol content is increased while protein, DNA and RNA contents and epididymal sperm counts are decreased. The acid phosphatase content is decreased 10.12 $\pm$ 0.11 in caput, 9.26 $\pm$ 0.30 in cauda of epididymis and in vas deferens 8.14 $\pm$ 0.15 in 0.5 mg treated groups and in 0.75 mg treated rats shows 9.52 $\pm$ 0.27 in caput, 9.14 $\pm$ 0.18 in cauda of epididymis and in vas deferens 7.84 $\pm$ 0.11is decreased, whereas alkaline phosphatase is increased. The surface epithelial cell height of these ducts is reduced and secretory activity is inhibited with the disruption of epithelial cell projections. The gravimetric and histometric changes of epididymis and vas deferens may be due to non-availability of androgens in morphine treated rats.

Induction of Integrin Signaling by Steroid Sulfatase in Human Cervical Cancer Cells

  • Steroid sulfatase (STS) is an enzyme responsible for the hydrolysis of aryl and alkyl sulfates. STS plays a pivotal role in the regulation of estrogens and androgens that promote the growth of hormone-dependent tumors, such as those of breast or prostate cancer. However, the molecular function of STS in tumor growth is still not clear. To elucidate the role of STS in cancer cell proliferation, we investigated whether STS is able to regulate the integrin signaling pathway. We found that overexpression of STS in HeLa cells increases the protein and mRNA levels of integrin ${\beta}1$ and fibronectin, a ligand of integrin ${\alpha}5{\beta}1$. Dehydroepiandrosterone (DHEA), one of the main metabolites of STS, also increases mRNA and protein expression of integrin ${\beta}1$ and fibronectin. Further, STS expression and DHEA treatment enhanced phosphorylation of focal adhesion kinase (FAK) at the Tyr 925 residue. Moreover, increased phosphorylation of ERK at Thr 202 and Tyr 204 residues by STS indicates that STS activates the MAPK/ERK pathway. In conclusion, these results suggest that STS expression and DHEA treatment may enhance MAPK/ERK signaling through up-regulation of integrin ${\beta}1$ and activation of FAK.

Expression of Human Liver 3,4-Catechol estrogens UDP-Glucuronosyltransferase cDNA in COS 1 Cells

  • The human cDNA clone UDPGTh2, encoding a liver UDP-glucuronosyltransferase (UDPGT), was isolated from a .gamma.gt 11 cDNA library by hybridization to mouse transferase cDNA clone, UDPGTm1. The two clones had 74% nlicleotide sequence identities in the coding region. UDPGTh2 encoded a 529 amino acid protein with an amino terminus membrane-insertion signal peptide and a carboxyl terminus membrane-spanning region. In order to establish substrate specificity, the clone was inserted into the pSVL vector (pUDPGTh2) and expressed in COS 1 cells. Sixty potential substrates were tested using cells transfected with pUDPGTh2. The order of relative substrate activity was as follows: 4-hydroxyestrone > estriol >2-hydroxyestriol > 4-hydroxyestradiol > $6{\alpha}$-hydroxyestradiol >$5{\alpha}$-androstane-$3{\alpha}$, $11{\beta}$, $17{\beta}$-triol=5${\beta}$-androstane-$3{\alpha}$ ${\beta}$, $17{\beta}$-triol. There were only trace amounts of gulcuronidation of 2-hydroxyestradiol and 2-hydroxyestrone, and in contrast to other cloned transferase, no gulcuronidation of either the primary estrogens and androgens (estrone, $17{\beta}$estradiol/testosterone, androsterone) or any of the exogenous substrates tested was detected. A lineweaver-Burk plot of the effect of 4-hydroxystrone concentration on the velocity of glucuronidation showed an apparent Km of $13{\mu}M$. The unique specificity of this transferase might play an important role in regulating the level and activity of these potent and active estrogen metabolites.

Premarin검사를 이용한 무월경환자에서의 시상하부-뇌하수체의 기능평가 (Evaluation of the Hypothalamic-pituitary Function by Premarin Test for the Patients of Amenorrhea)

  • Hypothalamic-pituitary function in patients of 6 selected groups of amenorrhea was evaluated by performing premarin test. Selected amenorrheic patients were divided into 6 groups of Turner's syndrome(5), adrenogenital syndrome(3), Sheehan's syndrome(4), moderate hyperprolactinemia(3), severe hyperprolactinemia(9) and functional oligoamenorrhea(9) the diagnoses of which were performed according to modified our own protocol for management of amenorrheic patients. As control 20 normally cycling women in mid follicular phase determined by their symptothermal charts during last 6 months designed by WHO were compared. The premarin test which is one of the tests evaluating the hypothalamic-pituitary function by the principle of negative and positive feed back effect's of estrogen was performed by injecting 20 mg of premarin in volus intravenously. The levels of serum LH before, 24, 48, 72, 96 and 120 hours after injection of premarin were measured by double antibody technique radioimmunoassay the reagents of which were supplied by WHO. The results were as follows: 1. Both negative and positive feed back effects by exogenous estrogen were well preserved even in the patients of gonadal dysgenesis although the baseline levels were much higher than normal. 2. In the patients of Sheehan's syndrome one could observe the minimal response of feed back effect in the case with minimal pituitary function. 3, Androgens in adrenogenital syndrome and prolactin in hyperprolactinemia may suppress mainly the positive feed back effect rather than the negative one. The suppressive effect can be abolished by proper treatments which can eliminate those suppressive hormones. 4. This premarin test may be beneficial for predicting the result of clomiphene in ovulation induction.
