철도 인적오류 분석방법론 개발 (A Methodology for Analyzing Human Error in Railway Operations)

  • Although human error is recognized as the primary cause of railway accidents and incidents, there have been limitations in finding the root causes of errors and developing effective corrective actions in the Korean railway industry, due to the absence of a systematic method and lack of professional knowledge and skills of investigators. Therefore, there has been a strong need for a systematic methodology for human error analysis. This paper introduces a methodology for analyzing human error m railway operations, called HEAR (Human Error Analysis and Reduction). HEAR is intended to help analysts identify the sequences and various levels of causes of operators' erroneous actions in railway accidents or incidents and make recommendations to eliminate or reduce the future possibility of similar errors and accidents. It was developed based on a thorough investigation of various techniques for human errors analysis and feedback from field investigators.

짚라인의 안전관리 실태와 개선방향에 관한 연구(I) (A Study on Status Investigation and Improvement for Safety Management of Zipline(I))

  • Recently, the number of facilities and users of zipline which is the most popular among extreme leisure facilities has been increasing year by year. Zipline is a means of transporting durable wire between two timber or stay and moving the trolley connected to the passenger at high speed in the opposite direction. However, due to the nature of the zip line facility, the risk of accidents is always present and safety accidents are frequently occurring. Therefore, this study investigates the current state of safety management and extract the problem in terms of safety management through analyzing the installation state of zipline facilities in the case of accidents and reviewing of similar facility regulations. In order to solve this problem, we will contribute to establishment of comprehensive and systematic countermeasures and establishment of safety improvement system by suggesting measures to improve safety management through analyzing foreign regulations and safety management regulations for zipline and other extreme leisure facilities.

  • 우리나라 자동차의 역사는 약 60년 정도로 주요 선진국에 비해 자동차의 역사는 짧지만 자동차와 연관되어 일어나는 교통사고의 발생률이나 심각도를 고려한다면 교통사고를 줄이기 위한 여러 가지 노력이 절실히 필요하다. 따라서 교통사고로 인해 발생되는 여러 가지 문제는 단순 개인의 피해를 넘어 사회적 문제로서 대두되고 있으므로 이를 해소하기 위해 교통사고 원인을 분석하는 작업은 무엇보다도 중요하다. 본 연구는 최근 2년 4개월간 SBS 방송 프로그램에서 일반 대중을 대상으로 방송한 차량용 영상기록장치(DVR, Driving Video Recorder) 제보 교통사고 영상을 통해 수집된 다양한 교통사고의 원인 중 하나인 운전자의 행태를 중심으로 분석하여, 교통사고 발생을 감소시킬 수 있는 방안을 제시하는 것을 목적으로 하고 있다. 이 연구는 교통사고 원인을 단순 데이터 중심으로 분석하고자 하는 기존의 논문과는 달리 다양한 센싱 정보와 비교적 고해상도의 영상정보 수집 내용을 바탕으로 교통사고 발생 시점 전후의 상황을 명확히 저장하고 있는 차량용 영상기록장치 영상을 개별적으로 일일이 직접 분석한 결과로 데이터베이스를 구축하여 이에 대한 통계학적 분석을 통해 실효성 있는 교통사고 감소 방안을 제시하고자 하였다.

  • In this paper, we proposed a simulator for reconstructing and analyzing vessel collision accidents happened at sea and we verified validity of the system by applying it to some collision accidents. This system will offer us to advantage of time and human power by contributing to automation of judge's decision.

  • In this paper, we propose the application of the Human Factors Analysis and Classification System(HFACS) to analyze an aircraft accident data. HFACS is a general human error framework originally developed and tested within the U.S military as a tool for investigating and analyzing the human causes of aviation accidents. It was examined that HFACS reliably accommodate all human causal factors associated with the commercial accidents. We found that the HFACS could be used as a reliable tool for investigating aircraft accidents including a single accident analysis.

  • Despite the government's various safety measures, the number of safety accidents continues to rise and the interior is more likely to occur indoors than outdoors. Safety accidents involving young children account for more than 70 % of the safety accidents in the safety accidents, and the ratio of safety accidents in the classroom or classroom is more than 50 %. In this thesis the author proposes the system managing the accidents notification service using LED lighting and access entry control. Utilizing IoT technology, remote control or access can be controlled remotely by controlling multiple lights and entrances from each room and by means of a number of lighting and entrances. By monitoring and analyzing real-time status via server PC and mobile interface, it can control the maximum control and incident prevention, automatic control, and automatic control.

선박 충돌사고 분석을 위한 시뮬레이터 구축 (Building of a Simulator for Analyzing Ship's Collision Accidents)

  • 해상에서 발생한 선박충돌사고 재결과정에서 당사자들의 진술을 통하여 해도나 플로팅용지에 충돌선박의 항적을 플로팅하는 과정을 삼각자 및 디바이더를 이용한 수작업에 의존함으로써 많은 시간이 소모되고 오류가 발생하는 문제점이 대두된다. 따라서 본 논문에서는 다양한 충돌상황에서도 충돌선박 당사자들의 진술내용을 컴퓨터에 입력하여 초인시부터 충돌에 이르기까지 양선박의 시간대별 거동을 그래픽과 수치 및 텍스트로 제시함으로써 충돌당사자 진술의 진위여부를 판단할 수 있고 해양안전심판의 정확도도 높일 수 있는 충돌사고 분석시뮬레이터를 제안하였으며, 해상선박충돌사고 실례들을 시뮬레이터에 적용하여 그 유효성을 확인하였다.

Automated Construction Activities Extraction from Accident Reports Using Deep Neural Network and Natural Language Processing Techniques

  • Construction is among the most dangerous industries with numerous accidents occurring at job sites. Following an accident, an investigation report is issued, containing all of the specifics. Analyzing the text information in construction accident reports can help enhance our understanding of historical data and be utilized for accident prevention. However, the conventional method requires a significant amount of time and effort to read and identify crucial information. The previous studies primarily focused on analyzing related objects and causes of accidents rather than the construction activities. This study aims to extract construction activities taken by workers associated with accidents by presenting an automated framework that adopts a deep learning-based approach and natural language processing (NLP) techniques to automatically classify sentences obtained from previous construction accident reports into predefined categories, namely TRADE (i.e., a construction activity before an accident), EVENT (i.e., an accident), and CONSEQUENCE (i.e., the outcome of an accident). The classification model was developed using Convolutional Neural Network (CNN) showed a robust accuracy of 88.7%, indicating that the proposed model is capable of investigating the occurrence of accidents with minimal manual involvement and sophisticated engineering. Also, this study is expected to support safety assessments and build risk management systems.

인적오류가 관여된 철도 사고의 체계적 분석을 위한 FRAM의 활용 (A FRAM-based Systemic Investigation of a Rail Accident Involving Human Errors)

  • There has been a significant decline in the number of rail accidents in Korea since system safety management activities were introduced. Nonetheless, analyzing and preventing human error-related accidents is still an important issue in railway industry. As a railway system is increasingly automated and intelligent, the mechanism and process of an accident occurrence are more and more complicated. It is now essential to consider a variety of factors and their intricate interactions in the analysis of rail accidents. However, it has proved that traditional accident models and methods based on a linear cause-effect relationship are inadequate to analyze and to assess accidents in complex systems such as railway systems. In order to supplement the limitations of traditional safety methods, recently some systemic safety models and methods have been developed. Of those, FRAM(Functional Resonance Analysis Method) has been recognized as one of the most useful methods for analyzing accidents in complex systems. It reflects the concepts of performance adjustment and performance variability in a system, which are fundamental to understanding the processes of an accident in complex systems. This study aims to apply FRAM to the analysis of a rail accident involving human errors, which occurred recently in South Korea. Through the application of FRAM, we found that it can be a useful alternative to traditional methods in the analysis and assessment of accidents in complex systems. In addition, it was also found that FRAM can help analysts understand the interactions between functional elements of a system in a systematic manner.

  • Many industrial disasters and safety-accidents are occurred constantly in current industrial society of domestic country. Although there are many attempts to reduce the occurrence of various industrial disasters and safety-accidents through exhaustive safety trainings and strict rules applied to fieldwork in electrical construction, still those accidents are not eradicated completely. The purpose of this study is to take counter measures against similar or same safety-accidents by analyzing the types of accidents and reasons among critical accident cases, which are occurred in electrical construction field.
