Various autogenous fresh demineralized tooth forms for alveolar socket preservation in anterior tooth extraction sites: a series of 4 cases

  • The aim of this study was to evaluate the clinical relevance of autogenous fresh demineralized tooth (Auto-FDT) prepared at chairside immediately after extraction for socket preservation. Teeth were processed to graft materials in block, chip, or powder types immediately after extraction. Extraction sockets were filled with these materials and dental implants were installed immediately or after a delay. A panoramic radiograph and a conebeam CT were taken. In two cases, tissue samples were taken for histologic examination. Vertical and horizontal maintenance of alveolar sockets showed some variance depending on the Auto-FDT and barrier membrane types used. Radiographs showed good bony healing. Histologic sections showed that it guided good new bone formation and resorption pattern of the Auto-FDT. This case series shows that Auto-FDT prepared at chairside could be a good material for the preservation of extraction sockets. This study will suggest the possibility of recycling autogenous tooth after immediate extraction.

Vasovagal Syncope before General Anesthesia in a Patient with Alveolar Bone Deficiency on Maxilla and Mandible (상하악 치조골 결손 환자에서 전신 마취 전 발생한 혈관미주신경성 실신)

  • Vasovagal syncope accounts for the majority of adverse events that occur in dental offices and is normally related to emotional or somatic factors. The factors trigger intense parasympathetic state, leading to bradycardia and hypotension. We experienced a case of vasovagal syncope just before general anesthesia. A 49-year-old woman with alveolar bone deficiency on maxilla and mandible was planned to undergo an alveolar bone graft with mandibular body under general anesthesia. She didn't have any histories of disease, medication or syncope. Though she showed a little anxiety from admission, she had no pre-operative medication. After she was guided to the operating room, she had signs and symptoms of vasovagal syncope without any prodromes. The patient was resuscitated soon only by the conservative treatment and was operated under general anesthesia.

Clinical Comparative Study for Maxillary Sinus Augmentation Using Deproteinized Bovine Bone Mineral and Mineralized Allograft (탈단백우골과 비탈회 동종골을 사용한 상악동 거상술의 임상적 연구)

  • Purpose: The purpose of this study was to compare the clinical efficacy of popular bone graft materials mineralized allograft and deproteinized bovine bone mineral. Materials and Methods: One hundred seven implants of 78 patients, accompanied by sinus lift using the lateral window technique and simultaneous implantation, were sampled. In addition, some patients with severe systemic conditions were excluded. The initial bone heights of all patients ranged from 3-6 mm. All of the sample cases were treated at our hospital from January 2005 to January 2008. Techniques other than the lateral window technique were excluded, and only one graft material ($Tutoplast^{(R)}$ or Bio-$Oss^{(R)}$) was accepted for inclusion. $Tutoplast^{(R)}$ was used in 63 implants of 41 patients, whereas Bio-$Oss^{(R)}$) was chosen for the remaining 44 implants of 37 patients. The diameters of the particles used ranged from 0.25-1.0 mm, and the volume was 0.5-2 cc (mean, 1.5 cc). Results: The survival rate of the implant fixtures was 99.07% when the lateral window technique was used. Among all of the cases, cases in which $Tutoplast^{(R)}$ was used demonstrated a survival rate of 98.4%, whereas Bio-$Oss^{(R)}$) resulted in 100% survival. With respect to the alveolar bone height, no significant differences were detected between the two graft materials that failed. Conclusion: According to the result reported above, the two common materials for sinus augmentation do not have clinically significant difference. Rather, host factors, such as the height of residual bone, which could be disclosed during questioning patients' systemic conditions, might have greater effects on the prognosis.

Scanning Electron Microscopy and Energy Dispersive X-ray Spectroscopy Studies on Processed Tooth Graft Material by Vacuum-ultrasonic Acceleration

  • Purpose: The current gold standard for clinical jawbone formation involves autogenous bone as a graft material. In addition, demineralized dentin can be an effective graft material. Although demineralized dentin readily induces heterotopic bone formation, conventional decalcification takes three to five days, so, immediate bone grafting after extraction is impossible. This study evaluated the effect of vacuum ultrasonic power on the demineralization and processing of autogenous tooth material and documented the clinical results of rapidly processed autogenous demineralized dentin (ADD) in an alveolar defects patient. Methods: The method involves the demineralization of extracted teeth with detached soft tissues and pulp in 0.6 N HCl for 90 minutes using a heat controlled vacuum-ultrasonic accelerator. The characteristics of processed teeth were evaluated by scanning electron microscopy (SEM), and energy dispersive X-ray spectroscopy (EDS). Bone grafting using ADD was performed for narrow ridges augmentation in the mandibular area. Results: The new processing method was completed within two hours regardless of form (powder or block). EDS and SEM uniformly demineralized autotooth biomaterial. After six months, bone remodeling was observed in augmented sites and histological examination showed that ADD particles were well united with new bone. No unusual complications were encountered. Conclusion: This study demonstrates the possibility of preparing autogenous tooth graft materials within two hours, allowing immediate one-day grafting after extraction.

Alveolar ridge preservation using granulation tissue for esthetic implant restoration on maxillary anterior tooth (상악 전치부의 심미적 임플란트 수복을 위한 육아 조직(Granulation tissue)을 이용한 치조제 보존술)

  • Esthetic factors are very important in the success of maxillary anterior implant restoration. However, achieving esthetic results is difficult, especially in cases where periodontitis has resulted in severe alveolar bone loss. In the case of maxillary anterior teeth, the alveolar ridge resorption that begins immediately after tooth extraction interferes with the esthetic implant restoration. Therefore immediate implant placement can be performed to minimize the alveolar ridge resorption. However, in severe bone loss cases, immediate implant placement could result in esthetic failure, and this result might cause irreparable problems. We can also perform alveolar ridge preservation and then place implants later. On JCP published in 2019, there is the consensus of European academy of periodontology on the extraction socket management and the timing of implant placement. This consensus states that alveolar ridge preservation should be considered when there is severe labial bone loss in an esthetically important area such as maxillary anterior region. On performing the alveolar ridge preservation, we cannot obtain the primary wound closure, so secondary wound healing is induced with open membrane technique or soft tissue grafting should be performed for primary wound closure. However, the secondary wound healing can have a negative impact on bone regeneration, and soft tissue grafting such as FGG or CT graft can be burdensome for both patients and dentists. On the other hand, by using the granulation tissue in the extraction socket, primary closure can be achieved without soft tissue grafting. Also some studies have shown that granulation tissue in periodontal defects contains stem cells that may help in tissue regeneration. Based on this, implant restorations were performed on maxillary anterior teeth with severe alveolar bone loss by alveolar ridge preservation using granulation tissue. In spite of the severe bone defect of the extraction socket, relatively esthetic results could be obtained in implant restorations.

Results of Maxillary Sinus Elevation for Endosseous Implant Placement (임플란트 식립을 위한 상악동 점막 거상술의 결과)

  • Background: Although dental implantation has become widespread and acceptable treatment for dental prosthodontics, maxillary posterior jaw region is often complicated by the pneumatization of the maxillary sinus and physiological resorption of the alveolar bone. When this occurs, the residual bone between the floor of the sinus and the crestal ridge is inadequate for the placement of implants. The sinus elevation procedure provides a way to increase the amount of available bone and to allow the placement of longer implants. Materials & methods: We studied 11 patients requiring the implant placements and the maxillary sinus elevation simultaneously from 1996 to 2003 in our clinic. Nine patients were males and two patients were females, aged from 39 to 72(mean=51.6). Four patients had medical compromised states; angina pectoris, diabetes, hypertension, hepatitis. Patients didn't show any pathologic findings clinically or radiographically. We studied the success and survival rate of implants, factors increasing the osseointegrating capacity of implants. Results: The success rate of osseointegration of implants was 93%. At least 6 months after loading on implants, the survival rate of implants was 78.5%. Autogenous bone graft and adequate residual bone height(>6mm) increased survival rate of implants. Conclusion: Successful implant placement with maxillary sinus elevation mainly depends on sufficient residual bone height, healthy maxillary sinus, autogenous bone graft.

Functional Endoscopic Sinus Surgery for a Patient with Maxillary Sinusitis Occurring after Implant Placement

  • Maxillary sinus membrane elevation and bone graft have been performed routinely in alveolar bone with insufficient residual bone height. There are a number of causes for development of maxillary sinusitis after these procedures. When maxillary sinusitis is caused by sinus membrane elevation, bone graft, and implant placement, various treatment such as medication, incision and drainage (I&D), implant removal, and the Caldwell-Luc procedure can be considered. Removal of an implant or the Caldwell-Luc procedure can be harmful if inflammation is not present in the oral cavity and survival of grafted bone and implant osseointegration can be expected despite the presence of maxillary sinusitis. In this case, functional endoscopic sinus surgery, which was often used in the otorhinolaryngology department, was performed without removal of the implant for a patient with maxillary sinusitis after one month following implant placement. Thus, we report on this case with a review of the literature.

Novel design of implant prosthesis considering esthetics and periodontal maintenance: case report (심미성 및 치주 조직의 유지 관리를 고려한 임플란트 보철 증례)

  • After extraction of tooth, alveolar ridge resorption is inevitable in most cases. Clinicians confront with horizontal and vertical resorption of alveolar bone. Without massive amount of bone and soft tissue graft, dental implant will be placed apically to gain stability. In those cases, not only white esthetic part, but also pink esthetic part should be restored with pink porcelain. The margin of prosthesis should be located apically to reproduce natural look with adjacent teeth. However, when the margin is located apically, it is always hard to remove remaining cement, thus complications may arise. In this report, a novel design of implant prosthesis is introduced to solve those issues. The novel design is consisted of zirconia framework with pink porcelain and separate crowns on top of the framework. It eliminates the possibility of cement remnants by bringing the crown margin coronally. Pink esthetic part is incorporated in abutment part instead of crown part and the screw hole is covered with separate crowns.

Orthodontic treatment and management of adult patient with cleft lip and palate (성인 구순구개열환자의 교정치료 및 관리)

  • Patients with cleft lip and palate require interdisciplinary treatment to achieve successful rehabilitation. However, there are special difficulties in orthodontic treatment of adult cleft lip and palate patients: 1. Lack of Tissue, Bone, and Soft tissue; 2. Heavy Scar Tissue, Vestibule, and Palate; 3. Severe Anteroposterior discrepancy and Impaired Maxilla; 4. Distortion of Alveolar Ridge; 5. Abnormal Eruption Path and Malalignment of Tooth. Solving these problems, orthodontist should have differential diagnosis on extent of cleft site and residual deformities of adult cleft lip and palate patient. The tooth missing area in cleft site was commonly treated with a removable or fixed prosthesis, but this method is not stable to retain maxillary arch shape. To establish the more stable arch shape in cleft lip and palate, endosseous implants in the alveolar clefts with bone graft is helpful for management of adult cleft lip and palate patient.

EFFECT OF COMPOSITE GRAFT OF CALCIUM CARBONATE AND CALCIUM SULFATE ON THE PERIODONTAL REGENERATION OF 3-WALL INTRABONY DEFECTS OF ADULT DOGS (성견의 3면 골내낭에 calcium carbonate와 calcium sulfate의 혼합이식이 치주조직 치유에 미치는 영향)

  • Synthetic bone graft materials have been used for the regeneration of periodontal tissue lost due to periodontal disease, but the limitations of these materials had prompted the use of composite grafts. Among those, a composite graft of calcium carbonate(CC) and calcium sulfate(CS) is one of those materials that has not been studied extensively. CC, which is extracted from a natural coral, is known to possess osteoconductive property. SC can play an adjunctive role in the regeneration of bone tissue, and has shown good resorbability and biocompatibility. This study was conducted in order to investigate the effects of CC and CS composite graft to the regeneration of bone in the intrabony defects of dogs. 3-wall intrabony defects ub size of $4mm{\times}4mm{\times}4mm$ were created in the alveolar bone in the premolar areas. Then those defects that were treated with root planning only were designated as control, while the experimental group 1 and 2 each received the CC and CS composite grafts in the ratio of 8 : 2 and 5 : 5 the animals were sacrificed after 8weeks and the specimens were histologically analyzed. The results were as follows ; 1. No inflammation or foreign body reaction were observed in all subjects. CS has not been seen due to complete resorption, and resorption pattern of CC was observed. 2. Significant differences(p<0.05) in new cementum formation were observed between control($1.42{\pm}0.64mm$) and experimental groups(group 1 ; $2.53{\pm}0.94mm$, group 2 ; $2.23{\pm}0.96mm$) but the difference between the two experimental groups was not significant. 3. Significant differences(p<0.01) in new bone formation were observed between control($0.59{\pm}0.55mm$) and experimental groups(group 1 ; $2.27{\pm}0.61mm$, group 2 ; $2.05{\pm}0.56mm$) but the difference between the two experimental groups was not significant. 4. The extent of apical epithelial migration has shown no significant difference between control($1.18{\pm}1.24mm$) and experimental groups(group 1 ; $0.51{\pm}0.54mm$, group 2 ; $0.73{\pm}0.70mm$). 5. The extent of bone formation was generally limited to the extent of cementum formation for all groups, and significant correlation was found in the amount of bone formation and cementum formation in experimental group 1.(Co.=0.86, p<0.01) These results suggest that the composite graft of CC and CS is biocomplatible and effective in the new bone and new cementum formations. In the case of 3-wall intrabony defects of dogs, the composite ratio of 8 : 2 and 5 : 5 had shown no significant differences in the healing.
