수축성 심막염에 대한 심막절제술 전후의 수술방법에 따른 혈역학적 비교 (Pre- and Postoperative Hemodynamic Studies in the Patients with Constrictive Pericarditis)

  • 15 patients with constrictive pericarditis who underwent interphrenic pericardiectomy from January, 1981 to April, 1983 and 11 patients who underwent radical pericardiectomy from May, 1983 to September, 1984 were compared to the clinical improvement and the results of pre- and postoperative cardiac catheterization. In the group of partial pericardiectomy the pericardium was removed anteriorly from the left phrenic nerve to the right phrenic nerve and in the group of radical pericardiectomy the pericardium was removed from almost entire surface of the heart including diaphragmatic surface and posterior wall of the left ventricle. The following results were obtained. 1. Both group of the patients showed marked symptomatic improvement early after operation. 2. The central venous pressure was decreased significantly after operation in both group of the patients. 3. The right atrial mean pressure and pulmonary arterial mean pressure decreased significantly after operation in both group of the patients and there was no significant difference in the amplitude of decrease between the two groups. 4. The right ventricular end-diastolic pressure and left ventricular end-diastolic pressure were decreased postoperatively in both group of the patients and the patients of the radical pericardiectomy showed more decrease than the patients of interphrenic pericardiectomy, and in the group of radical pericardiectomy the right and left ventricular end-diastolic pressure were normalized postoperatively but in the group of partial pericardiectomy they showed abnormally high pressure persistently. 5. The ejection fraction showed normal level pre- and postoperatively in both group of the patients.

Imaging Diagnosis of Emphysematous Pyelonephritis in a Non-Diabetic Dog

  • An 11-year-old, 4.3 kg, mixed breed, intact female dog exhibiting vomiting and a reduced appetite was presented. On physical examination, palpation of the right kidney elicited discomfort. Blood analysis revealed leukocytosis with neutrophilia, and elevation of alkaline phosphatase and blood urea nitrogen. Escherichia coli was detected in urine culture, whereas urine analysis showed absence of glucose. Radiography and ultrasonography suggested that there was gas in the right renal parenchyma, and computed tomography was performed while the dog was awake to investigate the suspected gas in the right kidney. After unilateral ureteronephrectomy, a histological diagnosis of right emphysematous pyelonephritis (EPN) was made. EPN refers to a severe necrotizing infection of the renal parenchyma with gas accumulation in the tissue. It is rare in both human and veterinary medicine, and almost all EPN patients have uncontrolled diabetes mellitus. This case report describes the imaging-based diagnosis of a rare occurrence of unilateral EPN in a non-diabetic dog.

교합기능시 흉쇄유돌근과 교근의 관계에 대한 근전도학적 연구 (AN INTEGRATED EMG STUDY OF THE RELATIONSHIP OF STERNOCLEIDOMASTOID AND MASSETER MUSCLES DURING OCCLUSAL FUNCTION)

  • The purpose of this study was to investigate the relationship between sternocleidomastoid (SCM) and masseter muscles during occlusal functions by means of EMG recordings of examined muscles. For the study, eighteen normal subjects were selected and the Bio-electric Processor EM2 (Myo-tronics Research, Inc., U.S.A.) with the surface electrodes was used to record the EMG activity from the right and left middle of masseter and insertion of SCM of each subject during right and left gum-chewing and isometric contraction by changing the biting force at right eccentric position of jaw. The amount of biting force ranged from 5 to 70kg during isometric contraction were measured by use of Jaw Force Meter. (Nihon Koden Kogyo, Japan.) The results were as follows: 1. The activity onset of SCM and masseter on the same side was almost at the same time, and integrated EMG values of two muscles on the chewing side were higher than the same named muscles on the non-chewing side during gum-chewing. (p<0.01) 2. The regression correlation was not present between both masseters (p>0.05), but between both SCM muscles or muscles of two kinds on the chewing or non-chewing side. ($p{\leqq}0.05$) 3. The integrated EMG value of SCM on chewing or non-chewing side were about 10 percent of that of ipsilateral masseter. 4. Mean voltage of each examined muscles were almost proportional to biting force during isometric contraction and the slope of voltage/biting force line was steepest at the ipsilateral masseter, followed by contalateral masseter, ipsi- and contra-lateral SCM muscles. 5. Mean voltage of ipsilateral masseter was highest during isometric contraction, followed by ipsilateral masseter, contra- and ipsi-lateral SCM muscles.

인체에서 기혈(氣穴) 순환 회복을 위한 기혈테라피의 효과 탐색 (Exploring the Effect of Qihyul-therapy for Recovery of Qi Circulation in Body)

  • 목적: 본 연구의 목적은 둔부근육의 협착, 틀어진 체형, 어깨 통증이 있는 기혈순환이 나쁜 대상자들을 상대로 기혈테라피의 효과를 탐색하는 것이었다. 방법: 기혈테라피와 교정 기혈운동을 사용 하였다. 결과: 기혈테라피를 시술하기 전에 둔부 근육이 거의 움직이지 않았지만, 시술한 후에는 근육의 탄력이 회복되었고 회색으로 경화되었든 피부의 색상도 밝아지었고 항문 괄약근에도 힘이 생기었다. 대상자의 체형이 우측으로 -7°가량 틀어지면서 우측 팔이 좌측팔에 비해서 약 6 cm정도 길었었으나, 체형교정 후에는 우측 팔의 길이가 약 5 cm 정도 줄어들었고, 틀어졌던 체형이 거의 평형을 이루었다. 목 및 어깨교정 후에는 쇄골과 목의 간격이 7 cm로 간격이 넓어졌고, 승모근을 풀어준 후에는 10 cm로 넓어졌으며, 어깨통증이 많이 완화 되었다. 골반교정 후 둔부통증이 완화되었고, 고관절의 위치가 바르게 돌아왔다. 좌측으로 45°각도 틀어졌던 우측다리가 바르게 교정이 되었으며, 4 cm 정도 길었던 우측 다리의 길이도 좌우가 같아졌다. 둔부통증 및 무릎통증도 완화되었다. 골반교정 기혈운동 후에 대상자 1의 고관절의 위치가 바로 잡히어 좌측으로 45°각도 틀어졌던 우측다리가 바르게 교정이 되었고, 4 cm 정도 길었던 우측 다리의 길이도 좌우가 거의 같아졌다. 대상자2는 교정 전에는 무릎이 붙지가 않았고, 오른쪽 무릎이 왼쪽에 비해서 위로 5 cm 정도 올라갔었으며, 발목이 좌측에 비해서 30° 각도로 굽어져 있었는데, 교정 후에는 두 다리의 무릎이 안정되게 붙일 수 있게 되었고, 무릎의 높이도 같아 졌으며, 발목꺾임이 각도도 정상으로 교정이 되었다. 결론: 꾸준하게 기혈테라피와 척추교정 및 골반교정 기혈운동을 한다면 틀어졌던 체형도 바르게 회복되고, 아픈 통증도 완화되어 관련 질환들을 예방 및 치유하는데 효과가 있다고 본다.

상악전치에 발생한 치근단낭종 치험례 (A Case Report of the Radicular Cyst in Maxillary Anterior Teeth Treated by Enucleation)

  • A radicular cyst is a slowly-growing epithelial sac lining a pathologic cavity in the bone at the apex of a tooth. Large radicular cyst revealed by radiographic examination almost always appear to involved several teeth. The author observed a 31 year old woman with a labial swelling and palatal mucosal expansion of the maxillary anterior region. Radiographic examination indicated that the lesion extended from the left maxillary lateral incisor to the right lateral incisor. Cyst consisted of tan to gray yellow slightly firm to partly hard plaque like tissue measuring 3.4 x 2.2 x 0.3㎝. The author has treated the cyst by means of enucleation with apicoecomies of the involved teeth except the right central incisor. Recovery proceed satisfactorily and there were no clinical or radiologic signs of recurrence 8 months later.

  • Endosseous implants have been used to provide anchorage control in orthodontic treatment without the need for special patient cooperation. However these implants have limitation like space requirement, cost, equipments. Recently titanium micro-implant for orthodontic anchorage was introduced. Micro-implants are small enough to place in any area of the alveolar bone, easy to implant and remove, and inexpensive. In addition, orthodontic force application can begin almost immediately after implantation. The mandibular first, maxillary first, mandibula second, and maxillary second molars were the four most commonly missing teeth in adult sample. In case of posterior molar teeth missing, deflective contacts in any position, over time, has produced pathologic change of occlusal scheme because of extrusion of opposing teeth. This case had interocclusal space deficiency by mandibular right molars missing over time. The micro-implants had been used for intrusion of maxillary right molars for interocclusal space. The micro-implant would be absolute anchorage for orthodontic movement. Therefore, the micro-implant would be effective method for correction of occlusal plane.

  • The model of rotary tillers under side loads established here is statically in determinate system. By means of FEM method, the deformation of side gearbox and right side board are calculated. Therefore the side deformations of rotary tillers under different lateral loads are discussed systematically. The results show that the rotary tiller system would bear the loads and deform unequally. Author's calculation also indicates that the lateral deforming values of right side board and side gearbox are almost the same, and more than 98% of the loads is born by the side.

  • 서처성 기관지 폐쇄증은 발생학적으로 태아의 어떤 시기에 기관지 계통에 대한 혈관이 차단됨으로 인해 유발된다고 여겨지는 아주 드문 폐-기관지 질환의 하나로, 이 질환은 주기관지와 쒜구역 또는 페엽 사이에 기도의 연결이 되어 있지 않은 이상증으로 정의된다. 그간 보고된 모든 경우의 기관지 폐쇄증은 측부 순환 이 In재함으로 인해 폐쇄된 폐구역이나 페엽이 흉부 단순 촬영상 과투과 음영을 보였다고 하였으나, 본원에 서는 그간 이론적으로만 가능하다고 했던 폐염의 허탈을 동반한 기관지 폐쇄증을 수술 치험하였기에 보고하 는 바이다.

Vascular anatomy and their variations in Situs inversus totalis using postmortem computed tomographic angiography

  • Studies describing the vascular systems and their variations in Situs inversus totalis (SIT) from a whole-body computed tomographic (CT) angiography perspective are lacking. We report a case of SIT in which postmortem CT angiography (PMCTA) was performed as a part of the forensic death investigation and incidentally detected several vascular variations in it. The PMCTA procedure was performed using the multiphase PMCTA protocol. Almost all major vessels were visualized, indeed in a completely reversed pattern. Contrast mixture flow interruptions were noted in the right coronary arterial branches suggesting possible blockage, upon which autopsy revealed >90% vessel occlusions at several locations. As such the cause of death was due to ischemic heart disease. Anomalous origins of the right internal mammary artery; abnormal left thyrocervical trunk and variations in the drainage of testicular veins were noted. Our findings might be helpful to clinicians and add to the body of literature on SIT.

  • We analysed the operating properties of resistive type superconducting fault current limiters (SFCLs) based on YBCO thin films with a single line-to-ground fault. When a single line-to-ground fault occurred, the short circuit current of a fault phase increased up to about 6 times of transport currents immediately after the fault instant and was effectively limited to the designed current level within 2 ms by the resistance development of the SFCL. The fault currents of the sound phases almost did not change because of their direct grounding system. The unsymmetrical rates of a fault phase were distributed from 6.4 to 1.4. It was found that the unsymmetrical rates of currents were noticeably improved within one cycle after the fault instant. We calculated the zero phase currents for a single line-to-ground fault using the symmetrical component analysis. The positive sequence resistance was reduced remarkably right after the fault but eventually approached the balanced positive resistance component prior to the system fault. This means that the system reaches almost the three-phase symmetrical state in about 60 ㎳ after the fault. The ground currents were almost 3 times of the zero phase mts since most of the fault currents flowed through the grounding line.
