Food protein-induced enterocolitis syndrome (FPIES) is an under-recognized non-IgE-mediated gastrointestinal food allergy. The diagnosis of FPIES is based on clinical history, sequential symptoms and the timing, after excluding other possible causes. It is definitively diagnosed by an oral food challenge test. Unfortunately, the diagnosis of FPIES is frequently delayed because of non-specific symptoms and insufficient definitive diagnostic biomarkers. FPIES is not well recognized by clinicians; the affected infants are often mismanaged as having viral gastroenteritis, food poisoning, sepsis, or a surgical disease. Familiarity with the clinical features of FPIES and awareness of the indexes of suspicion for FPIES are important to diagnose FPIES. Understanding the recently defined clinical terms and types of FPIES is mandatory to suspect and correctly diagnose FPIES. The aim of this review is to provide a case-driven presentation as a guide of how to recognize the clinical features of FPIES to improve diagnosis and management of patients with FPIES.
Cysteinyl leukotrienes are inflammatory mediators having important role in pathophysiological conditions such as asthma and allergic rhinitis. CysLT1 receptor mediates most of the disease regulatory actions of the CysLTs and it is been implicated in a number of inflammatory conditions including gastrointestinal and cardiovascular diseases. Hence in the present study, molecular docking of CysLT1 was performed with its potent and orally efficacious antagonist CP-199330 and CP-199331. The aim of this study was to compare the interaction of CP-199330 and CP-199331 with known drugs such as Zafirlukast, Pranlukast and Montelukast which had already showed clinical efficacy in the treatment of asthma. The residues such as TYR83, GLN274, LYS311 and SER313 were found to interact with both the antagonist and the known drugs. Also, we noticed the docking scores and interaction of the antagonists were comparable with the known drugs. Hence these antagonists could serve as better drugs for the treatment of allergy.
The present study was conducted to investigate the anti-allergic activity of Saingheylyunbooem(SHU)). We investigated the anti-allergic effects of SHU in RBL-2H3 basophilic leukemia cells by compound 48/80, a mast cell degranulator in mice. SHU inhibited ${\beta}$-hexosaminidase and histamine release from compound 48/80 stimulated RBL-2H3 cells. The in vitro anti-inflammatory activities of SHU in LPS-stimulated RAW 264.7 cells were investigated. SHU inhibited NO production effectively dowregulated the expression of iNOS mRNA and iNOS protein expression in LPS-stimulated RAW 264.7 cells. These result provide evidences that SHU may be beneficial in the treatment of allergic inflammtory disease.
기관지결핵에 의한 기도폐쇄, 협착은 기관지결핵 치료 초기보다는 치료 도중 또는 치료 후기에 주로 발생하는 합병증이다. 23세 여자환자가 기관결핵을 진단받고 항결핵제 치료 시작 2주 후 급성으로 악화된 심한 기침, 호흡곤란으로 병원에 왔다. 시행한 기관지내시경에서 용종과 같은 기관종괴가 기관 내에서 호흡에 따라 상, 하 운동을 하면서 호기 시에 기도폐쇄를 일으켰고 이 종괴는 기관지 내시경을 통해 성공적으로 제거되었다. 기관결핵의 치료경과 중 발생한 드물게 보이는 형태의 급성 기도폐쇄로 사료되어 문헌고찰과 함께 보고한다.
CR asthma is associated with disease chronicity, a positive family history of asthma and in vitro and in vivo defects in mononuclear cell function. The HPA axis in CR asthmatics is suppressed normally by dexamethasone and the pharmacokinetic profile of an oral dose of prednisolone is similar to that found in CS subjects. In addition, competitive binding studies have shown that the ligand binding and nuclear translocation functions of the GR are similar in the two groups. Studies using gel retardation assay have indicated a defect in DNA binding in CR subjects. Chemical mutational analysis of the GR has shown that is not due to a defect in its structure at the cDNA level. Scatchard analysis of the GR/DNA and GR/ligand interactions suggests that there may be transcriptional interference of the GR with other transcriptionally active molecules leading to defective gene transcription.
The management of severe asthma presents a significant challenge in asthma treatment. Over the past few decades, remarkable progress has been made in developing new treatments for severe asthma, primarily in the form of biological agents. These advances have been made possible through a deeper understanding of the underlying pathogenesis of asthma. Most biological agents focus on targeting specific inflammatory pathways known as type 2 inflammation. However, recent developments have introduced a new agent targeting upstream alarmin signaling pathways. This opens up new possibilities, and it is anticipated that additional therapeutic agents targeting various pathways will be developed in the future. Despite this recent progress, the mainstay of asthma treatment has long been inhalers. As a result, the guidelines for the appropriate use of biological agents are not yet firmly established. In this review, we aim to emphasize the current state of biological therapy for severe asthma and provide insights into its future prospects.
Objectives : Atopic dermatitis is a chronic inflammatory skin disease which is characterized by severe pruritis, erythema, edema, effusion and scabs. The aim of this study was to understand effects of Arctium lappa Linne, which is well known for its efficacy for various skin diseases, on atopic dermatitis Methods : We conducted this experiment using the DNFB-induced NC/Nga mice. After two weeks of applying DNFB to NC/Nga mice, severe symptoms of atopic dermatitis occurred. We divided the mice into three different groups: a control group which was given no treatment at all, a group treated with dexamethasone (1mg/kg), and another group treated with Arctium lappa Linne (300mg/kg). After one week of treatment, results were recorded according to their improvement on skin, itching behavior, IL-4, and INF-$\gamma$ measurement, which is a significant criterion for diagnosing atopic dermatitis. Results : Itching behavior showed significant improvement in the Arctium lappa Linne group. However Arctium lappa Linne failed to reduce IL-4 and INF-$\gamma$ count. Also there was no satisfying improvement on AD-like skin lesions on the rostral back of the NC/Nga mice treated with Arctium lappa Linne. Conclusions : Arctium lappa Linne showed improvement in itching behavior in NC/Nga mice induced by DNFB. Arctium lappa Linne showed neither significant improvement on skin lesions nor in IL-4 and INF-$\gamma$ measurement.
Objectives : Atopic dermatitis (AD) is a chronic inflammatory skin disease characterized by eczematous inflammtion of the skin. The chestnut inner shell extracts (CI) has been used as a cosmetic material for a long time in Korea. However, the precise anti-allergy effects of CI have yet to be clearly elucidated. In this study, we attempted to evaluate the effect of CI on mast cell-mediated allergy inflammation. Methods : To find the anti-allergy and inflammatory effect of CI, we investigated the inhibitory effect of CI on the production of inflammatory mediators using by enzyme-linked immunosorbent assay in phorbol 12-myristate 13-acetate (PMA) plus calcium ionophore (A23187) stimulated-human mast cell (HMC-1). Results : In this study, we found that CI did not show cytotoxic effect at up to 10 ug/ml on HMC-1. CI inhibited the production of tumor necrosis factor (TNF)-${\alpha}$, interleukin (IL)-6 and IL-8 in stimulated HMC-1. Maximal rate of TNF-${\alpha}$, IL-6 and IL-8 inhibition by CI (10 ug/ml) were about 47.6%, 44.1% and 22.5% respectively. In addition, we showed that Fr.3 isolated from n-Butyl alcohol layer of CI attenuated the production of TNF-${\alpha}$, IL-6 and IL-8 in HMC-1. Conclusion : Taken together, the findings of this study provide us with a novel insight action of CI as a potential molecule for use in the treatment of allergic inflammation diseases.
연구배경: COPD에서 보이는 호흡곤란의 주된 원인이 폐의 과팽창이고 이 과팽창을 치료하는 것이 호흡곤란의 향상에 중요한 것으로 밝혀지고 있다. 그러나 $FEV_1$, FVC만으로는 호흡곤란의 정도와 과팽창을 정확히 측정할 수 없다. 따라서 단순 흉부 방사선에서 관찰되는 폐의 과팽창의 정도와 고해상도흉부 단층촬영에서 폐기종의 정도가 호흡곤란을 객관적으로 알 수 있는 지표가 되는 지를 알아보고자 하였다. 방 법: COPD로 진단 받은 50명(평균 연령 69세, 남자 47명, 여자 3명)을 대상으로 하였다. 폐의 종축, 흉골 뒤공간, 횡격막의 높이를 각각 측정하여 총 0에서부터 3까지로 폐의 과팽창 정도(chest score)를 측정하였고, 고해상도 흉부단층 촬영에서 폐기종의 정도(HRCT score)를 구하여 이를 환자의 호흡곤란의 정도 및 폐용적 검사를 비롯한 폐기능 검사와의 상관관계를 구하였다. 결 과: 호흡곤란의 지수(Borg scale)는 폐기능 검사의 지표인 FVC, $FEV_1$, $FEV_1$/FVC, RV, RV/TLC, DLCO와 유의한 상관관계가 관찰되었다. Borg scale은 HRCT score와 상관관계가 있었으나, chest score와는 상관관계가 없었다. 그리고 Borg scale은 체질량 지수와 역 상관관계가 있었다. 결 론: 고해상도 흉부 단층촬영의 폐기종의 정도는 COPD 환자에서 호흡곤란의 정도를 알 수 있는 객관적 지표가 될 수 있다.
