Arthroscopic Repair versus Non-operative Treatment of First-time Traumatic Anterior Shoulder Dislocations: A Numbers-needed-to-treat Analysis for Prevention of Recurrent Dislocations

  • Background: Arthroscopic surgical repair is a better intervention than non-operative (conservative) treatment for patients with shoulder dislocations. This systematic review determined the numbers-needed-to-treat (NNT) and relative risk reduction (RRR) associated with arthroscopic surgical repair versus non-operative treatment in reducing recurrence rates among patients with first-time traumatic anterior shoulder dislocations. Methods: We searched Google Scholar, MEDLINE, SPORTDiscus, and CINAHL from inception in 2015. All articles had to compare arthroscopic surgical repair and non-operative treatment and be written in English. We used the total number of subjects and the number of recurrent dislocations within each treatment to calculate the NNT and RRR for each study and the pooled data. Results: Six articles were selected and all clearly demonstrated that the arthroscopic surgical repair was more effective than non-operative treatment in reducing the recurrence episodes. The pooled NNT was 1.76 (95% confidence interval [CI]=NNT to benefit 1.50-2.13) and the pooled RRR was 86.0% (95% CI=77.0%-92.0%) among individuals who underwent arthroscopic repair. The average follow-up time was 56 months. Conclusions: A Strength of Recommendation Taxonomy level of evidence of 1 with a grade A recommendation supports the use of arthroscopic surgical repair over non-operative treatment in prevention of first-time traumatic anterior shoulder dislocations. We suggest that sports medicine practitioners consider the patients' age, occupation, and physical activity level when making a clinical decision.

견관절 외상성 전방 불안정성에 대한 Bio-knotless 봉합 나사못을 이용한 관절경적 Bankart 병변 봉합술 (예비 보고) (Arthroscopic Bankart Repair in Traumatic Anterior Shoulder Instability with Bio-knotless Anchor (Preliminary and Technical Report))

  • Purpose: This study reports the clinical results of the arthroscopic Bankart repair in traumatic anterior instability of the shoulder with bio-knotless anchor. Materials and Methods: 21 cases of 21 patients (20 male and 1 female) were included in this study. The average age was 24.8 years old and the period from the first injury to operation was average 37.2 months. All cases had Bankart lesion and 12 cases had Hill-Sachs' lesion. The SLAP lesion was associated in 6 cases. Preoperative Rowe score was average 29.1. Arthroscopic Bankart repair with bio-knotless anchor were performed in all cases; 3 anchors at 3, 4, 5 O'clock position of the glenoid were used in 11 cases and 2 anchors at 4, 5 O'clock position were used in 10 cases. All the associated SLAP lesions were repaired arthroscopically with bio-knotless anchor. Thermal capsular shrinkage at the anterior and inferior shoulder capsule after the Bankart repair was performed in 3 cases. The average follow up period was 20.2 months. Results: The Rowe score improved to 92.8, excellent in 17 cases and good in 4 cases, at last follow up period and 20 cases had full range of motion of the shoulder. 1 case had mild limited range of motion of the shoulder (150 degrees in flexion, 60 degrees in external rotation and T12 level in internal rotation) without any problem in normal activity. The arthroscopic revision surgery of the shoulder was performed in 1 case because of multiple traumatic injuries of the shoulder with pain postoperatively. Conclusion: Arthroscopic Bankart repair with bio-knotless anchor in traumatic anterior shoulder instability is one of the good methods because of the good clinical results.

견관절 불안정성의 관절경적 치료에 있어 Beach chair position의 유용성 (The Usefulness of Beach-chair position in the Arthroscopic Treatment of Shoulder Instability)

  • Purpose: The purpose was to identify the effectiveness of beach-chair position in the arthroscopic Bankart repair over conventional lateral decubitus position with distal traction. Materials & Methods: 36 arthroscopic Bankart repair through July 2000 to July 2001 was done under beach chair position. All cases were shoulder instability. Male patients were 6 and female were 4 with average age of 25 years. Arthroscopic suture anchor was used in 24 cases and average number was 3. Results: Interscalene block was tried in 29 patients and 1 case was changed to general anesthesia. Arthroscopic examination to identify Bankart lesion and associated pathology was done without difficulty Bankart lesions were easily reduced to anatomic position and placed suture anchor and hooking approprately. After the arthroscopic examination,3 cases were converted to open procedure without any positional change. Conclusion: Under interscalene block, the preparation was more simple and the patient could watch arthroscopic procedure with confidence. There was no hindrance in arthroscopic examination and arthroscopic repair could be dont: in more anatomic position. It can be easily changed to open repair if it needed

Effects of Therapeutic Massage and Home Exercise Program on Pain, Range of Motion, Depression, and Quality of Life after Arthroscopic Rotator Cuff Repair

  • Purpose: This study examined the effects of therapeutic massage (TMP) and home exercise program (HEP) on pain, range of motion (ROM), depression, and quality of life (QOL) after arthroscopic rotator cuff repair in middle-aged women. Methods: Nineteen middle-aged women who underwent arthroscopic rotator cuff repair were enrolled in the study. All subjects were assigned randomly to the experimental group (n=9) or the control group (n=10). The subjects in both groups performed HEP for 30 minutes, three times per day, for eight weeks. The subjects in the experimental group also performed HEP, with TMP being performed for 30 minutes, 16 sessions for eight weeks. All subjects were evaluated with a visual analogue scale (VAS), shoulder ROM, self-rating depression scale (SDS), and Korean brief version of WHO quality of life scale (WHOQOL-BREF). A paired t-test was performed to test the significant differences between before and after the intervention. An independent t-test was conducted to test the significant differences between the groups. Results: Following the intervention, both groups showed significant differences than before the intervention in the VAS, shoulder ROM, SDS, and WHOQOL-BREF (p<0.05). There were significant differences in the ROM of shoulder flexion, SDS, and WHOQOL-BREF between the groups (p<0.05). Conclusion: These findings suggest that intensive TMP is an effective method for improving shoulder flexion mobility, depressive symptom, and QOL in middle-aged women after arthroscopic rotator cuff repair.

  • Purpose: To evaluate the results of arthroscopic repair of traumatic anterior shoulder instability with glenoid bone defect. Materials and Methods: Nineteen patients who had underwent arthroscopic repair for the shoulder with traumatic anterior instability and glenoid bone defect were retrospectively reviewed. Mean age was 24.6 years(range, 20 to 39) and mean follow-up was 23 months(range, 19 to 55). No glenoid bone defect was greater than 7mm in length and 20% of the glenoid. The results were evaluated according to stability, range of motion and function. Results: All patients obtained excellent-good results according to Rowe scoring system. Two patients(10.5%) had instability. The mean loss of external rotation was 15 degrees (range, 0 to 25). Functionally, 17 patients could participate in preinjured work or sports to the same level with or without mild discomfort. The remained 2 patients who had 25 degree loss of external rotation could not play sports. Conclusion: Though arthroscopic repair is a good treatment for traumatic anterior shoulder instability with small glenoid bone defect, it is possible to cause loss of external rotation

  • Object: To evaluate the efficiencies of the arthroscopic rotator cuff surgery which is Performed without the traction system in the lateral decubitus position. Methods: Twenty-nine cases of the arthroscopic rotator cuff surgery performed without the traction system in the lateral decubitus position were studied from February, 2002 to January, 2005. We performed a repair using the arthroscopic debridement and the arthroscopic rotator cuff repair, or using the mini-open incision technique after the confirmation of rotator cuff tear, then, the arthroscopic subacromial decompression was performed after the confirmation of subacromial lesions Results: We could easily find the subscapularis tear which was often overlooked in the arthroscopic rotator cuff surgery performed with the traction surgery by the relaxation of the subscapularis, as the arm position was internally rotate about 45 to 70 degrees from abducted position. We found that the operation time was reduced 14 minutes shorter than the operation time of the controlled group which had the surgery with the traction system on the average. We also found that there were no neurovascular complications from all cases. Conclusions: The arthroscopic rotator cuff surgery without traction system in the lateral decubitus position provided the better visual field, easy manipulation of the joint and reducing operation time.

Peri-anchor cyst formation after arthroscopic bankart repair: comparison between biocomposite suture anchor and all-suture anchor

  • Background: The purpose of this study is to investigate clinical outcomes and radiological findings of cyst formation in the glenoid around suture anchors after arthroscopic Bankart repair with either biocomposite suture anchor or all-suture anchor in traumatic anterior shoulder instability. We hypothesized that there would be no significant difference in clinical and radiological outcomes between the two suture materials. Methods: This retrospective study reviewed 162 patients (69 in group A, biocomposite anchor; 93 in group B, all-suture anchor) who underwent arthroscopic Bankart repair of traumatic recurrent anterior shoulder instability with less than 20% glenoid defect on preoperative en-face view three-dimensional computed tomography. Patient assignment was not randomized. Results: At final follow-up, the mean subjective shoulder value, Rowe score, and University of California, Los Angeles shoulder score improved significantly in both groups. However, there were no significant differences in functional shoulder scores and recurrence rate (6%, 4/69 in group A; 5%, 5/93 in group B) between the two groups. On follow-up magnetic resonance arthrography/computed tomography arthrography, the incidence of peri-anchor cyst formation was 5.7% (4/69) in group A and 3.2% (3/93) in group B, which was not a significant difference. Conclusions: Considering the low incidence of peri-anchor cyst formation in the glenoid after Bankart repair with one of two anchor systems and the lack of association with recurrence instability, biocomposite and all-suture anchors in Bankart repair yield satisfactory outcomes with no significant difference.

회전개근 순수 관절경적 봉합술과 국소절개 구제봉합술 비교분석 : 2~6년 추시결과 분석 (Arthroscopic Versus Mini-Open Salvage Repair of the Rotator Cuff Tear : Outcome Analysis at Two to Six Years Follow-up)

  • The purpose of this study was to compare the outcomes between arthroscopir repair and mini-open repair of medium and large rotator cuff tears in which arthroscopic repair was technically unsuccessful. We evaluated 76 patients of full-thickness rotator cuff tears, among them 42 patients had all-arthroscopic and 34 patients had mini-open salvage repairs. Patients who had acromioclavicular arthritis, subscapularis tear, or instability were excluded. There were 39 males and 37 females with mean age of 56 years (range,42 to 75 years). At a mean follow-up of 39 months (range, 24 to 64 months), the results of both groups were compared with regard to the UCLA and ASES shoulder rating scale s. Shoulder scores improved in all ratings in both groups (p > 0.05). Overall, sixty-six patients showed excellent or gr)of and ten patients showed fair or poor scores by the UCLA scale. Seventy-two patients satisfactorily returned to prior activity. Four showed unsatisfactory return. The range of motion, strength, and patient's satisfaction were improved postoperatively. There were no difference in shoulder scores, pain, and activity return between the arthroscopic and mini-open salvage groups (p > 0.05). However, Patients with larger size tear showed lower shoulder scores and less predictive recovery of the strength and function (p < 0.05). Postoperative pain was not different with respect to the size of the tear (p : 0.251). Arthroscopic repair of medium and large full-thickness rotator cuff tears had iln equal outcome to technically unsuccessful arthroscopic repairs, which were salvaged by conversion to a mini- open repair technique. Surgical outcome depended on the size of the tear, rather than the method of repair.

당뇨병 환자에서 관절경적 회전근 개 봉합술 후 발생한 화농성 견관절염의 치료 - 증례 보고 - (Treatment of the Septic Shoulder after Arthroscopic Rotator Cuff Repair in Diabetes Mellitus - A Case Report -)

  • 당뇨환자로서 관절경적 회전근 개 봉합술을 시행 후 견관절의 화농성 관절염으로 진단된 2예에 대해 관절경적 치료 및 추가적인 개방성 치료를 시행하였다. 관절경적 회전근 개 봉합술 후 감염은 평균 18일 뒤에 발생하였으며, 증상 발현으로부터 수술까지의 평균 기간은 65일이었다. 또한 기능 평가는 KSS, ASES, UCLA, Constant 점수를 이용하였다. 관절경적 변연 절제술과 추가적인 시술 후 평균 22.5일의 항생제 치료로 4개월에 염증이 치료되었으며 최종 결과는 KSS점수가 82점, UCLA점수가 33점, ASES점수가 91점, Constant점수가 71점이었다. 관절경적 회전근 개 봉합술 후 발생한 화농성 견 관절염은 관절경적 치료 및 추가적인 시술로 치료되었고, 관절 운동 범위 및 기능적 결과와 통증 및 만족도는 많이 개선되었다.

  • Purpose: To evaluate the usefulness of arthroscopic repair that was related with full thickness rotator cuff tear and assess clinical result. Materials and Methods: Twenty-one cases of arthroscopically repaired full thickness tear of rotator cuffs were studied. Between October 1998 to July 2002 we have analysed 21 repairs of FTRCT the average age 54(42∼74) years old, mean follow-up was 24(12∼41) months We analyzed the results statistically by paired t-test. Results: Postoperative VAS of pain improved average 7.2 to 1.9, UCLA score improved 13.9 to 31.9, ADL improved 11.5 to 25.5 respectively(all, p<0.001). Eighty-seventh % of the patients showed excellent St good results at the final follow-up. The satisfied rate was 90.5%(19cases). Conclusions: Arthroscopic repair in full thickness rotator cuff tear is effective surgical methods.
