The Relation of Onset-Age and ${\beta}$-Endorphin Levels in Alcohol Dependence (주정의존 환자에서 초발연령에 따른 ${\beta}$-Endorphin의 반응)

  • Objectives : To evaluate the relation of onset-age and plasma level of ${\beta}$-endorphin, Ethanol, ${\beta}$-endorphin, cortisol and blood glucose were compared in 48 male alcoholics and 29 normal controls Methods : Alcoholics are divided into two groups by onset-age of alcoholism. Blood samples were obtained before and after 0.75gm/kg of ethanol consumption at the 7th admission day. Results : 1) The ratio of onset-age before 25 years to after 25 years of the patient group was 2 to 3. 2) There was no significant difference in age at admission between two patient groups. 3) There was no significant difference in change of plasma ethanol levels among three groups. 4) There was no significant difference in change of plasma ${\beta}$-endorphin levels among three groups. 5) There was no significant difference in change of plasma cortisol level among three groups. 6) There was no significant difference in change of fasting blood sugar level among two patient groups.

  • Objectives : To evaluate the relation of familial history of alcoholism and plasma level of ${\beta}$-endorphin, ethanol, ${\beta}$-endorphin, cortisol and blood glucose were compared in 48 male alcoholics and 29 normal controls. Survey of Knowledge on Insomnia for Sleep Clinic Clients (수면클리닉을 방문한 환자들의 불면증에 대한 인식조사)

  • Objectives: Insomnia is not only the most common sleep-related disorder, but also is one of the most important. Knowledge of the comorbidities of insomnia is essential for proper treatment including pharmacological and non-pharmacological methods to prevent disease chronification. This study aimed to determine sleep clinic patients' knowledge of insomnia. Methods: This study recruited 44 patients (24 males and 20 females; mean age $54.11{\pm}16.30years$) from the sleep clinic at National Center for Mental Health. All subjects were asked to complete a self-report questionnaire about their reasons for visiting a sleep clinic and about their knowledge of treatment and comorbidities of insomnia. Results: The reasons for visiting the sleep clinic were insomnia symptoms of daytime sleepiness, irregular sleeping time, nightmares, snoring, and sleep apnea, in that order. Of the responders, 72.7% had a comorbidity of insomnia, and 22.7% showed high-risk alcohol use. In addition, 70.5% of responders chose pharmacological treatment of insomnia as the first option and reported collection of information about treatment of insomnia mainly from the internet and medical staff. More than half (52.3%) of the respondents reported that they had never heard about non-pharmacological treatments of insomnia such as cognitive behavioral treatment (CBT-I) or light therapy. The response rate about comorbidities of varied, with 75% of responders reporting knowledge of the relation between insomnia and depression, but only 38.6% stating awareness of the relation between insomnia and alcohol use disorder. Of the total responders, 68.2% were worried about hypnotics for insomnia treatment, and 70% were concerned about drug dependence. Conclusion: This study showed that patients at a sleep clinic had limited knowledge about insomnia. It is necessary to develop standardized insomnia treatment guidelines and educational handbooks for those suffering from insomnia. In addition, evaluation of alcohol use disorders is essential in the initial assessment of sleep disorders.

Studies on Depletion Layer of Probe Particles in the System of Poly(vinyl acetate)/Dimethyl Sulfoxide by Dynamic Light Scattering (폴리(비닐 아세테이트)/디메틸설폭사이드 계에서 동적 광산란법에 의한 탐침입자의 배제층 연구)

  • In the system of poly(vinyl alcohol) (PVA)/dimethyl sulfoxide, the refractive index of polymer was very well matched to that of solvent and thus its scattered intensity could be minimized. After adding small amount of polystyrene latex particle (nominal diameter 200 nm), diffusion behavior of only probe particle was investigated against the concentration of polymer matrix by means of dynamic light scattering. The polymer concentration dependence of its reduced diffusion coefficient was able to be analysed with the stretched exponential function of the reduced concentration $C[{\eta}]$. In very dilute concentration regime, the depletion layer kept constant but at the early semi-dilute regime of $1{\leq}C[{\eta}]{\leq}2.5$, the concentration-dependent exponent of depletion layer ${\delta}$ was appeared to be -0.8 which was very close to theoretical one of -0.85. However it was also observed at the higher concentration that its layer thickness decreased more abruptly than theoretical expectation and this phenomenon was ascribed to Oosawa type attractive interaction between adjacent latex particles.

A Study on Activities of Daily Living and Perceived Health Status of Disabled Persons at Home in Rural Areas (농촌 지역 재가 장애인의 일상생활 수행능력과 지각된 건강 상태)

  • Purpose: This study was attempted to provide basic data required to develop community-base rehabilitation program for disabled persons at home by investigating their characteristics. ADL, and perceived health status. Method: The subjects were 146 disabled persons at home in rural areas. Data were collected from January 22 to January 31, 2004. Data were analyzed using frequencies, percentages, means, standard deviations, minimums, maximums, $\chi^2$-test, Fisher's exact test, t-test, one-way ANOVA, Scheffe test, Contingency coefficient, Pearson's correlation, Stepwise multiple regression. Results: The average score of ADL was $5.51{\pm}1.92$: sphincter control $(5.85{\pm}2.10)$, communication $(5.66{\pm}2.02)$, transfers $(5.67{\pm}2.21)$, self care $(5.44{\pm}2.05)$, social cognition $(5.33{\pm}2.08)$, and locomotion $(4.85{\pm}2.32)$. This means that the respondents were in the state of dependence. The main items requiring others' help were 'bathing' (57.7%) and 'going up and down stairs' (51.1%) ADL was significantly different according to age, sender, occupation, medical fee payer, type, severity and duration of disability, BMI, and alcohol drinking. Among the respondents, 82.8% perceived that their health status was bad. Main factors influencing the perceived health status of disability were age and ADL$(R^2=.343)$. Conclusion: Perception of health status among the disabled is related to their characteristics and ADL, so it is necessary to develop community-base rehabilitation programs in order to improve ADL and the perception of health status.

  • Wernicke's encephalopathy is an acute neurological disorder characterized by mental confusion, oculomotor dysfunction, and ataxia. It has been reported in individuals with alcohol dependence, hyperemesis gravidarum, and prolonged parenteral nutrition without vitamin supplementation. Here we present the case of a 13-year-old male patient with neuroblastoma and a history of poor oral intake and nausea for 3 months. After admission, he showed gait disturbances, nystagmus, and excessive dizziness; his mental state, however, indicated he was alert, which did not fit the classical triad of Wernicke's encephalopathy. A diagnosis of Wernicke's encephalopathy was made only after brain magnetic resonance imaging and serum thiamine level analyses were performed. The patient's symptoms remained after 5 days of treatment with 100-mg thiamine once daily; thus, we increased the dosage to 500 mg 3 times daily, 1,500 mg per day. His symptoms then improved after 20 days of replacement therapy. This case report describes a pediatric patient who was promptly diagnosed with Wernicke's encephalopathy, despite only 2 suspicious symptoms, and who completely recovered after high doses of thiamine were given intravenously.

Non-monotonic Size Dependence of Electron Mobility in Indium Oxide Nanocrystals Thin Film Transistor

  • Indium oxide nanocrystals ($In_2O_3$ NCs) with sizes of 5.5 nm-10 nm were synthesized by hot injection of the mixture precursors, indium acetate and oleic acid, into alcohol solution (1-octadecanol and 1-octadecence mixture). Field emission transmission electron microscopy (FE-TEM), High resolution X-Ray diffraction (X-ray), Nuclear magnetic resonance (NMR), and Fourier transform infrared spectroscopy (FT-IR) were employed to investigate the size, surface molecular structure, and crystallinity of the synthesized $In_2O_3$ NCs. When covered by oleic acid as a capping group, the $In_2O_3$ NCs had a high crystallinity with a cubic structure, demonstrating a narrow size distribution. A high mobility of $2.51cm^2/V{\cdot}s$ and an on/off current ratio of about $1.0{\times}10^3$ were observed with an $In_2O_3$ NCs thin film transistor (TFT) device, where the channel layer of $In_2O_3$ NCs thin films were formed by a solution process of spin coating, cured at a relatively low temperature, $350^{\circ}C$. A size-dependent, non-monotonic trend on electron mobility was distinctly observed: the electron mobility increased from $0.43cm^2/V{\cdot}s$ for NCs with a 5.5 nm diameter to $2.51cm^2/V{\cdot}s$ for NCs with a diameter of 7.1 nm, and then decreased for NCs larger than 7.1 nm. This phenomenon is clearly explained by the combination of a smaller number of hops, a decrease in charging energy, and a decrease in electronic coupling with the increasing NC size, where the crossover diameter is estimated to be 7.1 nm. The decrease in electronic coupling proved to be the decisive factor giving rise to the decrease in the mobility associated with increasing size in the larger NCs above the crossover diameter.

The Comparison of the Neurocognitive Functions between Dysthymic Disorder and Major Depressive Disorder (기분부전장애 환자군과 주요우울장애 환자군의 신경인지학적 기능 비교)

  • Neurocognitive research focusing on cognitive deficits in Depression has resulted in several important but yet potentially contradictory findings. Much literature documents the presence of significant neurocognitive impairments in depressive patients. Studies have shown that dysthymic disorder patients demonstrate a diffuse pattern of cognitive impairment which is frequently indistinguishable from that of focal braindamaged patients. Some reports have suggested that there is a focal pattern of deficit, such as anterior cingulate dysfunction, frontal lobe impairment, or dysfunction of the temporal-limbic cortex. The aim of this study is to evaluate the neurocognitive functions in dysthymic disorder patients, and to compare the functions with those of major depressive disorder patients. The subjects are 17 dysthymic disorder patients. And their neurocognitive functions are compared with those of 23 major depressive episode patients. Patients with a history of neurologic disease, alcohol dependence, substance abuse and mental retardation are excluded. They are assessed with a part of Vienna Test System which is computerized neurocognitive function tests and can evaluate attention, eductive ability, reproductive ability, visuoperceptual analysis, vigilance, visual immediate memory, the speed of information-processing, judgement, and fine motor coordinations. There are no other specific difference between two groups, except the result of cognitrone test. This study provides information about the neurocognitive functions and some difference between major depressive disorder patients and carefully diagnosed dysthymic disorder patients.

Temperature Dependence of Activation and Inhibition of Mushroom Tyrosinase by Ethyl Xanthate

  • A new alkyldithiocarbonate (xanthate), as sodium salts, C2H5OCS2Na, was synthesized by the reaction between CS2 with ethyl alcohol in the presence of NaOH. The new xanthate was characterized by 1H NMR, IR and elemental analysis. Then, the new synthesized compound was examined for functional study of cresolase activity of Mushroom Tyrosinase (MT) from a commercial source of Agricus bisporus in 10 mM phosphate buffer pH 6.8, at three temperatures of 10, 20 and 33℃ using UV spectrophotemetry. 4-[(4-methylphenyl)- azo]-phenol (MePAPh) was used as a synthetic substrate for the enzyme for cresolase reaction. The results show that ethyl xanthate can activate or inhibit the cresolase activity of mushroom tyrosinase depending to the concentration of ethyl xanthate. It was concluded that the enzyme has two distinct sites for ethyl xanthate. The first one is a high-affinity activation site and the other is a low-affinity inhibition site. Activation of the enzyme in the low concentration of ethyl xanthate arises from increasing the affinity of binding for the substrate as well as increasing the enzyme catalytic constant. The affinity of ligand binding in the activation site is decreased by increasing of the temperature, which is the opposite result for the inhibition site. Hence, the nature of the interaction of ethyl xanthate is different in two distinct sites. The binding process for cresolase inhibition is only entropy driven, meanwhile the binding process for cresolase activation is not only entropy driven but also enthalpy driven means that hydrophobic interaction is more important in the inhibition site.

Clinical Study on Personality Characteristics of the Alcoholic Patients -with MMPI & Sasang Constitution(四象體質)- (알콜리즘 환자(患者)의 인격특성(人格特性)에 관(關)한 임상적(臨床的) 고찰(考察) -MMPI와 사상체질(四象體質)을 중심(中心)으로-)

  • To study the personality characteristics of alcoholic patients, they were screned with the MAST(Michigan Alcoholism Screening Test) and administered with the MMPI (Minnesota Multiphasic Personality Inventory) in the Dept. of Neuropsychiatry, Oriental Medical Hospital in Kyung Hee Univ. form March 1991 to June 1991. Patients admitted for alcoholism were examined and analyzed using the above tests and then studied by Sasang Constitutional approach. Results were as follows; 1. In the MAST score, the cases of obvious alcohol dependence was 53.3% and overall mean score was 28.1 point. 2. In the MMPI scale, the mean of the T-scores for 2(D)-4(Pd) profile was high, but it was not statistically significant. 3. The results obtained by multivariate cluster analysis of MMPI T-scores can be divided into 3 subgroups; 1) 9% showed the psychiatric tendency of F-6(Pa)-9(Ma)profile, 2) 44.2% showed no significant profile, 3)46.5% showed the neurotic tendency of 1(Hs)-2(D)-3(Hy)-4(Pd) profile. 4. Since 44.2% showed no significant profile on the MMPI, much more alcoholic patients were caused not by personality factors but by social tolerance, and also by increasing apportunities of drinking in Korean society. 5. When analyzed with Sasang Constituional approach, 1) for the Soyangin (少陽人): 6(Pa) scale was particularly high, 1(Hs) and 3(Hy) scales were low compared to others, 2) for the Taiumin(太陰人): 1(Hs)-2(D)-3(Hy)-4(Pd) neurotic profile was high, the K-scale and 4(Pd)scale were high compared to the Soyangin, 3) for the Soumin(少陰人): 1(Hs)-2(D)-3(Hy)-4(Pd) neurotic profile and 7(Pt) scale were also high.
