Comprehensive review of the literature was conducted to determine 1) selected risk factors and its impact that affect pregnancy outcome such as smoking, alcohol consumption, and substance abuse 2) these factors can facilitate future strategies for health promotion and prevention for both pregnant women and fetus. Review of literature were extracted from searching MEDLINE(1966 - Oct. 2000). CINAHL (1982 - Oct. 2000) and the domestic literature. The following factors were identified: 1. The effects of risk behaviors on pregnancy. ${\cdot}$Maternal smoking was associated with the occurrence of premature or LBW delivery, fetal growth retardation, extremities defects, heart defects and sudden infant death syndrome. ${\cdot}$Maternal alcohol consumption was associated with spontaneous abortion, premature or LBW delivery, morphologic/neurologic problems, especially fetal alcohol syndrome. ${\cdot}$Heroin was associated with withdrawal after birth in which were born to heroine addicts for gestational age and lung maturation in animal studies. ${\cdot}$Cocaine was associated with spontaneous abortion, abruptio placenta and a poor response to environmental stimuli. ${\cdot}$So far, the effects of caffeine on pregnancy was controversial, but severe caffeine consumption was associated with premature or LBW delivery, spontaneous abortion, still birth and dystocia. 2. Intervention methods and its effects identified were as follows ${\cdot}$Conducted intervention for smoking, alcohol and drug consumption were single or combined. ${\cdot}$Intervention methods were counseling, phone contact, mailing, use of educational videotape, booklet, support person and alternatives such as nicotine patch. ${\cdot}$The interventions increased the rates of smoking cessation during pregnancy and awareness of the risk of drug consumption, and decreased amount of alcohol consumption. ${\cdot}$The intervention outcome found positive effect on birth weight and length. 3. Our recommendations were as follows ${\cdot}$The personal and social cognition should be enhanced through education and the mass media. ${\cdot}$It's necessary to educate and give information of preconceptional care, planned pregnancy and early prenatal care for optimal pregnancy outcome. ${\cdot}$It's necessary to develop comprehensive assessment tool which is reliable and valid on smoking, alcohol consumption and substance abuse to identify supportive or interventional program.
Purpose: In this study the effects of forgiveness therapy on the resilience, self-esteem, and spirituality of wives of men suffering from alcohol abuse was examined. Methods: The study design was a quasi-experimental design. Forgiveness therapy was conducted once a week for 12 weeks. Data were obtained from March 2012 to December 2013. Participants were chosen from women in two Alcohol Counseling Centers. Of the 29 participants, 16 were assigned to the experimental group and 13 to the control group. Data were analyzed using descriptive statistics, t-test, ${\chi}^2$-test, and repeated measure ANOVA. Results: There were statistically significantly differences for resilience, self-esteem, and spirituality between the experimental and control groups. Forgiveness therapy improved the resilience, self-esteem, and spirituality in the experimental group compared to the control group (p<.05). In follow-up tests, the experimental group had a statistically significantly increase only in self-esteem. Conclusion: This study results show that forgiveness therapy is effective in improving resilience, self-esteem, and spirituality in wives of men suffering from alcohol abuse. Therefore, forgiveness therapy can be considered a useful nursing intervention to promote improvements in emotional stability and provide pain relief for these wives.
Purpose: Non-Suicidal Self-Injury (NSSI), a highly prevalent behavior in adolescents, refers to the direct destruction of one's body tissue without suicidal intent. To date, the prevalence of adolescent self-injury in South Korea and its associated factors remain unknown. This study aims to determine the prevalence of self-injury in Korean adolescents as well as its associated factors. Methods: We assessed 717 middle school students by means of an anonymous self-report survey. Information about demographic characteristics, lifestyle, anxiety and depression, self-esteem, and parenting behavior was obtained. Data were analyzed using $x^2$ test, t-test and multiple logistic regression. Results: NSSI was reported by 8.8% of respondents. Univariate analyses showed associations of exposure to alcohol use, anxiety, depression, self-esteem, and parenting methods with self-injury. In multiple analyses, alcohol use, anxiety, and parental abuse were associated with lifetime self-injury. Conclusion: The rate of NSSI in the South Korea was found to be lower than those of other countries. As our study suggests that alcohol use, anxiety, and parental abuse are associated with lifetime self-injury, health care providers at school should take these factors into account when developing prevention and intervention programs for adolescents.
Purpose: This study was aimed at finding factors influencing cognitive impairment which is one of the typical symptoms among the demented elderly. Methods: The number of subjects was 417 elderly residents aged over 65 yr in a community. A cognitive function and a depression level were measured using the Korean Version of Mini-Mental State Examination (MMSE-K) and the Korean Form of Geriatric Depression Scale (KGDS). Alcohol abuse was measured using the CAGE instrument. Chi-square test and Logistic regression analysis were conducted to identify factors affecting cognitive impairment. Results: The prevalence rates of cognitive impairment of the sample was 43.5%. Sex, age, educational level, perceived health and alcohol abuse were strong factors influencing cognitive impairment. However, the effects of smoking, living alone, depression, family history in dementia and stroke were not strong. Conclusion: Risk for cognitive impairment were increased by being female and older than 70 yr, having low education, perceiving health as poor, and drinking alcohol abusively. Therefore, a reinforcement system, continuous research and the development of proper programs should be preformed in order to prevent cognitive impairment.
Background: Non-conversion of sputum smear and culture prolongs the infectivity of the patient and has been associated with unfavorable outcomes. We aimed to evaluate factors associated with persistent sputum positivity at the end of two months of treatment of new case pulmonary tuberculosis (TB). Methods: Data of 87 human immunodeficiency virus-negative patients with culture-positive drug-susceptible pulmonary TB admitted to local university hospital between September 2015 and September 2016 were reviewed. Factors associated with sputum smear and/or culture positivity at the end of the second month of treatment were analyzed. Results: Twenty-two patients (25.3%) remained smear and/or culture-positive. Male sex, lower body mass index (BMI), unemployment, alcohol abuse, higher number of lobes involved and cavities on chest X-rays, shorter time to detection (TTD) on liquid cultures, higher respiratory sample smear grading and colony count in solid cultures, higher C-reactive protein, erythrocyte sedimentation rate, leukocytosis, thrombocytosis, and anemia were all significantly associated with persistent sputum positivity. However, in the logistic regression analysis only male sex, lower BMI, alcohol abuse, higher radiological involvement, cavitation, higher smear grading, higher colony count in solid cultures and shorter TTD were determined as independent factors associated with persistent sputum positivity at the end of 2 months of treatment. Conclusion: In conclusion, higher sputum smear and culture grading at diagnosis, shorter TTD, higher number of lobes involved, cavitation, male sex, alcohol abuse, and lower BMI were independently associated with persistent sputum positivity. These factors should be sought when distinguishing which patients will remain infectious longer and possibly have worse outcomes.

Background: Carcinoma of the tongue is the most common intra-oral malignancy in Western countries. Incidence and mortality rates have increased in recent years, and survival has not improved. This study aimed to determine etiologic factors for tongue cancer with age-sex matched case-control data. Materials and Methods: 47 patients with carcinoma of the tongue referred to our oral medicine clinic between years 2005-2006 were analyzed and compared with control group data. The medical records, including family history of cancer, dental trauma, and history of abuse of alcohol and tobacco products was recorded for all subjects. Chi square comparison tests and linear regression analysis were performed using the SPSS program for statistics. Results: Patient and randomly selected control groups each consisted of 30 male and 17 female subjects with mean ages 53.2 (${\pm}12.6$) and 52.6 (${\pm}11.5$) years respectively. Smoking and alcohol abuse proportions were significantly higher in the patient group (p=0.0001, p<0.0001 respectively). Chronic mechanical trauma was observed in 44.7% of the patients and 17.0% of the control group (p=0.004). Similarly, family history of cancer of any type (for the first degree relatives) was found to be more common in the patient group (p=0.009). On regression analysis, alcohol abuse, family history of cancer, smoking, chronic mechanical traumas appeared as significant etiologic factors (p=0.0001). Conclusions: We believe that field cancerization may become evident in oral and oropharyngeal mucosa with multiple steps of molecular changes starting from the first sign of dysplasia with chronic exposure to etiological factors. Chronic trauma cases need particular attention to search for very early signs of cancer.
Motivation has been identified as an important factor predicting long-term outcomes of alcohol abuse treatment. Whether a patient agreed on his/her inpatient treatment at time of hospitalization could be an indicator of their motivation for treatment. This study focused on this aspect of treatment motivation and examined whether this factor predicts post-discharge abstinence. A sample of 145 individuals who were hospitalized for alcohol abuse treatment participated in the baseline data collection, of which only 66 successfully completed the 8 month follow-up telephone interview. The findings of survival analysis suggest that voluntarily admitted individuals had significantly longer duration of post-discharge abstinence compared to their counterparts. A Cox proportional hazards regression model revealed that voluntary admission and family support were significant predictors for post-discharge relapse of drinking, after controlling for the effects of marital status, living alone, and working status. Implications for social work practice are discussed.
The purposes of this study were to compare middle school students' alcohol expectancies with high school students' alcohol expectancies and to identify factors (family history of alcohol problems, television advertisements, age, gender, drinking experience, alcohol prevention education) related to their alcohol expectancies. A total of 1,164 middle school students in seven metropolitan areas in Korea participated in this study. The major finding of this study was that the respondents scored higher on negative alcohol expectancies than high school adolescents. As found in previous studies, factors related to adolescents' alcohol expectancies were also significantly related to respondents' alcohol expectancies in this study. Respondents who had family histories of alcohol problems, had drinking experiences, and were older expect drinking in a more positive manner. This study also found that alcohol advertising affected respondents' alcohol expectancies. However, this study found that current substance abuse prevention programs did not affect respondents' alcohol expectancies at all. This finding may suggest that some changes on current prevention programs should be needed in order to be more effective. Finally the result on the relationship gender and alcohol expectancies in this study was different from previous studies. Female respondents expected drinking more positively than male counterparts. This study suggests that further studies on this issue are necessary.
Since the alcohol involves a wide variety of adverse health and social consequences, there is a strong need for improved estimates on economic costs and benefits of alcohol drinking to reduce the social alcohol abuse and to obtain some useful informations for targeting programs. However, there is no research for the intangible costs and benefits so far that are essential to estimate the real overall costs and benefits incurred by alcohol use. Thus this paper finds out and represents a general model for estimating the intangible costs and benefits by use of alcohol demand curve. The calculated monetary values, as a example, of them in 2002 show as big as 7,900 billion won and 4,200 billion won respectively.
Purpose: The purpose of this study was to identify a path diagram for the influence of family, personality, sexual abuse, drug abuse, coping strategies, and aggressive impulsiveness on aggression, and to develop a sublimation program for Korean adolescent's aggression. Method: Data was collected by self-report questionnaires. Subjects consisted of 2,111 adolescents. A proportional stratified random sampling method was used. The major instrument was the Mental Health Questionnaire for Korean Adolescents, and the Cronbach's Alpha ranged from .54 to .95 for each subscale. Statistical methods were Chi-square, correlation analysis, and path analysis. Results: The strongest contributing variables on aggression were person-related aggressive impulsiveness, antisocial personality, self-injured aggressive impulsiveness, gender, sexual abuse, psychosomatic symptoms II, drug abuse, age, parent-child relationship, alcohol abuse and cognitive avoidance coping strategies in the order named. Also the author developed a multi-systemic sublimation program for Korean adolescents's aggression. The multi-systemic sublimation program involves four domains including adolescents, parents, peers and community, and has several therapeutic sub-programs for each domain. Conclusion: The ecology of human development is composed of multiple, integrated levels of organization, including biological, individual-psychological, social-interpersonal, cultural, and historical levels. Therefore, this multi-systemic sublimation program will prevent and decrease the rate of aggressive behavior among Korean adolescents.
