Background and Objectives : Positional OSAS is characterized by an apnea-hypopnea index (AHI) score >5, which, while sleeping in the supine position, is double that in non-supine position. This study was performed to compare the clinical characteristics of positional OSAS and non-positional OSAS patients, and the effects of the modified jaw thrust maneuver during drug-induced sleep endoscopy (DISE) between positional OSAS and non-positional OSAS patients. Materials and Methods : 68 positional OSAS patients and 19 non-positional OSAS patients were included. They all underwent full-night polysomnography and DISE. The modified jaw thrust maneuver was introduced during DISE. Airway structural changes induced by the modified jaw thrust maneuver were evaluated and documented. Results : There were no statistically significant differences in Friedman stage or tonsil grade, body mass index, Epworth sleepiness scale (ESS) score, blood pressure, AHI, or obstructive pattern between the positional and non-positional OSAS patients. However, mean arterial oxygen saturation (SaO2), lowest SaO2, and total arousal index values were more severe in the non-positional OSAS patients. After introduction of the modified jaw thrust maneuver, retrolingual level obstruction showed a tendency toward a higher rate of airway opening in positional OSAS patients than in non-positional OSAS patients. Conclusions : The effects of a mandibular advancement device (MAD) can be estimated by carrying out a modified jaw thrust maneuver during DISE. The tendency toward a higher rate of airway opening in positional OSAS patients than non-positional OSAS patients in retrolingual level obstruction after jaw thrust maneuver introduced during DISE may be clinically important for MAD.
연구배경: Auto-PEEP 혹은 intrinsic PEEP은 호기말에 폐용적이 전체 호흡기계의 이완 용적으로 돌아오지 않음으로써, 증가된 호흡기계의 탄성반도압만큼 호기말 폐포내압(alveolar pressure) 이양의 값을 보이는 것을 말한다. Auto-PEEP 이 존재하는 만성폐쇄성폐질환 환자에게 externa1 PEEP을 적용하면 환자의 호흡 일을 줄일 수 있어서, 질환의 급성악화시 혹은 기계호흡으로부터의 이탈시 환자의 자발호흡을 보조하기 위한 요법으로 제시되고 있다. 이에 기계호흡중인 환자에서 auto-PEEP의 존재가 호흡 일에 미치는 영향을 알아보고, externa1 PEEP의 사용이 auto- PEEP에 의해 증가된 호흡 일을 줄이는지를 알아보기 위해 본 연구를 시행하였다. 방법: 호흡부전으로 기계호흡을 하고 있는 환자 15명을 대상으로 연구가 이루어 졌으며, 이들 7명에서 auto-PEEP이 관찰되었고(auto-PPEP군) 8명에서 auto-PEEP이 auto-PEEP군). 양군 간의 환자의 호흡역학적 지표의 차이를 조사하였으며, auto-PEEP이 존재하는 환자들에 대해 3cm $H_2O$의 external PEEP을 적용한 뒤 호흡역학적 지표들의 변화를 조사하였다. 호흡역학적 지표는 상시호흡량(tidal volume, 이하 $V_T$), 분당 호흡수, 분당환기량 (minute ventilation 이하 $V_E$), 최고흡기유량(peak inspiratory flow rate, 이하 PIFR), 최고호기유행peak expiratory flow rate, 이하 PEFR), 최고흡기압(peak inspiratory pressure, 이하 PIP), $T_I/T_{TOT}$, auto-PEEP, 폐 동적탄성 (dynamic compliance of lung, 이하 Cdyn), 호기 기도저항(expiratory airway resistance, 이하 RAWe), 평균 기도저항(mean airway resistance, 이하 RAWm), $P_{0.1}$, 환자에 의해 수행되는 호흡 일 (work of breathing performed by patient, 이하 호흡 일), pressure-time product(이하 PTP)등이었다.
폐쇄성 수면 무호흡 환자들의 양압기 사용을 위한 적정 양압측정은 편리함과 경제성뿐만 아니라 다양한 원인으로 CPAP 측정법보다는 APAP 측정법을 더 선호한다. 그러나 PSG 감시하에 진행하는 CPAP 측정법보다 정확성에 대한 의문이 아직까지 남아있다. 이에 본 연구에서는 동일한 대상자에 대한 CPAP과 APAP 측정법의 적정 압력과 무호흡-저호흡 지수를 변수로서 두 방법간의 효율성과 정확성을 비교하였다. 대상자는 분할 수면다원검사 중 CPAP 양압 측정과 7일 이상 APAP 양압 측정에 성공한 79명이 모집되었다. CPAP과 APAP 측정법의 적정 양압은 $7.0{\pm}1.8cmH_2O$, $7.6{\pm}1.6cmH_2O$ 무호흡-저호흡 지수는 $1.3{\pm}1.5/h$, $3.0{\pm}1.7/h$로 CPAP 측정법이 두 변수 모두 통계적으로 유의하게 낮게 나타났다(P<0.001). 그러나 미국수면학회 가이드라인 무호흡-저호흡 지수 5/h이하의 적정 양압 도달 비율은 CPAP 측정법이 96.2% (76명), APAP 측정법이 94.9% (75명)으로 유의미한 차이를 보이지 않았다(r=-0.045, P=0.688). 본 연구를 요약하면, CPAP은 APAP 측정법보다 좀 더 효율적인 측정법으로 나타났다. 그렇지만 적정 양압에 도달하는 비율은 두 방법 모두 통계적 차이를 나타내지 않아 정확성을 가진다. 따라서 CPAP과 APAP 측정법 중 요구되는 상황에 따라 적절한 방법을 선택하여 적정 양압을 측정할 수 있겠다.
연구배경 : 만성 폐쇄성 폐질환을 대변하는 폐기종에서 주요 기능적 장애는 호기의 장애이며, 최대 호기류량은 기도의 크기, 폐의 탄력반동압 그리고 기도의 함몰성에 의해 결정된다. 기도의 항몰성은 폐기종에서 기류를 제한하는 하나의 기전으로 작용하지만, 폐용적과 폐의 과팽창을 유지하는 기전으로 작용하여 가스교환을 증진시킨다는 주장도 있다. 따라서 폐기종에서 기관이 호흡에 미치는 생리학적 역할이 무엇인지를 알아보고자 본 연구를 수행하였다. 방 법 : 1997년 1월 1일부터 8월 31일까지 보라매병원 호흡기내과를 방문하여 단순 흉부방사선검사, 체간용적기록계(body plethysmography)를 포함하는 폐기능검사 및 HRCT를 통해 폐기종으로 진단된 20명을 대상으로 하였다. HRCT에서 대동맥궁의 정상부위에서 기관의 단면적을 호흡주기에 따라 측정하고 이를 체표면적으로 보정한 값과 동맥혈의 이산화탄소분압 및 산소분압, 기도저항, 폐유순도 등 폐기능지표와의 상관 관계를 분석하였다. 결 과 : 폐기종에서 기관의 단면적은 호기시 동맥혈의 이산화탄소분압(r=-0.61, p<0.05) 및 산소분압(r=0.6, p<0.05) 그리고 매분환기량(r=0.73, p<0.05)과 유의한 상관관계가 있었지만, 흡기시에는 상관관계가 없었다(이산화탄소분압과는 r=-0.22, p>0.05, 산소분압과는 r=0.26, p>0.05, 매분환기량과는 r=0.44, p>0.05). 매분환기량은 상시호흡량(tidal volume)과는 r=0.45(p<0.05)로 유의한 상관관계가 있었지만, 호흡수와는 r=-0.31(p>0.05)로 상관관계가 없었다. 폐기종에서 기관의 단면적은 호흡주기와 상관없이 $FEV_1$ FVC, $FEV_1$/FEC, 최대 호기류량, 잔기용적, 폐확산능, 기도저항, 폐유순도 등의 다른 폐기능지표와는 상관관계가 없었다. 결 론 : 폐기종에서 호기시 기관의 단면적은 주로 가스 교환(gas exchange)의 지표들과 유의한 상관관계가 있었지만, 폐용적이나 폐의 과팽창을 시사하는 지표와는 상관관계가 없었다. 따라서 폐기종에서는 호기시 동일 압력점의 개념에 의해 발생하는 기도압박으로 기관의 단면적이 감소하고 이러한 현상이 기류를 제한하는 하나의 기전으로 작용하는 것이지 폐용적이나 폐의 과팽창을 유지시켜 가스교환을 증진시키는 것은 아니라고 생각된다.
구강내 장치 요법은 가벼운 혹은 중간 정도의 폐쇄성 수면 무호흡증의 일차 치료법이며 심한 폐쇄성 수면 무호흡증의 경우에도 CPAP 또는 수술요법에서 효과가 없을 때 좋은 대안적 치료법이 될 수 있다. 하지만 다양한 구강내 장치들마다 치료 효과가 다르며 또한 모든 환자들에서 효과를 기대하기 어려운 것도 사실이다. 본 연구는 목젖입천장인두절제술(uvulopalatopharyngoplasty, UPPP) 을 받았으나 증상의 개선이 없는 심한 폐쇄성 수면 무호흡증 (obstructive sleep apnea, OSA) 환자에서 단국대 치대 구강내과에서 개발한 구강내 조절성 하악전방이동형 코골이 방지장치 (Adjustable Anterior Positioner, AAP)를 사용하여 치료한 증례를 보고하고 그에 대한 고찰을 한다.
Objective: The objective of this study was to investigate the effects of miniscrew-assisted rapid palatal expansion (MARPE) on changes in airflow in the upper airway (UA) of an adult patient with obstructive sleep apnea syndrome (OSAS) using computational fluid-structure interaction analysis. Methods: Three-dimensional UA models fabricated from cone beam computed tomography images obtained before (T0) and after (T1) MARPE in an adult patient with OSAS were used for computational fluid dynamics with fluid-structure interaction analysis. Seven and nine cross-sectional planes (interplane distance of 10 mm) in the nasal cavity (NC) and pharynx, respectively, were set along UA. Changes in the cross-sectional area and changes in airflow velocity and pressure, node displacement, and total resistance at maximum inspiration (MI), rest, and maximum expiration (ME) were investigated at each plane after MARPE. Results: The cross-sectional areas at most planes in NC and the upper half of the pharynx were significantly increased at T1. Moreover, airflow velocity decreased in the anterior NC at MI and ME and in the nasopharynx and oropharynx at MI. The decrease in velocity was greater in NC than in the pharynx. The airflow pressure in the anterior NC and entire pharynx exhibited a decrease at T1. The amount of node displacement in NC and the pharynx was insignificant at both T0 and T1. Absolute values for the total resistance at MI, rest, and ME were lower at T1 than at T0. Conclusions: MARPE improves airflow and decreases resistance in UA; therefore, it may be an effective treatment modality for adult patients with moderate OSAS.
Background and Objectives : Aerodynamic investigation is valuable information about the efficiency of the larynx in translating air pressure to acoustic signal. The normal data of the Korean has been reported, but there is no basic data of professional western classical singers who have learned how to control the flow of expiratory air for singing. The purpose of this study was to investigate the normal aerodynamic data of korean professional western classical singers and compare this with that of the Korean Materials and Methods : 50 Korean western classical singers were studied. Expiratory lung pressure combined with measurements of the mean air flow rate, voice frequency and intensity were measured with the aerodynamic test using airway interruption method. These data were compared with normal data of untrained normal adults. Results and Conclusions : The voice frequency and the voice intensity were increased in the western classic singers, but the mean air flow rate and the expiratory air pressure of the classical singers were within the same range of the untrained normal adults. This result means that western classical singers can change the loudness and pitch with a little increased or decreasing the mean air flow and the expiratory air pressure.
Acquired tracheal dilatation is a state of abnormal tracheal dilatation developing from various causes. Tracheomalacia and tracheal dilatation can develop in respiratory distress patients with prolonged endotracheal intubation with assisted positive-pressure ventilation due to positive airway pressure and high cuff pressure. The authors have recently experienced one case of respiratory failure, cardiac arrest, and whole body emphysema after tracheostomy and portex tube insertion were performed to patient with the endotracheal intubation with assisted positive-pressure ventilation for two weeks in the septic shock resulted from colon perforation, who developed tracheal dilatation. We summarize diagnostic and therapeutic strategies of acquired tracheal dilatation for the prevention of emergency status and the management for that patients.
Management of mechanical ventilation is essential for patients with neuro-critical illnesses who may also have impairment of airways, lungs, respiratory muscles, and respiratory drive. However, balancing the approach to mechanical ventilation in the intensive care unit (ICU) with the need to prevent additional lung and brain injury, is challenging to intensivists. Lung protective ventilation strategies should be modified and applied to neuro-critically ill patients to maintain normocapnia and proper positive end expiratory pressure in the setting of neurological closed monitoring. Understanding the various parameters and graphic waveforms of the mechanical ventilator can provide information about the respiratory target, including appropriate tidal volume, airway pressure, and synchrony between patient and ventilator, especially in patients with neurological dysfunction due to irregularity of spontaneous respiration. Several types of asynchrony occur during mechanical ventilation, including trigger, flow, and termination asynchrony. This review aims to present the basic interpretation of mechanical ventilator waveforms and utilization of waveforms in various clinical situations in the neuro-ICU.
Purpose: The neck is a particularly critical region for penetrating injuries due to the close proximity of the trachea, esophagus, blood vessels, and the spinal cord. An open neck injury has the potential for serious morbidity and mortality. The purpose of this study is to evaluate the assessment and management of open neck injuries. Methods: In this retrospective study, open neck injury patients who were admitted to the Trauma Center of Daejeon Eulji Medical Center, Eulji University between December 2015 and December 2017 were analyzed for epidemiology, the mechanism of trauma, the injured organ, complications, and mortality. Results: Thirty-two patients presented with open neck injuries. All patients underwent computed tomographic angiography to evaluate their injuries once their vital signs stabilized. Among these patients, 27 required surgical treatment. The most commonly injured organ was the airway. There were five deaths, and the main cause of death was bleeding. Mortality was associated with the initial systolic blood pressure at the hospital and Glasgow Coma Scale. Conclusions: Mortality from open neck injuries was associated with initial systolic blood pressure at the hospital and Glasgow Coma Scale.
