Thoracoscopic Sympathectomy for a Patient with Facial Hyperhidrosis -A case report- (흉강경하 흉부교감신경절제술을 이용한 안면다한증 치료 -증례보고-)

  • Endoscopic transthoracic sympathectomy (ETS) has recently become estabilished as a successful treatment for severe palmar and axillary hyperhidrosis. Descriptions have been published of neurolytic, operative and alternative endoscopic procedures involving thermocoagulation, laser coagulation, or or nonvideo-assisted ganglionectomy using equipment not widely available, with low morbidity and excellent results. All methods have advantage and disadvantages. A 19-year-old male who suffered from severe hyperhidrosis on face, palms and axillary areas, has been initially treated with stellate ganglion block in other pain clinic. He was transfered to our pain clinic for endoscopic thoracic sympathectomy. The patient was intubated left side 34 Fr. double lumen tube and positioned left semi-lateral position for right sympathectomy. Right side pneumothorax was created by clamping the ipsilateral side of the double lumen tube and aspiration of air. 11-mm trocar was introduced through incision at the third intercostal space in anterior axillary line, and then additional two 11-mm and 5-mm trocar was introduced through second and fifth intercostal space in mid axillary line. The lung was gently retracted and the parietal pleura over the heads of the appropriate ribs excised using 5-mm sharp insulated coagulating microprocesss. The T4, T3, and T2 ganglions, as well as accompanying rami communicantes, and other branchs arising from upper thoracic nerves to the brachial plexus and surrounding tissues were carefully dissected, coagulated. During sympathectomy, skin temperature of middle was continuously monitored. Elevation of palmar skin temperature intraoperatively indicated an adequate sympathectomy with a definite therapeutic effect. A No. 28 Fr. thoracotomy tube was introduced through a troca under video guidance, placed under water seal after the lung was reinflated. the controlateral side was performed same procedure. After bilateral sympathectomy, chest tubes were removed, and then, he was discharged 2 days after operation with great satisfaction. The ETS provides a well-tolerated, cost-effective alternative to thoracic sympathectomy for primary hyperhidrosis and sympathetic mediated neuropathic pain disorder. And T2 ganglion is considered the key ganglion for the treatment of primary hyperhidrosis. The low incidence of compensatory sweating may by explained by the limited extent of the sympathectomy.

Efficacy of a 14Fr Blake Drain for Pleural Drainage Following Video-Assisted Thoracic Surgery (비디오 흉강경 수술에 적용된 14Fr Blake Drain의 효능 분석)

  • Background: Pleural drainage following video-assisted thoracic surgery has traditionally been achieved with largebore, semi-rigid chest tubes. Recent trends in thoracic surgery have been toward less invasive approaches for a variety of diseases. The purpose of this study was to evaluate the safety and efficacy of drainage by means of small, soft, and flexible 14Fr Blake drains. Material and Method: Between December 2007 and March 2008, 14Fr silastic Blake drains were used for drainage of the pleural cavity in 37 patients who underwent a variety of video-assisted thoracic surgical procedures at our institution. Result: The average postoperative length of hospital stay was 3.26 days (range, 2~12 days), Blake drains were left in the pleural space for an average of 3.15 days (range, 1~7 days), and the average amount of drainage was 43.8 ml/day. The maximal amount of blood removed daily by a Brake drain was as much as 290 mL. There were no drain-related complications. Blake drains seemed to cause less pain while in place, and particularly at the time of removal. Conclusion: The use of a Blake drain following minor thoracic surgery appeared to be safe and effective in drainage of fluid or air in the pleural space, and were associated with minimal discomfort.

A Study on the Effects of Design Parameters of Vertical Ground Heat Exchanger on the Borehole Thermal Resistance (수직밀패형 지중열교환기의 설계인자가 보어홀 전열저항에 미치는 영향에 관한 연구)

  • Currently, vertical closed ground heat exchangers are the most widely utilized geothermal heat pump systems and the major influencing parameters on the performance of ground heat exchangers are the ground thermal conductivity(k) and borehole thermal resistance($R_b$). In this study, the borehole thermal resistance was calculated from the in-situ thermal response test data and the individual effects of design parameters (flow rate, number of pipe, grout composition) on the borehole thermal resistance were analyzed. The grout thermal resistance was also compared with the correlations in the literatures. The borehole thermal resistance of the investigated ground heat exchanger results in 0.1303 W/m.K and the grout thermal resistance (66.6% of borehole thermal resistance) is the most influencing parameter on borehole heat transfer compared to the other design parameters (pipe thermal resistance, 31.5% and convective thermal resistance, 1.9%). In addition, increasing the thermal conductivity of grout by adding silica sand to Bentonite is more effective than the other design improvements, such as an increase in circulating flowrate or number of tubes on enhancing borehole heat transfer.

Evaluation of a Tunneling Technique under the Latissimus Dorsi Muscle for Thoracostomy Tube Placement in Eleven Dogs (흉강 튜브 삽입을 위한 넓은 등근 아래 터널 만들기 방법에 대한 평가)

  • The present study evaluated the outcome of use of thoracostomy tube tunneling technique under the latissimus dorsi muscle for the evacuation of postoperative pneumothorax induced by thoracotomy in 11 dogs. A stab incision was made through the skin and the latissimus dorsi muscle over the rib in the fifth intercostal space caudal to a surgical window. The thoracostomy tube with a Kelly hemostat was advanced into the thoracic cavity in a cranioventral direction through the sublatissimal tunnel. After tube placement, a # 1 nylon horizontal mattress suture was placed around the skin incision. The thoracostomy tube was removed after creating a negative pressure in the thoracic cavity. Dogs were monitored after surgery for pneumothorax, subcutaneous emphysema, clinical signs including dyspnea, and tube kinking in a muscle tunnel using physical examination and postoperative radiography. There was no tube kinking in the sublatissimal tunnel in 11 dogs on introducing the tubes into the thoracic cavity. The mean (${\pm}SD$) follow-up period was $19{\pm}10$ months. On postoperative radiography, there was no evidence of pneumothorax in 11 dogs. Subcutaneous emphysema was identified around the stab incision in a dog postoperatively. The subcutaneous emphysema disappeared spontaneously within 3 days. On postoperative physical examination, there was no evidence of dyspnea in 11 dogs. Our results suggest that the sublatissimal tunneling technique for thoracostomy tube placement is effective to prevent air leakage around the thoracostomy tube while the tube remains in the thoracic cavity and along the thoracostomy tunnel after tube removal. Tunneling under the latissimus dorsi muscle should be considered the thoracostomy tube placement technique to prevent iatrogenic pneumothorax with first priority.

Condensation Heat Transfer Characteristics and Pressure Drop of R-290, R-600a, and R-22 in Horizontal Smooth Pipes with a Small Diameter (수평평활 세관 내에서의 R-290, R-600a, R-22의 응축열전달과 압력강하 특성)

  • The condensation heat transfer coefficients and pressure drops of hydrocarbon refrigerants (R-290 and R-600a) and hydrochlorofluorocarbon (HCFC) refrigerants were measured in the two horizontal double pipe heat exchangers with inner diameters of 10.07 mm and 5.80 mm at a mass flux of $35.5{\sim}210.4\;kg/m^2s$ and the condensation temperature of $40^{\circ}C$. The average condensation heat transfer coefficients of hydrocarbon refrigerants were higher than that of HCFC refrigerant(R-22). The pressure drop had a magnitude in the order of R-600a > R-290 > R-22. The pressure drops in the tubes with inner diameter of 10.07 mm were approximately $6{\sim}15%$, $9.8{\sim}12.5%$ and $2.1{\sim}4.6%$ higher for R-600a, R-290 and R-22, respectively, than those with inner diameter of 5.80 mm. The condensation heat transfer coefficients were compared with the published experimental data, and showed the best agreement with Haraguchi et al.'s correlation.

Emission Rates of Biogenic Volatile Organic Compounds (BVOCs) from Various Tree Species in Korea (I) (국내 수종별 BVOCs 방출량 (1))

  • This research focused on the investigation of isoprene and terpene emissions from 30 major urban tree species. We conducted sampling using a specific dynamic enclosure system between August and September 2020. Seedlings less than three years old were enclosed in a chamber consisting of a 400 L transparent Tedlar bag. The air-flows from the chamber's outlet were sampled using Tenax-filled sorbent tubes in the presence of standard conditions (temperature: 30℃, PAR: 1,000 μmol/m2/sec). A thermal desorption GC/MS system was used to analyze 38 BVOC compounds (isoprene, monoterpene, sesquiterpene, oxygenated monoterpene, and sesquiterpene). Isoprene emitters included Phyllostachys bambusoides, Quercus serrata, Daphniphyllum macropodum, and Buxus Koreana. Monoterpene emitters included Pinus rigida, Acer pictum subsp. mono, Larix kaempfer, Magnolia denudata, Metasequoia glyptostroboides, Pinus koraiensis, Pinus densiflora, and Abies holophylla. The monoterpene emission profiles were dominated by α-pinene, myrcene, limonene, β-pinen, and sabinene, while caryophyllene and farnesene were the prominent sesquiterpenes. Predominant oxygenated monoterpene compounds were also discovered as pulegone, borneol, menthol, eucalyptol, and nerol, while caryophyllene oxide were the prominent oxygenated sesquiterpenes. Sesquiterpenes and oxygenated sesquiterpenes had relatively lower contributions for all species.

  • Thermal power generation accounts for the highest percentage of domestic power generation, among which coal-fired boiler generation accounts for the highest percentage. Coal boilers generate harmful substances and fine dust during coal combustion and have a serious effect on air pollution. So, fluidized-bed boilers have been introduced as eco-friendly coal boilers. It uses a fluid medium which affect the combustion temperature of coal. Because of it fluidized-bed boilers emit less pollutants than original one. Water-wall tubes play an important role in this fluidized bed boiler. Due to the fluid medium, the wall damage is more severe than the existing boiler. However, there is no quantitative maintenance technique in Korea yet. Remote field eddy current testing is a non-destructive evaluation technique that is often used for inspection of inner and outer wall of tube. it can inspect with non-contact and high speed. However, it is an inspection that proceeds from inside the pipe, and the water-wall tube is not able to enter the interior. In this study, we designed and simulated an external remote field eddy current sensor suitable for water-wall tube of a fluidized - bed boiler using simulations. By obtaining a signal similar to the existing remote field eddy current test, the criteria for the external remote field eddy current sensor design can be presented.

Seedlings aged < three years were enclosed in a chamber consisting of a 400 L transparent Tedlar bag. The air flow from the outlet of the chamber was sampled using Tenax-filled sorbent tubes under standard conditions (temperature: 30°C; PAR: 1,000 μmol/m2/sec). A thermal desorption gas chromatography/mass spectrometry system was used to analyze the following 38 biogenic volatile organic compounds: isoprene, monoterpenes, sesquiterpenes, oxygenated monoterpenes, and oxygenated sesquiterpenes. Isoprene emitters included Quercus mongolica, Salix koreensis, Robinia pseudoacacia, and Salix chaenomeloides. Monoterpene emitters included Pinus strobus, Cedrela sinensis, and Cercis chinensis. The monoterpene emission profiles were dominated by á-pinene, myrcene, camphene, and limonene. The predominant oxygenated monoterpene and oxygenated sesquiterpene were eucalyptol and caryophyllene oxide, respectively. For all species, the contributions of sesquiterpenes and oxygenated sesquiterpenes were relatively low.

In-Vitro Fertilization and Culture of Pig Oocytes Matured In-Vitro by Liquid Boar Sperm Stored at 4$^{\circ}C$

  • This study was carried out to investigate the effects of liquid boar sperm stored at 4$^{\circ}C$ on sperm motility, normal acrosome, and in-vitro fertilization and culture of pig oocytes matured in-vitro. The sperm-rich fraction (30~60 ml) of ejaculate was collected into an insulated vacuum bottle. Semen was slowly cooled to room temperature (20~23$^{\circ}C$) by 2 h after collection. Semen was transferred into 15 ml tubes, centrifuged at room temperature for 10 min at 800$\times$g, and the supernatant solution was poured off. The concentrated sperm was resuspended with 5 ml of lactose, egg yolk and N-acetyl-D-glucosamine (LEN) diluent to provide 1.0$\times$10$^{9}$ sperm/ml at room temperature. The resuspended semen was cooled in a refrigerator to 4$^{\circ}C$ and preserved for 5 days to examine sperm motility and normal acrosome. The medium used for oocyte maturation was modified tissue culture medium (TCM) 199. After about 22 h of culture, oocytes were cultured without cysteamine and hormones for 22 h at 38.5$^{\circ}C$, 5% $CO_2$ in air. Oocytes were inseminated with liquid boar sperm stored at 4$^{\circ}C$ for 2 days after collection. Oocytes were coincubated for 6 h in 500 ${mu}ell$ mTBM fertilization media with 0.2, 1, 5 and 10$\times$10$^{6}$ /ml sperm concentration, respectively. At 6 h after IVF, oocytes were transferred into 500 ${mu}ell$ Hepes-buffered NCSU-23 culture medium for further culture of 6, 48 and 144 h. There were significant differences in sperm motility and normal acrosome among preservation days and incubation times, respectively. The rates of sperm penetration and polyspermy were higher in 5 and 10$\times$10$^{6}$ sperm/ml than in 0.2 and 1$\times$10$^{6}$ sperm/ml. Male pronuclear formation was lower in 0.2$\times$10$^{6}$ sperm/ml than in 1, 5 and 10$\times$10$^{6}$ sperm/ml. Mean numbers of sperm in penetrated oocyte were highest in 10$\times$10$^{6}$ sperm/ml compared with other sperm concentrations. The rate of blastocysts from the cleaved oocytes (2~4 cell stage) was highest in 1$\times$10$^{6}$ sperm/ml compared with other sperm concentrations. In conclusion, we found out that liquid boar sperm stored at 4$^{\circ}C$ could be used for in-vitro fertilization of pig oocytes matured in-vitro. Also, we recommend 1$\times$10$^{6}$ ml sperm concentration for in-vitro fertilization of pig oocytes.

A Study of Working Environments and Workers Exposed to Chromium (크롬 취급 사업장의 작업환경 및 근로자 폭로 실태에 관한 연구)

  • For the purpose of providing the basic data for health management of workers who are exposed to chromium and for improving the quality of working environment, the authors evaluated blood and urinary level of chromium, the occupational history. AST, ALT, Hb, Hct, nasal specular examinaton on 287 workers who have been dealed chromium compounds in 56 manufacturing Industries of five types, that is, 38 metal plating services(plating),4 manufacture of other fabricated metal products (fabricated metal product), 5 manufacturing of dyestuff(dyestuff), 6 dressing and dyeing of leather(leather), 3 others(manufacture of pottery and ceramic househod wares, motor vehicles, electronic valves and tubes and other electronic components) and also measured the level of chromium in air from February to October 1993. The results were as follows ; 1. The utilized type of chromium compounds was the hexavalent state in plating fabricated metal product dyestuff, leather and the trivalent state in .other, and atmosperic chromium concentration as geometric mean was $0.0138mg/m^3(0.001{\sim}0.068mg/m^3)$ in plating, $0.0115mg/m^3(0.006{\sim}0.015mg/m^3)$ in fabricated matal product, $0.068mg/m^3(0.002{\sim}0.019mg/m^3)$ in dyestuff, $0.0083mg/m^3(0.002{\sim}0.028mg/m^3)$ in leather $0.0039mg/m^3(0.003{\sim}0.005mg/m^3)$ in other by the type of industry and it exceeded TLV-TWA ($0.05mg/m^3$) in five (13.6%) of plating services. 2. The geometric mean of chromium in blood was $1.54{\mu}g/dl(0.10{\sim}3.62{\mu}g/dl)$ in Plantng, $0.94{\mu}g/dl(0.27{\sim}2.82{\mu}g/dl)$ in fabricated metal product, $0.51{\mu}g/dl(0.10{\sim}3.25{\mu}g/dl)$ in dyestuff, $0.87{\mu}g/dl(0.15{\sim}8.00{\mu}g/dl)$ in leather, $0.55{\mu}g/dl(0.20{\sim}2.28{\mu}g/dl)$ in other by the type of industry(p<0.001). 3. The geometric mean of chromium in urine was $14.47{\mu}g/l(6.90{\sim}28.00{\mu}g/l)$ in planting, $4.63{\mu}g/l(0.24{\sim}43.00{\mu}g/l)$ in fabricated metal product, $5.93{\mu}g/l(1.00{\sim}33.00{\mu}g/l)$ in dyestuff, $11.09{\mu}g/l(0.80{\sim}48.00{\mu}g/l)$ in leather, $12.41{\mu}g/l(10.10{\sim}41.00{\mu}g/l)$ in other by the type of industry(p<0.001). 4. As the result of nasal specular examination, twenty four cases (8.4%) of nasal septal perforation among 287 total subjects was observed, and there were 17 (9.7%) cases in plating, 4 csaes (14.3%) in dressing and dyeing of leather. In the comparison of chromium concentration in blood and urine between the perforated group and non-perforated group, the perforated group showed a significantly higher value as $1.883{\pm}3.055{\mu}g/dl\;and\;0.793{\pm}0.815{\mu}g/dl$(P<0.001), $21.31{\pm}34.610{\mu}g/L\;and\;9.304{\pm}11.079{\mu}g/L$ (P<0.001). 5. The mean concentration of chromium in blood, urine and the mean level of AST, ALT, Hb and Hct in exposure group were higher than those of control group(p<0.001).
