Variables influencing older people's participation in exercise - Based on Transtheorethical Model (TM) -

  • The purpose of this study is to identify differences in processes of change, decision making, and self-efficacy by exercise behavioral stages and determine variables significantly affecting the older people's participation in exercises. The subjects of this study included 299 people aged 65 or over who were residents of G and S districts in Busan Metropolitan City. They were selected through purposive quota sampling at colleges and centers for old people and homes in order that they could be evenly distributed over stages of pre-contemplation, contemplation, preparation, action and maintenance. Data were analyzed using descriptive, ANOVA, Logistic Regression. Variables that discriminate between participants and non-participants in exercise include self-reevaluation, reinforcing management, cons and self-efficacy. Thus if variables that discriminate between participant and non-participation in exercise such as self-reevaluation, reinforcing management, cons and self-efficacy are fully considered in designing nursing interventions for inducing old people to become exercise participants, it would provide guidelines for nursing intervention programs as appropriate for the people's exercise stages.   • Purpose: The purpose of this study was to identify factors associated with physical activity in older adults living alone. Methods: This is cross-sectional, correlational study used secondary data from the 2019 Community Health Survey. The participants were 19,134 older adults aged 65 years or older and living alone. Data were analyzed using descriptive statistics, the 𝜒2 test, and linear regression with the SPSS/WIN 27.0 program. Results: The participants' average amount of physical activity was 1,359.32 MET-min/week; 50.7% were inactive, 38.6% were minimally active, and 10.7% were health-promoting active. Demographic factors that predicted physical activity were gender, age, education level and monthly income, whereas health behavior factors included subjective health status and high risk drinking (R2=.055, p<.001). Psychological factors were sleeping time, stress, depression and fear of falling, and social factors included social contact with neighbors or friends, and participation in social or leisure activites(R2=.070, p<.001). Conclusion: It is important to determine the level of physical activity in older adults living alone. Demographic characteristics, health behaviors, psychological factors, and social factors should be considered in the development of specific and integrated nursing interventions to increase physical activity in older adults.

국민건강보험공단 진료비 자료를 활용한 65세 이상 방문건강관리사업 대상자의 비용-편익분석 (A Cost Benefit Analysis of Visiting Health Care for People 65 Years and Over Using Total Medical Expense from Health Insurance Claims Data)

  • Purpose: This study aimed to investigate the effect of home visiting care service and to evaluate the effect from the cost-benefit among older people. Methods: The target participants were enrolled in 2007 and they were classified into 1 year, 2 years, 3 years, and 4 years according to their service provision period. We analyzed health insurance claims data and entitlement data from the National Health Insurance Service databases between 2006 and 2010. This study examined the participants' social-economic and health status factors related to total medical expense. Cost-benefit analysis was done using the net benefit and benefit/cost ratio. Results: Based on the use of home visiting service, the effect was reduced by 223,914 won. The cost for 952,109 people aged 65 or older was 39,891,462,882 won and the benefit was 213,190,534,626 won. The net benefit was 173,299,071,744 won and the benefit/cost ratio was 5.34 times, which was very economical. Conclusion: Home visiting health care should continue to expand as a means of economically effective health care for people aged 65 and older and to ensure health equity for vulnerable groups.

65세 이상 노인의 국가건강검진 수검 여부에 미치는 요인: 제8기 국민건강영양조사(2019~2021년) 자료를 이용하여 (Factors Influencing the national health screening program in Seniors over 65 years old: Using the eight Korea national health and nutrition examination survey(KNHANES VIII 2019~2021))

  • Objectives: This study attempted to examine the factors that affect the national health checkup rate of the elderly, analyze the impact of these factors, and discuss countermeasures to problems that may arise based on this. Methods: This study used the '8th National Health and Nutrition Examination Survey (2019-2021)', a nationwide survey, as the main data. The subjects of the study are seniors aged 65 years or older, the dependent variable is whether or not they underwent a national health checkup, and the independent variables are gender, marital status, residence, education level, income level, economic activity, type of medical coverage, private insurance subscription, subjective health, High blood pressure, diabetes, depression, stress, and weight change were selected. The statistical analysis package for data analysis is SPSS ver. 27.0 was used, and a p value of less than 0.05 was considered significant. Results: Of the 2,806 seniors aged 65 years or older, 2,074 (73.9%) took the national health screening over the past two years. In terms of marital status, married was 1.451 times higher than single (p<0.001), and in terms of residence, myeon was 1.240 times higher than dong (p<0.01). In terms of education level, college graduates were 2.053 times higher than elementary school graduates (p<0.001), and in terms of economic activity, the employed were 1.325 times higher than the unemployed (p<0.01). Private insurance was 1.883 times higher than non-subscription (p<0.001), weight change was 1.234 times higher for change than no change (p<0.05), and current smoking rate was 2.003 times higher for non-smokers than for former smokers. It was high (p<0.001). Conclusion: In conclusion, if differentiated promotion and health education are strengthened for the elderly who cannot participate in the national health screening, the participation rate of the elderly in the national health screening is expected to increase.

Falls in Community-dwelling Korean Older Adults: Prevalence and Associated Factors: The 2019 Community Health Survey Data

  • Objectives: This study aimed to identify the prevalence of falls in community-dwelling older adults and to identify associated factors using the 2019 Community Health Survey. Methods: The original data was from the 2019 Community Health Survey, and the study sample comprised 1,642 older adults aged 65 years and older in Jeju province. Data collection was conducted from August 16 to November 20, 2019, through an interview done by a trained investigator. Respondents were queried about demographic characteristics, riding bicycles, hospital treatment due to an accident or poisoning in the previous year, fall experiences in the past year, fear of falling, self-management status, and pain and discomfort. Multivariate logistic regression analysis was used to evaluate for associations between potential risk factors and falls. Results: The prevalence of falls in this community-dwelling older adults was 13.1%. Falls were associated with riding bicycles (odds ratio = 4.7; 95% confidence interval: 2.26-9.81), fear of falling (odds ratio = 0.3; 95% confidence interval: 0.24-0.49), hospital treatment due to an accident or poisoning in the previous year (odds ratio = 7.8; 95% confidence interval: 5.02-12.19), self-management status (odds ratio = 0.6; 95% confidence interval: 0.34-0.89), and pain and discomfort (odds ratio = 0.6; 95% confidence interval: 0.40-0.87). Conclusions: We found that the prevalence of approximately about 13% of older adults living in a community has experienced falls. Based on the results of the study, we provided primary data to develop the care management intervention program to prevent falls and avoid risk factors that cause falls in community-dwelling older adults.

Risk Factors for Falls among the Community-Dwelling Elderly in Korea

  • Community-based centres were surveyed to determine the frequency of and risk factors for falls among elderly Koreans. We examined fall-related risk factors, including physiological and physical health, psychosocial functions, self-reported physical capacity and activity, vision, and the use of medication, among 351 elderly people aged 65 years or older, with ambulatory. Forty-two per cent of elderly Korean subjects reported at least one episode of falling in the previous 12 months, $38\%$ of whom had consequences that required either the attention of a physician or hospitalization. Factors significantly associated with an increased risk of falling were a restricted activity during the previous five years (adjusted OR 1.3), use of alternative therapy (adjusted OR 2.7), low knee flexor and extensor-muscle strength (adjusted OR 1.21 and 1.20), and poor balance with closed eyes (adjusted OR 8.32). We conclude that falls among older persons living in the community are common in Korea and that indicator of bad health and frailty or variables directly related to neuromuscular impairment are significant predictors of the risk of falling.

한국 노인의 외로움 측정도구 개발 (Development of the Korean Geriatric Loneliness Scale (KGLS))

  • Purpose: The purpose of this study was to develop and psychometrically test the Korean Geriatric Loneliness Scale (KGLS). Methods: The initial items were based on in-depth interviews with 10 older adults. Psychometric testing was then conducted with 322 community-dwelling older adults aged 65 or older. Content, construct, and criterion-related validity, classification in cutoff point, internal consistency reliability, and test-retest reliability were used for the analysis. Results: Exploratory factor analysis showed three factors, including 15 items explaining 91.6% of the total variance. The three distinct factors were loneliness associated with family relationships (34.3%), social loneliness (32.4%), and a lack of belonging (24.9%). As a result of confirmatory factor analysis, 14 items in the three-factor structure were validated. Receiver operating characteristic analysis demonstrated that the KGLS' cutoff point of 32 was associated with a sensitivity of 71.0%, specificity of 80.2%, and area under the curve of .83. Reliability, as verified by the test-retest intraclass correlation coefficient, was .89, and Cronbach's ${\alpha}$ was .90. Conclusion: As its validity and reliability have been verified through various methods, the KGLS can contribute to assessing loneliness in South Korean older adults.

노인의 평생교육 참여동기와 진로장벽의 관계에서 소득의 조절효과 (The Moderating Effect of Income on the Relationship between the Participation Motives of the Elderly in Lifelong Education and the Career Barriers)

  • 본 연구는 노인의 평생교육 참여동기가 진로장벽에 미치는 영향을 파악하고, 이 영향에 대한 소득의 조절효과를 실증적으로 분석하는 것을 목적으로 수행되었다. 이러한 목적을 달성하기 위해 본 연구에서 확인한 구체적인 연구문제는 다음과 같다. 첫째, 노인의 평생교육 참여동기가 진로장벽에 미치는 효과는 어떠한가? 둘째, 노인의 소득이 진로장벽에 미치는 영향은 어떠한가? 셋째, 노인의 평생교육 참여동기와 진로장벽의 관계에서 소득의 조절효과는 어떠한가? 이를 위해 D시에서 평생교육프로그램에 참여하고 있는 65세 이상의 노인 187명을 대상으로 연구를 진행하였다. 연구의 결과를 위해 주요변수의 상관관계를 확인하였으며, 조절효과를 분석하기 위해 중다회귀분석을 실시하였다. 자료를 분석한 결과는 다음과 같다. 첫째, 노인의 평생교육 참여동기는 진로장벽에 부적인 영향이 있음을 확인했다. 평생교육에 참여하거나 참여한 노인은 능력을 발전시키고, 인정받음으로써 진로장벽이 감소됨을 알 수 있었다. 둘째, 노인의 소득은 진로장벽에 유의한 부적인 영향을 미치는 것으로 분석되었다. 소득이 많을수록 진로장벽의 부적 관계가 강화됨을 확인했다. 셋째, 노인의 평생교육 참여동기와 진로장벽의 관계에서 소득은 조절효과를 갖는 것으로 나타났다. 이와 같은 연구결과를 토대로 논의와 시사점을 제시하였다.

Use of Information Technologies to Explore Correlations between Climatic Factors and Spontaneous Intracerebral Hemorrhage in Different Age Groups

  • Spontaneous intracerebral hemorrhage (sICH) has a high mortality rate. Research has demonstrated that sICH occurrence is related to weather conditions; therefore, this study used the decision tree method to explore the impact of climatic risk factors on sICH at different ages. The Taiwan National Health Insurance Research Database (NHIRD) and other open-access data were used in this study. The inclusion criterion was a first-attack sICH. The decision tree algorithm and random forest were implemented in R programming language. We defined a high risk of sICH as more than the average number of cases daily, and the younger, middle-aged and older groups were calculated as having 0.77, 2.26 and 2.60 cases per day, respectively. In total, 22,684 sICH cases were included in this study; 3,102 patients were younger (<44 years, younger group), 9,089 were middle-aged (45-64 years, middle group), and 10,457 were older (>65 years, older group). The risk of sICH in the younger group was not correlated with temperature, wind speed or humidity. The middle group had two decision nodes: a higher risk if the maximum temperature was >$19^{\circ}C$ (probability = 63.7%), and if the maximum temperature was <$19^{\circ}C$ in addition to a wind speed <2.788 (m/s) (probability = 60.9%). The older group had a higher risk if the average temperature was >$23.933^{\circ}C$ (probability = 60.7%). This study demonstrated that the sICH incidence in the younger patients was not significantly correlated with weather factors; that in the middle-aged sICH patients was highly-correlated with the apparent temperature; and that in the older sICH patients was highly-correlated with the mean ambient temperature. "Warm" cold ambient temperatures resulted in a higher risk of sICH, especially in the older patients.

한국노인의 고혈압 유병률과 위험요인 (Prevalence of Hypertension and Its Risk Factors among Aged 65 and over in Korea)

  • Purpose: The purpose of this study was to identify the prevalence of hypertension and its risk factors among community dwelling aged 65 and over in South Korea. Methods: A total of 1,367 subjects from the 2010 Korean National Health and Nutrition Examination Survey (KNHANES) were selected. The data analysis was conducted by SPSS WIN 19.0 using descriptive statistics, ${\chi}^2$-test, and logistic regression analysis. Results: The prevalence of hypertension was found to be 47.70%. Logistic regression analysis shows that hypertension was more prevalent as people became older; in females; in elderly who were not employed as compared to those employed; elderly with diabetes, and obesity as compared to those without these health conditions; as elderly who performed more moderate intensity exercise. While, education, household income, high risk alcoholic drinking, smoking, stress perception, usual activity, and eating out behavior were not associated with prevalence of hypertension in this study. Conclusion: The finding of prevalence of hypertension and its risk factors is expected to promote the screening or prevention strategy for community dwelling aged 65 and over at risk of hypertension in Korea.
