Background: Intra-articular distal humeral fractures can be surgically challenging. It remains under discussion whether open reduction and internal fixation (ORIF) or total elbow arthroplasty (TEA) is more beneficial for treatment of the elderly. This study aimed to compare the clinical and functional outcomes of ORIF and TEA for managing intra-articular distal humerus fractures in patients aged 65 years or older. Methods: Patients who underwent ORIF (n=28) or TEA (n=43) for in intra-articular distal humerus fracture between May 2008 and December 2018 were reviewed. Range of motion, Mayo Elbow Performance Score (MEPS), Disabilities of the Arm, Shoulder, and Hand (DASH) score, radiologic outcomes, and surgical complications were evaluated at the final follow-up visit. Results: The ORIF and TEA groups showed a mean arc of flexion-extension of 97°±21° and 101°±12°, respectively. The mean MEPS and DASH scores were 94±15 and 27±12 points, respectively, in the ORIF group and 81±27 and 47±28 points in the TEA group. This difference was statistically significant. The incidence of total complications was similar between the groups. Conclusions: In patients older than 65 years with intra-articular distal humerus fracture, ORIF had better outcomes than TEA.
The purpose of this study is to prepare desirable elderly housing facilities system in compliance with various characteristics of each region and to help establish policies regarding elderly housing facilities. Present realities and problems of the eight free welfare facilities of two sorts, a free home for the aged and free nursing home in Jeju Province have been studied. In Jeju Province, the average life span and the ratio of the aged people older than 65 are longer and higher than other provinces and the mentality of self-reliance is more widespread. The results are as follows : 1) The average age of the aged is 70 or older and more facilities are located in rural districts. 2) Some facilities are located in the places where are not adequate for life of the aged, and not even suitable as welfare facilities for the aged. 3) Housing environmental conditions of the facilities are relatively good, while indoor facilities for recreation and social life were not well equipped. 4) Life in the facilities can be assessed relatively free, while individual privacy is not well protected. 5) Major community contacting activities included home help service, participation in regional activities, opening facilities to the community.
Purpose: The purpose of this study was to investigate sleep quality in older adults in nursing home with objective data collection. Methods: Participants included 74 older adults in nursing homes in Korea aged 65 years or above. Data were collected using a wearable device (Fitbit), salivary melatonin level and Sleep Disorder Inventory (SDI). The Pearson correlation coefficient was calculated to examine whether there was any correlation between sleep-related variables such as Total Sleep Time (TST), Rapid Eye Movement (REM) sleep, shallow sleep, deep sleep, salivary melatonin level and SDI. Results: There were distortion of sleep structure, as TST comprised short REM sleep (15.93±5.47%), long shallow sleep (74.18±8.08%) and short deep sleep (9.89±5.03%). Also, salivary melatonin levels were low (15.06±7.77 pg/mL). Moreover, we found than melatonin was significantly associated with TST (r = .251, p= .044), REM sleep (r= .294, p= .020) and deep sleep (r= .391, p= .002). But there was no correlation between SDI and other sleeprelated variables. Conclusion: These findings highlight that insufficient sleep structure is associated with the salivary melatonin level among older adults in nursing home. We suggest developing programs to promote sleep quality of older adults in nursing homes.
Objective : To examine the association between dietary factors and cognitive impairment in older Korean women living in the community. Methods : Wave 2 data, from the Suwon Longitudinal Aging Study (SLAS), of 365 women aged 65 years or over, were used. The Korean version of the 30-point Mini-Mental State Examination (MMSE-K) was used to assess cognitive impairment (score$\geq$19). Dietary habits and frequencies of food group consumption were also examined. Results : A total of 67 women (18.4%) were found to be cognitively impaired. In bivariate analyses, nondietary factors, such as age, marital status, education, income, self-rated health, depression, emotional support, social activity, exercise, and dietary factors, such as self-rated nutritional status, frequency of beans and bean products and milk and dairy products consumption were associated with cognitive impairment. In the multivariate analysis, a higher frequency ($\geq$1 vs. <1 time/day) of beans and bean products (OR=0.48, 95% CI: 0.23-0.99) and milk and dairy products (OR=0.25, 95% CI: 0.10-0.61) consumption was inversely associated with cognitive impairment, after adjustment for non-dietary factors. Conclusion : These results suggest that dietary factors may play a significant role in cognitive impairment of older Korean women.
Objectives: While the risk of depression is expected to substantially increase among older adults receiving community care, leisure life satisfaction can be regarded as a key component in enhancing the mental health of those receiving community care. However, it is not yet known whether community care utilization increases the risk of depression, or what role is played by leisure life satisfaction in these settings. This study investigated the relationship between community care utilization and depression, as well as the main effect and the moderating role of leisure life satisfaction on the link between community care utilization and depression among older adults. Methods: This study, using the 2019 Korean Welfare Panel Survey, conducted multiple regression analysis on data from 4494 elderly people aged 65 years or older. Results: After controlling for potential covariates, older community care recipients were more likely to report symptoms of depression than those who did not receive community care. Meanwhile, leisure life satisfaction was negatively associated with depression in older adults. The test for interaction between community care utilization and leisure life satisfaction revealed that leisure life satisfaction significantly attenuated the link between depression and community care utilization. Conclusions: The findings of this study imply that leisure life satisfaction could play a meaningful role in improving the mental health of older adults receiving community care. Welfare policies affecting older adults should consider leisure life satisfaction as an important resource for reducing depression in community care settings.
Background: The purpose of this study is to analyze the factors affecting the unmet healthcare needs of older people with chronic diseases in Korea and provide a basic research report to strengthen their access to medical care. Methods: In the 2020 older people survey data, 8,182 older people aged 65 or older who were diagnosed with one or more chronic diseases were the final subjects of the study. According to Andersen's behavioral model used in unmet healthcare needs, independent variables were composed of predisposing factors, possible factors, and necessary factors, and whether or not unmet healthcare needs was set as dependent variable. Results: Of the older people with chronic diseases, 1.6% experienced unmet healthcare needs, of which 55.9% experienced unmet healthcare needs for reasons related to economic burden, 31.6% physical constraints, and 12.5% time constraints. As a result of the analysis, older people with chronic diseases were more likely to experience unmet healthcare needs if they were relatively low in age, low in education level, no spouse, low in household income, poor subjective health, complex chronic diseases, and functional restrictions. However, by major reasons for experiencing unmet healthcare needs, living in rural areas were more likely to experience unmet healthcare needs due to physical constraints, and those who participated in economic activities and who had were more likely to experience unmet healthcare needs due to time burden. These results were not derived when only unmet healthcare needs was set as the dependent variable. Conclusion: This study emphasizes the need for an approach by cause of unmet medical occurrence by suggesting that there are differences in influencing factors by reason for experiencing unmet healthcare needs.
Purpose: This study was done to examine factors affecting quality of life among community-dwelling elderly women who live in rural areas. Methods: The design of this research was cross-sectional descriptive study. The participants were 92 community-dwelling women aged 65 or older. Data were collected from November 1 to 15, 2013. ADL (Kart's Index), GDS-SF (Geriatric Depression Scale Short Form) and GQOL (Geriatric Quality of Life Scale) were used to measure variables. Data were collected using self-administered questionnaires and analyzed using descriptive statistics, Spearman's coefficient and stepwise multiple regression. Results: The participants' mean age was 77.5, and 85.9% had elementary school graduation or less education, and for 64.1%, their economic status was low. Spearman's rho coefficient analysis found that QOL was significantly associated with depression (r=-.72, p<.001), perceived health (r=.58, p<.001), regular exercise (r=.47, p<.001), education level (r=.29, p=.005), and ADL (r=-.21, p=.043). Multiple regression analysis showed that 65.9% of their QOL was explained by depression (${\beta}=-.72$), perceived health (${\beta}=.24$), ADL (${\beta}=-.16$), exercise (${\beta}=.22$) and number of diseases (${\beta}=.19$). Conclusion: These results indicate that older women who live rural areas need support for ADL, and prevention of depression to improve their quality of life.
Objectives: The purpose of this study was to identify the health promotion activities of the elderly Korean aged 65 or older and to examine the related factors associated with the health promotion activities. Methods: Data were obtained from 2008 Social Statistics Survey of Korea National Statistical Office of 6,207 people aged 65 or older. We measured the socio-demographic characteristics, physical health status, social health status, and health promotion activities. Statistical analyses were employed through the $X^2$-test and Odds ratio using Logit Model. Results: In our study, health promotion activity practice rates were varied among the socio-demographic characteristics, physical and social health status. Our findings also support that better socio-demographic and physical health status explain the higher practice rates of health promotion activities. In addition, the higher social health status was associated with better practice rates of health promotion activities. Conclusion: We found that the health promotion activities of the elderly could be encouraged by better socio-demographic status and physical and social health status. To better accomplish the health promotion for the elderly in our community, policy-makers should need careful political deliberation for executing health promotion services considering the distinctions of programme and target groups.

The purpose of this study is to suggest a plan to establishment and management for a library for old adults. Therefore, the library for old adults defined to the concept of providing a specialized service by providing the necessary material with an elderly user over 65 focus. And presenting the basic direction and specific direction, organizational plan, and ordinance on establishment and management of a library for older adults. In addition, after conducting a semi-structured interview based on a pre-prepared questionnaire targeting 30 elderly people aged 65 or older who had experiences using public libraries, the results were analyzed, and 20 responses indicated that the purpose of using the library was a way to participate in the program as an essential service for the elderly. 12 respondents stated that the purpose of using the library was possibly a necessity for such things as writing autobiographies and reflections of historical events. Lastly, investigating the operational cases of two public libraries that already utilize elderly learning and health information rooms, it was concluded that the operation of the elderly library also needs to reflect the regional differences and characteristics unique to older adults.
Unhealthy dietary behavior such as insufficient calcium intake can be one of risk factors of osteoporosis and chronic diseases in older people. This study evaluated the recent trends in dietary calcium intake and the food source in Korean adults aged 50 years or older using the Korea National Health and Nutrition Examination Survey (KNHANES, 2015-2019) data. This study used 24-hour recall survey data to investigate the calcium intake, the major food groups and main dishes contributing to the calcium intake. The mean calcium intake was 479.55-506.81 mg/day. The major food groups that contribute to calcium intake were vegetables, milks and fishes. Dairy and frozen desserts and kimchi were the major dish groups that contributed to the calcium intake. Calcium intake from milk in dairy and frozen dessert group has been high in last 5 years (50-64 years old: 34.71-47.68 mg, 65-74 years old: 29.72-43.65 mg, over 75 years old: 22.91-42.93 mg). In addition, baechu-kimchi is the most contributed to the calcium intake in kimchi group (50-64 years old: 35.10-41.47 mg, 65-74 years old: 29.62-34.96 mg, over 75 years old: 23.79-29.13 mg). In conclusion, various source of calcium needs to be recommended to increase intake calcium in over 50 years, which may reduce chronic diseases and improve quality of life.
