Association between Visual Impairment and Nutritional Risk among Older Adults with Diabetes: A Population-Based Cross-Sectional Study

  • Purpose: Despite the high prevalence of visual impairment caused by diabetic retinopathy and nutritional problems among older adults with diabetes, evidence regarding factors related to nutritional risk in this population is limited. Therefore, this study aimed to identify the correlates of nutritional risk among older adults with diabetes, focusing on visual impairment. Methods: This study was a secondary data analysis of the 2020 National Survey of Older Koreans aged 65 years and above. The sample comprised 2,376 older adults with diabetes, and complex sample ANOVA and Rao-Scott chi-square tests were used to compare the groups according to visual impairment. Complex-sample logistic regression analyses were conducted to verify the association between visual impairment and nutritional risk. Results: Older adults with diabetes, who also have severe visual impairment, are more likely to have nutritional risk status than those without impairment after controlling for covariates (odds ratio [OR] = 2.44, 95% confidence interval [CI] 1.16~5.13). Among the covariates, depression (OR = 3.58, 95% CI 2.60~4.94), dependent activities of daily living status (OR = 2.79, 95% CI 1.60~4.86), and experience of hospitalization during the past year (OR = 2.51, 95% CI 1.57~4.03) were strongly associated with nutritional risk. Conclusion: Severe visual impairment increases the nutritional risk among older adults with diabetes. Therefore, it is essential to prevent visual impairment due to exacerbation of diabetes through appropriate management. Additionally, tailored nutritional interventions for visually impaired older adults with diabetes that consider visual characteristics are required.

Effect of Work on Medical Expenditures by Elderly: Findings From the Korean Health Panel 2008-2013

  • Background: This study was conducted to investigate the effects of work on medical expenditures by the elderly. Methods: Data pertaining to individuals aged 65 or older collected by the Korean Health Panel 2008-2013 were used. The effects of work on medical expenditures were analyzed in a panel tobit model adjusted for several variables of demographic factors, socioeconomic factors, and health factors for health care. Data were also analyzed based on age groups (65-74, $75{\leq}$), type of work (waged or self-employed), and working time (daytime work or night time work). Results: Among the elderly older than 65 years, 34-37% were workers. Work among the elderly reduced medical expenditures relative to nonworking elderly. Specifically, medical expenditures were lower in individuals older than 75 years, as well as among those who were self-employed insured and had medical aid insurance and those who exercised. However, medical expenditures were higher among females, married individuals, those with a higher household income, and those with a chronic disease. Elderly wageworkers showed reduced medical expenditures than nonworking elderly and elderly daytime workers did. Conclusion: The elderly population's work, especially wage work and daytime work, reduced medical expenditures relative to no work. These results provide valuable information for policymakers by indicating that work was associated with lower medical expenditures than no work. If elderly work is to be encouraged, it is necessary to provide a variety of high-quality wage work.

  • According to the provisions of the United Nations, the proportion was 65 years of age or older population is 7% more than in the total population aged society. Due to the rapid decrease fertility and medical technologies and the improvement of living standards, the average life expectancy has emerged as an aging society. In the case of our country, and in 2011 the proportion of the population aged 65 and over 11.4%, the future is even faster aging rate is expected to launch in 2026, the super aged society. The days went by relying on caring for personal piety. Nursing crime is often due to the patriarchal family culture and afford. To care for patients with severe human or plant in the home, not to assume that society. Expanding the scope of long-term care insurance service person and the family burden reduced through measures to apply for health insurance, nursing care can prevent crime.

Association Between Objective Social Isolation and Unmet Medical Needs: A Nationwide Cross-sectional Study in Korea

  • Objectives: The aim of this study was to analyze the relationship between objective social isolation (SI) and unmet medical needs (UMN) in adults aged 19 and older. Methods: A cross-sectional analysis was conducted of 208 619 adults aged 19 and older, excluding missing data, using the 2019 Korea Community Health Survey. To analyze the association between objective SI and UMN, the chi-square test and logistic regression analysis were performed. Results: The prevalence of UMN was 1.14 times higher (odds ratio [OR], 1.14; 95% confidence interval [CI], 1.06 to 1.23) among those with SI than among those without SI, and the OR for groups with 5 SI types was 2.77 (95% CI, 1.86 to 4.12) compared to those with no SI types. In addition, a stratified analysis by age group showed that the association between SI and UMN existed even in groups under 64 years old. However, among those aged 65 and older, SI was associated with an OR of 1.53 (95% CI, 1.37 to 1.71) for UMN compared to non-SI. As the number of SI types increased, the prevalence of UMN also increased, indicating a strong association between SI and UMN in older adults. Conclusions: This study found that individuals with SI experienced UMN due to fear and anxiety about interpersonal relationships. Therefore, based on the results of this cross-sectional study, it is necessary to investigate the causal relationship between SI and UMN through future longitudinal data.

  • Purpose: The purpose of this study was to investigate the factors affecting medication errors and the medication management educational needs of community-dwelling older adults. Methods: From February 20 to February 23, 2017, 150 elderly people aged 65 or older were surveyed using a structured questionnaire. Results: A total of 85.7% of the older adults were taking medication, but their drug knowledge was found to be low. The medication error rate was 24.9%, and the score for medication management education requirement was 3.61 out of a possible 5points. Factors affecting medication errors were perceived health status and knowledge of medication, and their explanatory power was 43% in total. Conclusion: It was concluded that nursing intervention is needed to reduce older adults' medication errors and to increase their knowledge of medication. Additionally, groups of older adults with high medication errors should be intensively educated, and when developing a medication management education program, the contents of the sub-areas and items in which the participants' needs were high should be reinforced.

Correlation Between Physical Activity Measured by a Wearable Device and Quality of Life in Older Adults

  • Background: Physical activity and quality of life (QOL) influence the health status of older adults. Recently, the use of wearable devices to monitor physical activity has increased. Objects: This study examined the relationship between the amount of physical activity, measured using a wearable device, and QOL among older adults. Methods: In total, 71 older adults (aged ≥ 65 years) were enrolled. The amount of physical activity was measured using a wearable device with a wrist strap, and daily physical activity was classified according to intensity (sedentary, light, moderate, or very active). Self-reported QOL was evaluated using the Short Form 36 (SF-36) questionnaire. Pearson and Spearman correlation analyses were conducted to analyze parametric and non-parametric variables, respectively. The relationship between amount of daily physical activity and SF-36 scores was assessed. Results: The correlation analyses revealed positive correlations between the amount of moderate-intensity and very active physical activity (minutes/day) and SF-36 scores (p < 0.05). Conclusion: Physical activity of at least moderate intensity is associated with better QOL in older adults. Further studies are required to verify the effects of increased physical activity on QOL in older adults.

  • Germany became a super-aged society which has the number of people aged 65 or older reached 19 percent of population in 1997. Similarly, there is a tendency in Korea to increase the number of older people coupled with an increase in the number of households of the senior citizen who lives alone due to a rapid change of family unit. Research on geriatric in Germany is being carried out in many different ways, like medical, welfare, and social aspects because of the fact that Germany is in the middle of a super-aged society. Therefore, analysis and evaluation of elderly care facilities in Germany will be used as a fundamental resource for overall planning of elderly care and welfare facilities which are expected to grow demand dramatically.

지역사회 거주 노인의 낙상 경험 영향요인: 2021년 지역사회건강조사 활용 (Factors Influencing Fall Experiences among the Older Adults in Community: Using the 2021 Community Health Survey )

  • Purpose: This study aims to identify the factors that influence the experience of falls among older adults living in the community. Methods: The study participants were 70,887 65-year-olds who participated in the 2021 Community Health Survey. The study employed the Rao-scott x 2 test to examine the variation in fall experiences based on the characteristics of the older adults. Multiple logistic regression analysis was conducted to investigate these characteristics' impact on older adults' fall experiences. Results: The proportion of subjects in fall experience was 16.6%. The factors influencing the subject's fall experience were sex (odds ratio [OR]=1.47, 95% confidence interval [CI]=1.37~1.57), age (OR=1.48, 95% CI=1.34~1.65), family structure (OR=1.23, 95% CI=1.15~1.31), body mass index (OR=1.13, 95% CI=1.06~1.20), diabetes (OR=1.12, 95% CI=1.06~1.20), depression experiences (OR=1.56, 95% CI=1.42~1.70), stress (OR=1.12, 95% CI=1.05~1.19), subjective health status (OR=1.77, 95% CI=1.63~1.92), life satisfaction (OR=1.57, 95% CI=1.41~1.76), and chewing discomfort (OR=1.29, 95% CI=1.21~1.38). Conclusion: Efforts should be made to effectively educate and develop various programs aimed at reducing falls among older adults. It is essential to emphasize the importance of continuous and active attention to falls in the older adult population.

  • Objectives: This study investigated the association between oral health and suicide attempts in adults aged 65 years or older using the 7th Korea National Health and Examination Survey (KHANES, 2016-2018). Methods: Of the total 24,269 participants of the 7th KHANES (2016-2018), 1,488 adults aged 65 years or older who had an oral health examination were selected. Variations in suicide attempts with demographic characteristics and perceived oral health status were analyzed using the χ2 test, and the association between oral health and suicide attempts was analyzed using the logistic regression model. Results: The suicide attempt rate was higher among older adults with chewing (p<0.01) and speaking (p<0.05) difficulties. The odds ratio for a suicide attempt was 1.05 times higher among those with tooth loss (p<0.05). Conclusions: Our results showed that older adults with tooth loss had an increased risk of suicide. Thus, it is important to prevent tooth loss, in the first place, through proper oral health management. If tooth loss occurs, appropriate treatment should be prioritized to facilitate functional recovery. Finally, national support is required for older adults with limited access to dental care.

Factors affecting dental biofilm maturity assessed with Quantitative Light-induced Fluorescence-Digital in Korean older adults

  • Objectives: The study aimed to analyze the factors affecting the maturity of dental biofilm, which was assessed with quantitative light-induced fluorescence-digital(QLF-D), in a sample of Korean older adults. Methods: This cross-sectional study included 67 participants, aged 65 years and older. All participants completed a questionnaire and tests to measure their manual dexterity and handgrip strength, which are parameters that indicate hand function abilities. To evaluate dental biofilm maturity, 804 surfaces of six index teeth were imaged using QLF-D and then quantified as ${\Delta}R$ values. All data were collected from May 25, 2017 to April 30, 2018. The independent t-test, one-way analysis of variance, and step-wise multiple linear regression were performed to analyze the factors associated with the maturity of dental biofilm (${\Delta}R$). Results: The multivariate linear regression analysis revealed that the factor most strongly related to dental biofilm maturity(${\Delta}R$) was manual dexterity (${\beta}=-0.326$), followed by handgrip strength (${\beta}=-0.303$) and use of interdental cleaning devices (${\beta}=-0.283$) (p<0.05). Conclusions: Manual dexterity, handgrip strength, and use of interdental cleaning devices are factors that can predict dental biofilm maturity in adults aged 65 years or older. Therefore, the hand function of a patient should be evaluated first, before assessing the oral hygiene status of the patient or providing him/her with oral health education, and the dental hygienist should provide differentiated oral hygiene care depending on the patient's hand function ability. Finally, dental hygienists should help older adults to recognize the importance of auxiliary oral hygiene devices such as interdental brushes and keep motivating them to use the devices more frequently.
