본 연구서는 인지장애나 경증 치매로 의심되는 65세 이상 90세 이하의 남녀 노인 12명을 실험집단으로 선정하여 6개월간 독서치료 프로그램을 실시하고, 그 결과를 토대로 독서치료가 치매노인의 인지도와 우울증에 미치는 효과에 대해 연구하였다. 측정도구는 신뢰도와 타당도가 검증된 한국판 MMSE-K와 우울정도를 측정하기 위해 단축형 노인 우울척도를 사용하였다. 치매노인을 위한 독서치료 프로그램을 실시한 결과 인지도는 향상되고 우울증은 감소함으로써 독서치료가 경증 치매노인의 증상을 호전시키는데 효과가 있음을 알 수 있었다.
연구배경: 본 연구는 지역박탈지수(Area Deprivation Index, ADI)와 우리나라 노인의 노쇠와의 관련성을 분석하는 것을 목적으로 하였다. 방법: 2014년부터 2019년까지 국민건강영양조사 자료 중 65세 이상 노인을 대상으로하였다. 노쇠는 Fried phenotype을 기준으로 평가하였다. ADI와 노쇠의 관련성을 검증하기 위하여 다중 로지스틱 회귀분석을 실시하였다. 결과: 연구대상자 9,825명 중 ADI가 낮은 지역의 노인보다 높은 지역의 노인이 1.23배 더 (odds ratio [OR], 1.23; 95% confidence interval [CI], 1.09-1.37). 기혼자의 경우 ADI가 낮은 지역에 거주하는 노인보다 높은 지역에 거주하는 노인이 1.35배(OR, 1.35; 95% CI, 1.16-1.57) 더 노쇠한 것으로 나타났다. 결론: 본 연구는 65세 이상의 노인을 대상으로 ADI와 노쇠의 관련성을 파악하였다. 특히 기혼자의 경우 ADI가 높은 지역에 거주할 수록 더 노쇠한 것으로 나타났다. 따라서 건강불평등 해소를 위해 박탈지수가 높은 지역과 낮은 지역 간 격차를 감소시키기 위한 정책적 개입의 필요성을 제언한다.
Neutralized antibody titer of Akabane disease virus were performed from 810 dairy cows in 45 farming households from May 1992 to December 1993 in Incheon area. The 503 dairy cows at the 21 farming household were conducted from May to December 1992, 307 cows were from January to December 1993 The results obtained are summarized as follows 1. Evaluation of neutralized antibody titer of 810 dairy cows tested during two yews revealed that 68.1% (552 hearts) , during the four year revealed that 69.6% (350 heads), the later year was 65.8%(202 heads ) was more than antibody titer 16. 2. Antibody investigation according to area and years, showed no significant difference in the whole area and both years as 65-73% from 1992, and as 65-70% from 1993 had an antibody titer of above 16 or more. 3. Antibody investigation according to age, showed that 34.4% of cows aged below 2 had a titer of above 16, compared with 80.8% for cows aged above 5. It demonstrated that the younger cows had the lower titer level, and the older the higher. 4. Monthly variation of antibody titer showed that the highest level of antibody titer was observed in September, the lowest was in June. It meant that the best periods of vaccination were April, May, June. 5. The result of epidemidogical study to 40 farming households showed that 35 farms (87.5%) had abortions of which were 14 stillbirths, 5 abnormal births, and 13 farms (32.5%) had vaccination.
Purpose: To improve pulmonary function and decrease in balance ability with increasing forward head position and vertebral curvature, we applied Figure-8 brace to confirm the immediate effect on vital capacity and balance and to see if it is applicable. Methods: A total of 34 elderly women aged 65 or older and young women in their 20s with FHP were screened to measure vital capacity, measuring the forced expiratory volume in 1 second (FEV1) and forced vital capacity (FVC), and measuring the foot pressure to see the change in balance. For statistical analysis, the difference between pre and post values was compared using pared t-test. Results: As a result of vital capacity measurements, there was no significant difference between FEV1 and FVC for women over 65 years old (p>0.05). Young women in their 20s had no significant difference in FEV1 (p>0.05), and FVC had significant differences (p<0.05). In measuring foot pressure to measure balance, both women aged 65 and above and young women in their 20s had a significant decrease in anterior foot pressure, and a significant increase in posterior foot pressure (p<0.05). Conclusion: The results of this study did not positively affect the vital capacity of elderly women with FHP. However, the significant increase in vital capacity of young women in their 20s suggests that contraction of the abdominal muscle is necessary during forced expiration. Therefore, it is believed that proper application and therapeutic interventions should be combined when applying Figure-8 brace.
고령화가 급격한 속도로 진행되며 발생하는 문제 중 고령 운전자 사고는 해결되어야 하는 중요한 사회문제이며 사고를 감소하고자 하는 노력은 앞으로 맞이할 초고령 사회를 대비하기 위해 꼭 필요하다. 현재 고령자는 65세 이상으로 규정되어 있으며 이에 따라 고령 운전자 사고에 관련된 다양한 연구들은 65세를 기준으로 일반운전자와 고령 운전자를 구분하여 분석한다. 이러한 배경으로 인해 수용 가능한 정확도의 수준에서 실질적으로 교통약자로 분류되어야 하는 신체 능력을 갖춘 고령 운전자를 구분하는 기준은 제시되고 있지 않다. 따라서 본 연구에서는 상관분석을 이용하여 고령 운전자의 연령별 사고특성을 분석하여 연령군집별 사고 위험성을 비교하도록 한다. 분석결과 차종별 주요 사고특성에 대해 75세 이상의 경우 사고 발생에 미치는 영향력이 높아지는 것으로 나타났으며, 70세에서 80세 사이 구간에서 사고 발생 빈도가 상대적으로 낮았던 승용차에 대해서도 주요사고특성을 분석한 결과 75세 이상 84세 이하 구간에서 사고 발생에 대한 영향력이 높아지는 것이 두드러지게 나타났다. 이는 고령화가 진행되고 평균 수명이 늘어남으로써 고령자 구간 폭도 증가하는바, 고령자 내에서도 특성이 나뉘게 되며 본 연구를 통해 연령군집별로 사고 발생에 대한 영향력이 달라지는 것을 확인하였다.
Objectives : This study aimed to investigate the associations between physical disorders and prevalent/incident suicidal ideation in a community dwelling older population aged 65 years or over. Methods : 1204 people aged 65 years or over evaluated at baseline. Suicidal ideation was identified using the questions from the community version of the Geriatric Mental State Schedule ("GMS B3"). Reported physical disorders covering 11 common and generally chronic health problems were ascertained. Covariates included were depression, age, gender, years of education, accommodation status, past occupation, and current occupation. Of 1066 without suicidal ideation at baseline, 805 (76%) were followed 2 years later, and incident suicidal ideation was evaluated. Results : Prevalent suicidal ideation was significantly associated with 4 of 11 physical disorders: eyesight problems, persistent cough, heart disease and paralysis or weakness in one leg or arm. Incident suicidal ideation was associated with 3 physical disorders: asthma, high blood pressure and paralysis or weakness in one leg or arm. Both prevalent and incident suicidal ideation were significantly associated with increased number of physical disorders. Conclusions : Certain physical disorders were comorbid and precipitating factors of suicidal ideation in elders. And appropriate intervention and treatment of physical disorders might prevent suicidal ideation in elderly.
목적: 한국인을 대상으로 장년층과 노년층에서 남녀 각각 백내장 발병의 관련 요인을 알아보고자 하였다. 방법: 국민건강영양조사 자료를 이용하여 40-95세까지 총 5,024명 (남자 2,163명, 여자 2,861명)을 대상으로 하였고, 남녀각각 연령에 따라 두 그룹으로 분류하였다(그룹 1: 40-64세, 그룹 2: 65-94세). 백내장은 적어도 한 눈에 수정체의 혼탁이 발견된 경우로 정의하였다. 남녀 각각 두 연령 그룹에서 당뇨, 고혈압, 고중성지방혈증, 실외 활동, 흡연과 음주 습관을 보정한 후에 비만과 사회경제적 요인으로 가구 소득수준과 최종학력을 기준으로 교육수준이 백내장의 발병에 영향을 미치는지 비차비 값을 구하였다. 결과: 소득수준과 교육수준은 남녀 모든 연령 그룹에서 백내장 발병과 강한 관련이 있었다. 모든 다른 요인들을 보정한 후에 최종 다중분석 모델에서 남자의 경우는 저 소득(그룹 1:OR, 1.84[1.17-2.91], 그룹 2: 3.47[2.53-4.74]), 저 학력(그룹 1: OR, 3.00[1.90-4.74], 그룹 2: 7.44[5.41-10.23])과 실외 활동시간(그룹 2: OR, 1.26[1.06-1.49]이 백내장의 발병 위험을 높였다. 여자의 경우에는 저 소득(그룹 1: OR,1.72[1.14-2.60], 그룹 2: 2.32[1.70-3.16]), 저 학력(그룹 1: OR, 4.48[2.72-7.38], 그룹 2: 29.99[20.31-44.28])과 비만(그룹 1: OR, 1.40[1.10-1.82], 그룹 2: 1.27[1.04-1.54])이 유의한 위험 요인이었다. 결론: 한국 성인 남녀에서 낮은 사회경제적 상태는 백내장의 위험 요인이었고, 특히 여성의 경우는 비만도 백내장과 관련이 있었다.
BACKGROUND/OBJECTIVES: The objective of this study was to determine dysphagia risk among community-dwelling elderly people living at home. We also examined the impact of socio-demographic variables on dysphagia risk as well as the relationship between dysphagia risk and dietary characteristics. SUBJECTS/METHODS: The study sample included 568 community-dwelling individuals, aged 65 years and above, who were living independently in their own home in Seoul, Gyeonggi, or Gwangju in South Korea. We used a dysphagia risk assessment scale to screen for dysphagia risk and the Mini nutritional assessment to evaluate the nutritional status. Associations between dysphagia risk and other variables were assessed using logistic regression analysis. RESULTS: Of the 568 subjects, 350 (61.6%) were classified into the dysphagia risk group (DR) and 218 (38.4%) were classified into the normal group (non-DR). Being female (odds ratio (OR) = 1.82, 95% confidence interval (CI) = 1.28-2.59), being 75 years and older (OR = 2.40, 95% CI = 1.69-3.42), having a lower education level (OR = 2.29, 95% CI = 1.33-3.97), and having a lower perceived economic status (OR = 2.18, 95% CI = 1.32-3.60) were more frequently observed with dysphagia risk compared to those who did not have such characteristics. Lowered mastication ability (OR = 14.40, 95% CI = 4.43-46.95), being at risk of malnutrition or malnourished (OR = 2.72, 95% CI = 1.75-4.23), lowered appetite (OR = 3.27, 95% CI = 2.16-4.93), and decreased food intake (OR = 2.95, 95% CI = 1.83-4.78) were observed more frequently in the DR group than in the non-DR group when adjusting for potential confounding factors. CONCLUSIONS: It is necessary to develop and apply integrated programs to improve the dietary habits and nutritional status of elderly individuals at risk for dysphagia, especially for women aged 75 years or older with lower educational and economic levels.
Background: The purpose of this study is to examine the characteristics of and factors associated with long-term care (LTC) utilization under public long-term care insurance (LTCI) among end-of-life older adults in Korea. Methods: Using a 5% sample of older people aged 65 or older and their health and LTC insurance data, two-part model analyses were conducted. We compared LTC uses and their determinants during the last year of life among decedents in the year 2010 with those of survivors. We also compared the medical uses of the same sample with their LTC uses. Results: The end-of-life elderly were more likely to use LTC, and their expenditure on LTC was higher than their counterparts. Whether or not older people used LTC during their last year of life was significantly affected by age, sex, health insurance, household income, and living alone; however, LTC costs of the decedents were only affected by functional status, which may have been due to the reimbursement scheme of the current LTCI, which is mainly based on functional dependency level. For the survivors, having chronic diseases significantly increased the likelihood of LTC use, which was not the case for the decedents. End-of-life elderly with relatively low social economic status were more likely to use the LTC other than medical services, while the health conditions affected their medical uses most significantly. Conclusion: The study findings provide key information for predicting demand related to the increasing LTC needs of Korean older people at the end of life.
Objectives: To identify the factors affecting the cognitive function according to the education level of people aged 65 or older. Methods: This study used the data from the 2014 survey of living conditions and welfare needs of Korean older adults. A total of 10,248 people were analyzed in the final analysis, not including those who responded by proxy and those who did not participate in cognitive function tests. Cognitive function was measured by Mini Mental State Examination -Dementia Screening(MMSE-DS), and logistic regression was performed using the SPSS/WIN 24.0 program. Results: There were differences in factors affecting cognitive function depending on the education level. Overall, regular exercise had a positive effect on maintaining cognitive function, and the higher the education level, the more important it was to maintain the Instrumental Activities of Daily Living(IADL). In addition, for lower education levels, improving the life satisfaction was important to prevent the decline of cognitive function. Conclusions: Based on the results of this research, it is suggested that customized approaches shoud be taken according to education level when developing and implementing programs to prevent the decline of cognitive function.
