Donation after Circulatory Death in Lung Transplantation

  • The shortage of donor lungs has become a serious obstacle to implementing lung transplantation (LTx). Donation after circulatory death (DCD) donors are among the several donor pools utilized to overcome the problem posed by the shortage of donation after brain death (DBD) donors. The active use of DCD donors is expected to significantly reduce mortality on the waiting list for LTx, as LTx from DCD donors has comparable outcomes to LTx from DBD donors. Further studies on efforts to shorten the warm ischemic time and use uncontrolled DCD are required.

간호대학생의 죽음에 대한 의식 변화 연구 -호스피스 간호 학습 전.후 비교- (A Study on Change in Death Orientation of College Student Nurses -comparison of pre and post hospice care class-)

  • Purpose: The purpose of this study was to identify the death orientation of college student nurses and to evaluate the effectiveness in death orientation of a class on 'hospice care'. this was done by investigating the difference between the death orientation before and after the class. Method: The instrument used in this Investigation was the death orientation scale developed by Thorson and Powell (1988). The subjects of the study were the 47 college student nurses in a 'hospice care' class. The research was carried out between March 2, 2001, when the class began, and June 21, 2001 when the class ended The collected data were examined using arithmetic mean, percentage, t-test, ANOVA. Result: The results of the study are as follows : 1. The average death orientation score of the subjects before the class was 2.80. 2. There were a significant difference in the death orientation scores before and after the class (p<0.001). Before the class, 'hospice care', the students had an average score of 2,80. Their average score decreased to 2.55 after the class. Of 25 death orientation items on the scale, 14 items decreased significantly after the class as compared to before the class. 3. According to personal attributes (religion. the existence of religion, the experience of a death in recently) of subjects, there were no significant statistical differences in death orientation (p>0.05). Conclusion: This study showed that a class on hospice care was effective in changing the death orientation of student nurses.

  • We developed an experimental model of brain death using dogs. Brain death was caused by increasing the intracranial pressure[ICP suddenly by injecting saline to an epidural Foley catheter in five female mongrel dogs[weight, 20-25Kg .Hemodynamic and electrocardiographic changes were evaluated continuously during the process of brain death. 1. Abrupt rise of ICP after each injection of saline followed by a rapid decline to a new steady-state level within 15 minutes and the average volume required to induce brain death was 7.6$\pm$0.8ml.2. Body temperature, heart rate, mean pulmonary arterial pressure, left ventricular[LV enddiastolic pressure and cardiac output was not changed significantly during the process of brain death, but there was an increasing tendency.3. Mean arterial pressure and LV maximum +dP/dt increased significantly at the time of brain death.4. Hemodynamic collapse was developed within 140 minutes after brain death.5. Marked sinus bradycardia followed by junctional rhythm was seen in two dogs and frequent VPB`s with ventricular tachycardia was observed in one dog at the time of brain death. Hyperdynamic state develops and arrhythmia appears frequently at the time of brain death. Studies on the effects of brain death on myocardium and its pathophysiologic mechanism should be followed in the near future.

  • 5마리의 잡견(18~22kg)을 이용하여, 경막외강에 생리식염수를 연속 점적하여 뇌사를 유발시킨다. 뇌사발생시 점의 뇌압은 122.0$\pm$6.25mmHg이며 뇌사후 30분 최고치에 도달하였다. 뇌사를 유발시키는데 필요한 생리식염수의 양은 4.8$\pm$1.0ml이었고, 143.0$\pm$30.9분이 필요하였다. 뇌사가 올때까지 동맥압은 변하지 않지만 그후 점차 떨어지고, 맥박수는 뇌사 30분 후 안정시 보다 50% 정도 증가한 최고치에 달한다. 체온, 심박출량, 폐동맥압, 좌심실 이완말기압등의 혈역학적 지표는 뇌사진행 중에는큰 변화가 없었고, 심실 기외수축이 일시적으로 나타난 이외에는 부정맥도 관찰되지 않았다. 급작스러운 뇌압 상승 모델에서 보였던 혈역학적 변화는 관찰되지 않았다.

재고해 보는 근사 체험(Near-Death Experience)과 그 종교적 의미 (On Religious Significance of the Near-Death Experience)

  • Until 1970's, mankind have not had definite perspectives on what life after the death was like, which is one of the most important problems for them. Their concepts of the 'life and death problem' has been distorted by the dogmas of the established religions such as Buddhism or Christianity on heaven and hell. But the mankind came to have wholly different views on the life after death thanks to the studies by Raymond Moody Jr. or Elizabeth Quebler-Ross in the mid-1970's. This is the studies on the so-called 'near-death experience(NDE)' which made humankind be able to have scientific approach to the life after death for the first time in their history. What attracts our attention at this point is, however, that the arguements of the NDErs on humman destiny accurately coincide with those of the established religions. In the NDE, most of the experiencers have an encounter with the personal being, symbolized by the Light, through whom they learnt that the devotion(or love) to the neighborhood and the gain of the wisdom are the sole meaning of life. With this result, we can recognize why essential virtues maintained by the established religions until now such as ultimate compassion, unconditional love, forgiveness, or insightful learning are so significant, and that our studies of the NDE are very important in this respect.

일부 간호대학생들의 죽음에 관한 인식유형 분석 (An Analysis of Factors about Nursing Students' Attitudes Toward the Perception of Death : Q-sort method)

  • The aim of this study was to identify the attitude of nursing students' toward death using Q-methodology and to obtain baseline data to improve nursing students' education program on death. Twenty-nine participants at a college classified 40 Q-statements on a one to nine scale. The resultant Q-sort was a matrix representing the participant's operant subjectivity on the issue under consideration. The results of above procedures were analyzed by PQ Method. The results revealed that there are three types of perception about nursing students' attitudes toward the death. The categories were labeled positive-perspective, pain-avoid and preparation-deficiency. Positive-perspective individuals have a positive sense of their life and death. Thus, they would not fear death, believing in an after-life world, while being positive towards donation of intestines after death. Pain-avoid individuals hope life and death without pain. Preparation-deficiency individuals are unprepared to die. In conclusion, this study discovers three types of the perception about nursing students' attitudes toward the perception of death. By identifying the nature of each of these types, this study can be useful to develop efficient strategies for education program on death.

개의 뇌사와 신장이식 (Brain Death and Kidney Transplantation in Dogs)

  • Brain dead (BD) patients remain the largest source of solid organs for transplantation. BD has shown to decrease graft function and survival in rodent models. The aim of this study was to evaluate how brain death affects graft viability in the donor and kidney tolerance to cold preservation as assessed by survival in a canine transplantation. 13 Beagle dogs were used for the study. Brain death was induced by the sudden inflation of a subdural balloon catheter with continuous monitoring of arterial blood pressure and eletroencephalographic activity (n=3). Sixteen hours after conformation of brain death, kidney graft were retrieved (n=6). Non-BD donors served as controls (n=4). All kidneys were flushed with University of Wisconsin (UW) solution and preserved for 24 hours at 4$^{\circ}C$ before transplantation. Recipient survival rates, serum creatinine level were analyzed. Brain death induced the well-known Cushing reaction with a severe increase in blood pressure and tachycardia. Thereafter, cardiac function returned progressively to baseline within 8 hours and remained stable until the end of the experiment. All of dogs in both group transplanted were survived until 7 days (100%), and the kidneys showed functional early rejection at 8.3$\pm$0.5 days and 8.5$\pm$0.5 days after transplantation, in BD and allograft group, respectively. BD kidneys were functionally similar to control kidneys for 7 days after transplantated. Brain death has no deleterious effect on preservation injury and survival of dog kidney transplantation, although it induces changes in hemodynamic parameters. This study reveals that kidneys from BD donors do not exhibit more ischemia reperfusion injury, and support good early function and survival.

대학생들의 죽음 교육 전과 후의 웰다잉 인식과 결정에 관한 연구 (A Study on the Well-Dying Recognition and Decision of Death before and after Education Among University Students)

  • 본 연구는 대학생들이 죽음학 강좌 수강 이전과 이후 웰다잉 인식과 결정이 어떻게 변화되었는가를 비교 연구하고자 한다. 대전광역시 소재 일개 대학교에서 죽음학 관련 교양강좌를 2016년 8월부터 12월까지 15주 동안 수강한 대학생 교육 전 93명, 교육 후 117명 대상으로 연구 참여 희망자에 한해 설문조사를 실시하였다. 설문구성은 일반적인 특성은 성별, 연령, 학년, 전공, 결혼상태, 종교, 가족 동거여부, 건강상태 8항목이었다. 죽음의 인식 측면에 관한 4항목, 죽음의 수용 측면 5항목, 죽음의 결정 측면 7항목, 죽음 교육의 관심도와 중요도 12항목을 리커트 척도로 구성하였다. 통계 검정법으로는 카이제곱검정, 독립표본 t-검정, 의사결정나무 분석을 실시하였다. 연구결과를 의사결정나무 토대로 종합해보면, 죽음 준비 시기가 죽음에 직면한 시기(암환자, 말기환자 등)와 노인(65세 이상)에서 교육 전이 66.7%로 나타났으나, 교육 후에는 성인과 중 고등학교, 초등학교 이하, 대학교, 대학원에서 교육 후가 65.3%로 나타났으며 이는 유의한 차이를 보였다. 따라서 죽음교육의 효과성과 죽음교육의 시기와 내용 등을 어떻게 구체화 시킬 것인가에 대한 방향 제시를 할 수 있을 것으로 기대되며, 초 중 고등학교, 대학교에서의 죽음교육 실시에 대한 부정적인 시각과 우려를 해소하고 죽음 교육이 학생들의 삶에 태도 변화에 긍정적인 영향을 미칠 것으로 사료된다.

암환자 가족의 죽음 태도 유형에 관한 연구 (A Classification of Death Orientation of Cancer Patient's Family Members : A Q-Methodological Approach)

  • This study was designed to identify, describe and classify orientations of cancer patient's family members to death and to identify factors related to their attitudes on death. Death to the male is understood as a comprehensive system and believed to be highly subjective experience. Therefore attitude on death is affected by personalities. As an attempt to measure the subjective meaning toward death, the unstructured Q-methodology was used. Korean Death Orientation Questonaire prepared by Kim was used. Item-reliability and Sorting-reliability were tested. Forty five cancer patients' family members hospitalized in one university medical center in Seoul were sampled. Sorting the 65 Q-itmes according to the level of personal agreement ; A forced normal distribution into the 11 levels, were carried out by the 45 P-samples. The demographic data and information related to death orientation of the P-sample was collected through face to face in depth interviews. Data was gathered from August 30 till September 22, 1995. The Z-scores of the Q-items were computed and principal component factor analysis was carried out by PC-QUANL Program. Three unique types of the death orientation were identified and labeled. Type I consists of twenty P-samples. Life and death was accepted as people's destiny, They firmly believed the existence of life after life. They kept aloof from death and their concern was facing the and of the life with dignity, They were in favor of organ donation. Type II consists of Nine P-Samples. They considered that death was the end of everything and did not believed the life after life. They were very concerned about the present life. Type III consists of Sixteen P-samples. They regarded the death as a natural phenomena. And they considered that the man is just a traveller and is bound to head for the next life which is believed to be free of agony, pain or darkness. They neither feared death nor its process. Their conserns were on the activities to prepare themselves for the eternal-life after death. Thus, it was concluded that there were three distinctiven type of attitudes on death among cancer patient family members, and their death attitudes were affected by demographic and socio-cultural factors such as sex, education, and religion.

학령기 아동의 죽음인식에 관한 탐색적 연구 (A Inquiry of the Perception of Death in School Age)

  • 목적: 본 연구는 학령기 아동의 죽음인식에 대한 탐색적 연구를 통하여 그들의 죽음인식에 대한 주관적 구조와 유형을 탐색하여 학령기 아동에게 죽음인식을 이해하며 효과적인 죽음준비교육 프로그램을 개발 시행하기 위한 기초자료를 제공하고자 시도하였다. 방법: 학령기 아동의 죽음인식에 관한 주관적인 구조와 유형을 탐색할 수 있는 Q방법론을 사용하였다. Q-모집단은 학령기 아동 20명을 대상으로 중립적 면담과 개방형 질문지를 이용하고 문헌연구를 통하여 총 132개의 진술문을 수집하였으며. Q표본은 비 구조화된 방법을 통해 23개의 진술문(Q-표본)을 도출하였다, P표본은 학령기 아동 31명(8세-13세)이었으며 Q카드를 이용하여 Q-분류를 실시하였으며, 수집된 자료는 PC QUANL 프로그램을 사용하여 분석하였다. 결과: 학령기 아동의 죽음인식 유형은 5개의 유형으로 분류되었다. 제 1 유형은 기능적형으로 죽음의 구성요소인 비역행성, 보편성, 비기능성, 인과성에 대한 주관적 인식이 두드러진 유형이었다. 제 2 유형은 사후 세계형으로 기독교의 종교적 배경을 가진 아동들과 직계가족의 죽음을 경험을 했으며 사후세계에 대하여 강한 내세 중심의 죽음인식이 두드러진 유형이었다. 제 3 유형은 종교형으로 죽어서도 가족과 친구들을 지켜볼 수 있다는 믿음이 강하기 때문에 사후세계에 대한 긍정적인 믿음이 확실한 유형이었다. 제 4 유형은 공포형으로 죽음에 대한 공포가 타 유형에 비하여 강한 유형이었다. 제 5 유형은 현실형으로 좋은 죽음에 대한 인식에 강한 긍정적인 동의를 보이는 유형이었다. 결론: 본 연구결과가 주는 간호학적 의의는 다음과 같다. 간호실무면에서 학령기 아동의 죽음인식의 주관성을 이해하는데 있어서 기존의 문헌이나 연구에서 중점적으로 제시하고 있는 죽음의 구성요소에 대한 이해의 차원에서 좀 더 확대되어 학령기 아동이 인지하는 죽음의 정의, 사후세계, 좋은 죽음에 대한 인지수준을 이해하고 유형별 특성에 따라 학령기 아동들의 효과적인 죽음준비교육 프로그램을 개발하는데 유용한 자료로 사용하여, 청소년기에 나타나는 죽음 경시풍조 및 만연된 자살을 예방하고 올바른 삶을 살아갈 수 있도록 밑거름이 되는 역할을 담당 할 것이다. 간호이론 면에서 학령기 아동의 죽음인식의 주관적 구조와 유형별 특성을 규명하고 탐색함으로써 학령기 아동의 죽음인식 모델 구축과 나아가 생명존중의 유용한 자료로 활용될 수 있다. 간호연구 면에서 학령기 아동에게 유형별로 긍정적인 죽음인식과 더 나아가 생명 존중의 측면에서 심리사회적 간호를 제공하기 위한 도구의 개발과 간호중재 전략의 효과를 규명하는 연구에 기여할 수 있다.
