Effects of Robot-Assisted Arm Training on Muscle Activity of Arm and Weight Bearing in Stroke Patients (로봇-보조 팔 훈련이 뇌졸중 환자의 팔에 근활성도와 체중지지에 미치는 영향)

  • Background: This study investigated the effect of robot-assisted arm training on muscle activity of arm and weight bearing in stroke patients. Methods: The study subjects were selected 20 stroke patients who met the selection criteria. 10 people in the robot-assisted arm training group and 10 people in the task-oriented arm training group were randomly assigned. The experimental group performed robot-assisted arm training, and the control group performed task-oriented arm training for 6 weeks, 5 days a week, 30 minutes a day. The measurement tools included surface electromyography and smart insole system. Data were analyzed using independent sample t-test and the paired sample t-test. Results: Comparing the muscle activity of arm within the group, the experimental group and the control group showed significant differences in muscle activity in the biceps brachii, triceps brachii, anterior deltoid, upper trapezius, middle trapezius, and lower trapezius. Comparing the muscle activity of arms between the groups, the experimental group showed significant difference in all muscle activity of arm compared to the control group. Comparing the weight bearing within the groups, the experimental group showed significant difference in the affected side and non-affected side weight bearings and there were significant differences in anterior and posterior weight bearing. The control group showed significant difference only in the non-affected side weight bearing. Comparing the weight bearings between groups, the experimental group showed significant difference in the affected side and non-affected side weight bearings compared to the control group. Conclusion: This study confirmed that robot-assisted arm training applied to stroke patients for 6 weeks significantly improved muscle activity of arm and weight bearing. Based on these results, it is considered that robot-assisted arm training can be a useful treatment in clinical practice to improve the kinematic variables in chronic stroke patients.

  • Objective: This study is designed to examine how an application of a kinesio taping to the foot drop of a hemiplegic patient affects the functional recovery of a gait. Method: The patient was a man with left hemiplegia of about 11 months' duration. a single subject design (ABAB design) was used to investigate the timing difference at a stance phase between an unaffected side and an affected side in the gait ability of the hemiplegic patient by using the kinesio taping. The study was divided into four phase: an initial base-line, an experimental, a second base-line, and second experimental phase. Result: The timing difference at a stance phase between an unaffected side and an affected side in the gait was decreased in the case of the affected side by following the result of applying the kinesio taping to a lower extremity. Conclusion: The kinesio taping applied to the foot drop of a hemiplegic patient affects the improvement of the gait ability.

  • The aim of this study is to design and develop the gait rehabilitation equipment that judge patient's movement of his/her center of gravity using pressure sensors, and to aid hemiplegic patients to balance themselves using an automatic stepper that changes the patient's center of gravity. It is hard to bear the weight on the affected side for hemiplegic patients. The gait rehabilitation equipment detects the footing phase of hemiplegic patient during training and moves the unaffected footing side of the stepper up and moves the affected footing side down simultaneously so that the patient's center of gravity can shift from unaffected side to affected side. The gait rehabilitation system was developed and applied for hemiplegic patients during exercise. Eight hemiplegic patients and one normal adult were studied. The developed gait rehabilitation system could judge not only the normal adult's intention but also the patient's intention to move his/her center of gravity. Even though the most of hemiplegic patients exercised in automatic mode and a few hemiplegic patients exercised in manual mode, the developed gait rehabilitation system can aid the hemiplegic patients to train more easily.

하악편측절근환자의 교근활성도에 대한 근전도학적 연구

  • The electrical activities of masseter muscle were recorded on 9 subjects with unilateral fracture of mandible. The electromyographic studies were executed with 2 channel RS dynograph recorder for electromyography. The graphs were recorded in the physiologic rest position, incisal occlusion, molar occlusion, left lateral excursion, right lateral excursion, and protraction. The following conclusions were drawn; 1. In the physiologic rest position, incisal occlusion, molar occlusion, and protraction, the electrical potentials of the masseter muscle were greater in the affected side. 2. In the right lateral excursion, the electrical potentials of the masseter muscle were greater in the right side, and in the left lateral excursion, greather in the left side. There is no correlation mutually between the affected side and unaffected side.

Neurophysiological and Clinical Features of the Pusher Syndrome: Review Article

  • Patients with the pusher syndrome show severe misperception of their own upright body orientation although visual vestibular processing is almost intact. They recognize their body as oriented upright when it is actually tilted nearly 20 degrees to the affected side. These patients resist any attempts to passively correct their tilted body posture towards an earth vertical upright orientation. They use the non-affected side arm and/or leg to actively push towards the affected side. Pusher syndrome patients have different prognoses and symptoms than general stroke patients without pusher syndrome. Pusher syndrome patients have a poor prognosis, so they need a long duration of treatment. Therefore, accurate diagnosis and proper treatment are important. In this study, we reviewed the symptoms, causes, evaluation, and treatment for pusher syndrome.

Comparison of Incipient Grade and Improvement Rate between Each Opposite Gender and Lesion on Peripheral Facial Nerve Paralysis (구안와사(口眼喎斜) 환자에서 성별 및 발병부위가 병세 및 호전에 미치는 영향)

  • Objectives : This study was to compare the incipient grade and improvement rate between each opposite gender and part on peripheral facial nerve paralysis. Methods : We investigated 64 cases of patient with peripheral facial nerve paralysis and divided into four groups with each gender and affected part and compared their incipient grade and rate of improvement. All groups were evaluated by Yanagihara's unweighted grading system before treatment and in everyday and after treatment. Results & Conclusion : 1. There's a difference of incipient grade between each opposite affected lesion in male. But there was not significant statistics. 2. There's no difference of incipient grade between each opposite affected lesion in female. 3. There's a difference of incipient grade between each opposite gender, affected their right side. But there was not significant statistics. Left side affecting palsy is more severe than right in male, and reversed results in female. But there were not significant statistics. There's no differences between each affected lesion in female. 4. There's no differences of improvement rate between each affected lesion in female. 5. The group, affected right side had better rate of improvement than another in male. But it's not significant statistics. 6. Male group had better rate of improvement than female in both gender, affected its left lesion. But it's not significant statistics.

A Study on the Electromyographic Activity of Masticatory Muscles, Muscle Tenderness and Occlusal Contacts in Patients with Temporomandibular Disorders (측두하악장애환자에서 근활성, 근압통 및 교합접촉에 관한 연구)

  • The purpose of this study was to investigate the peak electromyographic activity(EMG) and time intervals in firing sequence of Anterior temporalis(TA) and Masseter(MM) on clenching, the number of tender points in the head and neck muscles, the occlusal contact state on clenching, and their relationship. 78 patients with Temporomandibular Disorders(TMDs) participated and were classified into articular or muscular group by clinical signs and symptoms. BioEMG$\textregistered$ of integrated masticatory function analyzer, Biopak$\textregistered$ system(Bioresearch Inc., USA), was used to measure EMG and related items, and T- Scan$\textregistered$(Tekscan, USA), computerized occlusal analyser, was used to record occlusal contact state on maximum voluntary clenching. EMG and occlusal contact were synchronously recorded and analysed with SAS Statistical program. The results of this study were as follows : 1. In total subjects, EMG of TA was lower than that of MM in articular group but in muscular group, vice versa, As a result, the ratio TA to MM was significantly different between the two groups. no significant differece. 3. The number and force of occlusal contact were more in articular group, but there was no significant difference in Total Left-Right statistics(TLR) between the two groups. 4. In unilaterally affected subjects for muscle function, no significant difference was observed between the affected side and the contralateral side in articular group, but in muscular group, the item of the number of tender points showed significant difference. However, for occlusal contact items, contact force in articular group shows significant difference between the two sides. 5. Rate of coincidence of the first firing side of TA with affected side or preferred chewing side was higher and not different between the two groups, but no significant correlation was showed between the first firing side and the first occlusal contact side.

The analysis of tissue elasticity using computer-controlled ultrasonography in the affected upper limb of patients after breast cancer surgery

  • In this paper, we propse a quantitative research by investigating the subcutaneous tissue elasticity by using ultrasonography in lymphedema patients after breast cancer surgery. Lymphedema patients who took breast cancer operation were included. Thickness of subcutaneous tissue was assessed at two spots; 10cm below elbow (forearm) and 10cm above elbow (upper arm), not only in affected side but also in sound side. By using probe attached to real-time pressure sensor, stress-strain curves were obtained. We defined tissue elasticity as slope of that curve at range of 7.5~15% of strain to avoid toe region. By comparing the elasticity of normal side and that of affected side, lymphedema tissues were classified into 'softer' and 'harder' tissues. Overall 30 cases of lymphedema tissues and 30 cases of sound tissues were checked. The difference of the elasticity between normal and affected side ranged from -3.98 N/m2 to 1.40 N/m2. The lymphedema tissues were classified into 17 softer tissues and 13 harder tissues. No demographic and clinical values, including clinical stage of lymphedema, showed statistically meaningful differences between two groups. Evaluation of subcutaneous tissue elasticity with ultrasonography and real-time pressure sensor could be one of the useful tools for investigation of lymphedema tissue characteristics.

Effect of Asymmetrical Tonic Neck Reflex on Weight Bearing of the Extremities (비대칭성 긴장성 경반사가 사지의 체중지지에 미치는 영향)

  • The purposes of this study were 1) to compare weight distribution and ratio of the extremities between normal and hemiplegic children in quadruped position and 2) to compare the effect of ATNR on weight bearing of the extremities between normal and hemiplegic children. The subjects fer the study were 48 children(24 normal, 24 hemiplegic) between the ages 3 to 6. They were teated weight distribution and ratio of the extremities in the neutral position of head and by passive right and left rotation of the head in the quadruped position. The data wert analized by paired t-test. The results were as follows: 1. In the neutral position of head, normal group was not difference on weight distribution and ratio of the extremities and hemiplegic group was more weight bearing on the sound upper extremity than the affected upper extremity(p<.01). 2. When the head rotated to the dominant side or sound side passively, there was not a significant difference between normal and hemiplegic group. 3. When the head rotated to the nondominant side or affected side passively, there was a significant difference between nondominant upper extremity nf normal and affected upper extremity of hemiplegic group(p<.05).

The Effect of Dong-gi Acupuncture(DGA) on Rehabilitation after Stroke (중풍(中風) 편마비(偏痲痺) 환자(患者)의 기능적 회복에 있어 침시술(鍼施術) 시(時) 동기요법(動氣療法)의 효과)

  • Objectives : The purpose of this study is evaluate of the effect of Dong-gi Acupuncture(DGA) on rehabilitation after stroke. Methods : 32 patients who had a stroke of paralysis were randomized into two groups. 17 patients were treated with DGA on sound side combined active or passive exercise on affected side. The other 15 patients were exclusive DGA group and the method retaining needles on acupoints of affected side for about 20 minutes were applied. We evaluated by Manual muscle test, Range Of Motion(ROM), Visual Analog Scale(VAS) and Modified Barthel index(MBI) before treatment, after 10 days and after 20 days. Results : In terms of power of lower limb, active ROM of shoulder abduction and MBI, the sample group showed statistically significant improve. In terms of power of lower limb ROM of hip joint abduction and affected side pain, significant difference between two groups was not shown. Conclusions : In this clinical study, Dong-gi Acupuncture therapy can be used for improving the power of upper limb, active ROM of shoulder abduction and MBI score after hemiplegic stroke.
