Diagnostic Power of Blood Parameters as Screening Markers in Gastric Cancer Patients

  • Background: Gastric cancer (GC) is the fifth most common cancer worldwide. Since development is usually asymptomatic, it is generally diagnosed at an advanced stage. The value of screening in patients with nonspecific symptoms for GC is controversial. Aim: The study aimed to evaluate whether hematological parameters (platelet count (PC), mean platelet volume (MPV), MPV/PC ratio, red blood cell distribution width (RDW), neutrophil to lymphocyte ratio (NLR), and platelet to lymphocyte ratio (PLR)) are useful markers to differentiate between gastric cancer patients and healthy individuals. Materials and Methods: Sixty-one patients with gastric cancer and sixty-one healthy individuals were enrolled to the survey and retrospective analysis of selected blood parameters were performed. Results: The mean values of PC, MPV, RDW, NLR, and PLR were significantly higher in GC patients compared to the control group. No statistical differences were observed in MPV/PC ratios. Likewise, no significant statistical differences were revealed in values of blood parameters among TNM stage groups. The RDW showed the highest diagnostic specificity and sensitivity. Conclusions: Hematological parameters: PC, MPV, RDW, NLR, PLR have diagnostic power and can discriminate patients with gastric cancer from patients without cancer. Blood parameters compared with clinical symptoms might alert physicians and patients and lead to performancce of upper gastrointestinal endoscopy, the gold standard in gastric cancer screening and therebly increase the early detection of cancer.

Transanal Tube Drainage as a Conservative Treatment for Anastomotic Leakage Following a Rectal Resection

  • Purpose: We evaluate the role of transanal tube drainage (TD) as a conservative treatment for patients with anastomotic leakage (AL). Methods: Patients treated for AL who had undergone a low or an ultralow anterior resection with colorectal or coloanal anastomosis for the treatment of rectal cancer between January 2013 and January 2017 were enrolled in this study. The data were collected prospectively and analyzed retrospectively. The primary outcomes were the diagnosis and the management of AL. Results: Two hundred thirteen consecutive patients, 122 males and 91 females, were included. The mean age was $66.91{\pm}11.15years$, and the median body mass index was $24kg/m^2$ (range, $20-35kg/m^2$). The median tumor distance from the anal verge was 8 cm (range, 4-12 cm). Ninety-three patients (44%) received neoadjuvant therapy for nodal disease and/or locally advanced rectal cancer. Only 13 patients (6%) developed AL. Six patients developed subclinical AL as they had a defunctioning ileostomy at the time of the initial procedure. They were treated conservatively with TD under endoscopic guidance in the endoscopy unit and received intravenous antibiotics. Six weeks after discharge, these 6 patients underwent follow-up flexible sigmoidoscopy which showed a completely healed anastomotic defect with no residual stenosis. Seven patients developed a clinically significant AL and required reoperation with pelvic abscess drainage and Hartmann colostomy formation. Conclusion: These results suggest that TD for management of patients with AL is safe, cheap, and effective. Salvaging the anastomosis will help decrease the need for Hartmann colostomy formation. Proper patient selection is important.

  • Purpose: Laparoscopy-assisted distal gastrectomy for early gastric cancer has gained acceptance and popularity worldwide. However, laparoscopy-assisted distal gastrectomy for advanced gastric cancer is still controversial. Therefore, we propose this prospective randomized controlled multi-center trial in order to evaluate the safety and feasibility of laparoscopy assisted D2-gastrectomy for advanced stage gastric cancer. Materials and Methods: Patients undergoing distal gastrectomy for advanced gastric cancer staged cT2/3/4 cN0/1/2/3a cM0 by endoscopy and computed tomography are eligible for enrollment after giving their informed consent. Patients will be randomized either to laparoscopyassisted distal gastrectomy or open distal gastrectomy. Sample size calculation revealed that 102 patients are to be included per treatment arm. The primary endpoint is the non-compliance rate of D2 dissection; relevant secondary endpoints are three-year disease free survival, surgical and postoperative complications, hospital stay and unanimity rate of D2 dissection evaluated by reviewing the intraoperative video documentation. Discussion: Oncologic safety is the major concern regarding laparoscopy-assisted distal gastrectomy for advanced gastric cancer. Therefore, the non-compliance rate of clearing the N2 area was chosen as the most important parameter for the technical feasibility of the laparoscopic procedure. Furthermore, surgical quality will be carefully reviewed, that is, three independent experts will review the video records and score with a check list. For a long-term result, disease free survival is considered a secondary endpoint for this trial. This study will offer promising evidence of the feasibility and safety of Laparoscopy-assisted distal gastrectomy for advanced gastric cancer. Diffuse-Type Histology Is Prognostic for All Siewert Types of Gastroesophageal Adenocarcinoma

  • Purpose: The optimal treatment for gastroesophageal junction adenocarcinoma (GEJA) remains controversial. We evaluated the treatment patterns and outcomes of patients with locally advanced GEJA according to the histological type. Materials and Methods: We conducted a single-institution retrospective cohort study of patients with locally advanced GEJA who underwent curative-intent surgical resection between 2010 and 2020. Perioperative therapies as well as clinicopathologic, surgical, and survival data were collected. The results of endoscopy and histopathological examinations were assessed for Siewert and Lauren classifications. Results: Among the 58 patients included in this study, 44 (76%) were clinical stage III, and all received neoadjuvant therapy (72% chemoradiation, 41% chemotherapy, 14% both chemoradiation and chemotherapy). Tumor locations were evenly distributed by Siewert Classification (33% Siewert-I, 40% Siewert-II, and 28% Siewert-III). Esophagogastrectomy (EG) was performed for 47 (81%) patients and total gastrectomy (TG) for 11 (19%) patients. All TG patients received D2 lymphadenectomy compared to 10 (21%) EG patients. Histopathological examination showed the presence of 64% intestinal-type and 36% diffuse-type histology. The frequencies of diffuse-type histology were similar among Siewert groups (37% Siewert-I, 36% Siewert-II, and 33% Siewert-III). Regardless of Siewert type and compared to intestinal-type, diffuse histology was associated with increased intraabdominal recurrence rates (P=0.03) and decreased overall survival (hazard ratio, 2.33; P=0.02). With a median follow-up of 31.2 months, 29 (50%) patients had a recurrence, and the median overall survival was 50.5 months. Conclusions: Present in equal proportions among Siewert types of esophageal and gastric cancer, a diffuse-type histology was associated with high intraabdominal recurrence rates and poor survival. Histopathological evaluation should be considered in addition to anatomic location in the determination of multimodal GEJA treatment strategies.

국소진행된 식도암에서 동시적 항암화학방사선요법의 결과 (Outcome of Locally Advanced Esophageal Cancer Treated with Concurrent Chemo-radiotherapy)

  • 목 적: 동시적 항암화학방사선치료를 받은 식도암 환자의 치료성적과 예후인자를 분석하였다. 대상 및 방법: 1994년 1월부터 2007년 7월까지 식도암으로 방사선치료를 받은 환자 246명 중 근치적 목적으로 45Gy이상의 방사선치료와 함께 동시적 항암화학요법을 받았으며 생존확인이 가능하였던 78명을 대상으로 후향적 분석을 시행하였다. AJCC 병기 IIA~IVB까지의 환자를 대상으로 하였는데, IVB는 상부식도암이 아닌 환자에서 쇄골상부림프절 전이가 있거나 하부식도암이 아닌 환자에서 복강림프절 전이가 있는 경우만을 포함시켰다. 방사선치료는 중앙값 54 Gy를 조사하였고, 항암화학요법은 대부분의 환자(88%)에서 5-FU와 cisplatin (FP) 병합요법으로 시행되었다. 추적관찰기간은 2~117개월(중앙값 14개월)이었다. 결 과: 흉부단층촬영이나 식도내시경을 통해 치료에 대한 반응을 확인할 수 있었던 54명의 환자중 완전관해가 17명, 부분관해가 18명이었다. 완전관해를 보인 환자에서 재발할 때까지의 기간은 중앙값 20개월이었고 첫 재발부위로 국소재발이 3명, 원격전이가 7명이었다. 1년, 2년, 5년 전체 생존률은 각각 58.9%, 21.7%, 12.2%이었고 중앙 생존기간은 14개월이었다. 전체 생존률에 대한 단변량분석에서 치료에 대한 반응과 FP병합요법의 횟수가 통계적인 유의성을 보여주었다. 환자의 숫자가 적었으나 방사선감작제로 일단위 혹은 주단위의 cisplatin이 FP 병합요법보다 좋은 결과를 보였다. 결 론: 원격장기로의 전이가 없는 국소진행성 식도암 환자에서 동시적 항암화학방사선치료는 기존의 연구와 유사한 결과를 보여주었다. 일단위 혹은 주단위의 cisplatin이 전신상태가 FP 병합요법에 적절하지 않은 환자에서 대안으로 사용될 수 있을 것으로 생각된다.

내시경과 Cerclage Wire를 이용한 식도내 낚시바늘 제거: 증례보고 (Endoscopic Retrieval of Esophageal Fishhooks Using Cerclage Wire: A Case Report)

  • 1년 6개월령의 수컷 비글견이 3일간의 연하통증과 식욕부진으로 내원하였다. 환축은 병력청취와 방사선학적 검사결과 낚시바늘에 의한 식도와 위내 이물로 진단되었다. 위식도 내시경을 시행한 결과 하나의 낚시바늘은 흉부식도의 심장기저 부에 나머지 하나의 낚시바늘은 위 분문부에 위치하고 있었다. 그리고 두 낚시바늘은 하나의 낚싯줄로 서로 연결되어 있었다. 연결된 낚싯줄을 끊고 위내 낚시바늘을 제거하기 위해 위 절개술을 시행하였다. 위 절개술 후 식도내 바늘을 제거하기 위해 세 갈래, 다섯 갈래 내시경 포셉과 생검 포셉을 이용한 내시경적 제거를 시도하였으나 낚시바늘이 식도의 점막에 깊게 박혀있어 견인에 실패하였다. 낚시바늘을 더욱 단단히 잡아줄 수 있는 장치를 위해 정형외과용 스테인리스 와이어(16G)를 올가미 포셉 형태로 제작한 뒤 내시경을 통한 시야가 확보된 상태에서 바늘에 걸어 제거하는데 성공하였다. 시술 10일 후 환축은 회복되어 퇴원조치 되었다.

Burnt-out Metastatic Prostate Cancer

  • A burnt-out prostate cancer tumor is a very rare clinical entity. The term 'burnt-out' refers to a primary tumor that has spontaneously and nearly completely regressed without treatment. Since metastasis of prostate cancer is usually encountered in the presence of advanced disease, distant metastasis with an undetectable primary tumor is very rare. We report herein a case of a burnt-out prostate cancer tumor that metastasized to the thoracic (T) spine and caused cord compression. A 66-year-old man visited the Emergency Department due to weakness of both legs for the past two days. His blood and urine tests were normal at the time. His spine magnetic resonance imaging (MRI) scans looked like bone metastasis that involved the T-7 vertebral body and a posterior element, and caused spinal cord compression. Other images, including from the brain MRI, neck/chest/abdomino-pelvic computed tomography (CT) scan and 18F-fluorodeoxyglucose (FDG)-positron emission tomography (PET) and endoscopy, revealed no lesions that suggested malignancy. After total corpectomy T-7 and screw fixation/fusion at T5 to T10, the pathology report revealed a metastatic carcinoma that was strongly positive for prostate-specific antigen (PSA). The serum PSA value was 1.5 ng/mL. The transrectal 12-core prostate biopsy and ultrasonography showed no definitive hypoechoic lesion, but one specimen had slight (only 1%) adenocarcinoma with a Gleason score of 6 (3+3). The final diagnosis was burned-out prostate cancer with an initial normal PSA value. Although metastatic disease with an unknown primary origin was confirmed, a more aggressive approach in seeking the primary origin could provide a more specific treatment strategy and greater clinical benefit to patients.

합성곱 신경망을 활용한 위내시경 이미지 분류에서 전이학습의 효용성 평가 (Evaluation of Transfer Learning in Gastroscopy Image Classification using Convolutional Neual Network)

  • Stomach cancer is the most diagnosed cancer in Korea. When gastric cancer is detected early, the 5-year survival rate is as high as 90%. Gastroscopy is a very useful method for early diagnosis. But the false negative rate of gastric cancer in the gastroscopy was 4.6~25.8% due to the subjective judgment of the physician. Recently, the image classification performance of the image recognition field has been advanced by the convolutional neural network. Convolutional neural networks perform well when diverse and sufficient amounts of data are supported. However, medical data is not easy to access and it is difficult to gather enough high-quality data that includes expert annotations. So This paper evaluates the efficacy of transfer learning in gastroscopy classification and diagnosis. We obtained 787 endoscopic images of gastric endoscopy at Gil Medical Center, Gachon University. The number of normal images was 200, and the number of abnormal images was 587. The image size was reconstructed and normalized. In the case of the ResNet50 structure, the classification accuracy before and after applying the transfer learning was improved from 0.9 to 0.947, and the AUC was also improved from 0.94 to 0.98. In the case of the InceptionV3 structure, the classification accuracy before and after applying the transfer learning was improved from 0.862 to 0.924, and the AUC was also improved from 0.89 to 0.97. In the case of the VGG16 structure, the classification accuracy before and after applying the transfer learning was improved from 0.87 to 0.938, and the AUC was also improved from 0.89 to 0.98. The difference in the performance of the CNN model before and after transfer learning was statistically significant when confirmed by T-test (p < 0.05). As a result, transfer learning is judged to be an effective method of medical data that is difficult to collect good quality data.

Expression Levels of Vascular Endothelial Growth Factors A and C in Patients with Peptic Ulcers and Gastric Cancer

  • Purpose: Vascular endothelial growth factor (VEGF) is one of the most important growth factors for metastatic tumors. To clarify the role of VEGF-A and C in patients with peptic ulcer disease (PUD) or gastric cancer (GC), we evaluated the expression levels of these two molecules. We also analyzed the effect of Helicobacter pylori infection on VEGF-A and C expression levels. Materials and Methods: Patients with dyspepsia who needed diagnostic endoscopy were selected and divided into three groups: nonulcer dyspepsia (NUD), PUD, and GC, according to their endoscopic and histopathological results. Fifty-two patients with NUD, 50 with PUD, and 38 with GC were enrolled in this study. H. pylori infection was diagnosed by the rapid urease test. After RNA extraction and synthesis of cDNA, the expression levels of VEGF-A and C were determined by quantitative reverse transcriptase polymerase chain reaction. Results: The VEGF-C expression level in the PUD and GC groups was significantly higher than that in the NUD group. Moreover, the VEGF-A expression level in the PUD and GC groups was higher than in the NUD group, although the differences were not statistically significant. Significant positive correlations were also observed between the expression levels of these two molecules in the PUD and GC groups. In addition, the expression levels of these two molecules were higher in H. pylori positive patients with PUD or GC than in H. pylori negative patients of the same groups; however, these differences did not reach statistical significance. Conclusions: Up-regulation of VEGF-C expression during gastric mucosal inflammation may play a role in the development of peptic ulcers or GC.

VR, AR 시뮬레이션 및 3D Printing을 활용한 어깨와 팔꿈치 수술실습 (VR, AR Simulation and 3D Printing for Shoulder and Elbow Practice)

  • 최근 의료 영상 기술의 발전은 진단, 수술계획, 또는 교육에 도움이 되는 수술 시뮬레이션을 만들어 왔다. 개선된 고화질 영상과 3차원 시각화는 의료 영상 가용성을 향상시키고 수술, 교육 분야에서 더 잘 이용할 수 있게 되었다. 실제 인간의 시각은 입체이다. 따라서, 외과의사의 판단을 통해 2차원 영상을 스테레오로 재구성하여 처리하는 것이 함께 필요하다. 이러한 과정을 줄이기 위해, 3차원 (3D) 이미지가 사용되어 왔다. 3D 영상은 복잡한 상황에서 외과 의사가 매우 짧은 시간에 판단할 수 있도록 3D 시각화를 강화하여 제공한다. 3D 화상 데이터 세트에 기초하여, 가상 내시경 수술 계획, 실시간 상호 작용 가상 의료 시뮬레이션이 가능하게 되었다. 본 논문은 새로운 이미징 기술의 최근 응용 프로그램을 설명하고 이의 기본과 특별히 주목할만한 의료 3D 복원 기술에 관한 것이다. 최근 CT, MR 및 기타 영상 양식의 기술발전은 흥미로운 새로운 솔루션과 어깨 영상의 활용 가능성을 넓혀왔다. 특히, 의료 기기에서 파생 된 3차원 (3D) 이미지는 고급 정보를 제공한다. 이 프레젠테이션은 어깨와 팔꿈치의 수술실습에서 원리, 3D 영상기술의 잠재적 응용가능성, 시뮬레이션, 3D프린팅을 설명한다.
