The Effectiveness of Non-pharmacologic Interventions for Chronic Nonspecific Low Back Pain: A Systematic Review and Meta-analysis (국내 비특이적 만성 요통 환자에게 적용한 비약물적 중재의 효과: 체계적 문헌고찰 및 메타분석)

  • Purpose: The purpose of this study was to evaluate the effectiveness of non-pharmacologic interventions for chronic nonspecific low back pain (CLBP) in adults aged 18-64 years. Methods: We searched for potentially relevant randomized controlled trials and non-randomized controlled trials through five Korean electronic databases (i.e., Korean Studies Information Service System, Research Information Sharing Service, Korean Medical Database, KoreaMed, and National Assembly Library) published from January 2010 to May 2019. Two investigators independently selected the studies based on the criteria and assessed risk of bias in the included studies. We estimated the effect size of interventions using Comprehensive Meta Analysis 3.3. Results: Of 10,151 studies, 26 studies met the inclusion criteria and 15 studies were included in the meta-analysis. Exercise reduced low back pain (Hedges's g=-1.53, 95% CI: -2.22 to -0.85) and pain-related disabilities (Hedges's g=-0.92, 95% CI: -1.40 to -0.45). We found that taping was effective in decreasing low back pain (Hedges's g=-1.12, 95% CI: -1.51 to -0.73) and pain-related disabilities (Hedges's g=-0.50, 95% CI: -0.93 to -0.07). Manual therapy yielded a marginally significant reduction in low back pain (Hedges's g=-2.32, 95% CI: -4.64 to 0.00), the therapy was not effective in decreasing pain-related disabilities. Conclusion: Although there was little evidence for the effectiveness of manual therapy in adults with CLBP, exercise and taping were effective to relieve pain and pain-related disabilities. Based on these findings, we suggest the development of non-pharmacologic interventions or a nursing intervention protocol for the CLBP management. Also, nurses should consider implementation of effective non-pharmacologic interventions for CLBP.

  • The purpose of this study is to analyze the general characteristics, effect size, and qualitative indicators of self-advocacy studies of people with developmental disabilities published in domestic academic journals and theses. For this purpose, among a total of 2153 papers related to self-advocacy published from 2000 to 2023, 41 studies with developmental disabilities as the keyword were selected, and the specific research results are as follows. Based on the results of this study, when developing a language intervention program related to self-advocacy for people with developmental disabilities, it is recommended to develop an intervention program based on the number of sessions of 10-19 in a learning situation with 20-30 people in adolescents and adults, or during the transition period. There are many studies limited to educational aspects such as special education and integrated education, and by applying this, it is hoped that a self-advocacy language intervention program will be developed at the level of language rehabilitation that can effectively and sophisticatedly assert self-assertion and self-rights after experiencing difficulties in communication.

Life Experiences of the Disabled Adults in Public Education Yahak Program (성인 장애인의 야학교육프로그램 참여 일상경험)

  • This study was to explore the living experiences of the disabled adults who were participating in public education Yahak program held at evening class. The study designed in-depth interviews with ten disabled people using a grounded theory approach. Through analyzing process, 34 concepts, 15 subcategories, and eight categories were deduced. In axial coding, casual condition, 'Suffering from unknown cause disabilities' and 'Isolated by social cause', context condition, 'Taking discriminative treat for disabilities' impacted on phenomenon, 'Overcoming their conditions by themselves'. Intervening conditions was 'Taking social supports' and action-interaction condition, 'Enjoying public programs' totally lead to consequence in 'Controlling daily life' and 'Exploring their own social roles'. The periods of process were divided three stages, reflecting disabled situation, formation phase of social relation, and self-developing phase. The core category, 'Trying to be recognized as a member of society' incorporated the relationship between and among all categories and explained the process. The study indicates that social education program for the disabled helped to develop themselves as a member of society. Therefore, we suggest there may be a need for training for professionals who work with disabled people to develop social adaptation.

The Study of Sensory Processing Skills in People with Intellectual Disabilities (지적 장애인의 감각처리능력에 관한 연구)

  • The purpose of this study was to examine the sensory processing of adults with intellectual disabilities by Adolescent/Adult Sensory Profile(AASP). AASP was executed for 98 adults with intellectual disabilities residing in nursing home. The score of sensory processing ability expressed the conditions of being 'Less Than Most People' and 'Much Less Than Most People' in the all sensory processing areas of Low Registration, Sensory Sensitivity, Sensory Seeking, Sensory Avoiding. There was not significant difference in the sex. The score of sensory processing ability between teenagers(11 years to 17 years) and adult(above 18 years), teenagers statistically showed lower scores in the three sensory processing areas of Low Registration, Sensory Seeking, and Sensory Sensitivity. In addition, there was significant difference in the sensory processing areas of Low Registration and Sensory Avoiding according to the disability grading. In the future research, it is suggested to examine the sensory processing within residual setting and it need a study comparing sensory processing skills according to the disability grading by increasing subjects.

Factors on Help-seeking Behaviors among the Disabled Experiencing Domestic Violence (가정폭력 피해 장애인의 외부도움요청 영향 요인)

  • This study aims to investigate the factors on help-seeking behaviors among the disabled experiencing lifelong domestic violence and suggest practical plans to address the problems. According to an analysis of the data of the "2010 Domestic Violence Survey of South Korea", the occurrence rate of lifelong physical violence is 12.1% and that of severe physical violence is 6.2% among 273 adults with disabilities. The rate of help-seeking among the disabled experiencing lifelong domestic violence is 26.3%, which means two-thirds of the disabled experiencing domestic violence do so for a long time. The results of the study show that the experience of severe physical violence(p<.05), attitudes toward violence(p<.05), and awareness of domestic violence and related laws(p<.05) were found to have an impact on help-seeking behaviors among adults with disabilities experiencing domestic violence. However, an accepting attitude of disability and social connections of disabled were not found to have an impact on help-seeking behaviors. Based on these results, this study suggested raising awareness about domestic violence among the disabled, educating people with disabilities not to tolerate violence and informing them about the domestic violence-related legal system, and training service professionals (social workers, health professionals etc.) to screen the disabled for domestic violence and prevent them from becoming domestic violence victims.

The Effect of 12 Weeks of Combined Training on Body Composition, Health-Related Physical Fitness, and Bone Mineral Density of Obese and Osteoporotic Intellectual Disabilities-Case study (12주간 복합트레이닝이 비만과 골다공증 지적장애인의 신체조성, 건강체력, 골밀도에 미치는 영향-사례연구)

  • The purpose of this study was to investigate the effect of exercise training on health-related physical fitness and osteoporosis after 12 weeks of combined training for adults with intellectual disabilities diagnosed with obesity and osteoporosis. The subjects conducted a combined training program consisting of aerobic exercise and strength training twice a week for 12 weeks. Body composition and bone mineral density were measured before exercise, 6, 12 weeks, and health-related physical fitness was measured before and after exercise. Body weight and body fat decreased after exercise and bone density increased after exercise compared to before exercise. Muscle strength, muscle endurance and flexibility were improved after exercise compared to before exercise. It was confirmed that the 12 week compound training increased the leg strength and improved the functions such as walking and running, and increased the health-related fitness and increased bone mineral density.

A Study on the Relationship between Body Function and Prelusive Movement to Falls to Promote Wellness in Chronic Stroke Patients (만성뇌졸중 환자의 웰니스 증진을 위한 신체기능과 낙성전조동작의 관련성 연구)

  • This study was conducted to investigate the effects of a participatory rehabilitation program on sit-rise and rise-to-walk test performances, and perception and motor skills in adults with medically vulnerable individuals and, adults with developmental disabilities in particular. Seventeen adults with developmental disabilities participated in a participatory rehabilitation program using resistance bands and exercise balls, for 60 minutes once weekly over 13 weeks. Their performances were measured before and immediately after the intervention, and 12 weeks after. The findings were as follows. In the sit-rise test, the number of times rising from sitting posture increased after the intervention versus before, but the difference was not statistically significant. In the rise-to-walk test, the performance showed statistically significant difference over time, and the post-hoc test showed a significant effect after the intervention versus before. There was no significant difference in perception and motor skills. In sum, the participatory rehabilitation program positively influenced dynamic balancing related to functional activities but had no significant effect on perception and motor skills, which is related to motor control and motor learning. It is suggested that to increase the participation in community activities, reduce fall risk, and improve dynamic balancing abilities in adults with developmental disabilities, participatory rehabilitation programs should be utilized to promote the physical wellbeing.

The Effects of Therapeutic Horse Riding, Adaptive Behavior in Adult with Intellectual Disabilities (재활승마가 지적장애 성인의 적응행동에 미치는 영향)

  • The purpose of this study is to examine the effects of therapeutic horse riding on adult with intellectual disability in terms of adaptive behavior. The subjects were 16 adults with intellectual disability. They participated in a therapeutic horse riding program of 60 minutes long, Twice a week for 12 weeks. The participants' adaptive behavior level were measured before and after 12 weeks of program with Social Maturity Scale. The data was analyzed by paired t-test using the SPSS. After 12 weeks of therapeutic horse riding program, there was a significant improvements on self-help, task, self-regulation & socialization. However, locomotion, communication changes did not reach statistical significance. The result indicates that therapeutic horse riding has a positive effect on adults with intellectual disability in self-help, task, self-regulation & socialization. This study suggests that therapeutic horse riding could be considered an effective method on adults with intellectual disability to improve adaptive behavior.

  • This study was to examine the effects of individual and environmental character among adults with intellectual disabilities on self-determination and to address issue of self determination in practical terms. This study was analyzed by the Structural Equations Models with 219 individuals with intellectual disabilities in residential home in the Metropolitan area, the Yeongnam area, the Honam area. The results showed that individual factors such as adaptive behavior and provider-user relationship recognition, and environmental factors such as institution size and institution location were directly affected to the self-determination of them. The opportunity was a full mediating effect in this process. In conclusion, this study stated how importance of the consumerism. Also it was suggested that some practical strategies to improve their self-determination in changing residential environment and support methods of the service in way to emphasis their opportunities.

Effects of Community-based Rehabilitation Programs on Activities of Daily Living, Quality of Life and Assistive Technology Satisfaction for Disabled Adults in South Korea

  • The purpose of this study was to examine the effects of community-based rehabilitation (CBR) program on activities of daily livings (ADLs), quality of life and assistive technology satisfaction for disabled adults in South Korea. Fourteen community-dwelling subjects were participated in a home-based rehabilitation program which performed once a month for 2 months. The outcome measures included Modified Barthel Index (MBI) for assessing ADLs, EuroQol five-dimensions questionnaire (EQ-5D) for quality of life (QoL) and Quebec User Evaluation of Satisfaction with assistive Technology 2.0 (QUEST 2.0) for assistive technology. Significant improvements in ADLs, quality of life and assistive technology satisfaction were observed after the program. This study indicated that community-based rehabilitation program is an effective method for improving ADLs, quality of life and assistive technology satisfaction for adults with disabilities.
