Acute Respiratory Distress Syndrome as the Initial Clinical Manifestation of an Antisynthetase Syndrome

  • Antisynthetase syndrome has been recognized as an important cause of autoimmune inflammatory myopathy in a subset of patients with polymyositis and dermatomyositis. It is associated with serum antibody to aminoacyl-transfer RNA synthetases and is characterized by a constellation of manifestations, including fever, myositis, interstitial lung disease, mechanic's hand-like cutaneous involvement, Raynaud phenomenon, and polyarthritis. Lung disease is the presenting feature in 50% of the cases. We report a case of a 60-year-old female with acute respiratory distress syndrome (ARDS), which later proved to be an unexpected and initial manifestation of anti-Jo-1 antibody-positive antisynthetase syndrome. The present case showed resolution of ARDS after treatment with high-dose corticosteroids. Given that steroids are not greatly beneficial in the treatment of ARDS, it is likely that the improvement of the respiratory symptoms in this patient also resulted from the prompt suppression of the inflammatory systemic response by corticosteroids.

굴곡성 기관지경을 이용한 성인의 기도내 이물 제거 (Fiberoptic Bronchoscopy for Removal of Endobronchial Foreign Bodies in Adults)

  • Foreign body aspiration occurs uncommonly in adults. A review of recent reports reveals only a few case reports on this topic. We have experienced 8 cases of endobronchial foreign bodies in adults from June 1988 to February 1991 which were removed successfully with fiberoptic bronchoscope and biopsy forceps. Only one of them had the primary disorder predisposing aspiration. Foreign bodies were located in right lower lobe (3 cases), right intermediate (2 cases), left lower lobe (2 cases) and left upper lobe bronchus (1 cases). Removal of endobronchial foreign boides in adult would be accomplished by fiberoptic bronchoscopy successfully.

Clinical Year in Review 2014: Critical Care Medicine

  • Severe sepsis is the most common cause of death among critically ill patients in non-coronary intensive care units. In 2002, the guideline titled "Surviving Sepsis Campaign" was published by American and European Critical Care Medicine to decrease the mortality of severe sepsis and septic shock patients, which has been the basis of the treatment for those patients. After the first revised guidelines were published on 2008, the most current version was published in 2013 based on the updated literature of until fall 2012. Other important revised guidelines in critical care field such as 'Clinical Practice Guidelines for the Management of Pain, Agitation, and Delirium in Adult Patients in the Intensive Care Unit' were revised in 2013. This article will review the revised guidelines and several additional interesting published papers of until March 2014, including the part of ventilator-induced lung injury and the preventive strategies.

  • The incidence of pediatric inflammatory bowel disease (IBD) is increasing worldwide, especially in the developing countries. It differs from adult disease in clinical manifestations, especially with regard to genetic predisposition in monogenic IBD. Pediatric disease also have a tendency to show more aggressive inflammation and greater extent of lesion. Newer drugs such as anti-tumor necrosis factor ${\alpha}$ have been known to make a difference in treating pediatric IBD. Recent studies suggested that the patients with high risk factors might have some benefits from earlier use of biologics. To achieve treatment goals such as relieving symptoms, optimizing growth, and improving quality of life while minimizing drug toxicity, more research is needed to develop tools for risk stratification in the use of biologics for pediatric IBD.

Introducing the general management of glomerular disease from a pediatric perspective based on the updated KDIGO guidelines

  • In 2021, a new chapter on the general management of glomerulonephritis (GN) was added to the Kidney Disease: Improving Global Outcomes (KDIGO). It emphasizes the importance of early general management of GN for improving long-term kidney outcomes and prognosis. The chapter introduces the management of glomerular diseases in 18 subchapters. Here, kidney biopsy for the diagnosis and evaluation of kidney function and the management of complications, such as hypertension, infection, and thrombosis, are presented. Moreover, the adverse effects of glucocorticoids and immunosuppressive therapy, which are commonly used drugs for glomerular disease, are mentioned, and a guideline for drug selection is presented. Each subtheme focused on items reflecting the interpretation of the "practice points" of the expert working group are introduced. In this review of the general treatment for GN in the KDIGO guidelines, excluding pregnancy and reproductive health, we focused on and compared various references pertaining to pediatric GN management.

RNF43 and ZNRF3 in Wnt Signaling - A Master Regulator at the Membrane

  • The Wnt 𝛽-catenin signaling pathway is a highly conserved mechanism that plays a critical role from embryonic development and adult stem cell homeostasis. However, dysregulation of the Wnt pathway has been implicated in various diseases, including cancer. Therefore, multiple layers of regulatory mechanisms tightly control the activation and suppression of the Wnt signal. The E3 ubiquitin ligases RNF43 and ZNRF3, which are known negative regulators of the Wnt pathway, are critical component of Wnt signaling regulation. These E3 ubiquitin ligases control Wnt signaling by targeting the Wnt receptor Frizzled to induce ubiquitination-mediated endo-lysosomal degradation, thus controlling the activation of the Wnt signaling pathway. We also discuss the regulatory mechanisms, interactors, and evolution of RNF43 and ZNRF3. This review article summarizes recent findings on RNF43 and ZNRF3 and their potential implications for the development of therapeutic strategies to target the Wnt signaling pathway in various diseases, including cancer.

  • 과거에 비해 최근에는 우리나라에서 기생충 감염의 빈도가 낮아지긴 하였지만, 우리나라에서 비교적 흔한 감염원인 민물 게장등을 섭취한 병력이 있고, 호산구성 흉막 삼출과 말초혈액에서 호산구증다증이 있을 경우, 반드시 기생충 감염의 가능성을 생각해 보아야 한다. 피하조직에서의 폐흡충의 이소기생 례가 빈번한 것은 아니지만, 본 증례와 같이 이동성의 복부 종괴가 있을 경우 조직검사와 면역학적 검사를 통해 기생충 감염 여부에 대한 확인 절차는 반드시 필요하다. 그럼으로써, 결핵이나 폐암등 다른 질환으로의 오진에 의해 발생하는 경제사회적 손실을 최소화할 수 있을 것으로 사료된다.

Metorchis orientalis의 병아리내 성장발육 및 성충의 형태 (Growth and development of Metorchis orientalis in chicks and its adult morphology)

  • Meterchis orientalis의 병아리 숙주내 감염력 및 성장발육상을 알아 보고 성충의 형태학 적 특징을 관찰하였다. 참붕어로부터 분리 수집한 피리유충을 부화된지 3일된 병아리 40마리에 각각 100개씩 경구감염시킨 후 1.5, 3, 5, 7, 9, 11, 14일 및 21일에 충체를 누수하여 고정, 염색, 탈수 및 봉입 등의 과정을 거친 다음 관찰 및 계측하였다. 총체는 모두 병아리의 담낭에서 발견되었으며 평균 32U의 회수율을 나타내었다. 감염 후 9일까지는 비교적 완만히 성장하였으나 9일에서 11일 사이에는 급속히 성장하였고 11일에서 14일 사이에 다시 성장이 둔화하였으며 21일에 회수한 충체는 14일된 충체보다 크기가 작았다. 감염 후 1.5일파 5일에 요수한 충체에서는 비생식기관과 생식원기가 관찰되었고 5일된 충체에서는 비정형적인 난락와 엽상의 고구이 관찰되었으며 이후 차차 발육하여 감염 후 11일에 회수한 충체들은 완전히 성숙하였다. 성숙한 충체는 나뭇잎 모양이었으며 1∼3굽이의 저정낭, 타원형의 난소, 주머니 모양의 수정낭,517엽의 고환 및 소포형의 난황선 등을 보유하고 있었다. 충란은 평균 31.gx15.Sum 크기이었고 최대 폭이 가운데 지점에 있었으며 난개 반대편의 난각이 비후되어 있었다. 이상의 결과로 병아리가 M orienta비의 새로운 숙주로 추가되었고 이 흡충이 병아리 내에서 감염 후 21일까지 sigmoid형으로 성장하며 성충이 되기까지는 l1일 정도가 소요됨을 알 수 있었다.

Ectopic Schistosomiasis Involving Axillary Lymph Nodes

  • Axillary lymph node schistosomiasis is a form of ectopic schistosomiasis, in which the Schistosoma adult worms or eggs are infected in the axillary lymph nodes. A 53-year-old woman visited our hospital with a small mass in the right armpit, which had been present for over 1 month. Histological examination of biopsy samples from the right axillary lymph nodes revealed granulomas containing calcified Schistosoma eggs. This is the first authentic case of ectopic schistosomiasis identified in the axillary lymph nodes.

Morphological Studies of Developmental Stages of Oculotrema hippopotami (Monogenea: Polystomatidae) Infecting the Eye of Hippopotamus amphibius (Mammalia: Hippopotamidae) Using SEM and EDXA with Notes on Histopathology

  • The present study was performed to observe histopathological effects of Oculotrema hippopotami Stunkard, 1924 infection in the eye of Hippopotamus amphibius, as well as to reveal new details of morphology and structural features of this monogenean and its comparison between 2 age stages of the parasite. This was done using both light and scanning electron microscopy, energy dispersive X-ray analysis (EDXA) and histopathology. The presence of a mixture of different generations (adult and sub-adult) in one host individual is common for Oculotrema Stunkard, 1924 in contrast to Polystoma Zeder, 1800. New metrical and graphical information obtained for adults and sub-adults compared with the previous studies. Here we show the presence of genital papillae in adults, metrical data on the distal part of the vas deferens. SEM micrographs of sperm ejaculatory structures and information about the flattened dorsal side of the body provided for the first time. Histopathological changes, such as necrosis and hemorrhage in host tissues as a result of O. hippopotami attachment structures are described. Structural analysis of different body parts of O. hippopotami of both age groups are also included. We show qualitative differences in the presence of hardening ions (S, P, Ca) in attachment structures (oral and haptor suckers) that increase with the age of the worm. The presence of sub-adults and adults on the same host, together with high levels of infection without high pathogenicity may account for Oculotrema being one of the most successful parasites among the Monogenea.
