부신증식으로 인한 원발성 Aldosterone증 1예 (1 Case of Primary Aldosteronism Caused by Adrenal Hyperplasia)

  • 본 환자는 전신쇠약감 피로감, 고혈압과 혈청 potassium치가 저하되어 있고 혈청 Renin활성도가 감소되어 있으면서 Aldosterone억제 시험에도 억제가 되지 않았음을 볼 때 원발성 Aldosterone증을 의심할 수 있었다. 또 자세변화 시험에서 혈청 Renin활성도가 떨어져 있었고 자세변화에 따라 혈청 Aldosterone이 증가했으며 컴퓨터 단층촬영에서 별다른 병변이 뚜렷이 보이지 않음을 볼때 부신증식성 Aldosterone증으로 의심된다. 따라서 본 환자에게 spironolactone을 투여했고 이후 외래를 통해 추적검사 중이며 혈압 조절이 양호할 뿐 아니라, 불편을 느끼던 증상들도 호전이 되었음을 안 수 있었다. 끝으로 원발성 Aldosterone증에 대한 관심을 더 기울인다면 더 많은 증례가 발견될 것으로 사료되고, 본 저자들은 보다 많은 증례를 찾아 추후 증례보고및 추적검사할 예정이다.

연령에 따른 흰쥐 부신 수질의 전자현미경적 관찰 (Ultrastructural Studies on the Adrenal Medulla of The Developmental Rats)

  • 흰쥐 부신수질의 발생에 따른 미세구조적 변화를 관찰하기 위하여 임신 20일령, 출생직후, 출생후 7일, 15일 및 30일에 각각 조직을 채취하여 전자현미경적으로 관찰한 바, 흰쥐 부신 수질에서는 과립내용물의 전자밀도가 중등도 또는 낮고, 한계막에 싸여 있으며, 한계막과 과립 내용물간에 비교적 넓은 halo를 형성하거나 얇은 core를 형성하는 원형 내지 타원형의 소위 adrenaline 과립 $(123\sim200nm)$과 과립 내용물의 전자밀도가 adrenaline 과립보다 높고 한계막과 과립내용물간에 대체로 넓은 halo를 형성하는 부정형의 소위 noradrenaline 과립 (장경 $177\sim299nm$, 단경 $124\sim194nm$) 등 두 가지 형태의 과립이 관찰되었다. 이들 두 종류의 과립은 임신 후기 및 출생직후에서는 두종의 과립이 한 세포내에서 흔재하고 있었으나, 출생후 7일령에서부터 명확한 두종의 세포를 구별할 수 있었으며 과립의 크기와 수는 일령에 따라 다소 차이를 나타내었고, 핵내에서는 치밀체가 출현하였다.

비 유전독성 간발암물질일 Clofibrate의 F344 랫드에 있어서 경구 아급성독성시험 (Oral Subacute Toxicity of Nongenotoxic Hepatocarcinogen, Clofibrate in F344 Rats)

  • Clofibrate, a peroxisome proliferator, is hepatocarcinogenic in rats in a dose-dependent manner. A total of 70 male and female F344 rats, 5-week-old, were divided into three groups. Rats were fed clofibrate at 0, 0.25, or 0.5% in diet for 30 days. All rats were anesthetized with $CO_2$, blood samples were taken by cardiac puncture for hematology and clinical chemistry, and the rats were killed by exsanguination. Livers, kidenys, pancreas, adrenal glands, spleen, heart, lungs, thyroid gland, reproductive organs, and digestive organs were removed, weighed, later processed, and embedded with paraplast for histological examination. The relative liver and kidney weights with respect to final body weight in the clofibrate-treated group were significantly increased compared with those of control group at all dose levels (p<0.01). It has been suggested that clofibrate may influence on hepatotoxicity by increases in peroxisomal proliferation.

  • 저자 등은 임상증상, 지속적인 저칼률혈증 및 복부 질산화단층촬영으로 원발성 고-알도스테론증으로 밝혀졌는 42세 여자환자에서 주요 증상인 주기적 근마비와 상지 근죄악감이 혈청감사상 지명한 근육효소의 증가 및 근전도검사로 근육병임이 증명되었고 선종제거로 임상증상의 완전조직이 생긴 1례를 보고하였다.

  • The word of stress crime from Latin language as stringere and it was used in medical fields from 1935. According to Selye, all the biological bodies reveal physilolgical changes when some stimulation exceed normal levels, and consequently the pituitary gland and adrenal systems are activated. Jacob expressed that stress is the loss of homeostasis by physical, chemical, and emotional stimulation. When biological organisms receive extreme stress the amount of catecholamine excretion are increase. Author investigated the catecholamine contents in rat urine after giving the low temperature stress, noise stress, and water immersion stress. The 24 hours rat urine was collected by adding 1 ml 6 N-HCl and the sample is passed through Bio-Rex 70 samples treatment column to extract catecholamine and detected the catecholamine with HPLC-fluorescence detetor. The highest epinephrine concentration was 67.14 ng in water immersion stress condition and the dopamine concentration of 221.37 ng was shown in the low temperature stress condition.

  • The oral admistration of deer horn extract was proved to cause a considerable amelioration to the impairment of hepatic oxidative phosphorylation and to the decrease of ATP content in liver tissue induced by cholesterol ingestion. Deer horn extract was shown to accelerate considerably incorporation of $acetate-1-C^{14}$ into cholesterol in liver tissue and it tends to restore a decrease of GOT activity of liver tissue, caused by cholesterol administration. Histological examination showed that deer horn extract had a remakable preventive effect aginst fatty infiltration of organgs such as liver, heart, spleen, and adrenal gland of cholesterol given rabbits.

Anti-stress Effects of Kimchi

  • The anti-stress effects of kimchi were studied in the Sprague-Dawley rats dosed with kimchi. The rats in the stress groups were subjected to immobilization stress for 2 hr/day for 5 days. At the end of the experimental period, daily average food intake and body weight (BW) gain had been reduced in the stress group compared to the control group. Daily average food intake was significantly increased in the stress-kimchi diet group compared to the stress-only group. The weights of the thymus and spleen were decreased by immobilization stress, but this reduction was partially suppressed by the addition of kimchi. The weights of the adrenal gland and epididymal adipose tissue were increased in the stress group, but ingestion of kimchi completely and partially suppressed these stress-induced changes, respectively. Serum levels of total cholesterol and triglyceride, and plasma levels of corticosterone were increased in the stress group, but at control levels in the stress-kimchi diet group.

고령의 환자에서 발생한 다발성 후경부 신경절신경종 1예 (A Case of Multiple Posterior Cervical Ganglioneuromas in Elderly Patient)

  • Ganglioneuroma (GN) is benign neurogenic tumor arising from ganglia of the sympathetic nervous system. They are mostly found at posterior mediastinum, retroperitoneum, and adrenal gland, whereas only 1-5% occurred in the cervical region. GN usually present as a single, painless and slow-growing mass, but multiple cervical occurrences are extremely rare. An 80-year-old woman came to our clinic complained of posterior neck mass for three years. We performed surgical excision, and it was finally diagnosed as GN. We report the unique and rare disease entity with a brief literature review.

  • Neuroblastoma arises from the embryonic tissue of the adrenergic rest. It is commonly found in children and mostly in nonrenal tissue. We present a case of intrarenal neuroblastoma which was initially thought to be a Wilms' tumor. The patient was a 18 months-old girl treated with radical nephrectomy and adjuvant chemotherapy after operation. The neoplasm within the kidney in children cannot always indicate Wilms' tumor. Neuroblastoma of the adrenal gland or retroperitoneal tissue may often compress or invade the kidney directly or arise from the kidney. Clinical aspects that differentiate between neuroblastoma and Wilms' tumor are discussed with a review of the literatures.
