중·고등학생의 음주 실태와 학교 음주예방 교육의 영향: 2015년 청소년건강행태온라인조사를 활용하여 (Drinking Status and Effects of School-based Alcohol Prevention Programs in Middle and High School Students: Using the 2015 Youth Risk Behavior Web-based Survey Data)

  • Purpose: The purpose of this study was to examine effects of school-based alcohol prevention programs on drinking statuses of adolescents. Methods: The findings of this study was based on the data obtained from the '2015 11th Korea Youth Risk Behavior Web-based Survey. The number of study subjects were 68,043. Results: It was figured that 35.6% of the study subjects had experienced school-based alcohol prevention programs within the last 12 months. As the students got older, the chances to participate in the programs decreased (p<.01). For both middle and high school students, current drinking rates for the educated was lower than those of the uneducated students(6.6% vs 8.0%; 22.2% vs 25.9%) and it was statistically significant. A similar pattern was found for high-risk drinking rates. Those educated showed lower rates than the uneducated with statistical significance of p<.001. In addition, the educated had lower problem drinking rate than the uneducated for both middle (p<.05) and high school students (p<.001). The results of logistic regression analysis showed that school-based alcohol prevention programs had statistically significant effect on current drinking status of adolescents (p<.05). However, it had significant effect only on high-risk drinking status of high school students (p<.05) and had no effect on problem drinking. Conclusion: This study addressed effectiveness of school-based adolescent alcohol prevention programs and that it is important to develop means to implement school health education.

12주간의 수중운동이 경직성 뇌성마비 청소년의 어깨 관절 등속성 근기능에 미치는 영향 (The Effects of 12 Weeks Aquatic Exercise Training on Shoulder Joint Isokinetic Muscle Function in Adolescent Spastic Cerebral Palsied)

  • Purpose: The present study investigated the effects of 12-week aquatic exercise training on isokinetic muscle function of the shoulder in adolescents with cerebral palsy. Methods: The study included four male and four female adolescents with cerebral palsy. Isokinetic muscle function was measured at an angular velocity of $60^{\circ}/s$, using Biodex System VI Pro. The peak torques of internal rotation and external rotation were measured before and after training. Aquatic exercise training was performed once a day for 120 min, 4 times a week for 12 weeks. Results: The peak torque of external rotation according to body weight and mean power of internal rotation were significantly higher after training (p < 0.05). Conclusion: Our findings suggest that 12-week aquatic training for adolescents with cerebral palsy can improve isokinetic muscle function of the shoulder. Future studies should analyze the changes in isokinetic muscle function of the shoulder in more detail using various aquatic exercise programs to investigate their effects on individuals with cerebral palsy.

집단사회성놀이가 자폐청소년의 사회성에 미치는 효과 (The effect of group sociality play in sociality of autistic adolescent)

  • 집단사회성놀이는 놀이를 통한 사회성 증진을 목적으로, 그룹활동을 통하여 일상생활에서 필요로 하는 목적 있는 활동, 인지, 신체움직임 등 여러 요소들을 잘 조합하여 자유롭게 자아를 표현시키는 치료적 방법이다. 이를 타인과의 의사소통 능력이 저하된 자폐청소년들에게 적용하여 궁극적으로 자폐청소년들의 대인관계 개선과 사회성을 증진을 유도함으로써 그 효과를 검증하고자 본 연구를 실시하였다. 본 연구는 대전 소재 주간보호센터를 이용하는 사회성에 결함을 가지고 있는 8명의 자폐청소년을 대상으로 단일군 전후실험설계를 적용하였다. 집단사회성놀이 프로그램은 2013년 10월부터 12월까지 주 2회씩 7주간(총 14회기), 한 회기 당 60분씩 시행하였고 중재 전후의 사회성을 Wilcoxon 순위검정을 실시하여 비교분석 하였다. 연구결과 이화-자폐 아동 행동 발달 평가(E-CLAC)와 사회성숙도 검사의 점수의 총점이 $39.88{\pm}6.78$에서 $47.00{\pm}4.28$, $49.96{\pm}28.00$에서 $61.36{\pm}21.75$로 각각 통계적으로 유의한 변화가 있었음을 확인할 수 있었다. 세부적으로는 집단에서의 적응, 지시 따르기, 행동 빈도 증진 등의 하위항목에서 유의한 변화가 크게 나타났다. 결론적으로 집단사회성놀이에 포함된 집단활동 요소와 대상자들의 발달을 고려한 놀이 활동 요소가 복합적으로 작용하여 자폐청소년의 사회성 증진 효과에 효과적으로 나타났음을 확인할 수 있었다.

일부 대학생의 감각처리능력과 대인관계문제와의 관계 (The relation between sensory processing ability and interpersonal problem of some university students)

  • 이 연구는 성인 대학생의 감각처리 능력과 대인관계 문제와의 관련성을 조사하였으며 연구 대상자 177명이 작성한 청소년/성인 감각 프로파일 점수와 대인관계문제 검사 점수의 관련성을 분석한 결과 '낮은 감각 등록'영역은 KIIP 모든 척도와 관련성이 있는 것으로 나타났다 '감각찾기' 영역은 통제지배(r=0.150, p<0.05), 자기희생(r=0.175, p<0.05), 과관여(r=0.218, p<0.01), 대인적과민성(r=0.196, p<0.01), 사회적인정욕구(r=0.149, p<0.05)가 상관관계를 보였다. '감각민감성'영역은 KIIP 모든 척도와 관련성이 있는 것으로 나타났다. '감각회피' 영역은 KIIP 척도 중 통제지배(r=0.403, p<0.01), 자기중심성(r=0.420, p<0.01), 대인적비수용성(r=0.452, p<0.01), 공격성(r=0.417, p<0.01)과 비교적 높은 상관관계를 보였다. 본 연구를 통해 감각처리 능력과 대인관계 문제와는 전반적으로 관련이 있는 것으로 나타났으며 특히 낮은 감각등록과 감각민감성 영역이 대인관계 문제와 높은 상관관계가 있는 것으로 나타났다.

발달장애를 가진 아동·청소년의 성교육 연구에 대한 체계적 고찰 (A Systematic Review on Sex Education of Children and Adolescent With Developmental Disabilities)

  • 목적 : 본 연구는 발달장애를 가진 아동 청소년을 위한 성교육의 대상, 기간, 유형, 내용, 교수방법을 체계적으로 분석하여 아동작업치료사에게 성교육에 대한 기초자료와 방향성을 제공하는 것을 목적으로 한다. 연구방법 : 전자 데이터 베이스인 PubMed, EBSCO host(CINAHL Plus with full text), Medline (ProQuest), RISS, KISS를 사용하여 2008년 8월부터 2018년 9월까지 국내외 학회지에 개제된 연구를 대상으로 검색하였다. 주요 용어는 "(Sexual Education OR Sex Education) AND (Disability Disorder OR Autism) AND (Effect OR Effectiveness)"를 해외논문 검색에 사용하였고, 국내논문은 "장애 AND 성교육"으로 검색하여 최종 15편의 연구를 선정하였다. 결과 : 근거의 질적 수준은 대상연구 논문 15편 중 수준 I이 1편, II이 3편, 수준 III이 9편, 수준 IV가 2편이었다. 성교육의 내용으로 '신체와 성장', '대인관계 기술'이 가장 많았으며, 성교육 제공분야는 특수교육, 작업치료 순으로 나타났다. 성교육의 효과는 '성지식'과 '성태도', '문제가 되는 성행동'에 있었다. 결론 : 본 연구는 발달장애를 가진 아동 청소년을 위한 성교육의 내용과 경향, 효과를 고찰하여 작업치료 임상분야에서 실천하기 위한 기초자료로 제시하고자 하였다. 추후에는 성교육 중재를 통한 작업참여 효과 연구, 클라이언트의 성활동에 대한 아동작업치료사의 의식 조사, 성 발달 척도 개발이 필요할 것이다.

Enrollment in Private Medical Insurance and Utilization of Medical Services Among Children and Adolescents: Data From the 2009-2012 Korea Health Panel Surveys

  • Objectives: The purposes of this study were to examine the status of children and adolescents with regard to enrollment in private medical insurance (PMI) and to investigate its influence on their utilization of medical services. Methods: The present study assessed 2973 subjects younger than 19 years of age who participated in five consecutive Korea Health Panel surveys from 2009 to 2012. Results: At the initial assessment, less than 20% of the study population had not enrolled in any PMI program, but this proportion decreased over time. Additionally, the number of subjects with more than two policies increased, the proportions of holders of indemnity-type only ('I'-only) and of fixed amount+indemnity-type ('F+I') increased, whereas the proportion of holders with fixed amount-type only ('F'-only) decreased. Compared with subjects without private insurance, PMI policyholders were more likely to use outpatient and emergency services, and the number of policies was proportionately related to inpatient service utilization. Regarding outpatient care, subjects with 'F'-only PMI used these services more often than did uninsured subjects (odds ratio [OR], 1.69), whereas subjects with 'I'-only PMI or 'F+I' PMI utilized a broad range of inpatient, outpatient, and emergency services relative to uninsured subjects (ORs for 'I'-only: 1.39, 1.63, and 1.38, respectively; ORs for 'F+I': 1.67, 2.09, and 1.37, respectively). Conclusions: The findings suggest public policy approaches to standardizing PMI contracts, reform in calculation of premiums in PMI, re-examination regarding indemnity insurance products, and mutual control mechanisms to mediate between national health insurance services and private insurers are required.

국내 학령기 아동·청소년 대상 집단따돌림 중재에 관한 체계적 문헌고찰 (A Systematic Review of School-bullying Interventions for Children and Adolescents in Korea)

  • Purpose: Bullying is a global problem, and various programs are under way to prevent it. The purpose of this study was to review school bullying interventions for Korean school-age children and adolescents. Methods: Online databases such as RISS, KISS, DBpia, NDSL and KMBASE were searched, identifying 32 intervention studies published from January 2009 to November 2018. Results: Thirty-two intervention studies were identified: 23 included school bullying prevention and 9 included school bullying treatment for victims or youth at high risk for bullying. The main purpose of preventive intervention was to decrease the bystander's attitude toward group bullying and treatment program was to improve the psychosocial adaptation of bullying victims. The school bullying interventions varied from group counseling, social skills training, art therapy, bibliotherapy using role-play, game & activities. Classroom environment variables and self-esteem, peer-related variables improved significantly after the school bullying prevention programs and school bullying treatment programs, respectively. Conclusion: There is potential for enhancing the outcomes of the behavioral, interpersonal psychological variable. Integrated interventions considering the individuality, gender and physical health of children and adolescents will also be needed. However, a rigorous study design is required to compensate for the methodological limitations.

유연성 편평발 청소년에게 적용한 로우다이 테이핑 기법이 동적 균형과 제자리 멀리뛰기에 미치는 영향 (Effect of low-dye taping technique applied to flexible flatfoot adolescents on dynamic balance and long jump in place)

  • 유연성 편평발 청소년에게 적용한 로우다이 테이핑 기법이 동적 균형과 제자리 멀리뛰기에 미치는 영향을 알아보았다. 연구의 대상자는 유연성 편평발 청소년 35명으로 탄력 테이프 17명과 비탄력 테이프 17명으로 로우다이 테이핑 기법을 적용하였다. 본 연구의 자료분석은 t-test 활용하여 그룹 중재 적용 전과 적용 후, 그리고 각각의 그룹간을 비교하였다. 연구 결과 동적 균형과 제자리 멀리뛰기에서 중재 적용 전과 적용 후에 유의한 결과가 나타났으며(p<.05), 각각의 그룹 간 비교에서 유의한 차이가 나타나지 않았다(p>.05). 결론적으로 유연성 편평발의 청소년에게 적용된 로우다이 테이핑 기법은 동적 균형과 제자리 멀리뛰기에 효과가 있으며, 로우다이 테이핑은 안쪽 세로활 상승의 즉각적 효과를 보기 위한 중재 방법으로 적절하다.

청소년기 요추간판탈출증에 대한 한방 보존적 치료 12례의 후향적 분석 (Effects of Conservative Korean Traditional Medical Treatment on Lumbar Intervertebral Disc Herniation in 12 Adolescents : A Retrospective Study)

  • Objectives : This study was performed to assess the effectiveness of conservative Korean traditional medical treatment on lumbar intervertebral disc herniation in 12 adolescents. Methods : Data were collected from adolescent patients diagnosed with lumbar intervertebral herniation by MRI, hospitalized at Jaseng Korean Medicine Hospital from January 1, 2014 to December 31, 2015. A total of 12 patients were included in the study. Patients were treated by acupuncture, pharmacopuncture, electroacupuncture, herbal medicine, chuna manual therapy, physical therapy during hospitalization period. To measure treatment outcomes, a verbal numerical rating scale (VNRS), Oswestry disability index (ODI), range of motion (ROM), Straight leg raising test (SLR), and EQ-5D were used. Results : The average age of the participants was $16.05{\pm}1.62$. The average of duration of symptom was $19.25{\pm}26.83(weeks)$ and the average length of hospitalization was $23.8{\pm}16.4(days)$. 2 patients (17 %) suffered from only lumbar pain, while 10 patients (83 %) suffered from lumbar and leg pain. No participant suffered from only leg pain. 8 patients (67 %) were diagnosed with disc herniation in one segment, and 4 patients (33 %) were diagnosed with disc herniation in multiple segments. A total of 17 disc segments (28 %) were herniated. The most herniated lumbar level was L4-5. 3 segments were diagnosed with bulging (17 %), 6 with protrusion (35 %), and 8 with extrusion (47 %). Extrusion was the most frequent herniated type. After treatment, the average VNRS of lumbar pain significantly decreased from $5.58{\pm}1.62$ to $2.91{\pm}1.56$ (p<0.001) and the average VNRS of leg pain significantly decreased from $5.16{\pm}2.51$ to $3.08{\pm}1.8$ (p<0.001). ODI significantly decreased from $48.87{\pm}18.72$ to $28.57{\pm}15.05$ (p<0.05), and EQ-5D significantly increased $0.58{\pm}0.31$ to $0.80{\pm}0.12$ (p<0.05). Range of flexion significantly improved from $61.25{\pm}32.62$ to $68.33{\pm}26.22$ (p<0.05). Conclusion : Conservative Korean Traditional Medical Treatment may be effective in the treatment of lumbar intervertebral disc herniation in adolescence. However, more case reports and clinical research are needed.

뇌성마비 청소년의 자아개념과 자가간호수행의 관계 (The Relationship Between Self-Concept and Self-Care Activities of the Cerebral Palsied Adolescents)

  • The purpose of this study was to investigate the relationship between self-concept and self-care activities of the cerebral palsied adolescents and to gain the baseline data for development of effective rehabilitation nursing intervention program of the cerebral palsied adolescents. The design of this study was a descriptive correlational study. The subjects of the study were 160 cerebral palsied adolescents attending at special schools located in Seoul and Kyonggi and rehabilitation centers located in Seoul, Kyonggi and Kyongnam province. The data was collected from May 20 to July 20, 2000. The instrument used for this study were the self-concept scale(50items 4point scale) and self-care activities scale(29items 4point scale). Self-Concept Scale had developed by Fitt(1965), which was standardized by Chung(1968) and modified by Kim(1984). Self-Care Activities Scale developed by the researcher through out the consulting of expert and pilot study on the basis of ADL check list developed by Kang(1984) and LDSQ-3(Lambeth Disability Screening Questionnair-3) developed by Na et al. (1995). The data was analyzed by the SPSS/PC+program using frequency, percentage, mean, standard deviation, t-test, ANOVA & Scheffe test and Pearson's correlation coefficient. The results of this study were as follows; 1. The mean score of self-concept was $138.55{\pm}17.20$(range: 50-100), which the item mean score was $2.77{\pm}0.34$(range: 1-4). The score of subarea of the self-concept was the highest score in family self ($3.01{\pm}0.54$) and the lowest score in physical self ($2.52{\pm}0.42$). 2. The mean score of self-care activities was $95.25{\pm}21.69$ (range: 29-116), which the item mean score was $3.28{\pm}0.75$ (range: 1-4). The score of subarea of the self-care activities was the highest score in feeding($3.75{\pm}0.59$) and the lowest score in walking($2.64{\pm}1.21$). 3. There were statistically significant difference in the score of self-concept according to the age (F=3.24, P=.04), the grads (F=4.36, P=.01), and types of cerebral palsy (F=2.42, P=.03). 4. There were statistically significant difference in the score of self-care activities according to the age (F=8.29, P=.00), the grads (F=16.05, P=.00), types of living place (F=6.46, P=.00), types of cerebral palsy (F=48.92, P=.00), whether or not receiving a rehabilitation therapy (t=-3.64, P=.00), whether or not receiving a vocational training (t=2.14, P=.03), and whether or not using a device (t=-7.42, P=.00). 5. There was not significant correlation between self-concept and self-care activities (r=.081, P=.311).
