A Case of Adnexal Torsion in Pregnancy (임신과 동반된 자궁부속기 염전 1례)

  • Adnexal torsion is not a common surgical emergency in obstetrics and gynecology, and is not observed frequently during pregnancy with its incidence being one out of about 5,000 pregnancies: pregnancy is a factor that encourages torsion. The traditional treatment for ischemic and twisted adnexa consists of oophorectomy or salpingo-oophorectomy. With early diagnosis. a conservative approach is recommended in cases of young women to assure future fertility. With a brief review of literature, we report a case of unilateral salphingo-oophorectomy during pregnancy due to adnexal torsion with the delivery of a normal baby.

Orbital Meningioma in a Dog (개에서 발생한 안와 수막종 1례)

  • A 5-year-old, castrated male Yorkshire Terrier with strabismus and exophthalmos of the right eye was referred to the Veterinary Medical Teaching Hospital, Chungnam National University. Radiography, ultrasonography(US), computed tomography(CT) and magnetic resonance imaging(MRI), demonstrated a well-circumscribed, locally calcified mass of the retrobulbar region with contiguous bone structures invaded. Surgical removal of the adnexa, globe, optic nerve and the incorporated orbital neoplasm was performed. Primary orbital meningioma was confirmed by histopathologic examination.

MALT Lymphoma of Ocular Adnexa: A Case Report (안구 부속기의 점막연관 림프조직형 림프종의 증례보고)

  • Purpose: Lymphoma originated from mucosa associated lymphoid tissue(MALT) is most common in gastrointestinal system, and rarely found in salivary gland, thyroid, bronchus or orbit. We experienced a case of MALT lymphoma which was originated from conjunctiva and involving lower eyelid without metastasis. Methods: A 40-year-old man suffered palpable mass on right lower eyelid without pain. Orbital computed tomographic and ultrasonographic findings showed a conical mass($1.9{\times}1.2{\times}0.9cm$ size) inside lower eyelid. The mass was completely excised under local anesthesia and histopathological examination was followed. Results: Microscopic finding showed a multiple follicular colonization. In the follicle, small lymphocytes and plasma cells differentiated to centrocyte-like cell, monocyte B cell, plasma cell were diffusely infiltrated. Immunophenotyping was preformed on fixed section. The majority of the small cells were immunoreactive for the B cell marker CD20. Based on the typical histological findings supported by immunostaining, the mass was defined as MALT lymphoma. After excision, SPECT, abdominal CT was carried out and there were no evidence of extraorbital disease. Conclusion: Biopsy and pathological examination should be performed in patients who complain palpable mass on lower eyelid because of possibility of MALT lymphoma. Although MALT lymphoma is rarely metastasized, it is necessary to evaluate the extraorbital involvement using SPECT or other radiologic exams. For detecting extraorbital involvement, periodic follow-up examination is need.

The histologic features of the uterus and adnexa extirpated from gender identity disorder patients with depot androgen injection (남성호르몬 투여 받은 성 주체성 장애 환자에서 적출된 자궁 및 부속기의 조직학적 특징에 관한 고찰)

  • Objective: To investigate the histologic features of the uterus and adnexae extirpated from gender identity disorder (GID) patients that received depot androgen injection. Methods: We reviewed the histologic findings of the uterus and adnexae removed from sixteen GID patients, who had taken depot androgen injection for 5~168 months. Results: Fourteen patients (87.5%) showed the atrophied epithelium of exocervix and all of 16 patients (100%) showed the atrophy of endometrium. Seven patients (43.7%) showed multiple cystic follicles in the ovarian cortex and 6 patients (37.5%), 3 patients (18.7%) showed corpus albicans and corpus luteum, respectively. Conclusions: Exogenous androgen induced atrophy of cervix and endometrium. This effect was more prominent in the endometrium. In addition, PCO-like histologic features were observed in the ovary.

Efficient DRG Fraud Candidate Detection Method Using Data Mining Techniques (데이터마이닝 기법을 이용한 효율적인 DRG 확인심사대상건 검색방법)

  • Objectives : To develop a Diagnosis-Related Group (DRG) fraud candidate detection method, using data mining techniques, and to examine the efficiency of the developed method. Methods ; The Study included 79,790 DRGs and their related claims of 8 disease groups (Lens procedures, with or without, vitrectomy, tonsillectomy and/or adenoidectomy only, appendectomy, Cesarean section, vaginal delivery, anal and/or perianal procedures, inguinal and/or femoral hernia procedures, uterine and/or adnexa procedures for nonmalignancy), which were examined manually during a 32 months period. To construct an optimal prediction model, 38 variables were applied, and the correction rate and lift value of 3 models (decision tree, logistic regression, neural network) compared. The analyses were peformed separately by disease group. Results : The correction rates of the developed method, using data mining techniques, were 15.4 to 81.9%, according to disease groups, with an overall correction rate of 60.7%. The lift values were 1.9 to 7.3 according to disease groups, with an overall lift value of 4.1. Conclusions : The above findings suggested that the applying of data mining techniques is necessary to improve the efficiency of DRG fraud candidate detection.

  • Focal adnexal dysplasia(FAD) is a rare non-neoplastic lesion of adnexal appendages that occurs in dogs and has good prognosis. We report a case in which FAD diagnosis was favored by its histopathological features. A 8-year-old, castrated male American cocker spaniel dog presented with papillomatous masses on the distal phalanx of left forelimb. Histopathologically, the stroma was sharply demarcated from the adjacent dermis and subcutis. The adnexa was surrounded and interspersed with abundant, hyalinized collagen and the numerous sebaceous lobules distributed haphazardly around hair follicles, which is thickened, dilated and tortuous with abundant keratin. Epidermis was acanthotic. Proliferative lesions of the skin region are very common with similar gross features, thus the basic histopathological examination supported by additional diagnostic methods allow to establish the definitive diagnosis in most cases of cutaneous tumor or tumor-like lesions. And it is important that physicians and pathologists are aware of this unusual tumour-like nonneoplastic lesion.

Preoperative Staging of Endometrial Carcinoma by MRI (자기공명영상을 통한 자궁내막암의 수술전 병기 결정)

  • Background: In patients with endometrial carcinoma, preoperative evaluation of exact staging has important prognostic and therapeutic implications. The incidence of pelvic and aortic lymph node involvement in endometrial carcinoma depends on grade of tumor differentiation and depth of myometrial invasion. Material and method: To evaluate whether MRI provides a preoperative assessment for staging of endometrial carcinoma, MRI was undertaken in 28 patients, a few weeks before operation. Myometrial invasion was devided in three categories, and involvement of cervix, adnexa, and pelvic cavity were classified. Results: The results of MR imaging were compared with these of pathology. The preoperative MRI staging of endometrial carcinoma was correct in 22 out of 28 patients. In the evaluation of myometrial invasion, the MR imaging underestimated in 4 cases and overestimated in 1 case. Conclusion: In patients with endometrial carcinoma, MR imaging is very useful in the assessment of the depth of myometrial invasion, stromal invasion of cevix, lymphatic & pelvic metastases and extent of the lesion.

  • Mucosa-associated lymphoid tissue(MALT) lymphoma is thought to originate from marginal zone B-cells. In the WHO classification, Extranodal marginal zone lymphoma of MALT is classified B-cell Non-Hodgkin lymphoma. Common sites of MALT lymphoma include stomach, lung and the ocular-adnexa. Although less common in other sites, it is the most common low-grade lymphoma of the breast, thyroid, bowel skin and soft tissue. No strong age or gender predominance exists in MALT lymphoma. Dissemination to other sites can occur. In the oral cavity, MALT lymphoma is rare. Herein, we present a case of intra-oral MALT lymphoma. 66 year-old woman without any background of immunodeficiency or autoimmune disease admitted department of oral & maxillofacial surgery in Ulsan university hospital for evaluation of long-standing mild upper lip swelling. The lesion was completely resected and biopsied. Histological and immunohistochemical stains(CD3, CD5, CD20, CD21, CK) findings were used to confirm the lesion. Bone marrow biopsy was done and no bone marrow involvement was found. She did not receive chemotherapy and radiotherapy after surgery. No recurrence has been noted in the 22 months to date.

Idiopathic Postganglionic Horner's Syndrome in Dogs : 3 cases (개의 특발성 절후성 Horner's syndrome 3례)

  • Horner's syndrome is a group of clinical signs that results from interference with the sympathetic innervation of the globe and adnexa. Three dogs were presented with unilateral ptosis, miosis, enophthalmos and protrusion of the third eyelid. There were no other clinical signs on physical and neurological examination. On ophthalmic examination, the symptomatic eyes were diagnosed as Horner's syndrome. In order to localize the site of the lesion, pharmacological testing was performed through assessment of ocular response to the topical administration of 10% phenylephrine and clinical signs were resolved within 20 minutes. The pharmacological testing suggested that the deficit could be at the postganglionic neuron. Total resolution of clinical signs was observed within 6 months after their initial appearance without any treatment.

  • This study conducted a chi-square test and a logistic regression analysis by not only using a investigation data of discharged patients with damages from 2004 to 2006, but also using a census and a research data on the actual condition from health care system. The result follows; First, the centralization of medical utilization of operation patients is becoming apparent, centrally the capital area. To improve this, a policy that can not only provide medical centers and sickbed, but also improve the quality of local medical treatments for the localization of medical treatments. Second, propelling localization policy of certain diseases for the localization of medical utilization is needed as the rate of non-residential of operation patients that have diseases of the eye and adnexa, or cancers is high. Third, a localization policy for patients with damages is needed as the rate of treatments in other regions of operation patients with industrial accidents is increasing day by day.
