Anti-Biofilm Effect of Egg Yolk Phosvitin by Inhibition of Biomass Production and Adherence Activity against Streptococcus mutans

  • The formation of biofilms on the enamel surface of teeth by Streptococcus mutans is an important step in dental plaque formation, demineralization, and early caries because the biofilm is where other bacteria involved in dental caries attach, grow, and proliferate. The objectives of this study were to determine the effect of phosvitin (PSV) on the biofilm formation, exopolysaccharides (EPS) production, adherence activity of S. mutans, and the expression of genes related to the compounds essential for biofilm formation (quorum-sensing inducers and components of biofilm matrix) by S. mutans. PSV significantly reduced the biofilm-forming activity of S. mutans and increased the degradation of preformed biofilms by S. mutans. PSV inhibited the adherence activity of S. mutans by 31.9%-33.6%, and the production of EPS by 62%-65% depending upon the strains and the amount of PSV added. The expressions of genes regulating the production of EPS and the quorum-sensing-inducers (gtfA, gtfD, ftf, relA, vicR, brpA, and comDE) in all S. mutans strains were down-regulated by PSV, but gtfB was down-regulated only in S. mutans KCTC 5316. Therefore, the anti-biofilm-forming activity of PSV was accomplished through the inhibition of biofilm formation, adherence activity, and the production of quorum-sensing inducers and EPS by S. mutans.

한방병원에서 입원치료를 시행한 중증 아토피피부염 환자의 예후지표 탐색 (Prognostic Factors of Inpatients with Severe Atopic Dermatitis Treated in a Traditional Korean Medicine Hospital)

  • Objectives : This study was aimed to find significant prognostic factor of inpatients with severe atopic dermatitis (AD) who treated in a Traditional Korean Medicine (TKM) hospital. Methods : A retrospective review of medical records of 39 patients with severe AD who hospitalized for more than 7days was performed. All patients were treated with acupuncture, herbal decoction and herbal wet wrap dressing. Therapeutic effect was assessed by difference of objective SCORing of Atopic Dermatitis index (${\Delta}OSI$) at admission and at discharge. Patients were divided into two groups: improvement group (${\Delta}OSI{\leq}5$) and no-improvement group (${\Delta}OSI>5$), and clinical characteristics, severity, infectious complication, serum total IgE, eosinophil counts, adherence and concomitant treatment were investigated. Results : There were 7 cases in no-improvement group. There was no significant difference in OSI or infectious complication between the two groups. Also, there was no significant difference in total serum IgE and eosinophil counts, which are known to be related to severity of AD. However, the proportion of adherence group was significantly lower in no-improvement group than improvement group. Conclusions : This study suggest that treatment adherence is a significant prognostic factor for treatment outcome in inpatients with severe atopic dermatitis. If the adherence to treatment is improved, the therapeutic effect of hospitalization would increase in spite of severe AD or infectious complication.

  • Purpose: The purpose of this study was to evaluate the effects of periodic reminding interventions on medication adherence, self-efficacy, and pain intensity for home-based lung cancer patients. Methods: A quasi-experimental nonequivalent control group, pretest and posttest design was used. The intervention comprised of individual education by tailored image-combined medication instructions, daily reminding text message, and weekly telephone calls for four weeks. The subjects in this study consisted of 62 lung cancer patients (31 in the experimental group, and 31 in the control group). Mann-Whitney U-test was applied to analyze the data. Results: Experimental group who received periodic reminding intervention program better adhered to prescribed medication compared to the control group (z=-6.14, p<.001). Experimental group demonstrated higher level of self-efficacy compared to the control group (z=-6.74, p<.001). Experimental group experienced less intense average pain compared to the control group (z=-6.29, p<.001). Conclusion: The findings suggest that periodic reminding interventions can be applied as an effective nursing intervention to promote medication adherence and self-efficacy to improve and pain management for home-based lung cancer patients.

성정(性情)에 의한 표리병증(表裏病證)의 발생기전에 관한 고찰(考察) (Study for the mechanism of exteriror, interior disease by the nature and emotion)

  • 1. Objectives This paper is written for clear understanding on the concepts of 'Deep adherence of the Nature' and 'Sudden explosion of the Emotion' and for intensive comprehension of Mechanism of 'The Exterior Disease' and 'The Interior Disease' by The Nature and the Emotion. 2. Methods The study mainly based on "The exterior disease injured by the Nature, the interior disease injured by the Emotion" and deals with 'Seongmyungron, Sadanron, Hwarkchungron and Jangburon'. 3. Results and conclusions The conclusions as follows (1) Exterior Disease in each constitution breaks out by deep adherence of the Nature. Interior Disease brings out with sudden explosion of the Emotion. (2) 'Deep adherence of the Nature' is the state that the Ear, Eye, Nose, Mouth taking care of environmental frames can't work enoughly because of wrong minds on the Jaw, Breast, Umbilicus, Abdment. 'Sudden explosion of the Emotion' is the state that the Lung, Spleen, Liver, Kidney doing human affairs doesn't do appropriately for each constitution due to negative minds on the Head, Shoulder, Waist, Hip. (3) The result that the Ear, Eye, Nose, Mouth doesn't extract the clear energy from 'Jin, Go, You, Yaek sea' so they are lack of 'Shin, Qi, Hyul, Jeong' causes exterior disease in each constitution. (4) In each constitution, Interior disease results from that the Lung, Spleen, Liver, Kidney fails to bring on the clear juice from 'Lee, Mak, Hyul, Jeong sea', so shortage of 'Jin, go, You, Yaek' results in it.

손위생 교육프로그램이 간호대학생의 손위생 지식, 손위생 인식, 비강 내 황색포도알균 집락 및 손위생 이행에 미치는 효과 (The Effect of the Hand Hygiene Education Program on Hand Hygiene Knowledge, Hand Hygiene Perception, Nasal Staphylococcus aureus Colonization and Hand Hygiene Adherence in Nursing Students)

  • Purpose: This study was conducted to test the effect of the Hand Hygiene Education Program on hand hygiene knowledge, hand hygiene perception, nasal Staphylococcus aureus colonization and hand hygiene adherence in nursing students. Methods: A non-equivalent pre-post test of quasi-experimental design was used. 87 second grade nursing students participated in the study with 43 in the experimental group and 44 in the control group. We used the Hand Hygiene Education Program which was held 5 times over 5 weeks, taking 60 minutes per session. For the analysis, descriptive statistics, chi test, and t-test were used for statical analysis with SPSS 19.0. Results: There were significant increases in hand hygiene knowledge (p=.004) and hand hygiene adherence (p=.002) and there was a significant decrease in nasal Staphylococcus aureus colonization (p=.026) in the experimental group compared to the control group. However, hand hygiene perception (p=.543) was not significantly changed. Conclusion: Findings of this study suggest that the Hand Hygiene Education Program may be effective in enhancing hand hygiene knowledge and hand hygiene adherence. Also this program was effective in reducing nasal Staphylococcus aureus colonization in nursing students. Further studies are needed to evaluate the effects of the Hand Hygiene Education Program on hand hygiene perception in nursing students.

Role of eptC in Biofilm Formation by Campylobacter jejuni NCTC11168 on Polystyrene and Glass Surfaces

  • The complex roles of cell surface modification in the biofilm formation of Campylobacter jejuni, a major cause of worldwide foodborne diarrheal disease, are poorly understood. In a screen of mutants from random transposon mutagenesis, an insertional mutation in the eptC gene (cj0256) resulted in a significant decrease in C. jejuni NCTC11168 biofilm formation (<20%) on major food contact surfaces, such as polystyrene and borosilicate glass, when compared with wild-type cells (p < 0.05). In C. jejuni strain 81-176, the protein encoded by eptC modified cell surface structures, such as lipid A, the inner core of lipooligosaccharide, and the flagellar rod protein (FlgG), by attaching phosphoethanolamine. To assess the role of eptC in C. jejuni NCTC11168, adherence and motility tests were performed. In adhesion assays with glass surfaces, the eptC mutant exhibited a $0.77log\;CFU/cm^2$ decrease in adherence compared with wild-type cells during the initial 2 h of the assay (p < 0.05). These results support the hypothesis that the modification of cell surface structures by eptC affects the initial adherence in biofilm formation of C. jejuni NCTC11168. In motility tests, the eptC mutant demonstrated reduced motility when compared with wild-type cells, but wild-type cells with the transposon inserted in a gene irrelevant to biofilm formation (cj1111c) also exhibited decreased motility to a similar extent as the eptC mutant. This suggests that although eptC affects motility, it does not significantly affect biofilm formation. This study demonstrates that eptC is essential for initial adherence, and plays a significant role in the biofilm formation of C. jejuni NCTC11168.

  • 폐홉충(Paragonimus westermani) 초기 감염시의 숙주 면역 기전을 알아보기 위하여 폐흡충 피낭유충을 백서에 감염시켜 주별(주별)로 항혈청을 분리하고 등일 백서에서 복강 대식세포(homologous rat peritoneal macrophage)를 얻은 다음, 이들과 폐흡충 탈낭유충 및 숙주 조직 이행중인 유약충으로 effector system을 조작하여 폐흡충 유충에 대한 항체 의존성 대식세포 공격기전을 관찰하였다. 백서 복강 대식세포는 항혈청 출현 시에만 폐흡충 탈낭유충에 대한 세포부착 반응(cell adherence reaction)을 보였고 이 반응에는 보체가 관여하지 않았다. 폐흡충 감염 1주에서 8주까지의 백서 항혈청 중 2주째에 분리한 항쳔청 실험군에서 100%의 세포부착 반응을 나타내었고 사멸된 충체를 관찰할 수 있었으며, 세포부착 반응은 탈낭 유충기에서만 나타나는 반면 조직이행 유약충에서는 전혀 그 반응을 인정할 수 얼어 폐흡충 감염에 따른 항체의존성 숙주 세포 매개독성(antibodydependent cell·mediated cytotoxicity)은 발육단계 별 특이성을 보였다. 한편, 대식세포의 공격에 의하여 사멸된 폐흡충 탈낭유충의 형태학적 특징은 대식세포와 유충 사이에 형성되는 fuzzy material, tegumental syncytium 내의 미세구조의 변성 및 syncytium과 근육층을 연결하는 tubular tunnel의 형성 등이었다.

Comparison of five international indices of adherence to the Mediterranean diet among healthy adults: similarities and differences

  • BACKGROUND/OBJECTIVES: To compare five indices of adherence to the Mediterranean Diet (MD) among adults living in the Mediterranean region. SUBJECTS/METHODS: A total of 100 healthy Lebanese adults aged between 18 and 65 years. Face-to-face interviews to collect sociodemographic and medical information, to take anthropometric measurements, and to fill a validated, culturally adapted, food frequency questionnaire (FFQ). The score for each item was calculated following the recommendations for each corresponding index. The five MD indices were Mediterranean Diet Scale (MDScale), Mediterranean Food Pattern (MFP), MD Score (MDS), Short Mediterranean Diet Questionnaire (SMDQ), and the MedDiet score. RESULTS: Significant correlations were detected between items with P-values < 0.001. Minimal agreement was seen between MDScale and MedDiet score and maximal agreement between MDS and MedDiet score. Univariate and multivariate analyses showed that MDS and MedDiet scores had significant correlations with fiber and olive oil intake, main components of the MD. MDScale showed a significant correlation with waist-to-hip ratio and with total energy intake but none of the five indices was correlated to body mass index (BMI). CONCLUSIONS: The indices that showed the highest correlation with variables related to the MD are the MDScale and the MedDiet score; therefore, they can be used to assess our future study populations. Based on the current results, more than half of the study population was non-adherent to the MD and adherence to this diet did not appear to protect against being overweight ($BMI{\geq}30$).

비만 진료 프로그램에 대한 수용성 태도 분석 (Review on Acceptability of Patients towards Obesity Treatment Program)

  • Objectives: Acceptability of patients towards obesity treatment program is associated with better weight loss outcomes. The purpose of this study was to review previously published study results of the predictive factors associated with patients' acceptability in obesity treatment. Methods: Authors searched for the articles related to acceptability reported as continuation, attendance and adherence, published from 2011 to 2018 found on Pubmed, Scopus, Research Information Sharing Service, and Koreanstudies Information Service System. A total of 23 articles were finally selected. From the study results, unchangeable and changeable predictors were extracted, and these predictors were examined according to detail categories. Results: Regarding the continuation of the treatment, unchangeable factors such as younger age, lower educational level, male sexuality and lower accessibility to physical activity predicted lower continuation. Furthermore, changeable factors such as early and half weight loss, better accessibility to the treatment and financial incentives for retention predicted higher continuation rate. Greater degree of attendance was predicted by unchangeable factors such as economical affordability, and changeable predictors such as half weight loss and proximity to the clinic. Main factors of adherence to the recommendation were unchangeable predictors such as weight loss experience, and changeable predictors such as more physical activity and appropriate dietary habits. Negative psychological state predicted lower continuation, attendance and adherence rate. Conclusions: Our review results suggest that unchangeable and changeable predictors of acceptability of patients should be carefully examined during treatments of obesity.

Large Variation in Clinical Practice amongst Pediatricians in Treating Children with Recurrent Abdominal Pain

  • Purpose: To evaluate intra- and inter-observer variability and guideline adherence amongst pediatricians in treating children aged between 4 and 18 years referred with recurrent abdominal pain (RAP) without red flags. Methods: The first part of the study is a retrospective single-center cohort study. The diagnostic work-ups of eight pediatricians were compared to the national guidelines. Intra- and inter-observer variability were examined by Cramer's V test. Intra-observer variability was defined as the amount of variation within a pediatrician and inter-observer variability as the amount of variation between pediatricians in the application of diagnostic work-up in children with RAP. Prospectively, the same pediatricians were requested to provide a report on their management strategy with a fictitious case to prove similarities in retrospective diagnostic work-up. Results: A total of 10 patients per pediatrician were analyzed. Retrospectively, a (very) weak association between pediatricians' diagnostic work-ups was found (0.22), which implies high inter-observer variability. The association between intra-observer diagnostic was moderate (range, 0.35-0.46). The Cramer's V of 0.60 in diagnostic work-up between pediatricians in the fictitious case implied the presence of a moderately strong association and lower inter-observer variability than in the retrospective study. Adherence to the guideline was 66.8%. Conclusion: We found a high intra- and inter-observer variability and moderate guideline adherence in daily clinical practice amongst pediatricians in treating children with RAP in a teaching hospital.
