Risk Factors for Recurrent Abdominal Pain in Children with Nonorganic Acute Abdominal Pain

  • Purpose: The purpose of this study was to identify the risk factors for recurrent abdominal pain (RAP) in children who presented with nonorganic acute abdominal pain. Methods: A retrospective, single study was conducted on 2-15-year-old children diagnosed with nonorganic acute abdominal pain at the pediatric outpatient department of Vajira Hospital, Nawamindradhiraj University, between January 2015 and December 2019. The potential risk factors were analyzed using univariate and multivariate analyses. Results: Of the 367 patients with nonorganic acute abdominal pain, 94 (25.6%) experienced RAP within three months. In this group with RAP, 76 patients (80.8%) were diagnosed with functional gastrointestinal disorders, including functional dyspepsia, irritable bowel syndrome, functional abdominal pain-not otherwise specified, and functional constipation. History of gastrointestinal infection (p=0.011), mental health problems (p=0.022), abdominal pain lasting ≥7 days (p<0.001), and change in stool frequency (p=0.001) were the independent risk factors associated with RAP in children with nonorganic acute abdominal pain; their odds ratios and 95% confidence intervals were 3.364 (1.314-8.162), 3.052 (1.172-7.949), 3.706 (1.847-7.435), and 2.649 (1.477-4.750), respectively. Conclusion: RAP is a common problem among children who first present with nonorganic acute abdominal pain. The identification of risk factors may provide proper management, especially follow-up plans for this group in the future.

Effects of Sweet Bee Venom Pharmacopuncture Combined with Korean Medicine Treatment for Acute Low Back Pain Syndrome Patient: A Case Report (급성 요통 증후군 환자에 대해 봉독약침요법을 병행한 한의학적 치료 효과: 증례보고)

  • Objectives : Acute low back pain syndrome causes pain and poor quality of life. There are various studies of SBV (Sweet bee venom) pharmacopuncture, but few have identified the therapeutic effects for patients unable to walk due to acute back pain. This case series report three cases of acute low back pain syndrome treated with SBV pharmacopuncture combined with Korean Medicine (KM) treatments. Methods : Three acute low back pain syndrome patients with no other acute abnormalities in the imaging were treated by KM treatments including SBV pharmacopuncture, acupuncture, cupping, herbal medicine and physical therapy. The improvement of symptoms was evaluated using Numerical rating scale (NRS), Oswestry disability Index (ODI), EuroQol-5 dimension index (EQ-5D) and EuroQol-visual analogue scale (EQ-VAS). Results : Patients who were unable to walk due to acute back pain were able to walk. There were significant improvements of NRS, ODI, EQ-5D and EQ-VAS after treatment. The patients experienced an average 90% decrease in low back pain after inpatient KM treatment for about 8 days. NRS decreased by an average of 78.9%, ODI decreased by an average of 49.4%, and EQ-VAS increased by an average of 92.6%. In the case 1, EQ-5D decreased in all categories, but in the case 2, it decreased in all categories except for pain/discomfort and anxiety/depression, but in the case 3, it decreased in all categories except anxiety/depression. There was no serious side effect after treatment of SBV pharmacopuncture. Conclusions : KM treatments including SBV pharmacopuncture may be effective for acute low back pain syndrome patients. However, randomized controlled trials are needed in the future to confirm the clinical effects of these interventions.

The Effect of the Treatment with Venesection on Treating Low Back Pain of Acute Stage (급성기(急性期) 요통(腰痛)의 치료(治療)에 자락요법(刺絡療法)의 병행(竝行)이 미치는 영향(影響))

  • Objective : This study is designed in order to evaluate the therapeutic effect of venesection on acute low back pain. Methods : From Nov. 1st 2003 to Apr. 30th 2004, 40 cases of low back pain patients at acute stage were divided into 2 groups; one group(control group) took only acupuncture therapy, and the other group(test group) took acupuncture and venesection therapy. Pain rating score(PRS) was used to assess the pain for its intensity, frequency, duration, and aggravating factors. And visual analog scale(VAS) was used, too. PRS and VAS was checked twice, before the treatment and after the treatment. Treatment was done twice a day. Results : Both acupuncture therapy and acupuncture with venesection therapy showed good effect on low back pain of acute stage. And the latter group showed better effect on decreasing pain than former. It was proved by the difference between PRS and VAS checked before treatment and what checked after treatment. But it was not proved statistically. Conclusion : Only acupuncture and acupuncture with venesection can be recommended as a useful therapy to treat low back pain of acute stage.

Combination Treatment of Korean Medicine with Chungpochukeo-tang for Acute Pelvic Pain by Ruptured Ovarian Cyst : 2 Cases Report (난소낭종 파열 후 발생한 급성 골반통에 대해 청포축어탕 가감방을 포함하는 한의치료 경과 : 증례보고)

  • Objectives: This case report aims to report the clinical effectiveness of the combination treatment of Korean medicine on acute pelvic pain followed by ruptured ovarian cyst. Methods: The patients who diagnosed with ruptured ovarian cyst complained of acute pelvic pain, lower abdominal pain, abdomen distention, constipation, lower back pain and dysuria. They received combination treatment of Korean medicine during hospitalization. The treatment included Chungpochukeo-tang, acupuncture, moxibustion, and pharmacoacupuncture. The effects were evaluated through Numeric Rating Scale (NRS). Results: After the treatment, the clincial symptoms such as acute pelvic pain, lower abdominal pain, abdomen distention, constipation, lower back pain and dysuria were improved. Conclusions: This case report shows that the combination treatment of Korean medicine with Chungpochukeo-tang may be effective for treating acute pelvic pain by ruptured ovarian cyst.

  • Granulocytic sarcoma (chloroma) is a rare, solid tumor of myelogenous stem cells, shows usually in patients with acute myelogenous leukemia and less commonly in patients with chronic myelogenous leukemia or myeloproliferative disorders. We report here a patient presenting acute paraparesis due to spinal epidural granulocytic sarcoma causing spinal cord compression in acute myelogenous leukemia with a brief review of literature.

Acute Malocclusion Caused by Articular Disc Perforation: A Case Report

  • Patients with temporomandibular disorder often present with acute occlusal change and properly managed with conservative treatment. If such change is caused by unusual etiology, differential diagnosis may be challenged. This article describes the diagnosis of a patient exhibiting pain and acute posterior open bite on the ipsilateral side after chewing hard food. After initial conservative treatment failed to resolve the complaint, magnetic resonance imaging was ordered and confirmed partial perforation of articular disc. Disc perforation itself is usually chronic in nature, but sudden macrotrauma may also cause the disorder. However, occlusal discrepancy caused by disc perforation is rare and seldom reported. We present a case of acute malocclusion caused by disc perforation with a review of related literature.

Pain, Disability, Emotional Status and Educational Needs between Acute and Chronic Low Back Pain Groups (급·만성 요통 환자군의 통증, 기능장애, 정서 상태 및 교육 요구)

  • Purpose: This study aimed to identify pain, disability, anxiety, depression and educational needs between acute and chronic low back pain groups. Methods: A total of 153 patients aged 18 to 64, recruited from S-neurosurgical clinic for low back pain in Gyeonggi-do. Out of 153 subjects, 70 were Acute Low Back Pain (ALBP) group and 83 were Chronic Low Back Pain (CLBP) group. The collected data was analyzed using the SAS System V 9.4 program by chi-square test/Fisher's exact test and t-test. Results: The pain and disability scores were higher in ALBP group while the depression score was higher in CLBP group. The educational needs score in the area for the time for lumbar operation was higher in CLBP group. In the Low Back Pain (LBP) treatment management, ALBP group visited clinic (60.0%) most frequently and CLBP group visited both clinic & traditional medicine (66.3%) regularly. Conclusion: In order to minimize the progression from acute to chronic LBP, it is necessary for patients who visited the clinic to be accompanied with an education program that reflects educational needs of patients and with proven alternative therapy.

  • Neurolytic splanchnic nerve block is a relatively safe and effective method for the relief of intractable pain caused by upper abdominal cancer. We have experienced a case of severe acute respiratory failure during splanchnic nerve block under control of X-ray fluoroscopy. We think that the most likely cause of the acute respiratory failure was an asthmatic attack due to anxiety and dyspnea from the injury or stimulation of the diaphragm and pleura in this case.

The Study on History Period and Treatment Outcomes of Patients Admitted to a Korean Medicine Hospital for Low Back Pain (요통으로 한방병원에 입원한 환자의 병력기간별 호전도에 관한 연구)

  • Objectives Low back pain causes disability and incurs large socioeconomic burden. Many cases of acute low back pain progress into chronic conditions. As the treatment period extends, burden of socioeconomic expenses also rises. Thus this study is to investigate whether history period influences improvement rate of pain and disability in the patients who receive conservative Korean medicine treatment. Methods 447 inpatients who received conservative Korean Medicine treatment for the management of low back pain have been grouped into acute, sub-acute and chronic according to the length of history period. NRS, ODI and improvement rate of each group were investigated. Treatment every patient has received as follow; acupuncture, herbal medicine, Chuna manipulation, bee venom, and physical treatment. Results Acute patients showed improvement with NRS of $3.98{\pm}2.50$ and ODI of $28.04{\pm}21.54$. The NRS of sub-acute patients showed improvement with NRS of $2.81{\pm}2.07$, ODI also showed improvement with $11.64{\pm}13.94$. Statistically significant difference was observed in NRS, ODI and improvement for acute patients. Improvement in pain and disability was observed to be larger. Conclusions It demonstrated that receiving valid conservative Korean medicine treatment is beneficial for low back pain patients in improvement of their pain and disability.

  • Objectives : The purpose of this study was to evaluate the effectiveness of Calculus Bovis Fel Ursi Moschus(BUM) pharmacopuncture on the treatment of acute low back pain patients. Methods : The subjects in this study were 30 patients with acute low back pain who were treated in Dept. of Acupuncture and Moxibustion, Jein Oriental Medicine Hospital from November 1, 2010 to June 30, 2011. These patients were randomly divided into two groups. In one group(control group), patients were treated with acupuncture. And in the other group(experimental group), patients were treated with both acupuncture and BUM pharmacopuncture. For evaluating change of pain, visual analog scale(VAS) and Oswestry disability index(ODI) were checked before and after. Results : Each group had significant decrease in VAS and ODI after each treatment. Experimental group had more significant decrease in VAS after each treatment than control group. Conclusions : We suggest that cotreatment of acupuncture and BUM pharmacopuncture on acute low back pain patients can be recommended as a useful therapy.
