Investigation on the Relationship in Acute Lateral Tortipelvis and Breaks in Low Back Pain Patient (요통 환자에서 급성 측방 골반 뒤틀림과 Breaks와 상관관계 분석)

  • Objectives : To investigate the relationship in acute lateral tortipelvis and breaks in low back pain patients with acute lateral tortipelvis were discussed. Methods : The subject of this study consisted of 43 patients who were diagnosed as acute lateral tortipelvis by X-ray and 23 patients who were diagnosed as none tortipelvis by X-ray. We investigated acute lateral tortipelvis and breaks on X-ray film. Results and Conclusions : In the distribution of acute lateral tortipelvis, L4 was 17 cases(39.5%), L3 was 14 cases(32.6%), L2 was 9 cases(18.6%), L1 was 4 cases(9.3%) in the sequence. In acute lateral tortipelvis group, the number of breaks was more than none tortipelvis group(p>0.05). The most common parity of breaks was L4-5 in low back pain patient. Between distribution of lumbar level of acute lateral tortipelvis and breaks, there was no significant relationship.

The Effects of Sacroiliac Joints Taping Therapy on Low Back Pain (천장관절 테이핑요법이 요통에 미치는 영향)

  • Purpose: This study was to figure out the effect of sacroiliac joints taping therapy on low back pain. Methods: 35 patients who visited to receive the treatments of the physical therapy in H orthopedic clinic in Kyoung-buk had been treated with spiral balance taping for 4weeks from 3 to 29, July 2006. They were divided 3 groups: acute, subacute, chronic. VAS(Visual Analogue Sacle) was analyzed for pain scale, and compared between the pre and post test among groups. Results: The acute patients' degree of the pain post therapy was decreased, the subacute patients' degree of the pain post therapy was decreased and the chronic patients' degree of the pain post therapy was decreased. There were statistically remarkable differences(p<0.05) in 3 groups. Conclusion: This results suggest that sacroiliac joints taping therapy to correct leg length inequality was effective on low back pain.

Comparison the Soyeom Pharmacopuncture Therapy Effects of Ultrasound Guided Group and Unguided Group on the Patients' Facet Joint with Acute Low Back Pain Caused by Traffic Accidents: A Retrospective Study (교통사고로 유발된 급성 요통 환자의 후관절 소염약침 치료에 대한 초음파 유도하군과 비유도군의 효과 비교: 후향적 연구)

  • Objectives This study aims to compare the effect of an ultrasound on a guided group and an unguided group on Soyeom pharmacopuncture therapy on the facet joint in patients with acute low back pain caused by traffic accidents. Methods 21 patients with acute low back pain caused by traffic accidents from March 1, 2021 to May 31, 2021 were included in this study. The study was conducted as a retrospective study which analyzes the patient's medical records. 11 patients (Group A) received ultrasound guided Soyeom pharmacopuncture therapy and 10 patients (Group B) received unguided Soyeom pharmacopuncture therapy on the facet joint. Visual analogue scale (VAS) and Oswestry disability index (ODI) was used to evaluate improvements in functions and pain, and five point Likert scale to evaluate patient's satisfaction. Results Both groups showed a statistically significant decrease in the VAS and ODI on the 5th day of hospitalization. However, there was no statistically significant difference between the groups. Difference in the Likert scale between the groups was not statistically significant either. Conclusions We found that ultrasound guided Soyeom pharmacopuncture therapy on the facet joint showed similar efficacy compared with unguided Soyeom pharmacopuncture therapy at facet joint on acute low back pain patients caused by traffic accidents.

The Effect of Lumbar Medial Branch Block on Low Back Pain

  • Objective : The goal of this study was to establish the benefit and prognostic factors of lumbar medial branch block[MBB] for low back pain. Methods : A retrospective analysis was based on the data obtained from 281 patients with low back pain, who visited our hospital between January 2001 and November 2004. Pain relief was evaluated at 2 weeks, 1 month and 3 months. The authors analyzed the results of MBB according to the patient's age, sex, symptom duration, pathologic condition, and presence of radiating pain. Results : Two hundred eighty one patients had sprain [151]. lumbar fracture [27], spinal stenosis [50], herniated lumbar disc [24] acute post-operative pain [8], and chronic post-operative pain [21] with success rate of 63.6%, 59.3%, 26.0%, 25.0%, 87.5% and 42.9%, respectively. The effects of MBB in sprain, lumbar fracture, and acute post-operative pain were significantly better than those in stenosis, herniated lumbar disc and chronic post-operative pain patients. The patients in young age group [<60 years], with short symptom duration [<6 months] and without radiating pain showed good response to lumbar MBB. Conclusion : The lumbar MBB appears to be safe and effective for low back pain in certain selected patients. Good prognostic factors were low back pain without surgical conditions and radiating pain, with short symptom duration [<6 months], and in relatively young age [<60 years] group.

Comparison of the Relationship Between Impairment, Disability and Psychological Factors According to the Difference of Duration of Low Back Pain (요통기간에 따른 손상, 장애, 심리적 요인들의 상관성 비교)

  • The purpose of this study was to investigate the correlations between pain intensity, physical impairments, disability, and psychological factors according to the difference in duration of low back pain. This study was a cross-sectional survey of 102 participants with low back pain, divided into two groups equal in number: The first group consisted of patients with acute and subacute low back pain, while the second group consisted of patients suffering from chronic low back pain. The results showed that gender, age, pain intensity, physical impairment, disability and Fear-Avoidance Beliefs (FABs) for work activities were not significantly different between two groups. FABs for physical activities of the first group were significantly more prevalent than in the second group. More than moderate correlations were found between pain intensity, physical impairment, and disability in the first group. Less than moderate correlations were found between pain intensity, physical impairment, disability, FABs, and depression in the second group. These findings suggest that we must consider psychological factors in the treatment of patients with chronic low back pain. Regression analyses revealed that pain intensity and FABs for work activities significantly contributed to the prediction of disability in the first group. Also, pain intensity and FABs for physical activities significantly contributed to the prediction of disability in the second group. Pain intensity was most important predictor of disability in two groups.

  • Acute intraabdominal aortic obstruction ends in progressive fatal course or severe permanent disability unless it is diagnosed and treated promptly. However, the incidence of such disease is very rare, so there is very little chance for a neurosurgeon to encounter a patient with acute intraabdominal aortic obstruction. The authors present a case of 62-year-old man with severe low back pain and acute paraplegia caused by acute intraabdominal aortic obstruction.

Relationships among Nursing Activities, the Use of Body Mechanics, and Job Stress in Nurses with Low Back Pain (종합병원에 근무하는 요통 간호사의 간호업무활동, 신체역학원리 및 직무 스트레스와의 관련성)

  • Purpose: This study was conducted to identify relationships among nursing activities, the use of body mechanics, and job stress in nurses with low back pain. Methods: Participants were 225 nurses with low back pain working at an university hospital. The data were collected with self-reported questionnaires at March, 2012 and analyzed by descriptive statistics, t test, ANOVA, Scheff$\acute{e}$e test and multiple regression analyses. Results: There was significantly a higher degree of pain in subjects who working at special units including intensive care units and emergency room than at general wards. Low back pain was negatively correlated with the use of body mechanical principles while it was positively associated with the degree of job stress. According to results of multiple regression analyses, low back pain was significantly associated with the use of body mechanics and job stress in nurses working at general ward and special units including intensive care units and emergency room. Conclusion: The use of the principles of body mechanics and reducing job stress are important to prevent low back pain in nurses. It is necessary to develop and apply stress management and education program about the use of the principles of body mechanics.

Therapeutic Exercise for Low Back Pain (요부통증에 대한 운동치료)

  • Even though back pain therapy has greatly improved as spinal bio-mechanics is introduced, many patients still have difficulties due to low back pain. At the initial therapeutic stage, the aim of rehabilitation therapy for low back pain is pain control, but, at the later therapeutic stage, the prime aims are to reduce the late complication and to prevent the recurrence of low back pain. Accurate diagnosis should be a first step before any therapy is planned. Thus, accurate physical, neurologic, E.M.G. and radiologic tests are required to give prescription for therapeutic exercise to the patients. In addition to this, the roles of theraphists and therapeutic exercise should be re-evaluated after the therapeutic exercise is performed. Fist of all, the most important things are to educate the patients to understand the low back pain and to let the patients join the therapeutical planning. 1. Bed rest and muscle relaxing exercise for releasing the muscle tention are required for the treatment of acute low back pain. An active exercise is recommended rather than a passive exercise. If the therapeutic exercise depravate the low back pain, the exercise should be immediately terminated and the therapeutical exercise should be replanned. 2. For the treatment of the chronic back pain, stretching exercise and para-spinal muscle strengthening exercise should be performed steadily and actively to prevent the recurrence of low back pain and the low back injury due to minor damage. The patients should be educated to do proper exercise and to maintain good posture in everyday life. 3. As the low back pain is released and the body function is recovered, control of whole body function is necessary. Swiming, bicycling and walking for $30\sim40$ minutes a day and $3\sim4$ days a week are recommended. Other exercise could be recommended depending on the patients condition.

  • Chronic low back pain (CLBP) is a distinct disease entity in that its clinical course and response to the treatment are quite different from acute low back pain (ALBP). CLBP is also closely related with systemic or preexisting psychosocial factors rather than focal or localized factors. Since there has been little consensus in practice regarding the proper management of CLBP, clinicians tend to approach and manage patients with CLBP in an empirical manner rather than an evidence-based one. In this article, I will review the difficulties of therapeutic choice in CLBP and provide superior aspects of medical treatment over invasive interventional treatments.

Effect of Exercise Programs for Chronic Low Back Pain Patients : A Systematic Review (만성 허리통증환자의 운동프로그램에 대한 효과)

  • Background: The purpose of this study was to systematic review the effect of exercise programs for chronic low back pain patients. We needs systematic development of low back pain exercise program to reduce economic cost further doing great service to public health promotion. Methods: We searched to the effects of exercise programs for chronic low back pain patients by Dankook University electronic library databases of DBPIA, KSI KISS, CINAHL, MEDLINE and PEDro combined with a hand search of papers published in relevant journals. Any type of study relevant to the topic published during time period from 1970 to 2007 was included. Results: The literature search identified 30 studies. 1.Performing the flexion exercise increased abdominal muscle activity but acute herniated intervertebral disc should be avoided. 2.The general lumbar extension exercise used lumbar extension machine and the muscle power increases, significant probability the change. 3.The spinal segments exercise for the patients offered significant efficacy and appeared to be a reasonable therapeutic option. 4.Spinal stabilization exercises appear to improve trunk endurance and balance to patients with chronic low back pain. This exercise programs had effective decrease pain and disability. Conclusion: The review suggests that although the exercise programs for chronic low back pain patients, and we expected the efficacy of the exercise programs for chronic low back pain patients used in this study should be further investigated in a long period study and objective outcomes.
