Analysis of Patients with Acute Industrial Toxic Exposure at an Emergency Department in an Industrial Complex (공단밀집지역에 위치한 일개 응급의료센터에 내원한 급성산업중독환자에 대한 분석)

  • Purpose: Surveys on poisoning usually involves intoxication rather than inhalation, skin contact, etc. Therefore, we examined the characteristics of patients who visited the emergency department in an industrial complex after acute industrial exposure to toxic materials. Methods: Medical records of patients exposed to toxic materials in the work places from April, 2006, to March, 2008, were analyzed retrospectively. Inhalation patients due to fire were excluded. Results: Subjects included 66 patients, with a mean age of $35.4{\pm}10.9$ years, mostly men (91%). Toxicity occurred in 51 patients (77%) by contact, 15 patients (23%) by inhalation, and none by oral ingestion. For toxic materials, 10 patients were exposed to hydrofluoric acid, 8 to hydrochloric acid, 7 to sodium hydroxide, 7 to metals, and others. The face and hands were the most frequent exposure site by contact. Most exposures were caused by accidents, with 29 cases (42%) exposed because of carelessness or not wearing protective equipment. Most complaints were pain on exposure site, but 7 of the inhalation patients complained of dyspnea. The majority of patients with contact exposure were discharged after wound care or observation. After inhalation exposure, 1 patient died and 5 patients were admitted to the intensive care unit. Conclusion: Major causes of workplace exposure were not wearing protective equipment or carelessness. Although contact exposures are usually benign, cautious observation and management are required in patients with inhalation exposure.

The Relationship between Pesticide Exposure and Central Nervous System Symptoms (농약 노출과 중추신경 증상과의 관련성)

  • Objectives: The acute toxic effects of pesticide are well known. Concern has also been expressed that long-term exposure may result in damage to the central nervous system. This study was conducted to test the hypothesis that central nervous system symptoms might occur due to pesticide exposure. Methods: In a cross-sectional study, first, cumulative exposure index (CEI) was estimated. Neurologic symptoms (Q-16 questionnaire) for 541 farmers (exposed to pesticides) were compared with 119 non-exposed persons in spraying season nine rural areas in Korea. Results: The pesticides poisoning rates for last 3 months were 67.2% for orchard farmers, 55.3%for dry field farmers, and 20.5% for husbandry farmers, respectively, showing significant difference (p<0.001). Compared with non-exposure group, exposure groups (especially, orchard farmers) reported significantly more neurologic symptoms and had a higher overall neurological symptoms score (p<0.001). Factors related to the positive neurological symptoms (answers "yes" to six or more of Q-16 questionnaire) adjusted for age, sex, education level, smoking and alcohol drinking were type of farming (OR 3.08, 95% CI 1.50-6.30 in orchard farmers vs non-exposure group), CEI (OR 2.75, 95% CI 1.12-6.78 in Q3 vs Q1), past poisoning (OR 1.97, 95% CI 1.21-3.20 vs normal), current mild poisoning (OR 3.03, 9500 CI 1.47-6.22 vs normal) and current moderate poisoning (OR 6.34, 95% CI 3.03-13.25 vs normal), respectively. Conclusions: These results suggest that long-term exposure to pesticides appears to be associated with subtle changes in the central nervous system.

A Fatal Case of Methylene Blue Threatment Failure in Methemoglobinemia (메틸렌블루에 반응하지 않는 메트헤모글로빈혈증 1례)

  • Acute toxic methemoglobinemia is an infrequent complication of the use of various drugs. Severe methemoglobinemia is very often fatal. Methylene blue is an effective drug in the treatment of methemoglobinemia patients. However, failure to respond to methylene blue has been described in patients with sulfhemoglobinemia, chlorate poisoning, and glucose-6-phosphate dehydrogenase deficiency. It is even possible that hemolysis may occur due to methylene blue treatment itself. We encountered a case of a 71-year-old woman who developed methemoglobinemia caused by alprazolam intoxication. She presented with hemolytic anemia and did not respond to methylene blue. In spite of concerted N-acetylcysteine therapy, the hemolytic anemia became aggravated and the patient died eleven days after intoxication.

  • For thestudying and utilizing the thorn sea cucumber synthetically, the method preparing polysaccharide and polypeptide from sea cucumber was studied. The comparatively rational craft for preparing crude polysaccharide is fresh raw materials deal with oar form, $60\%$ of the ethanol precipitate after hydrolyzed by pretense and vibrated with ultrasonic wave auxiliary. The purification of polysaccharide and removal of protein fromcrude polysaccharide were made through precipitation method using the acetate. The polypeptide is obtained by concentrating in vacuum, freeze-drying after mixing precipitations of two times. Polysaccharide acute poisoning experiment indicate there is no bad reaction when LD50>5000g/kg, there strain rate of liver tumor H11 approach present generally acknowledged tumor treat medicine cyclophosphamide, and there is no side effect. The fatigue resistance function of polypeptide experiment also indicates that the fatigue resistance ability of mouse which fed on added sea cucumber polypeptide has a great improvement.

  • The Sam Mool Injin extract is occasionally used as a kind of remedies for the treatment of hepatic discomfort in herb medicine. The study for the effect on the hepatic parenchymal cells, especially regenerating activity and even its toxicity have. little been done and only a very few pharmacological properties have been reported. So, an experimental study for the morphologic changes in the acute carbon tetrachloride poisoning of wister rat by administration of its extract has been performed. Male Wister rat weighing around 200gm were used in this experiment. Results were as follows; 1) In the $Ccl_4$ 0.3c.c 7day group, there is noted considerable increase of regenerating activity in the experimental group compared with non-extract treated group, in half of them. 2) Remaining all of the experimental group do not show significant difference of regenerating activity compared with control group. 3) In the group treated with Sam Mool Injin extract alone, there was noted some considerable increase of nonspecific leukocytic infiltrations compared with saline control group. This can be considered as a possibility of hepatic tissue reaction on the Sam Mool Injin extract.

  • The authors collected epidemiological and clinical informations on 1,307 CO patients from the questionairefilled by attendants of patients and from the clinical records kept in the hospital. All patients were treated by hyperbaric oxygenation at the Hyperbaric Chamber Unit of Seoul National University Hospital during 5 years, from January, 1969 to December, 1973. The following findings were obtained. 1. Female showed higher incidence rate than male. The highest incidence rate was observed in the age group of 15-29 years in both sex. 2. The most important variable influencing the recovery of the patients was the starting time of treatment. It was evident that earlier the starting time, shorter the recovery time. 3. Duration of admission was within 5 days in 57.1 percent of the admitted cases and 76.1 percent of them was discharged within 8 days. 4. As the complications, pulmonary edema, bronchopneumonia and trophic changes were observed. Also the neurological disorders were found. 5. The fatality rate followed by the hyperbaric oxygenation was 1.07 percent.

  • Methylene blue is a very effective reducer of drug-induced methemoglobinemia. It has dose-dependent oxidation or reduction properties. In most cases, a dose of 1 to 2 mg/kg IV given over 5 minutes and immediately followed by a 15- to 30-mL fluid flush to minimize the local pain is both effective and relatively safe. The onset of action is quite rapid, and the effects are usually seen within 30 minutes. The dose may be repeated after 30 to 60 minutes and then every 2 to 4 hours as needed. The total dose should not exceed 7 mg/kg as a single dose or 15 mg/kg within 24 hours. Repeated treatment may be needed for treating compounds that have prolonged elimination or those compounds that undergo enterohepatic recirculation (e.g., dapsone). Methylene blue can cause dose-related toxicity. At high doses, methylene blue can also induce an acute hemolytic anemia and rebound methemoglobinemia. The reasons for treatment failure with methylene blue include ineffective GI decontamination, the existence of other forms of hemoglobin (e.g., sulfhemoglobin), a low or high dose of methylene blue and the toxicokinetics of some agents, such as aniline, benzocaine or dapsone.

  • Incidents of poisoning caused by edible marine bivalve, Tapes philippinarum in Kojedo, Korea were reported in March 1968 and 1969. The results of the present investigation revealed that the incidents were caused by hemorrhagic diathesis and liver injuries, as those caused by acute yellow atrophic liver. The minimal lethal dose of the liver extract of the bivalve varied with seasons. The dose was 0.02ml in March and increased to 0.25ml in April-May 1969. After lune the extract showed no toxicity. Phthalein reaction of smashed liver solution of the bivalve showed a great variation with seasons. In March the values of $P_1$(crude solution) and $P_2$ (boiled one) were 0.1 and 10, at its strongest toxicity. In May those of $P_1\;and\;P_2$ were 1 and 1,000. However the mouse died after intraperitoneal administration of 0.25ml of the liver extract at the $P_2$ value of 1,000. For this reason, determination of the toxicity is difficult by judging from the phthalein reaction alone. The bivalves collected from Pusan, Kaduk, Koje, Hansan, Yeosu, Mokpo and Kunsan were found nontoxic during April-August 1969.

Postmortem Distribution of Methidathion in Human Specimens of a Acute Poisoning (Methidathion 중독사에 의한 사후혈액 및 조직중 분포)

  • Methidathion is one of the organophosphorus pesticides commonly used for stamping out harmful pests in farming areas. This paper presents a fatality due to methidathion intoxication and describes the distribution of methidathion in postmortem blood and tissues obtained at autopsy. Qualitative identification of methidathion was achieved by TLC, GC and GC/MS, and quantitative analysis was performed by GC with thermionic specific detector (TSD). The analytes in postmortem specimens were extracted by liquid-liquid extraction (LLE) with ethylether. After the ethylether layer was evaporated, the residue was partitioned into hexane and acetonitrile, and the acetonitrile layer was used for analysis. Tissue specimens were homogenized with 4% perchloric acid and applied for LLE. After extraction, the extracts were reconstituted 100 $\mu\textrm{g}$ pyraclofos (IS, 100 $\mu\textrm{g}$/ml in methanol) for GC and GC/MS analysis. On analysis of postmortem specimens, methidathion was identified and quantitated. The methidathion concentrations were 2.0 $\mu$l/ml in blood, 24.4 $\mu\textrm{g}$/g in liver, 13.9 $\mu\textrm{g}$/g in lung, 21.8 $\mu\textrm{g}$/g in kidney, respectively.

A Case of Intoxication of Ingested Formalin (포르말린에 의한 급성 중독 1례)

  • Formalin is a water-soluble, colorless, pungent, irritating and highly reactive gas. A 40% solution of formaldehyde in water, also known as formalin, is used as a disinfectant, antiseptic, deodorant, tissue fixative and embalming fluid. Ingestion can lead to immediate deleterious effects on almost all systems of the body including gastrointestinal tract, central nervous system, cardiovsacular system and hepato-renal system, causing gastrointestinal hemorrhage, cardiovsacular collapse, unconsciousness or convulsions, severe metabolic acidosis and acute respiratory distress syndrome. We treated a 39-year-old woman who ingested 300 ml formalin in a suicidal attempt. Despite hemodialysis, death occurred after 23 h.
