Effects of Indirect Moxibustion on Skeletal Muscles in Mouse Model of Skeletal Muscle Adiposity (간접구 시술이 골격근 Adiposity 유발 쥐의 근육조직에 미치는 영향)

  • Objectives : To observe the regenerative effects of indirect moxibustion, a traditional Korean medical treatment on skeletal muscles using mouse model of skeletal muscle adiposity. Methods : Twenty seven ICR male mice were randomly assigned into Intact control(n=3), glycerol treatment together without moxibustion(n=12), and glycerol treatment together with moxibustion (n=12) groups. Mice of glycerol treatment groups were injected with 50 ${\mu}l$ DW(distilled water) containing 50 % of glycerol into the two tibialis anterior. After injection, moxibustion was applied at 'Shenshu'($BL_{23}$) and 'Zusanli'($ST_{36}$) acupoints three times per each session, every days for twelve days(total 12 treatments). Phospho-Erk1/2, Myostatin protein levels were analyzed by western blotting and immunofluo-rescence staining techniques for tissues of the tibialis anterior muscle. Smad, phospho-Smad were analyzed by immunofluorescence staining. Results : 1. Histological analysis of sections from injected TA muscles showed that glycerol induced rapidly muscle necrosis, with a maximum at day 3. 6 days and 9 days after injection, muscle was regenerating. 2. According to western blotting and immunofluorescence staining, phospho-Erk1/2 protein signals in glycerol treatment with moxibustion group were stronger compared to Intact and glycerol treatment without moxibustion group. 3. According to western blotting and immunofluorescence staining, myostatin protein signals in glycerol treatment without moxibustion group were stronger compared to Intact and glycerol treatment with moxibustion group. 4. According to immunofluorescence staining, Smad protein signals in glycerol treatment without moxibustion group were stronger compared to Intact and glycerol treatment with moxibustion group. 5. According to immunofluorescence staining, phospho-Smad protein signals in glycerol treatment without moxibustion group were stronger compared to Intact and glycerol treatment with moxibustion group. Conclusions : These results confirm that indirect moxibustion of 'Shenshu'($BL_{23}$) and 'Zusanli'($ST_{36}$) influences muscle regeneration in mouse models of skeletal muscle adiposity. Further discussion, and the establishment of moxibustion mechanism will prompt clinical application of moxibustion.

Effect of GB 34-GB 39 Electro-acupuncture on Regional Cerebral Blood Flow in Stroke Patients and Normal Volunteers Evaluated by $^{99m}Tc-ECD$ SPECT (양릉천-현종 전침치료가 뇌경색환자 및 정상인의 뇌혈류에 미치는 영향 - SPECT와 SPM을 이용한 연구 -)

  • Objectives: Acupuncture has been applied in Asia for thousands of years, especially to rehabilitation after stroke. It has been reported that acupuncture increased cerebral blood supply and stimulated the functional activity of brain nerve cells shown by using brain imaging techniques. This study was to evaluate the effect of GB 34-GB 39 electro-acupuncture (EA) on regional cerebral blood flow (rCBF) in stroke patients and normal volunteers using single photon emission computed tomography (SPECT). Methods: The study procedure was divided into two parts: patients and volunteers studies. For the patients study, ten ischemic stroke patients (3 males, 7 females, mean age $68.5{\pm}8.9$ years old) were selected. Baseline brain SPECT was done with triple head gamma camera (MultiSPECT3, Siemens, USA) after intravenous administration of 1,110 MBq of $^{99m}Tc-ECD$. Fifteen-minute EA at GB 34 and GB 39 were applied on the affected limb. The same dose of $^{99m}Tc-ECD$ was injected during the EA, and the second set of SPECT images wasobtained. Using the computer software (ICON 7.1, Siemens, USA), 3 SPECT slices (upper, middle, lower) surrounding the brain lesion were selected and each slice was divided into 10-16 brain regions. Asymmetry indexes (AI) were analyzed in each brain region. We regarded over 10% changes of AI between before and after EA as significance. For the volunteers study, 10 healthy human volunteers (5 males, 5 females, mean age $28.1{\pm}6$ years old) were selected. In the resting state, $^{99m}Tc-ECD$ brain SPECT scans were performed. On the 7th day after the resting examination, 15 minute EA was applied at GB 34 and GB 39 on the right side of the subjects. Immediately after EA, the second SPECT images were obtained inthe same manner as the resting state. Significant increases and decreases of rCBF after EA were estimated by comparing their SPECT images with those of the resting state using paired t statistics at every voxel, which were analyzed by statistical parametric mapping with a threshold of p = 0.01, uncorrected (extent threshold: k=100 voxels). Results: In stroke patients, six of the eight (75%) had significantly increased perfusion in post-acupuncture scans compared to their baseline state. In normal volunteers, GB 34-GB GB EA increased rCBF in both hemispheres including right ventral posterior cingulate (Brodmann area (BA) 23), left superior temporal, anterior transverse temporal (BA 22, 41), left parastriate, peristriate (BA 18, 19), right occipitotemporal, angular (BA 37, 39), left rostral postcentral, caudal postcentral and preparietal (BA 2, 3, 5). However GB 34-GB 39 EA decreased rCBF in the right hemisphere including triangular and middle frontal lobes. Conclusions: The results demonstrated that OB 34-GB 39 EA increased cerebral perfusion in ischemic stroke patients and increased rCBF grossly in temporal lobes of normal volunteers. It is also suggested that there may be a correlation between the GB meridian and the territory of the middle cerebral artery.

  • It is generally understood that San Bu Jiu Hou is the pulse form at CunGuanChi(寸關尺) as in ${\ulcorner}$NanJing(難經)${\lrcorner}$. However, it is totally different in ${\ulcorner}$HuangDiNeiJing${\lrcorner}$. This only appears in tew chapters of ${\ulcorner}$SuWen(素問)${\lrcorner}$ and does not appear in ${\ulcorner}$LingShu(靈樞)${\lrcorner}$. SanBu in ${\ulcorner}$SuWen SanBuJiuHouLun${\lrcorner}$ refers to top, middle, bottom and each part is divided into 3 parts, Tian(天), Di(地), Ren(人) to form JiuHou, and through Jiu Hou, not only does it diagnose ShenZang(神臟) and XingZang(形臟), but also goes on to form a diagnostic system by fusing diagnostic skill and treatment into one. ${\ulcorner}$JiuZhenShiErYuan(九針十二原)${\lrcorner}$ discusses detailed shapes and functions of nine types of acupuncture, and the ${\ulcorner}$GuanZhen(官針)${\lrcorner}$ explains how to manipulate Jiu Zhen adequately, but there is more to it than just shape and function in techniques of acupuncture. It is because it fuses (or merges) pathology, diagnostics, treatment etc to form a single diagnosis system. ${\ulcorner}$JinFu(禁服)${\lrcorner}$ discusses about nine types of acupuncture of pulse form and effect, which are treatment means based on RenYingCunKouMaiFa(人迎寸口脈法). Various pulse daignosises exist in ${\ulcorner}$HuangDiNeiJing${\lrcorner}$, but those influence of future generations can be divided into SanBuJiuHouMaiFa(三部九候脈法) and RenYingCunKouMaiFa(人迎寸口脈法), and which medical ideologies this kind of pulse daignosis originates from should be discusssed. We will finally expolre and report the process its development into 寸尺脈(Cun Chi Mai).

A Study of EEG Analysis for the Moxibustion Stimulation (간접 뜸 자극에 관한 EEG 분석)

  • Although research efforts for brain waves have prospered in medicine and engineering, acupuncture still has a long way to go regarding researches on brain waves analysis. Thus this study set out to analyze brain waves stimulated by indirect mugwort moxibustion, which was part of acupuncture techniques, and to investigate their correlations with the automatic nervous system. For the experiments, stimulation was given to Jungwan, Shingwol and Gwanwon, which were some of the spots on the body suitable for acupuncture, through indirect mugwort moxibustion. The subjects' brain waves were measured before the stimulation, during the stimulation, and one hour and two hours after the stimulation. The measurements were analyzed with Matlab 7.0 for FFT and frequency power spectrum. Then the ${\alpha}$, ${\beta}$, ${\delta}$, and ${\theta}$ waves were analyzed and examined for changes to the percentage of each frequency and to the amplitude of vibration according to the stages of stimulation. The EEG data of the entire brain were translated into FFT to analyze the percentage of the ${\alpha}$, ${\beta}$, ${\delta}$, and ${\theta}$ waves. As a result, the ${\alpha}$ waves recorded a double increase after the stimulation. The power spectrum analysis results of the entire brain decreased the ${\alpha}$ and ${\beta}$ waves dropping in the energy level, which suggested that the parasympathetic nerves were activated. When the results of the study were compared with those of the previous study, it's confirmed that indirect moxibustion stimulation could cause changes to the automatic nervous system and bring stability to those who were nervous or under stress due to the proportionate increase of the ${\alpha}$ waves.

영국의 중의학: 의사, 의사가 아닌 시술자, 중국인 시술자

  • 중의학은 서양의 의료 다원주의의 한 부분이 되었다. 특히 침과 같이 서양의 개념과는 분명히 다른 의료 기법에 있어서는 두드러진다. 그밖에 뜸, 안마, 기공, 중약 및 복합 처방들도 있다. 서양에서 전문직업인화는 보건의료 관련분야에서 중심적인 인력양성 방법이 되었다. 이는 대학이 기반을 다진 중세까지 거슬러 올라가는데, 19세기에는 법인들이 전문직업인 모임으로 바뀌어 가면서 개인적 환경을 변화시키고 직업의 체계를 개발하였다. 변호사, 교사, 사제, 의사가 그 예이다. 의료에 있어서 학교교육을 통해 수련된 의사들이 조산사나 안마사들을 지배하게 되었는데 이런 경향은 1950-60년대에 극에 달한다. 그러다가 이들의 지배는 대중매체, 환자, 다른 주변 의료 보조인력들에 의해 도전을 받는다. 이와 비슷한 양상을 이단적인 의료에서도 관찰할 수 있다. 전문직업인의 우세와 시점이 비슷한 생물의학의 우월적 지배는 많은 다양한 의료에 의해 도전을 받는다. 그러나 생물의학의 우월함의 약세가 시술자들의 전문직업인화 까지를 포함하지는 않았다. 실제로 침술과 같은 치료술들을 전문직업인인 의사나 의사가 아닌 시술자들이 함께 쓰고 있기 때문이다. 이는 직능간의 분쟁을 야기할 가능성이 높다. 이단적 개념을 기존 의료계로 통합하는 것이 전문직업인들인 의사들의 노력으로 이해되었다. 이 견해는 두 가지 문제를 내포하고 있는데, 첫째 의사들을 단일한 속성으로 보고 있다는 것인데, 실제로 현대의학을 배우고 가르치는 나라들 사이에도 매우 다양한 차이가 있음을 간과하고 있다. 둘째로 의료인이 아닌 시술자들도 다양한 분야로 통합되었는데, 통합은 어쩌면 하급의 전문인들의 노력의 결과로 설명되어야 하는 것이 더 알맞을 수 있다. 그러나 이들도 전문직업인의 조직구조를 가져야 만 하고, 그렇지 않으면 파편으로 남을 것이다. 전문직업인주의는 언제나 과학과 연계하고 있는데, 독일 정부와 직업인 조직은 의료과학 집단에게 이단적 시술들의 임상효과를 검증하게 하였다. 이것은 이단 의료계에 우호적인 단체나 적대적인 단체 모두에게 적용되었다. 매우 강한 힘을 가진 냉소적인 조직 가운데 하나는 National Committee of SHI-Physicians and Sickness Funds(Bundesausschuss der Arzte und Krankenkassen).2 인데, 2001년에, 이 단체에서 출판한 자료에는 침의 효용성은 침을 시술하는 사람에 달린 것 일뿐 시술자의 수련이나 침 시술의 배경 개념에 달려있는 것이 아니다. 그러므로 침은 무작위 표본추출 대조군 실험을 통해 효과를 검증받지 못한 다른 치료법들의 범주에 머물러 있다.(cf. Bundesausschuss 2001: 8). 1990년대, 무작위 표본추출 대조군 실험은 생물의학이 아닌 의료의 효과를 검증하는데 알맞지 않고 비윤리적이라고 강한 비판을 받았다. 한편 1995년에 WHO는 침의 유효성 평가를 위해 이 실험기법을 추천하며, 이어 2001년에, the International Council of Medical Acupuncture and Related Techniques(ICMART)는 침연구와 시술을 위한 헌장(Acupuncture Charter Berlin an Evidence Based Medicine (EBM)3 for acupuncture)을 채택 공표한다. 독일 보험 회사들도 침의 효과와 효율성을 검증하기 위해 대규모 실험을 시작했다. 100,000 명이 넘는 환자들이 무작위 표본추출 대조군 실험을 통해 평가되고 있는데 이 실험은 병원이나 일차의료기관의 외래에서 진행되고 있다. 영국에서는 이 분야의 유력한 Edzard Ernst가 위의 개념과 비슷한 구도를 설정 공표하는데 보완의학 분야의 많은 다른 전문가들의 견해를 무릎 쓰고, 그는 증거기반 의학에 대한 그의 신념을 표방한다. "무작위 표본추출 대조군 실험이 보환의학 분야의 시술을 검증하기 위한 알맞은 방법이 아니라고 끝없이 주장하지만 우리는 거의 모든 치료법을 포괄하는 분야에 이들 실험을 발견하였으니, 이는 보완의학도 엄정한 방법으로 검증할 수 있다는 것을 보여준다고 할 수 있다."(Ernst et al. 2001: xiii). 적어도 독일에서 진행중인 효과 검증 연구가 향후 아시아 의학의 기반 마련에 중요한 역할을 하게 될 것이다. 영국에서는 이에 대한 정치적 수렴이 전문 직업인화에 달려 있을 것이다. 여기에서 나는 아시아에서 유래한 의료의 서양에서 어떻게 사회학적 관찰의 대상으로 분류되고 정의되는지 소개하고자 한다.

Effects of the Doin Gigong Exercise on the Shoulder-Arm Pain in Women (도인기공체조가 여성의 견비통(肩臂痛)에 미치는 효과(效果))

  • Objective : This study intends to identify the effects of the Doin Gigong Exercise on the shoulder-arm pain in Women. Methods : The study was performed with nonequivalent control group pretest-posttest design. A total of 45 subjects were divided into 25 persons in experimental groups and 20 persons in contrastive group by convenience sampling. The data were collected by using questionnaires and measured values from June, 2004, to August, 2004. Results : The results of the study are as follows: 1. The homogeneity testing revealed that most study variables were equally distributed between experimental and control groups. 2. The first hypothesis: The experimental group who performed the Doin Gigong Exercise and who decreased on the subjective symptom scale was supported(t=10.245, p=.000). 3. The scond hypothesis : The experimental group who received Doin Gigong Exercise and who decreased on the subjective pain(VAS) was supported(t=6.816, p=.000). 4. The third hypothesis: The experimental group who received Doin Gigong Exercise and who decreased on the muscular stiffness measured techniques(UNIQUE) was supported (t=7.114, p.000). Conclusions : Based on the results described above, it is considered that the Doin Gigon Exercise is clearly effective in all an values of the 5 measured. Therefore, the Doin Gigong Exercise can be thought of as effective nursing intervention for the reduction of shoulder-arm pain in women.

A Review of Recent Studies for Treatment of TMD Using CNKI Database (CNKI 검색을 통한 턱관절 장애 치료의 최신 연구 동향)

  • Objectives The purpose of this research is to investigate recent clinical studies of Temporomandibular Joint Disorder in China. Methods We investigated recent clinical studies of Oriental Medicine therapies in traditional Chinese medical journals for Temporomandibular Joint Disorder through CNKI search. 20 clinical articles published from 2010 to 2015 were analyzed. This study examined the authors, published years, types of study designs, criteria for diagnosis and evaluation, periods, purposes of study and classified articles by techniques of treatment. Results Most of articles were classified as RCT. TMD was diagnosed by symptoms in a high proportion of articles. The criteria for evaluation that most frequently used were grading scale, but there was a lack of objectivity. The techniques of treatment were Tuina, acupuncture, herb medicine, electrotherapy, splint, PNF, congnitive behavior therapy, laser therapy, magnetic therapy. Conclusions In order to develop treatment of TMD in the Korean medicine, clinical studies for various therapies on a high level and cooperative studies between medical communities are needed.

Evaluation of the Quality of Case Reports in Journal of Oriental Neuropsychiatry Using CARES Guideline (CARES 지침에 따른 동의신경정신과학회지의 증례보고에 대한 질 평가 연구)

  • Objectives: Case report is a description of medical experience that contains a patient's information and treatments. It has been used to determine the effectiveness and side effect of the treatments. Case report can be used for medical, scientific, or educational purposes. The objective of this study was to evaluate the quality of case reports published in Journal of Oriental Neuropsychiatry from 2013 to 2017 using CARE and CARES guideline. Methods: Case reports were selected from Journal of Oriental Neuropsychiatry and their quality assessed using CARE guideline and CARES guideline. The items of CARE and CARES guideline were assessed as 'Sufficient', 'Not sufficient', 'Not reported', 'Not applicable'. The outcome was analyzed using case reports and items. Results: 26 case reports were selected. For CARE guideline, 'Sufficient' percentage was min at 22.2%, max at 46.4% with a median of 39.3%. Sum of 'Sufficient', 'Not sufficient' percentage was min at 59.3%, max at 85.7% with a median of 75.0%. Items 'Timeline', 'Diagnostic assessment', 'Follow up and outcomes', 'Informed consent' were not reported in more than 50% of all case reports. Items 'Keywords', 'Abstract', 'Introduction', 'patient information', 'Diagnostic assessment', 'Therapeutic interventions', 'Follow up and outcomes' were not sufficiently reported in more than 50% of all case reports. For CARES guideline, 'Sufficient' percentage was min at 0%, max at 50% with a median of 0%. Sum of 'Sufficient', 'Not sufficient' percentage was min at 33.3%, max at 100% with a median of 92.9%. Items of herbal prescription 'herb', 'decoction', 'use', items of acupuncture treatment 'materials', 'techniques', items of moxibustion treatment 'materials', 'techniques' were not sufficiently reported in more than 50% of all case reports. Conclusions: The reporting rate of items was good. But the quality of reporting was low. Also, particular items were not reported frequently. Therefore, there is a need to improve the quality of case reports in Journal of Oriental Neuropsychiatry.

Review of Clinical Studies on Korean Medicine and Complementary and Alternative Medicine Treatment for Allergic Rhinitis in the Korean Literature (알레르기성 비염의 한의학 및 보완대체의학적 치료에 대한 국내 임상 연구 동향)

  • Objectives The purpose of this study is to analyze Korean clinical studies in Korean medicine and complementary and alternative medicine for allergic rhinitis and to propose for better methods of clinical studies and effective treatments on allergic rhinitis. Methods Electronic researches were performed with NDSL, RISS, KISTI, KISS, OASIS by keyword 'Allergic rhinitis'. Results The treatments used in 36 studies were herbal medicines, acupuncture, pharmacopuncture, moxibustion, cupping, inject medicine to nasal cavity, aroma therapy, ointment, nasal cleansing solution, laser treatment, infra-red therapy, oxygen nebulizer, diet treatment, moxa-pellet treatment, mindfulness meditation and space-spine manipulation techniques. The most common acupoints were LI4 and LI20 (Large Intestine Meridian). The most common methods of assessment is subjective nasal symptoms. All studies using subjective nasal symptoms and quality of life as assessment methods to show valid results on the improvement of allergic rhinitis. Conclusions This study shows that Korean medicine and complementary and alternative medicine on allergic rhinitis are effective. It will be expected to use that various methods for allergic rhinitis and more clinical studies for allergic rhinitis and pediatric patients needed in the future.

The Effects of Alphabet Exercise with SSP Electrotherapy on Lateral Ankle Sprain (족관절 염좌에 SSP 전자침 요법을 병행한 동기침법(動氣鍼法)의 효과)

  • The multiple acupuncture techniques have been used to relieve the pain of lateral ankle sprain. Recently, early functional rehabilitation is emphasized in rehabilitative concepts using active ROM, facilitated proprioception, alphabet exercise and so forth. Similarly, in oriental rehabilition medicine, traditional movement technique called "Dong-Ki" has been used to relieve pain of ankle sprain. So we used "Dong-Ki" technique with SSP(silver spike point) electrotherapy transformed from alphabet exercise and evaluated the effects in method of randomized controlled trials. Methods : 27 outpatients with lateral ankle sprains were randomized into two groups : 15 samples and 12 controls. Sample group was treated with "Dong-Ki" technique(writing his name with foot) with SSP electrotherapy to relieve pain during additionally. "Dong-KI". Outcomes were measured by 10cm VAS and ankle circumference(cm). Results : In terms of VAS, Sample group had inclination of pain relieving but not significantly meaningful. In terms of ankle circumference measurement, Sample group showed significantly improved.
