주사전자현미경(SEM)을 이용한 일회용침 끝의 미세 형태에 대한 관찰(2) (Investigation of Micromorphological Characteristics of Acupuncture Needle Tip Using SEM in Korea(2))

  • Objective: There have not been so many study about the quality of acupuncture needle tip. In order to ensure safe acupuncture treatment, the quality of needle tip is essential. Therefore we investigated the current condition of the tip of the acupuncture needles in Korea. Methods: We have selected the needles that made by 8 companies in Korea, and selected 50 pieces from 1000 pieces each company by randomized methods. and observed the tip of each needle using a scanning electron microscope at ${\times}800$ magnification. Results & Discussion: We found that needles had several defects such as scratch marks on the surface, metallic scuff, lumps and irregularities of the needle tips, stubbed or malformed tips, tips of point off-center, peeled off coated tips. There was much difference on the quality of needle among the manufacturer, and some needles seem to need to do- thorough quality control. It is necessary to intensify quality management and a concentrated control on manufacturing process of acupuncture needles.

전침 시술시 침 재료에 따른 부식성 평가 (The Corrosiveness Evaluation according to the Needle Material in the Electroacupuncture Operation)

  • Objectives : Electroacupuncture is widely used because of its diverse curative influence. However, its electrical safety is not guaranteed when applied on the human body. Therefore, in this study, we did research on the corrosiveness of electroacupuncture when applied on the body fluid. Methods : We did research using acupuncture needles which were made of STS304 or STS316 and which were coated or not coated in silicone. They were 0.25 mm in diameter and 40 mm in length. We immersed them to the depth of about 1cm in Hank's Solution and gave a stimulus in 120 Hz for 60 minutes. Then, we measured the pH of Hank's solution and mass of needles and observed their shape. Results : Acupuncture needles which were made of STS304 corroded more easily than those which were made of STS316 when they were applied on the human body. Acupuncture needles which were coated in silicone corroded much more easily than those which were not coated in silicone when they were applied on the body fluid. Conclusions : In this study, we did research on the corrosiveness of electroacupuncture when it is applied on the body fluid using several acupuncture needles. We made a conclusion that when acupuncture needles were applied on the body fluid, the coated one made of STS304 corrodes more easily than the uncoated one made of STS316. We hope that additional further researches on the effect of the corrosiveness of an acupuncture needle will be carried out.

Illustrations of the Nine Types of Needles based on Huangdi's Internal Classic Ling-shu

  • Background: The phrase "Nine Needles" refers to the 9 types of acupuncture needles describing their shapes, sizes, and uses in Huangdi's Internal Classic Ling-shu. The aim of this study was to present 3D illustrations of the "Nine Types of Needles" based on Huangdi's Internal classic Ling-shu, taking into consideration the conformation and application of the Nine Needles. Methods: Sketches of the "Nine Needles" were based on references to those needles cited in Huangdi's Internal Classic Ling-shu, the Great Compendium of Acupuncture and Moxibustion, and the Golden Mirror of Medicine. The computer programs Creo 3.0, Keyshot 5, Adobe Photoshop CS5, and Adobe Illustrator CS5 were used for 3D modelling and visualization. Results: Based on a review of Huangdi's Internal Classic Ling-shu, illustrations of the Spade needle, Round-Pointed needle, Pressure needle, Sharp-Edged needle, Sword-Shaped needle, Round-Sharp needle, Fine needle, Long needle, and Large needle, 3D models were created. The Spade needle had a sharp, large head, the Round-Pointed needle had an egg-shaped tip, and the Pressure needle had a blunt head like a grain of millet. The Sharp-Edged needle had a sharp blade with a triangular edge for bloodletting. The Sword-Shaped needle resembled a sword. The Round-Sharp needle resembled a horse's tail. The Fine needle and the Long needle had sharp points and thin bodies. The Large needle had a cylindrical shaft and rounded tip. Conclusion: This study demonstrated that 3D illustrations could be generated for the Nine Needles according to the descriptions and figures provided in the ancient literature.

거짓침을 이용한 침연구의 현황 (Current evidence on acupuncture from sham needle studies)

  • Objectives : There has been a considerable debate about how to best control for placebo effects in clinical trials of acupuncture. Recently several sham needles were developed and validated. This study aimed at summarising the validation studies of these needles and evaluating the outcomes of the randomised controlled trials (RCTs) using them. Methods : Computerised literature searches were performed using 'acupuncture' AND 'placebo OR sham' with a limitation of the results to RCTs in Medline via PubMed and the Cochrane Library. Only formally validated sham needle controlled studies were included. Data were extracted regarding study design, condition, sample size, credibility testing, intervention and outcomes. Methodological quality was assessed using a modified Jadad scale. Results : Three validated sham needles by Streitberger, Park, and Fink, were identified. Acupuncture's effectiveness for various conditions was tested using these needles in 12 RCTs. Real acupuncture was superior to sham acupuncture for rotator cuff tendonitis and hypertension. No significant differences between real acupuncture and sham acupuncture emerged for chemotherapy-related nausea and vomiting, postoperative nausea and vomiting, menstrually related migraine, acute stroke rehabilitation, chronic/episodic tension-type headache, neutrophil respiratory burst in healthy volunteers, alcohol withdrawal symptoms and chronic poststroke leg spasticity. Conclusions : The new sham devices have been adequately validated and may be useful tools for investigating specific research question. In spite of the sham needle's limits, the results of RCTs using such devices tend to suggest that the clinical effects of acupuncture are largely due to a placebo response.

STS304 스테인리스 강선 호침에서 전침 자극 후 침체에 나타나는 전기적 부식 연구 (Electric Corrosion of STS304 Acupuncture Needles Used for Electroacupuncture)

  • Objectives : The electroacupuncture(EA) was often used in Oriental medicine clinic and basic researches. However, there have been neither any national standard of acupuncture needles in Korea nor many studies about the property of acupuncture needles for applying electrical stimulations. Therefore we investigated the electrical corrosion property of acupuncture needles used for EA stimulation in clinic. Methods : We observed the occurrence of electrical corrosion like changes in the surface of acupuncture needles after EA stimulation with various parameters(electrical stimulation intensity, treatment duration, diameter of needle) under a light microscope. We performed EA stimulation on Baekhoe($GV_{20}$) or Joksamli($ST_{36}$) of rats with intensity of 0.025, 0.05, 0.075, 0.1, 0.25 or 0.5mA at a 10Hz frequency. Results : The electrical corrosion-like changes were found on the surface of needles when the EA stimulation was given over 0.05mA of intensity. When electrical stimulation was treated for 10min, the electrical corrosion-like changes were low, but the electrical corrosion-like changes gradually increased by the treatment duration(20, 30min). We also found that acupuncture needles of 0.2mm diameter were produced more extensive electrical corrosion-like changes than acupuncture needles of 0.3 or 0.4mm diameter by electrical stimulation. Conclusions: These results suggested that EA stimulation could produce electrical corrosion-like changes of acupuncture needles through intensity of current, duration of electric stimulation and/or diameter of needle.

SEM-EDX를 이용한 침 끝의 미세 부착물의 조성에 대한 관찰 (Investigation of Micromorphological Characteristics of Acupuncture Needle Tip Using SEM-EDX)

  • Objectives : There have been several studies about the quality of acupuncture needle tip recently. We have investigated the condition of the tip of the acupuncture needles in the last studies. In the former studies, we discovered the metallic scuff, lumps and irregularities of the acupuncture needle tips under the microscope. But, no information was available on those foreign materials' identity. Methods : We have selected 200 needles of 1000 pieces from several companies by randomized methods. And we observed the tip of the 6 needles selected finally at ${\times}1000\;or\;{\times}3000$ magnification and analyzed the components of the metallic scuff, lumps and irregularities of the needle tips with a SEM-EDX analyser. Results : We found that the identity of the metallic scuff, lumps and irregularities of the needle tips were metallic materials and silicon. For example, A point was composed of Fe(69.78%), Cr(17.71%), Ni(8.11%), Zn(2.04%), Si(1.23%), Mn(1.12%), and B point was composed of Si(66.40%), Fe(26.76%), Cr(6.84%). Conclusion : The results of this study confirm that there is a real possibility of the remaining of metallic materials and silicon in body of patient, after acupuncture treatment. Therefore, it is necessary to intensify our efforts to make needles of good quality and to concentrate on manufacturing process of acupuncture needles completely to be free from danger in acupuncture treatment.

구침(九鍼)의 형태와 용도에 대한 문헌연구 (Literature Study on the Conformation and Application of Nine Classical Needles)

  • Objective : Nine classical needles have been recorded in oriental medical classics as a diversified instrument for acupuncture to treat patients with various symptoms. Recently, it has suggested that doctors didn't make full use of acupuncture and that poor understanding of the nine needles has caused the lack of usage. Methods : It has studied bibliographically about the conformation including a length & shape and application & usages of the nine classical needles presented in oriental medical classics. Results : Chamchim (shear needle) has 1.6 chon (寸) length, sharp apex treating fever in the head and trunk; Wonchim (roun-pointed needle) has 1.6 chon, eggshaped apex treating disease of flesh by massaging with it; Sichim (spoon needle) has 3.5 chon, thick body with round sharp apex treating weakness of Qi; Bongchim (lance needle) has 1.6 chon, a triangled apex treating chronic disease with bleeding; Pichim (stiletto needle) has 4 chon, razor sharp applying to surgery of big pus; Wonlichim (round-sharp needle) has 1.6 chon, thin body, sharp apex treating acute arthralgia syndromes; Hochim (filiform needle) has various lengths which treats various disease of meridians and organs; Jangchim (long needle) has 7 chon, sharp apex treating chronic arthralgia syndromes in deeper place of body; Daechim (large needle) has 4 chon, nail like apex applying to excrete artheredema in joints. Conclusions : The conformation of nine classical needles has been changed from those recorded in oriental medical classics since it was developed. However, the usage of nine classical needles has remained the same. Therefore it is considered that intrinsic attribute of the nine needles has preserved even though the conformation of nine classical needles has changed.

체내에서 침이 발견된 사례 보고 (A Report on Three Cases of Acupuncture Needles Found Remaining in Patients' Bodies)

  • Objectives : To report the cases of forgotten needles found in the patients' bodies since Dong-guk University Ilsan Hospital opened, and arouse considerable attention to prevent recurrence. Methods : Oriental medical doctors in the hospital has applied acupuncture on patients' bodies in case of need, and the other staffs removed the needles after treatment. Results : All the patients in the cases had cerebrovascular disease with paralysis, aphasia, cognitive disorder and disturbance of consciousness as well as dysphagia requiring gastrotomy tube and dysfunction of respiration requiring tracheal cannula. The pathways the acupuncture needles were passing through are presumed to be transtracheal via the hole of tracheal cannula in two cases, and transgastrointestinal via mouth or the opening of gastrotomy tube. The needle was excreted with stool without any harm to the patient in the latter case, while the impact of the needles on the respiratory system or thorax of the patients in the former two cases is ambiguous. The hospital has taken some measures to prevent recurrence. Conclusions : There are not a few at high-risk of occurrence of forgotten needles that might induce medical problems among patients in oriental medical clinics or hospitals. Authors expect this report arouses all the people involved in the whole procedure of acupuncture treatment, including oriental medical doctors, nurses, nurse-assistants and caregivers considerable attentions to the possible occurrence and risk of forgotten needles.

이침 임상시험에서의 대조군에 대한 문헌고찰 (A literature review on controlling methods used in clinical trials of auricular acupuncture)

  • Objectives : To review the controlling methods used in RCTs (randomized controlled trials) of auricular acupuncture. Methods : To investigate the controlling methods used in clinical trials of auricular acupuncture, 46 articles were retrieved from PUBMED (from August 1976 to August 2007, with limits: english, clinical trial) using the key words of "ear acupuncture", "auricular acupuncture", "auriculo-medicine", and sorted out according to the types of control treatments. Among them, 10 articles were selected based on STRICTA (Standards for Reporting Intervention in Controlled Trials of Acupuncture), and the controlling methods used in those articles were reviewed. Results : In clinical trials of auricular acupuncture, 'sham acupoints (non-acupoints and non-treatment acupoints included)', 'minimal acupuncture', 'pseudo-intervention', and 'placebo devices' have been used as controlling methods, but their 'inactivity' have been in question. To compensate the disadvantages of 'minimal acupuncture', and 'pseudo-intervention' control, placebo auricular acupuncture needles have been developed, although these needles didn't satisfy the conditions of 'blinding' and 'physical inactivity' simultaneously. Conclusions : Further researches on 'sham acupoints' and new development of 'placebo auricular acupuncture needles' are needed.

침 금속재료에 대한 생체적합성 연구 -생물학적 특성을 중심으로- (A Study for Biocompatibility of Acupuncture′s Metal Materials, - Focused on Biological Character -)

  • Objectives : To present criteria for acupuncture standardization, which will improve quality of acupuncture and secure safety, through studies of the biocompatibility of acupuncture materials. Methods : Acupuncture needles distributed in Korea were studied. Intracutaneous reaction tests and sterility tests were executed to study the biocompatibility of acupuncture. Results & Conclusions : All 4 kinds of acupuncture needles distributed in Korea were found suitable for medical use, through intracutaneous reaction tests and sterility tests. However, the medical instrument standards of the Korea Food and Drug Administration lack criteria concerning intracutaneous reaction tests and sterility tests. The criteria for these tests should be standardized in order to secure the biocompatibility of acupuncture needles.
